Heart block in Marfan's syndrome.

Marfan's syndrome is classified as a heritable disorder of connective tissue in which the supporting structure is involved in clinical and pathologic alterations. In its classic form the syndrome is associated with abnormalities of the eye, aorta, and the skeleton. Conduction disturbances and ventricular arrhythmias have rarely been described in this syndrome. In this brief case report a patient with symptomatic heart block, prolonged atrial-His interval and ventricular arrhythmia in whom a permanent ventricular pacemaker has been implanted, is described.

Prenylamine-induced ventricular tachycardia and syncope controlled by ventricular pacing.

We describe eight patients treated for angina with prenylamine who developed life-threatening ventricular arrhythmias after QT interval prolongation. When prenylamine administration was stopped QT interval shortened to within normal values, while the ventricular arrhythmias were controlled by a temporary ventricular pacemaker and disappeared after several days. We stress the importance of surveillance of the QT interval and ventricular arrhythmias in patients receiving long-term treatment with prenylamine.

Amiodarone-induced thyrotoxicosis: four cases and a review of the literature.

Hyperthyroidism is a rare side effect of amiodarone treatment. Four patients with this untoward effect are described One patient with mild hyperthyroidism before treatment with amiodarone, developed prolonged severe resistant thyrotoxicosis 6 months after initiation of this drug. A second patient with Wolf-Parkinson-White syndrome and paroxysmal atrial fibrillation was successfully stabilized with amiodarone for 18 months. However, the arrhythmia recurred and the thyroid function tests were found to be elevated. In 2 other patients only slight elevation of the thyroid function tests were found without clinical manifestations 4 months after treatment with amiodarone had been initiated.

Maternal pulmonary edema occurring after therapy with ritodrine for premature uterine contractions.

Intravenous administration of beta-sympathomimetic drugs in combination with betamethasone or dexamethasone is a treatment often used for the suppression of premature uterine contractions and for the stimulations of fetal lung maturity. It has been suggested that the combination of these drugs may be responsible for maternal pulmonary edema and five such cases have been reported in the literature (1,2). We report a patient who developed pulmonary edema during ritodrine therapy. Corticosteroids were not given in this case.

Regional distribution of the MB isoenzyme of creatine kinase in the human heart.

Human myocardial tissue obtained at autopsy from ten patients was examined for content of the MB isoenzyme of creatine kinase (CK-MB). We wished to determine whether this isoenzyme is distributed homogeneously throughout the heart. In eight cases, there was no history or pathological evidence of heart disease. Two had a history of previous myocardial infarction; in these, tissue was obtained from sites distant from the scar. Difference was found between the CK-MB content of the right atrium and the left atrium, and between the right ventricle and left ventricle. In all cases, the CK-MB content of the right side of the heart significantly exceeded that of the left side of the heart. Statistically significant differences were also found between the CK-MB content of the anterior interventricular septum and that of the posterior septum. These topographical variations in CK-MB content may be related to differences in the density of contractile elements in various parts of the heart and, moreover, are not taken into account in the enzymatic estimation of infarct size.

The MB fraction of creatine phosphokinase. An indicator of myocardial involvement in acute pericarditis.

An elevated level of the MB fraction of creatine phosphokinase (CPK) with normal serum myoglobin and normal CPK values was found in a case of acute idiopathic pericarditis. The elevated serum CPK-MB isozyme is suggested to be an indicator of myocardial involvement accompanying acute pericarditis. The normal CPK and serum myoglobin values and the pattern of rapid decrease of CPK-MB level ruled out the possibility of acute myocardial infarction.
