Dissociation of digoxin-like immunoreactivity and Na+,K+-ATPase inhibitory activity in rat plasma.

We measured endogenous digitalis-like factor (EDF) in rat plasma during acute saline infusion by two different procedures. Na+,K+-ATPase inhibitory activity in the rat plasma significantly increased during saline loading (7.8 +/- 2.2 vs 2.5 +/- 0.9%, with and without acute saline loading, respectively, p less than 0.05). On the other hand, the plasma digoxin-like immunoreactivity significantly decreased during acute saline loading (16.9 +/- 1.6 vs 32.0 +/- 2.8 pg digoxin equivalents/ml, with and without acute saline loading, respectively, p less than 0.01). These results indicate that the major substances detected by digoxin-like immunoreactivity and direct Na+,K+-ATPase inhibitory activity are completely different, at least in rat plasma.

Purification and characterization of human urine-derived digitalis-like factor.

We were able to purify a digitalis-like factor to apparent homogeneity from human urine based on the inhibitory effect on [3H]-ouabain binding to intact human erythrocytes. This ouabain displacing compound closely resembles ouabain in its polarity, molecular weight, non-peptidic nature and mode of action except for its UV absorbance spectrum. This compound sharing many biological activities of ouabain may be the endogenous ligand for the Na+, K+-ATPase and serve as a specific regulator of the sodium pump.

Presence of digitalis-like factor in mammalian plasma.

We attempted to purify a digitalis-like factor from volume expanded dog plasma using an inhibitory effect on the binding of [3H]ouabain to intact human erythrocytes to monitor digitalis-like activity. A highly polar [3H] ouabain displacing compound was purified to a high degree using a combination of chromatographic procedures including reverse phase and gel filtration high performance liquid chromatography. This compound, a reversible inhibitor of [3H]ouabain binding, closely resembles ouabain in its polarity and significantly increases during saline infusion. Its molecular weight was estimated to be 343Da. Moreover, similar compound was consistently detected in other mammalian plasma.

Effects of human urine-derived digitalis-like factor on cultured renal tubular epithelial cells.

We purified a polar digitalis-like factor to apparent homogeneity from human urine using inhibition of 3H-ouabain binding to intact human erythrocytes to monitor digitalis-like activity. Since endogenous digitalis-like factor may act as a natriuretic hormone, we tested the binding characteristics of this urine-derived ouabain-displacing compound to the sodium pump in intact renal epithelial cells, in order to assess its potential physiological significance. Cultured canine and porcine epithelial cell lines, MDCK and LLC-PK1, had a high sodium pump density as estimated from 3H-ouabain binding sites. Human urine-derived ouabain-displacing compound showed a dose-related inhibition of 3H-ouabain binding to both of these cells, similar to the inhibition of unlabelled ouabain. These findings indicate that the ouabain-displacing compound is capable of acting on the sodium pump in intact renal epithelial cells and may be the circulating regulator of Na+,K+-ATPase.

The effects of urinary digitalislike factor on cultured vascular smooth muscle cells.

We attempted to purify a digitalislike factor from human urine. On the assumption that a natural ligand for the digitalis receptor should be searched for on the basis of the effects on intact cells, we used an inhibitory effect on the binding of [3H]ouabain to human erythrocytes to determine digitalislike activity. A highly polar [3H]ouabain displacing activity was obtained by a combination of chromatographic procedures including reverse-phase high performance liquid chromatography. Urine-derived [3H]ouabain displacing activity, a competitive inhibitor of ouabain binding to human erythrocytes, acted on human lymphocytes in a similar manner. The dose-response curve of this compound was parallel to that of ouabain. Urine-derived [3H]ouabain displacing activity significantly inhibited monensin-stimulated increase in ouabain-sensitive 86Rb uptake, a parameter of Na+,K+-adenosine triphosphatase (ATPase), by 95% (p less than 0.01) in cultured vascular smooth muscle cells (A10 cells). Furthermore, this compound enhanced net 45Ca influx by 30% (p less than 0.01) and reduced 45Ca efflux by 35% (p less than 0.01) in A10 cells. These results suggest that urine-derived [3H]ouabain displacing activity may be a regulator of Na+,K+-ATPase and a modulator of vascular tone.

[A cytologic diagnosis of breast secretions--application of cytology to the mass survey of breast cancer].

From January 1982 to March 1987, an exfoliative cytological examination of abnormal nipple discharge was carried out on 790 patients. Of 22 histologically confirmed mammary carcinomas, 7 cases (31.8%) were diagnosed as positive by conventional cytologic techniques. By using the concentration method, in which the nipple secretion was collected in a series of testings that lasted 3 to 5 days and stored in a glass tube containing a fixative, the diagnostic accuracy was seen to increase significantly and 9 out of 15 malignant cases (60.0%) were accurately diagnosed as having cancer. Moreover, all 5 cases without any palpable mass were found to be positive by the smears made by this procedure.

Analyses of adenosine and adenine nucleotides in biological materials by fluorescence reaction-high-performance liquid chromatography.

A previous method of determination of adenine compounds by high-performance liquid chromatography, using bromoacetaldehyde as a fluorescent reagent and a column of Hitachi gel No. 3012-N, was improved and extended to biological materials, especially to measure enzyme activities. A column packed with finer beads, Hitachi gel No. 3013-N, was found to be better than that of No. 3012-N, judging from the analysis time and resolution. ADP, from the hydrolysis of ATP by Na, K-ATPase, was determined quantitatively, and the enzyme activity was inhibited with ouabain. cAMP obtained from ATP by reaction with adenylate cyclase was also determined in the presence of various concentrations of L-epinephrine or sodium fluoride. The ATP levels in human blood were determined, and the cellular levels of ATP and ADP in neuroblastoma N1E 115 were examined as a function of cell growth.

Facial reconstruction with latissimus dorsi myocutaneous island flap following total maxillectomy.

Following total maxillectomy for maxillary cancer, facial reconstruction was performed using a latissimus dorsi myocutaneous island flap. Postreconstructive deformity was studied in 10 patients. In 5 patients, after simple total maxillectomy the inner raw surface of the facial skin and orbito was covered by the flap, and the other extended total maxillectomy patients where the orbital contents and facial skin were involved, reconstruction was by means of the folded flap. In the patients with simple total maxillectomy, cicatricial contracture of the facial skin and cicatricial ectropion of the lower eyelid were quite small, and in the patients with extended total maxillectomy, reconstructed facial skin did not give rise to cicatricial contracture.

Carnitine deficiency in lysinuric protein intolerance: lysine-sparing effect of carnitine.

Plasma carnitine deficiency (41% of normal control) was found in a 9 year-old boy with lysinuric protein intolerance. Following oral carnitine administration, the plasma level of lysine as well as carnitine has significantly increased (p less than 0.05). His well-being seemed to have improved on this substitutive remedy. Hitherto unrevealed these findings on LPI indicate that carnitine has a lysine-sparing effect and suggest that there might be a biosynthetic pathway of carnitine directly from free lysine in human.

[Complete remission of advanced renal cell carcinoma by chemotherapy and surgical treatment: a case report].

A rare case of metastatic renal cell carcinoma which represented complete remission by chemotherapy and surgical treatment is presented. A 59-year-old female was admitted to our hospital because of general fatigue, weight loss and appetite loss. The diagnosis of right renal tumor metastasized to both lungs and extending into the inferior vena cava was made by radiographic findings. Because of very poor general condition the first choice of treatment was chemotherapy with cisdichlorodiamine platinum, adriamycin, cyclophosphamide, 1-(2-tetrahydrofuryl)-5-fluorouracil) (UFT), and OK432. Five months after the beginning of chemotherapy both lung coin lesions disappeared completely, and radical nephrectomy including venacavotomy and tumor thrombectomy was performed. At present, 6 months after the radical nephrectomy, she is free from the disease and complete remission has been obtained by oral administration of 400 mg/day UFT and 5.0 KE OK432 intracutaneous injection every week.
