[Nontraumatic spondylolysis of the axis vertebra].

A case of nontraumatic spondylolysis of the axis vertebra is presented. A 55-year-old man suddenly experienced nausea and dizziness on January 15, 1984 when he rotated his neck toward the left. Since this episode, he experienced the same attacks twice. On examination, the patient was afraid of moving his neck abruptly and on gradual hyperextension of his neck, he started to complain of slight dizziness. But otherwise no abnormalities of motor, sensory and cerebellar functions were noticed. Lateral view of plain films of the cervical spine revealed a bilateral bony defect in the lamina of the axis vertebra. The surface of the bony margin around the defect seemed to be well corticated and smooth. There was no spondylolisthesis between the second and the third cervical vertebra. CT scan of the axis vertebra also showed bilateral true bony defect, not in the pars interarticularis but in the inferior articular process of the axis vertebra. Spondylolysis with or without spondylolisthesis of the cervical vertebrae is relatively rare and about 48 cases of the cervical spondylolysis have been reported in the previously published literature.

Studies on the metabolism of unsaturated fatty acids. XIII. Properties of 2,4-dienoyl-CoA reductase from beef liver.

It is demonstrated that 3-alkenoyl-CoA is the product of the enzymic reduction catalyzed by 2,4-dienoyl-CoA reductase with the highly purified enzyme preparation from beef liver mitochondria. Incorporation of deuterium atoms from deuterium-labeled NADPH and 2H2O during the reaction catalyzed by 2,4-dienoyl-CoA reductase from beef liver was investigated. When trans-2, trans-4-decadienoyl-CoA was incubated with the reductase in the presence of 4R-[4-2H1]NADPH and H2O, no deuterium was detected in the reaction product by gas chromatography-mass spectrometry after derivatization to pyrrolidine amides of fatty acids. When the substrate was incubated with the enzyme in the presence of 4S-[4-2H1]NADPH and H2O, one deuterium atom was incorporated into the product, 3-decenoate, at the C-5 position. In the case of 3-decenoate enzymatically prepared in the presence of NADPH and 2H2O, two deuterium atoms were incorporated into the product at the C-2 position. These results indicate that the reaction catalyzed by 2,4-dienoyl-CoA reductase from beef liver is a unique example of 1,4-addition of hydrogen to a 1,3-diene system conjugated with a carbonyl group of a thioester in biochemical fields. Chain length specificity of the reductase, isotope effects on reaction rates and competitive inhibitors are also discussed.

Studies on the metabolism of unsaturated fatty acids. XV. Purification and properties of 2,4-dienoyl-CoA reductase from rat liver peroxisomes.

Peroxisomal 2,4-dienoyl-CoA reductase was purified from rat liver to homogeneity. The subunit molecular weight of 33,000 was determined by sodium dodecyl sulfatepolyacrylamide gel electrophoresis. The native molecular weight close to 120,000 was estimated by gel filtration on Sephacryl S-300 Superfine. trans-2, trans-4-Decadienoyl-CoA was the most active substrate among the dienoyl-CoA's of various chain lengths. The total activity of peroxisomal 2,4-dienoyl-CoA reductase exceeded that of the mitochondrial one even in the livers of rats fed with a standard diet. Furthermore both reductases were remarkably and coordinately induced in the livers of clofibrate-treated rats.

Familial osteodysplasia associated with trigeminal neuralgia: case report.

A 24-year-old woman suffering from left trigeminal neuralgia presented with the familial occurrence of skeletal anomalies simulating Hajdu-Cheney syndrome. She was of a short stature and had low-set ears, protrusion of the bilateral temporal and occipital bones, platybasia, basilar impression, and dental anomaly. Vertebral angiography disclosed marked displacement to the left and dorsally of the basilar artery, which was confirmed during the operation to have compressed the left trigeminal nerve in the cerebellopontine angle narrowed by the unusually protruded bony structure. Cases of Hajdu-Cheney syndrome have occasionally been reported, a few combined with facial spasm, but Hajdu-Cheney syndrome with trigeminal neuralgia is very rare. The patient's family members were surveyed. The possible cause of the neuralgia with its relation to the osteodysplasia is briefly discussed.

[Ectopic ureterocele of an adult male].

A case of ectopic ureterocele in a 50-year-old male is reported. He was admitted to the hospital with complaints of miction pain, cloudy urine and fever attack. He had had pain at voiding from time to time during the past 10 years. The prostata was swollen, edematous and tender. Cystoscopy revealed a large bulge in the left trigone to bladder neck, and two ureteral orifices in the right, but none in the left. Complete duplication of the right ureter, lateral deviation of left ureter, and bladder deviation to the right were detected on the drip infusion pyelogram. An ectopic ureteral opening was present in the posterior urethra. A 5 Fr ureteral catheter inserted into this orifice revealed a dilated & tortuous left ureter which belonged to the left upper half kidney. No communication between the ureter & seminal vesicle was revealed by seminal vesiculography. Ureterectomy from left upper half kidney and resection of ureterocele were performed together with ureterovesicoanastomosis to drain urine from the left lower half of kidney.

Effect of prostaglandin on urethral resistance and micturition.

The effect of four exogenous prostaglandins, PGA1, PG802, PGE2 and PGF2 alpha, upon the lower urinary tract was investigated in female mongrel dogs without neurogenic lesions in vivo. The urethral resistance was studied by means of a urethral pressure profile, and the bladder function by evaluating whether or not the micturition was triggered. The reduction of urethral resistance in terms of the maximum urethral closure pressure was most significant with PGE2 given intraaterially. Micturition was most frequently provoked by the intravenous administration of PG802, a derivative of PGE1. PGE series seemed to be the most potent for the evacuation of urine in female dogs.

Children with unstable bladder: clinical and urodynamic observation.

A clinical analysis was done on 303 patients diagnosed by urodynamic study as having an unstable bladder, which was characterized by uninhibited contraction of the detrusor without any overt neurogenic lesions. An age distribution histogram demonstrated 2 peaks: children less than 10 years old and patients between 60 and 80 years old. Sex was not significant in the children but men predominated in the adults. There were 68 children less than 15 years old with the chief complaints of enuresis (54 per cent), urge incontinence (41 per cent) and fever (25 per cent). The objective findings were vesicoureteral reflux in 29 children, urinary tract infection in 20 and distal urethral stenosis in 8. An unstable bladder type I, the occurrence of uninhibited contractions during the filling phase of cystometry, was found in 53 children (78 per cent). We noted several postures that the children characteristically used to resist unheralded urgency and to prevent urine from leaking. The main therapeutic regimen consisted of anticholinergics with or without antimicrobials. Several children underwent surgical intervention. Once the antireflux operation was performed followup with anticholinergics was of primary importance. The unstable bladders of 63 of the 68 children (93 per cent) have been controlled satisfactorily. The etiology of bladder instability is discussed for both groups of patients.

Effects of alcoholic extract from Ma-Klua (Diospyros mollis) on adults and larvae of the dwarf tapeworm, Hymenolepis nana in mice and on the infectivity of the eggs.

The anthelmintic effect of an alcoholic extract from a shrub, Diospyros mollis, popularly known as Ma-Klua in Thailand, on the adults and larvae of the dwarf tapeworm, Hymenolepis nana, in mice was studied in comparison with that of flubendazole. The experimentally infected mice were given a single oral dose of 10-1000 mg of Ma-Klua extract or flubendazole/kg body wt 1, 2, 3, 4, or 12 days post-infection and autopsied 14 days post-infection. Ma-Klua extract was effective in the elimination of adults (ED50 = 79 mg/kg) but not larvae. Drastic effects of Ma-Klua extract on the motility and structure of adults were observed and the number of the adults in mice decreased with time after administration of the drug 12 days post-infection. The small numbers of adults remaining in the host intestine 2 days after the drug administration showed severe damage in the gravid segments. These facts were thought to be responsible for the significant reduction in egg output observed 1 and 2 days after medication. Fresh eggs exposed to Ma-Klua extract in vitro and in vivo, or in vivo alone showed reduced infectivity. The effect of flubendazole on adults and larvae was minimal.

Renotropic growth factor found in cancer patient sera after removal of cancer-bearing kidney.

We have used two sets of primary human kidney culture systems for assaying human renotropic growth factor in day 5 postnephrectomized human sera. The first set of primary kidney cell cultures was prepared from one hydronephrotic kidney and used for assaying the renotropic growth stimulating activity of six kidney cancer patients sera simultaneously. The level of renotropic growth-stimulating activity varies from patient to patient. The postnephrectomized sera of three patients showed rather small (27 to 68%) increase over their prenephrectomized sera. The postnephrectomized sera of the remaining three patients showed a much greater (2.69- to 3.19-fold) increase over their prenephrectomized sera. Variation in the growth-stimulating activities produced in these uninephrectomized kidney cancer patients is most probably due to different levels of renal function loss by removal of the cancer-bearing kidney. Each cancer-bearing kidney has a different proportion of healthy kidney region to diseased (nonfunctional) region. The second set of primary kidney cell cultures was prepared from a healthy noncancerous region of a cancer-bearing kidney and used to assay for renotropic growth-stimulating activities in two different unilaterally nephrectomized kidney transplantation donor sera. Increased levels of growth-stimulating activity are quite similar and high (4.19- to 4.79-fold increase above their prenephrectomized sera). These high and similar assay values of the renotropic growth factor produced in two healthy humans suggest the sensitivity and reliability of the assay method with the use of the noncancerous cells from the cancer-bearing kidney.

Total DNA transcription reveals the existence of highly repetitive transcribable sequences in higher animals.

Using HeLa S-100 extracts, we have transcribed total DNA isolated from several species of higher animals. Transcription of the total DNA from rat, mouse, or hamster gives a discrete product of 6S RNA (about 180 nucleotides) in addition to a smeared background, whereas transcription from chick, calf, or human DNA gives a discrete band of 5S RNA (about 120 nucleotides). In view of the sensitivity to a-amanitin, these transcripts are produced by RNA polymerase III. Fingerprint analysis demonstrated that the 6S RNA species transcribed from rodent DNA have conserved sequences. The composition of dinucleotides in these bands having a guanosine residue on the 3' side was analyzed by RNase T2 digestion. The results show that the CG ratios of the transcripts of rodent, calf and chick are quite low in comparison with AG, UG, or GG except in the case of the human transcript, suggesting that these highly repetitive transcribable sequences are derived from a common prototype sequence, except in human cells. We have isolated two phage clones and one phage clone from a rat and mouse genomic library, respectively, which give a transcript of 180 nucleotides in size, and whose pattern of RNase T2 digests is the same as that of the conserved transcriptional products of total DNA. These highly repetitive transcribable sequences appear to be reiterated at several to ten thousand copies per haploid genome. The relationship of these sequences with the highly repetitive interspersed sequence B1 or Alu family is discussed.

Generalized fibrosis associated with pulmonary asbestosis.

An autopsy case of pulmonary asbestosis with fibrosis of various organs was reported. Pulmonary carcinoma and glomerulonephritis were complicated. Asbestos bodies (ABs) and fibers (AFs) were identified in the organs with fibrosis such as the lung, liver, kidney, heart, and thyroid gland by electron microscopy and digestion method. Moreover, we identified AFs in the spleen by electron microscopy which had been resected due to idiopathic portal hypertension six years previously. It is suggested that AFs can be transported to various organs and persist for a long period of time to cause pathological changes there.
