Histiocytoid hemangioma of the innominate vein.

Primary malignancies originating in large veins are infrequent. Leiomyosarcomas and malignant vascular tumors have been occasionally reported. The recently described histiocytoid hemangioma has a predilection for medium-sized and large vessels. A case is reported of histiocytoid hemangioma arising in the innominate vein and presenting clinically as a mediastinal mass.

Characteristics of tuberculosis in HIV-infected patients: a case-control study.

To test the hypothesis that HIV infection can modify the clinical characteristics of tuberculosis, 65 consecutive cases of tuberculosis in HIV-seropositive patients diagnosed in Barcelona (Spain) were compared with 65 HIV-seronegative controls matched for age and sex. Thirty of the 65 cases were accepted as AIDS cases (August 1987 Centers for Disease Control criteria) only because of the tuberculosis. Among the cases 54 (83%) were parenteral drug addicts and 88% were males. The tuberculosis was pulmonary or pleural in 62 controls (96%) but in only 25 cases (39%; P less than 0.0001). Lymph nodes were involved in 25 cases (39%) and in none of the controls (P less than 0.0001). Disseminated forms of tuberculosis were present in seven cases (11%) and in no controls (P less than 0.007). Bone, joints and central nervous system involvement were also significantly (P less than 0.05) more frequent in cases. The treatment (isoniazid and rifampin for 6 months plus ethambutol and pyrazinamide during the first 2 months) was always effective. One relapse was detected after a median follow-up of 55 months in cases and none in controls after a median follow-up of 43 months. Twenty-five cases (39%) and 14 controls (22%) developed mild or severe side effects related to the treatment (P less than 0.004). In conclusion, most of the HIV-infected patients with tuberculosis were drug addicts with extrapulmonary or disseminated forms. A short course of treatment (6 or 9 months) may be enough but side effects were frequent.

Radiocesium concentration in migratory birds wintering in Spain after the Chernobyl accident.

Levels of 137Cs and 134Cs resulting from the Chernobyl nuclear accident were studied in 195 birds that winter in two regions of Spain. Only five of the 12 species examined were contaminated. The average values for 137Cs vary between 1.6 and 41 Bq kg-1 fresh. In particular, the contamination for song-thrushes (Turdus philomelos) are compared between the regions of Extremadura and Valencia, 350 km east of Extremadura at the same latitude. The results show that the contamination of birds wintering in Spain decreases from east to west. The whole-body dose commitment for humans consuming these contaminated birds was calculated. The values are well below the established ICRP guideline.

Childhood self-healing histiocytosis X.

A 6-month-old boy had histiocytosis X limited to the skin and oral mucosa, which regressed spontaneously over a period of six months. We review the subsets of autoinvolutive histiocytosis X with special emphasis on the dermatologic manifestations. Childhood self-healing histiocytosis X is a benign condition that should be differentiated from self-healing reticulohistiocytosis (Hashimoto-Pritzker type).

Erythema gyratum atrophicans transiens neonatale: a variant of cutaneous neonatal lupus erythematosus.

We cared for a patient with neonatal lupus erythematosus with cutaneous and hepatic involvement that was clinically and histologically indistinguishable from erythema gyratum atrophicans transiens neonatale, an entity described by Gianotti and Ermacora in 1975, which we consider to be a subtype of cutaneous neonatal lupus erythematosus. The differential diagnosis of any annular erythema in the newborn requires careful clinicopathologic evaluation, with direct immunofluorescence studies and determination of anti-Ro/SS-A, anti-La/SS-B, and anti-nRNP antibodies in both the baby and the mother.

Porokeratotic eccrine ostial and dermal duct nevus.

We report 2 cases of congenital porokeratotic eccrine nevus in a 5-year-old girl and a 7-year-old boy. The clinical manifestations were those of nevus comedonicus palmaris. The histologic picture consisted of comedo-like dilatations and cornoid lamellae involving the eccrine ostia and ducts. One of the cases showed anomalies in the dermal eccrine ducts. We believe that these lesions are eccrine hamartomas unrelated to porokeratosis. We review similar cases from the literature and discuss the differential diagnosis.

Biopsy↗

Eosinophilic histiocytosis. A subset of lymphomatoid papulosis.

A 43-year-old male with a 23-year history of small-plaque-type parapsoriasis developed a chronic recurrent self-healing papulonodular and tumoral cutaneous eruption. The microscopic findings were consistent with eosinophilic histiocytosis. Immunohistochemical and ultrastructural studies revealed a population of T lymphocytes, eosinophils and monocyte-macrophage cells in the dermal infiltrate. These findings confirm the similarities between eosinophilic histiocytosis and lymphomatoid papulosis suggesting that the former may be a clinicopathologic variant of the latter.

Eosinophilic pustulosis: a case report.

A 16-year-old female had a chronic cutaneous eruption clinically characterized by papules, pustules and large exudating plaques of 2 years duration. Multiple biopsies showed infiltration of the follicular epithelium by eosinophils and intraepidermal pustules with neutrophils and eosinophils that were accompanied either by acantholysis or spongiosis. Eosinophilia and hypoproteinemia were detected. A definite infectious etiology or autoimmune mechanism could not be established. Differential diagnosis with other entities that share some clinico-pathological and immunological features are discussed.

Familial multiple trichodiscomas.

A familial multiple trichodiscoma involving two sisters is reported. Trichodiscoma is a benign neoplasm of the mesenchymal component of the hair disk characterized clinically by asymptomatic papules and histologically by a dermal fibrovascular proliferation. Familial involvement and associations with other follicular neoplasia should be investigated in all cases.

Perforating lichen striatus.

A case of lichen striatus with transepidermal elimination of clusters of necrotic keratinocytes is reported. On the basis of the morphologic findings, we suggest that transepidermal elimination may be a mechanism of healing in some cutaneous lichenoid eruptions.

Intestinal angiodysplasia: retrospective study of 18 cases.

Eighteen patients operated on for intestinal angiodysplasia are reported. Visceral angiography was the diagnostic procedure of choice, providing evidence in most cases of lesions in the right colon. Right hemicolectomy was the surgical treatment normally provided. Mortality in our series was nil. This report looks at the main clinical features of these lesions, paying particular attention to their etiopathogenesis.

[Pigmented dermatofibrosarcoma protuberans (Bednar's tumor)].

Pigmented dermatofibrosarcoma protuberans, first described by Bednar under the term storiform neurofibroma, is an infrequent neoplasm accounting for 1 to 5% of cases of dermatofibrosarcoma protuberans. Bednar's tumor is composed of spindle shaped cells arranged in a distinctive storiform or carthwheel pattern, and melanin-containing dendritic cells scattered within the tumor. The differential diagnosis with (non-pigmented) dermatofibrosarcoma protuberans is based on the presence of this population of pigmented cells. Two cases of pigmented dermatofibrosarcoma protuberans (Bednar's tumor) are reported here in, and a discussion follows on the clinicopathological features of this neoplasm and the different hypotheses on its histogenesis.

[Multiple hamartoma syndrome].

We describe the clinicopathological features of multiple hamartoma and neoplasia syndrome (Cowden's syndrome) in a 58 year-old man and in a 56 year-old woman. The typical mucocutaneous lesions of this disorder were present in both cases. Visceral hamartomas involving thyroid and gastrointestinal system (polyposis) were present in both patients. Dermal or submucous fibromas are proposed as an additional diagnostic criterium of Cowden's syndrome. Associated abnormalities and visceral involvement in multiple hamartoma syndrome are reviewed.

Biopsy↗

[Actinic reticuloid. Immunohistochemical study].

A patient that fulfilled the clinical and histological features of actinic reticuloid has been studied. He presented a positive patch test for fragance mix. Typical cutaneous lesions were reproduced with a solar simulator. The immunochemical studies of the inflammatory infiltrates of these induced lesions gave a higher percentage of T8 in relationship to T4 lymphocytes. Most of the cells were HLA-DR positive. No clonality of these lymphocytes was detected. In the peripheral blood lymphocytes, an increased percentage of T8 lymphocytes was also observed.

Operative choledochoscopy in biliary surgery.

Forty-six patients submitted to operative choledochoscopy during biliary surgery are presented. Forty-two had benign diseases, principally calculi (38 cases), with only four patients presenting with malignant neoplasms. Our results bear out the effectiveness of operative choledochoscopy as a complement to cholangiography and conventional exploration of the bile duct, in significantly reducing the number of retained stones found, and in aiding other surgical procedures in the biliary tract.
