Sanded nuclei in delta patients.

Sanded nuclei are nuclei with eosinophilic inclusions identified by light microscopy in cases of chronic hepatitis B virus infection. In hematoxylin and eosin-stained sections, these inclusions have an almost homogeneous, finely granular texture giving a sandy appearance. They have been related to excess hepatitis B core antigen formation. We have studied liver biopsies from two HBsAg positive immunosuppressed patients with numerous sanded nuclei, morphologically identical to those previously described in hepatitis B. Immunohistochemically, sanded nuclei showed a strong nuclear positivity for delta antigen, but were negative for hepatitis B core antigen. Hepatitis B core particles were not demonstrable by electron microscopy. To our knowledge, this is the first time that sanded nuclei have been related to hepatitis delta virus (HDV) infection.

Veno-occlusive hepatic disease of the liver in renal transplantation: is azathioprine the cause?

Five male patients with veno-occlusive disease of the liver (VOD) were observed in 200 consecutive renal transplants (RT) treated with azathioprine and prednisone. Mild liver enzymatic increases not justified by other reasons were detected between 2 and 9 months after RT. All 5 patients developed portal hypertension and died between 18 and 79 months following RT. Diagnosis of VOD was histological; in 3 cases diagnosis was made while the patients were still alive. In our patients, 9 previous viral hepatotropic infections (5 during hemodialysis and 4 after TR) were demonstrated. Due to the reported low incidence of VOD in RT patients, when many of them have been treated with azathioprine, the etiological role of this drug must be questioned. However, the possible association of a previous hepatotropic viral infection and the use of an immunosuppressive agent should be considered as a probable cause of VOD in kidney grafts.

[Embolization of vertebral hemangioma. Technic-indications-results].

A series of 19 cases of vertebral hemangiomas treated by hyperselective embolization is reported. Localization is thoracic in 12 cases and lumbar in 7. Patients were referred for back pain in 10 cases, neurological signs of spinal cord compression in 5 and of radicular compression in 4. The first step is a precise analysis of the vascular pattern of each case with identification of the feeders of the spinal cord. Embolization as the only treatment, was proposed in 12 cases with painful manifestations. In 5 cases with symptoms of spinal cord compression, it was associated with surgery. The last 2 cases were not treated because of the close proximity of an Adamkiewicz artery. Results on painful symptoms were excellent in 9 cases and good in 3. There was no complications in this series. Therefore, we consider embolization to be a safe tool in the treatment of vertebral hemangiomas, with good efficiency on painful symptom and pre-operatively in case of spinal cord compression.

[Woolly hair nevus associated with systemic epidermal nevus].

A 22-year-old female with two woolly hair nevus of the scalp and a systematized epidermal nevus is reported. This association has been rarely described. Scanning electron microscopy of the woolly hair demonstrates oval and triangular hair shaft sections and longitudinal grooves. The structure of the cuticle was not disturbed.

Pseudoscleroderma secondary to phytonadione (vitamin K1) injections.

Two patients showed symmetrical sclerodermoid (morphea-like) skin lesions at sites of intramuscular phytonadione (vitamin K1) injections. The lesions appeared one and two years after the injections. A dense sclerosis involving the reticular dermis and subcutaneous fat was observed on histologic examination of biopsy specimens. Local adverse reactions to phytonadione are reviewed. Possible causative mechanisms for phytonadione-induced pseudoscleroderma are discussed.

[Multicentric abdominal angiofollicular hyperplasia: a clinical variant with exclusive involvement of the liver and abdominal lymph nodes].

Three patients with a non-malignant lymphoproliferative disease involving liver and abdominal lymph nodes, without peripheral lymphadenopathy, are presented. They had systemic symptoms, fever and/or arthralgia, a biochemical pattern of liver obstruction and signs of B-cell immune hyperactivity (hypergammaglobulinaemia and different autoantibodies). Abdominal lymph node enlargement was found on laparotomy; the lymph node histologic pattern was that of angiofollicular hyperplasia (AFH), malignant lymphoma being discarded. Several stages of involvement were present in all cases, made up of small lymphocytes, plasma cells, and centrofollicular cells forming portal nodules with cholangiole infiltrates in one case which gave rise to a lymphoepithelial lesion. Steroid therapy was effective in all cases, but the symptoms and the liver obstruction relapsed in two patients after steroid suppression. Persistent functional anomalies of B and T lymphocytes were present in all cases. No evidence of viral disease was seen. These patients form a clinicopathological variant of multicentric AFH on whom malignant lymphoma could be excluded only by the histological study of abdominal lymph nodes. The persistence of lymphocytosis (although polyclonal and lacking cytogenetic alterations) may suggest the possible evolution into true and malignant lymphoma in any case.

Factitial dermatosis of the breast: a possible dermatologic manifestation of Munchausen's syndrome.

We report on two cases of factitial dermatosis of the breast associated with benign fibrocystic disease. One of the patients could also be diagnosed as showing Munchausen's syndrome. The lesions were protracted and intractable, painful ulcers that recurred and eventually became bilateral, despite attempts at surgical correction. The management of factitial dermatosis of the breast is complex, and should include psychiatric referral as well as avoidance of further surgical treatment if possible.

A synthetic vaccine protects humans against challenge with asexual blood stages of Plasmodium falciparum malaria.

We have previously shown that a mixture of three synthetic peptides (83.1, 55.1, 35.1), corresponding to fragments of the relative molecular mass 83,000 (83K), 55K and 35K Plasmodium falciparum merozoite-specific proteins, induces protection in Aotus triviroatus monkeys experimentally infected with P. falciparum. Here we describe two polymeric synthetic hybrid proteins based on these peptides that delay or suppress the development of parasitaemia in immunized human volunteers.

Hepatic fibrin-ring granulomas in visceral leishmaniasis.

Hepatic fibrin-ring granulomas and leishmania parasites were found in the liver biopsy specimens of 3 patients with prolonged fever and hepatosplenomegaly. It was recognition of the leishmanias in the liver biopsy specimen that prompted the diagnosis in all cases. There was no evidence of Q fever, Hodgkin's disease, cytomegalovirus hepatitis, or allopurinol treatment, which are the recognized causes of hepatic fibrin-ring granulomas. This report extends the range of etiologies of hepatic fibrin-ring granulomas. As a result, leishmaniasis should always be a consideration to the pathologist and the clinician in the differential diagnosis of fibrin-ring granulomas.

The effect of human pregnancy serum on the synthesis and action of interleukin-1.

Supernatants from mixed lymphocyte reactions performed in the presence of pregnancy serum (PS) or normal human serum (NHS) were tested for interleukin-1 (IL-1) activity in a thymocyte assay. A lower response was obtained with PS-containing supernatants, suggesting that PS exerts its suppressive effect by inhibiting IL-1 synthesis or by blocking its mode of action. IL-1 activity generated by human peripheral blood mononuclear cells (PBMC) stimulated with LPS and cultured in the presence of PS or NHS was similar. However, when PS was present, the proliferation of thymocytes co-mitogenically stimulated with PHA and exogenous IL-1 was diminished compared with the control (NHS), exhibiting its maximum inhibition when PS was added at the highest concentration and at the beginning of the incubation period. The suppressive effect of PS in the thymocyte assay disappeared with the combined action of exogenous IL-1 and IL-2. The response of T lymphocytes allogeneically stimulated was decreased in the presence of PS even when IL-1 was added; the recovery of proliferation being strictly dependent on the presence of the appropriate dose of exogenous IL-2. These results indicate that PS was interfering with IL-1 activity by rendering IL-2-producer cells unable to synthesize IL-2 and subsequently to proliferate.
