Dermatobia hominis infestation.

A patient is reported who, after leaving Venezuela, developed some boils on the left upper limb inhabited by Dermatobia hominis larvae. The curious life-cycle of this tropical fly is described with some considerations about the diagnostic problem. A simple unreported way of larvae extraction is suggested.

Kaposi's sarcoma after immunosuppressive therapy for bullous pemphigoid.

Immunosuppressed patients are at risk of acquiring Kaposi's sarcoma. We describe a 86-year-old man who was receiving steroid therapy for bullous pemphigoid and rapidly developed Kaposi's Sarcoma. The authors review and discuss the role of acquired immunosuppression in the pathogenesis of Kaposi's sarcoma.

Lichen planus and chronic active hepatitis. A retrospective survey.

In a retrospective survey 95 in-patients with non erosive lichen planus (LP) were summoned to be investigated by means of routine liver tests. 44 patients responded. Remarkable abnormalities of liver tests were found in 6 patients who were further studied for antibodies to smooth muscle and mitochondria and submitted to liver biopsy. Histological diagnosis revealed chronic active hepatitis (CAH) in 5 cases (11.3%). Such an impressive prevalence and the histologic and immunologic similarities between the two diseases support the view that the association LP-CAH is not fortuitous and that both diseases may have the same autoimmune pathogenesis. Accordingly LP may be regarded as a major risk factor for liver cirrhosis.

Carpal tunnel syndrome appearing with prominent skin symptoms.

A case of carpal tunnel syndrome (CTS) occurred in which the skin manifestations predominated over the neurologic symptoms. The skin changes included cyanosis, anhidrosis, alopecia, nail dystrophy, and episodes of acute necrosis. Carpal tunnel syndrome is the most common neuropathy accompanying nerve compression. The results of therapy depend on early diagnosis and the rapid relief from compression. Dermatologists should be well aware of this syndrome, which is almost entirely ignored in the major textbooks of dermatology.

Immunofluorescence findings in haematoporphyrin-induced keloid.

A case of haematoporphyrin-induced keloid is described in which immunofluorescence (IF) revealed antibodies eluted from circulating lymphoid cells directed against fibroblasts. The antibodies eluted belonged to the main Ig classes IgD and IgM and they did not bind complement in vitro. Their possible activity in stimulating fibroblasts is discussed.

Is Dowling-Degos disease the same disease as Kitamura's reticulate acropigmentation?

A patient is described with the clinical and histopathological features of both Dowling-Degos disease and Kitamura's acropigmentatio reticularis. The possible identity of the two genodermatoses is discussed on the basis of the similarity of their clinical and histopathological features and of the presence of the two diseases in the family of the propositus.

The case of the felted wig.

A sudden matting of hair after shampooing of a hairpiece is reported, which led the patient to a lawsuit. Chemical analyses showed that hairs were weathered and led to a possible explanation of the curious phenomenon.

Flushing in rosacea: a possible mechanism.

Flushing in rosacea has been investigated by means of (a) pharmacological inhibition of some possible chemical mediators and (b) titration of bradykinin as a possible effector directly in the blood. Clonidine-inhibited flushing was seen in all patients (mean 45%), other drugs had poorer results. Bradykinin increased in all patients at the climax of flushing (mean 60%). These findings support the hypothesis that epinephrine promotes a bradykinin release responsible for vasodilation.
