[Arthroscopic meniscectomies. Short- and median-term results].

Two hundred and forty arthroscopic meniscectomies have been performed between 1980 and 1982. One hundred and ninety-eight cases have been reviewed with a follow up of between 3 and 6 months and 101 cases were reviewed with a follow up of between 6 months and 2 years. On eight occasions the arthroscopy had to be completed by an arthrotomy and one of these became septic. No sepsis was observed in the absence of arthrotomy. Three cases of phlebitis and one haemarthrosis were seen. Social and functional rehabilitation was very rapid. The stay in hospital was 2 days and resumption of sport and work was 2 or 3 times more rapid than after meniscectomy by arthrotomy. The results were excellent or good in 85 p. 100 of cases, slightly better than after arthrotomy. The prognosis was worse when there were patellar or tibio-femoral chondral lesions, and the results were not as good in lateral meniscus lesions, and particularly when there was an associated lesion of the anterior cruciate ligament. However, two patients out of three with this condition were noticeably improved. The remainder will possibly have a secondary repair of the anterior cruciate ligament. The present follow up is not sufficient to evaluate long-term results but the fact that meniscectomy was partial in 3 cases out of 4 supports the hope that the results will be stable. It is only later that it will be possible to evaluate the unobserved lesions or the incidence of recurrence.

Verification of impedance measurements by a volumetric and electromechanical model.

A dynamic model of the middle ear and mastoid cavity will be presented. A variety of precisely controlled volumetric changes were simulated by the model, singularly or in conjunction with pressure changes, mass and friction effects at the level of the drum. The contributions of each factor alone and in controlled combinations to the acoustic impedance were serially tested and recorded. The validity of static compliance and tympanometry in volumetric middle ear and mastoid estimates is questioned. The model demonstrated dependability of impedance tympanometric techniques in measuring intratympanic air fluid contents and pressure changes. The model provided objective evidence on the capability of impedance techniques to differentiate mechanical traction on the drum from traction brought about by negative pressures. Clinical applications of these findings will be discussed.

[Correlation of the pollen content of the air with the geography and vegetation in a mountain range and its allergologic importance].

For several years pollen traps have been installed at different sites in the Inn valley/Tyrol. Even within short distances significant differences in the pollen content of the air in respect to amount and time do occur. The pollen maxima differ even up to 6 weeks during subsequent years. The mean values of the maxima of the same species differ up to 12 days at the different sites. In mountain areas with small-scale vegetation patterns and climatic differences it is therefore necessary to set up a dense network of pollen traps especially in respect to the medical use of the results.

[Microbial sources of antigens in the environment of farmer's lung patients (author's transl)].

The actinomycetes Micropolyspora faeni and Thermoactinomyces vulgaris are important sources of antigens in extrinsic allergic alveolitis, the deuteromycete Aspergillus fumigatus is it to a lesser extent. These microorganisms have been sought in hay samples obtained from farmers who on the one hand show farmer's lung symptoms and on the other antibodies to these microorganisms. A relatively good correlation has been found between the immunological and microbiological findings. The conditions necessary for the occurrence of antigen sources in farmer's lung disease are discussed, and the need for prophylactic measures is pointed out.

[Exogenous allergic alveolitis of farmers (farmer's lung) from the viewpoint of an alpine pulmologic practice].

Extrinsic allergic alveolitis of the rural population is a relevant disease of the alpine region. Observations in a pulmonologic practice suggest that the disease may represent itself in a acute and a chronic form. Six patients with acute alveolitis, 8 cases of chronic farmer's lung, 16 cases of late disease stages and 33 patients of sensitization (with positive immunologic data) without overt disease are described. Additional 31 patients with bronchopulmonary symptoms but without specific antibodies served as controls. The entire patient population was actively working in farming jobs. Case history, clinical, radiographic and immunologic data as well as lung function testes are presented. Follow-up studies suggest that climatic and occupational factors of the region modify the incidence and the clinical course of the disease. Actually, the quantity of antigen present may determine the course of disease. The acute extrinsic alveolitis appears to be associated with massive antigenic exposure. A lower antigenic presence is assumed to initiate occult phases of alveolitis which ultimately may lead to the chronic variety of the disease. Measures for prevention and treatment of this occupational disease involve proper hay drying which abolishes the antigenic sources, prevention of exposure during working and the attempt of long term medical prophylaxis.

Systolic time intervals in measurement of inotropic response to drugs.

During periods of tachycardia induced by atrial pacing in eight patients, moderate increments in dP/dt(max) and (dP/dt)/CPIP (common produced intraventricular pressure) and moderate reductions in left ventricular ejection time (LVET) and Q-S2 were demonstrated. These changes varied between individuals, but reduction in systolic intervals was consistently less than that reported from populations showing a range of resting heart rates. Individual regression formulae relating each variable to paced heart rate were used to calculate rate-dependent and rate-independent changes induced by isoprenaline and ouabain. Despite technical difficulty in precise measurement of systolic intervals, there was an excellent inverse correlation between rate-independent changes in Q-S2 and in both dP/dt(mac) and (dP/dt)/CPIP. Rate-independent change in Q-S2 appears to be a practical, moderately sensitive, and reasonably precise measure of the inotropic effect of a drug which does not radically alter left ventricular end-diastolic pressure or blood pressure. Day-to-day variation in systolic intervals may limit the use of the technique to studies of short duration.

Fatalities on the liquid-protein diet: an analysis of possible causes.

Reports of deaths occurring in patients using variants of the liquid-protein diet (LPD) have raised some questions about the safety of this procedure for the management of obesity. Case reports involving 44 publicized deaths were reviewed. Possible causes of death including such factors as potassium and/or electrolyte deficiencies, inadequate or poor quality protein, imperfect patient selection or compliance, an associated medications or disease were examined. Data in eight patients are insufficient for analysis or show that the patients were not using a LPD. Of the remaining 36 patients 32 were inappropriately managed as judged by conservative standards. Four patients who may have been properly managed all had known preexisting cardiac disease. The analysis does not identify any single factor which is responsible for the deaths. It shows that the procedure was widely misused and suggests that this contributed in multiple ways to the fatalities. No properly managed patient died as a direct result of the LPD and the cardiac deaths could not be ascribed to the LPD. Since the LPD, when properly supervised, has demonstrated great promise in the management of obesity, it is suggested that caution be exercised in dismissing the LPD because of adverse publicity.

[Perennial coseasonal hyposensitisation (author's transl)].

The specific hyposensitisation in the causal therapy derived from the pathogenesis of pollinosis. The perennial-coseasonal treatment brings remarkable advantages compared with the single preseasonal treatment. The own method of hyposensitisation with coseasonal low-dosage with regard to the individual pollinosis season is described. The technique of the antigen application, the possible undesirable sidereactions and their treatment are mentioned. The results of the therapy in 50 patients with pollinosis are documented: 40 patients became free complaints within the first year of coseasonal treatment, 22 of them without any adjuvant therapy, 18 with adjuvant antihistaminic therapy.

[Metal pneumoconiosis (author's transl)].

The occurrence of two cases of metal pneumoconiosis in a metal works has been used to draw attention to this rare occupational disease. The technology for the manufacture and processing is described with special reference to dust exposure at the various stages. After an historical review, there is a detailed discussion of the clinical and radiological features of the two cases. Problems concerning the prognosis, differential diagnosis and the laws concerning compensation are discussed.

Adjusted single load exercise test. A new exercise test for improved diagnosis of coronary heart disease.

A new adjusted single load exercise test (ASLET) is presented. The test is aimed to improve diagnosis of coronary heart disease and should not be used in patients with a well-established diagnosis. 60--120 min after a multistage near-maximal ergometric test with a negative electrocardiographic outcome ASLET is performed. The single load used in ASLET is based on the last load reached at the multistage procedure. If for example a heart rate of 150 beats/min (end point parameter of the multistage test for the age group above 40 years) is reached at 100 W (after performing 25, 75, and 100 W) the ASLET will be a sudden load of 100 W. This procedure has proved to increase the sensitivity of our exercise test. In about one third of false negative results at the multistage procedure the ASLET test was positive in symptomatic patients.

Behavior of N-methylated allopurinols and related 4-thioxopyrazolo [3,4-d]pyrimidines towards bovine milk xanthine oxidase.

1. All available N-mono- and N,N'-dimethylallopurinols and the corresponding 4-thioxo derivatives have been tested as substrates or inhibitors of bovine milk xanthine oxidase (xanthine: oxygen oxidoreductase, EC 1.2.3.2). 2. None of the compounds tested revealed any inhibitory activity towards the enzyme. 3. All compounds were resistant to enzymic oxidation, with the exception of 7-methylallopurinol and its 4-thioxo analog. Both these compounds were attacked at position 6. 7-Methylallopurinol was oxidised nearly ten times faster than the isomeric 3-methylhypoxanthine. 4. These observations can be explained by assuming that for attack at C-6, the enzyme must bind both to N-1 and N-2 in the pyrazole ring and causes tautomerisation, which places a double bond at position 5,6 in the pyrimidine ring. This activation process resembles the activation of hypoxanthine.
