[Lithiasis of the main bile duct: change in surgical approach].

Should the papilla Vateri be explored during surgery for gallstones of the main biliary tract? A group of 672 choledochotomies were analyzed retrospectively; out of precautions we approached the papilla Vateri in 75% of these cases. This group was compared to a prospective study of 200 consecutive choledochotomies of which 21% required dilatation or sphinectotomy of the papilla Vateri. The number of remaining gallstones was approximately the same in both groups. There were 5 deaths due to postoperative acute pancreatitis in the first series and none in the second. Dilatation of the papilla or sphincterotomy should no longer be performed, routinely in surgery of the biliary tract.

[EEA stapler in colon surgery].

The EEA stapler, for accomplishing stapled anastomoses in the gastrointestinal tract seems of a great interest. It opens new possibilities in case of carcinoma of the middle part of the rectum. It also makes the re-anastomose after a Hartmann procedure much easier. The results of 63 anastomoses on the colon or the rectum are reported.

[Hereditary myeloperoxidase deficiency. Clinical, biological and genetic study].

A case is reported of hereditary myeloperoxidase deficiency in a diabetic patient suffering from a Candida albicans hepatic abscess. Myeloperoxidase (MPO) is completely absent from the neutrophils and monocytes, although it is present in the eosinophils. Functional granulocyte studies have revealed normalchemotactic and phagocytic activity, although the bacterial activity is partially diminished with regard to Staphylococcus aureus and is almost nil with regard to Candida albicans. The granulocytic metabolism, when stimulated by zymosan, is characterized by greatly increased oxygen consumption. Genetic investigations have been conducted in 28 members of the patient's family. Cyto- and biochemical determination of MPO is clearly diminished in close relatives of the patient. Genetic analysis indicates recessive autosomal transmission with variable expressivity of the gene for the heterozygote state.

[New therapeutic acquisitions in the surgery of the liver and bile ducts].

During these last years, there has not been sensational changes in biliary surgery, but rather a progressive improving of already known procedures. Conversely, the surgery of the liver has had a remarkable evolution. Even without mentioning transplantation, the possibility of resecting large parts of the gland with a reasonable risk demonstrates the progresses done these thirty last years. It is probable that technics introduced by liver transplant teams, such as bloodless resection by cold perfusion in situ, will allow to go even further in subtotal liver resection.

Epithelial liver hamartoma, systemic arterial hypertension and renin hypersecretion.

The case of a 14-year old girl presenting with headaches, severe progressive hypertension and high plasma renin levels, in whom a voluminous epithelial liver hamartoma or adenoma was discovered at surgery is documented. The morphological characteristics of the hamartomatous abnormality are described and evidences are put forward which would suggest that the liver lesion might have been the site of the abnormal renin production which was responsible for the systemic arterial hypertension.
