Hemodynamic effects of loud noise before and after central sympathetic nervous stimulation.

The hemodynamic effects of loud noise after central alpha 2-adrenoceptor stimulation were studied in 13 patients with mild (WHO 1) essential hypertension. The patients were randomized (double-blind) to treatment with either placebo or guanfacine 1-2 mg for four weeks and then crossed over and treated for another four weeks. All patients were exposed to a loud broad-band noise (105 dBA for 30 min) and all were studied both on placebo and guanfacine. Guanfacine significantly reduced the resting blood pressure from 141/92 to 134/88 mmHg (p less than 0.01) as well as heart rate at rest from 63 to 58 beats/min (p less than 0.05). Noise stimulation caused a significant increase in blood pressure and resistance in the placebo-treated group, while cardiac output decreased significantly. Pretreatment for one month with the central alpha 2-adrenoceptor stimulating agent guanfacine did not block the noise-induced pressor response nor the increase in peripheral resistance. A significant decrease in stroke volume was observed and cardiac output also tended to decrease in this group. It could be concluded that loud noise is a potent pressor stimulus which causes vasoconstriction and that the blood pressure response during noise could not be blocked by the centrally acting antihypertensive agent guanfacine. Since noise causes vasoconstriction it also induces an increased tone in the small arteries and, if the noise stimulus is sufficiently strong and repeated for a long time, it might cause structural changes in the resistance vessels and permanent arterial hypertension in humans.

Vasculitis simulating eczematous dermatitis due to C2 deficiency.

In a patient with total C2 deficiency and repeated respiratory infections a papulovesicular rash showed to be due to leukocytoclastic vasculitis. The patient's brother also had a total deficiency of C2 but no clinical symptoms. HLA-investigation in both him and the proband showed the genotype commonly reported in cases with homozygous C2 deficiency. Their heterozygous mother and sister had slightly decreased C2 levels.

Addition of the calcium antagonist PN 200-110 to pindolol markedly augments the antihypertensive effect in essential hypertension.

Several large-scale studies have recently drawn attention to the fact that arterial hypertension frequently is inadequately controlled and that therapeutic alternatives other than the commonly employed stepped-care treatment may be needed in order to obtain normotension. For this reason PN 200-110, a new dihydropyridine calcium antagonist--at two different dose levels (average 3.8 mg b.i.d. or 5.7 mg b.i.d.)--or placebo was added in a double-blind cross-over trial to pindolol, 10 mg per day, in 20 patients with essential hypertension, after an initial 3-week placebo period. Ionized calcium in serum was determined repeatedly during the study. From an initial level of 157/100 mm Hg, PN 200-110 at the first dose level reduced blood pressure by 14/11 mm Hg (p less than 0.01/0.001) and at the second dose level reduced blood pressure by 22/18 mm Hg (p less than 0.001/0.001). The reduction in mean arterial pressure was significantly correlated to age (=0.050, p less than 0.05). There was no significant increase in heart rate, nor were there any significant correlations between ionized calcium and the effect of PN 200-110 nor between the changes in ionized calcium and the changes in blood pressure. Adverse effects were few and mild. One patient had to be withdrawn because of side effects, probably not related to the investigated drugs. Thus, addition of PN 200-110 to hypertensive patients treated with pindolol caused highly significant and clinically relevant further reductions in arterial pressure. The results show that a combination of this kind offers the possibility of good blood pressure control.

The long-term effect of isradipine in pindolol-treated patients.

The long-term efficacy of isradipine, a new dihydropyridine calcium antagonist with marked vascular selectivity, was evaluated in 17 patients with essential hypertension. All had a supine diastolic blood pressure of greater than 95 mmHg with 10 mg pindolol once daily. After a short-term, double-blind, dose-finding, crossover comparison with addition of isradipine or placebo twice daily, they continued on pindolol and their optimal dose of isradipine in a single-blind, long-term follow-up study. Eighteen patients were recruited but one male patient discontinued treatment after 2 weeks due to ankle oedema and will not be accounted for in the overall evaluation. There were 11 males and six females with a mean age of 56 +/- 10 years. In the short-term study on the optimal dose of isradipine (5.1 mg twice daily) blood pressure was lowered by 24/18 mmHg (P less than 0.001). No change in heart rate was seen despite the substantial drop in blood pressure. In the long-term study the patients were seen for a mean follow-up time of 12.5 months (range 4-17 months). After the longest follow-up time mean arterial pressure was 107.0 +/- 7.4 compared with 120.1 +/- 8.2 mmHg after placebo baseline [delta = 13 mmHg (11%), P less than 0.001, n = 17]. The heart rate was unchanged (delta = 0.2 beats/min, 95% confidence limits -3, +3), and so was ankle circumference (delta = 0.12 cm, 95% confidence interval, -1, +1). On the other hand, mean weight was reduced by 2 kg from 90 kg (P less than 0.05, n = 17).(ABSTRACT TRUNCATED AT 250 WORDS)

Blood pressure, erythrocyte sodium and potassium concentrations and Na+K+ATPase activity in children with hypertensive mothers.

Fifty-four children aged 10-15 years were studied. Twenty-two children had mothers who had sustained hypertension after a hypertensive pregnancy (HT) and 17 had normotensive mothers who had previously had a hypertensive pregnancy (NT). A control group consisted of 15 children with normotensive mothers (C). Blood pressure was significantly higher in the HT group in comparison with the C group (P less than 0.01). Erythrocyte sodium (Na+) and potassium (K+) concentrations were similar in all groups and not related to blood pressure. The Na+K+ATPase activity in erythrocyte membranes was lower in the HT group than in the NT group (P less than 0.02), but not significantly different from the C group. There was no statistically significant correlation between Na+K+ATPase activity and blood pressure.

Metal sensitivity: positive history but negative test indicates atopy.

Among patients with a history of metal sensitivity, 50 were patch-test-negative to nickel, cobalt and chromate. 20 of them were further examined epicutaneously and intracutaneously. They were also studied with a clinical diagnostic method for symptoms and signs of atopy. Most patients in this category were found to be atopic.

Natural history of dermatitis herpetiformis in southern Sweden.

A retrospective clinical survey of 96 patients with dermatitis herpetiformis (DH) was performed in two defined populations of 425,000 in southern Sweden. The incidence of DH was 1.05-1.13/100,000 inhabitants/year and the prevalence was approximately 20-25/100,000 inhabitants. In one-third of DH patients the age at onset was greater than 60 years. In women with DH a strong connection to thyroid dysfunction was observed, but also other conditions of probable autoimmune pathogenesis were found in both sexes. No connection to malignant disease was observed. DH seems to be less active the later in life it starts. Several patients with DH manage without dapsone or need dapsone just occasionally in connection with bouts. This is the case even without a gluten-free diet. Many mild cases of DH were observed without a gluten-free diet; therefore, this restricting regimen should be prescribed only in more active cases of DH.

Bile acid transport by serum lipoproteins in patients with extrahepatic cholestasis.

Individual bile acid conjugates were determined by gas-liquid chromatography in very low density (VLDL), low density (LDL) and high density (HDL) lipoprotein fractions obtained by sequential ultracentrifugation of serum from cholestatic patients. In the lipoproteins were found 16-48% of the serum bile acids. In LDL, the amount found was twice that in VLDL and HDL together. More cholic than chenodeoxycholic acid was detected in the lipoproteins. The occurrence of bile acids in lipoproteins might be of importance for the transport of bile acids from blood to peripheral tissues during cholestasis.

Subtotal C4 deficiency and SLE-like disease.

Extremely low C4 values were found in a 65-year-old man with relapsing arthritis and skin lesions of many years duration of the scalp, face, hands and feet together with painful ulcerations of the toes and fingers. The discovery was made during an exacerbation, but the deficiency of C4 persisted in repeated controls after remission. The clinical findings in connection with these low C4 values are in congruence with the diagnosis of inherited deficiency of C4.

Device for measuring dryness of the oral mucosa. A preliminary report.

Following a survey of various methods used to register oral mucosal dryness--xerostomia--a new device, measuring surface slide friction, has been developed. An instrument is devised to register the twisting force as well as the force at right angle to the surface. Calculations from these parameters give standardized expressions for friction between the probe of the instrument and the oral mucosa. By using methylscopolamine, 3 students were tested for dryness in the mouth. The instrument showed a peak of increased friction after about 60 min. The reliability was tested by using various water solutions of a macromolecule as interphase between the instrument and the steel plate. These preliminary tests indicated that the instrument showed values of good validity and reliability. It should be further developed for registering oral mucosal dryness in clinical settings.

Long-term effects of captopril and atenolol in essential hypertension.

Fifty patients with mild or moderate essential hypertension were randomized (double-blindly) to treatment with either captopril (n = 26) or atenolol (n = 24). Their mean supine diastolic blood pressure after placebo was 100-125 mmHg. The study included an initial dose finding phase (12 weeks) during which the dosages of captopril and atenolol were increased stepwise every second week in order to obtain normotension (supine diastolic blood pressure less than 95 mmHg). Hydrochlorothiazide was added when necessary. During the second phase of the study the patients were followed on active treatment for 2 years. After the initial 12 weeks of active treatment, recumbent and standing blood pressures had fallen significantly both in the captopril group (by 31/20 and 33/19 mmHg, p less than 0.001) and in the atenolol group (by 24/18 and 30/20 mmHg, p less than 0.01 (systolic), p less than 0.001 (diastolic)). The antihypertensive effect was maintained in both groups during long-term treatment. The antihypertensive effect of both agents was potentiated to the same extent by addition of hydrochlorothiazide. Side-effects were few and mild. It can be concluded that both captopril and atenolol are safe and effective antihypertensive drugs.

Blood pressure and response to "stress" in 11-16 year old children.

Blood pressure at rest and cardiovascular response to "stress" were studied in 27 girls and 33 boys 11-16 years old. One group (HT, n = 23) had hypertensive mothers with a previous hypertensive pregnancy, another group (NT, n = 20) had normotensive mothers with a previous hypertensive pregnancy and the control group (C, n = 17) had normotensive mothers with normotensive pregnancies. Resting blood pressure was 124/71 mmHg (HT), 117/67 mmHg (NT) and 112/65 mmHg in the 3 groups. Systolic pressure was significantly different in all 3 groups (p less than 0.05 - p less than 0.001). Responses to noise stimulation (100 dBA) were identical in all groups with increases in diastolic and mean arterial blood pressure and cardiac output. During a video game session increases in blood pressure and heart rate were equal in the groups but during physical exercise a slight decrease in diastolic blood pressure was seen in the NT and C groups only. Differences in blood pressure in children with varying maternal history of hypertension do not seem to reflect alterations in the cardiovascular response pattern to "stress".

A diagnostic tool for atopic dermatitis based on clinical criteria.

Forty-seven patients with atopic dermatitis was compared to non-eczematous controls with regard to the occurrence of different symptoms and signs. This comparison made it possible to analyze statistically atopic symptoms and signs with regard to their diagnostic importance. Hereby, a diagnostic point system was constructed which in 96% of probands gave the correct diagnosis. This diagnostic tool might be used when examining patients with obscure eczematous disease.
