[Two cases of multilocular renal cell carcinoma].

Two cases of multilocular cyst of the kidney associated with clear cell carcinoma are described. Only 15 cases of multilocular cystic carcinoma of the kidney have been reported in the English literature including 6 Japanese cases. Their preoperative diagnosis is very difficult and nephrectomy is the only choice of treatment in many cases. The histopathogenesis of the multilocular cyst is reviewed.

[Antitumor effects of a podophyllotoxin derivative, VP 16-213].

Single and combination chemotherapies of VP 16-213, a new antitumor agent were evaluated for its antitumor effect against several murine tumors. Dose-dependent antitumor effects were observed when VP 16-213 was administered via any of the three routes, i.p., i.v. and orally, on days 1 and 5 after i.p. or s.c. inoculation of Ehrlich carcinoma and sarcoma 180. The drug also proved effective against i.v. inoculated EL-LP-12 (subline of Ehrlich carcinoma), i.p. or s.c. inoculated P388 and B16 melanoma, i.p. inoculated colon 26, and s.c. inoculated colon 38 and Lewis lung carcinoma. However, oral administration of the drug was not effective against B16 melanoma, colon 26 and Lewis lung carcinoma despite the fact that the doses employed for this route was higher than those employed for i.v. and i.p. routes. The optimum dosing schedule was also investigated with oral administration of the drug against s.c. inoculated Ehrlich carcinoma. A single dose (day 1) or two doses (days 1 and 5) were more effective than three doses (days 1, 3 and 5) or five consecutive daily doses. VP 16-213 showed additive and more than additive effects in combination with the antitumor agents, cyclophosphamide, BCNU, mitomycin C or cisplatin against s.c. inoculated Ehrlich carcinoma and i.v. inoculated EL-LP-12.

[General pharmacological studies of VP 16-213. Effect on the peripheral system].

The general pharmacological studies of VP 16-213, an antitumor agent, in the peripheral system were carried out by oral and i.v. administration with laboratory animals. VP 16-213 had no effect on the resting tone and spontaneous motility of all the isolated smooth muscle organs tested at concentrations of up to 10(-5) g/ml. VP 16-213 showed no significant influences on the neuromuscular transmission in the sciatic nerve-tibial muscle preparation of rabbits, on renal function in dogs and rats, on blood sugar level in rabbits, and on charcoal meal transport in mice. At a dose of 20 mg/kg, i.v., VP 16-213 reduced gastric secretion in rats. Antiinflammatory, local irritation and hemolytic effects were not observed after the administration of the drug. Single administration of VP 16-213 had orally no effect on the excretion of serum BSP in rats. However, successive administration of the drug (20 mg/kg/day) once daily for 10 days significantly depressed it. VP 16-213 decreased the number of hemolytic plaque forming cells in the spleens of immunized mice.

[Clinical evaluation of CMA in the treatment of prostatic cancer].

Forty five patients with previously untreated prostatic cancer were given chlormadinone acetate (CMA) at a dose of 100 mg/day (mean 12.6 months), and 42 of them were evaluated for effectiveness. Overall response evaluation revealed 19 complete responses (45.2%), 14 partial responses (33.3%), 4 minor responses (9.5%), and 5 were unresponsive (11.9%). Consequently CMA was judged to be effective in 33 patients (78.6%, complete and partial responses). Response rate was 100.0% for stage A, 88.9% for stage B, 77.8% for stage C, and 57.1% for stage D. The response rate for stage D was relatively high, possibly because the time of judgement of effectiveness was a little too early. As side effects, elevated triglyceride level, impaired liver function and impotence were noted in one patient each. All of them were slight. The testosterone level in both castrated and non-castrated patients were not higher than that in women, and no significant difference in the level was noted between the groups. Nevertheless, the clinical usefulness of castration is not denied. CMA is considered useful as an endocrine therapeutic drug, though its effect on the disease at stage D was insufficient. The administration method, such as sequential treatment and combination treatment of CMA and estrogen preparations, is a future subject.

Relationship between movements of the head and the center of gravity of the body in Menière's disease.

The body sway of the patients with unilateral and bilateral Ménière's disease was recorded by using a cephalograph and a stabilometer, and analyzed with a microcomputer. The results show that the locus type, locus area and locus length traced by the head's center and the body's center of gravity are useful parameters of the body sway, representing the characteristics of peripheral vestibular disorders. The noteworthy characteristics are shown in the right-and-left type and forward-and-backward type in statokinesigram, in the significant differences of locus area and/or locus length between the head movement and the movement of the center of gravity, and the change of the averaged divisional frequencies of the body sway with eye-closing.

Diffuse hypoplasia of the aorta as a possible cause of cardiac hypertrophy.

Left ventricular hypertrophy is caused by a wide variety of cardiovascular conditions, including systemic hypertension, aortic valvular disease, and hypertrophic cardiomyopathy. This paper describes a case of diffuse hypoplasia of the aorta in left ventricular hypertrophy. This condition has not previously been well documented. We emphasize the need for evaluation of aortic dimension when systolic hypertension and vascular bruits are observed in association with myocardial hypertrophy.

Effects of prazosin on platelet aggregation and plasma beta-thromboglobulin in essential hypertension.

Platelet aggregation in response to adenosine 5'-diphosphate (ADP) and plasma levels of beta-thromboglobulin (beta-TG) were followed in 10 subjects with essential hypertension without any manifest vascular complications and in 14 age-matched normotensive controls. The effects of prazosin on blood pressure and platelet function were also examined in the subjects with hypertension. Platelet aggregation in response to 5 microM ADP was significantly greater in the subjects with hypertension than in the controls. Plasma beta-TG levels were 30% higher in the subjects with hypertension than in the controls. The increased platelet aggregation and plasma levels of beta-TG in the subjects with hypertension returned to normal after the blood pressure had been controlled by prazosin therapy. Our data suggest that the aggregation and release reactions of platelets increase in uncomplicated essential hypertension, and that prazosin may have some beneficial effects on the prevention of vascular complications in hypertension by normalizing platelet function as well as by lowering blood pressure.

Amino acid sequence of protein alpha-amylase inhibitor from Streptomyces griseosporeus YM-25.

The amino acid sequence of a protein alpha-amylase inhibitor from Streptomyces griseosporeus YM-25 (Haim II), which consists of 77 amino acid residues, including two disulfide bridges, was determined by conventional methods. One of the disulfide bridges was found to be located between Cys(6) and Cys(22), and the other between Cys(40) and Cys(67) from the results of structure analyses of the two cystine-containing peptides obtained from the thermolysin digest of the native inhibitor.
