Inhaled beclomethasone dipropionate in acute infections of the respiratory tract.

100 patients with acute tracheitis, tracheobronchitis or bronchitis were randomly allocated to receive inhaled beclomethasone dipropionate (BDP) 100 micrograms qds or placebo as an adjunct to oral antihistamine and a tetracycline antibiotic. 2 patients were withdrawn from analysis, leaving 49 patients in each group. There was no evidence that inhaled BDP conferred any benefit or detriment on the progress of the condition as assessed by daily symptom scores and weekly clinic visits for up to 2 weeks. The same conclusion maintains when the patients were subdivided into two grades of severity as assessed by the physician when the patient first presented. Inhaled BDP would seem to have no role in the inflammatory process associated with those acute infections of presumed viral origins.

Radiation measurements aboard Spacelab 1.

The radiation environment inside Spacelab 1 was measured by a set of passive radiation detectors distributed throughout the volume inside the module, in the access tunnel, and outside on the pallet. Measurements of the low-LET (linear energy transfer) component obtained from the thermoluminescence detectors ranged from 102 to 190 millirads, yielding an average low-LET dose rate of 11.2 millirads per day inside the module, about twice the low-LET dose rate measured on previous flights of the space shuttle. Because of the higher inclination of the orbit (57 degrees versus 28.5 degrees for previous shuttle flights), substantial fluxes of highly ionizing HZE particles (high charge and energy galactic cosmic rays were observed, yielding an overall average mission dose-equivalent of about 150 millirems, more than three times higher than measured on previous shuttle missions.

Tapetum lucidum in the pigmented and albino ferret.

Light and electron microscopy showed that the tapetum lucidum in the pigmented ferret is morphologically indistinguishable from that in the albino ferret. The matrix of the rods of the tapetal cells was strongly osmiophilic, but glutaraldehyde fixation before osmium tetroxide treatment caused a dissolution of the matrix material. It has been proposed that the tapetal cells are modified melanocytes and that the tapetal rods are composed of melanin, but it can be concluded from our data that the matrix of the tapetal rods is not melanin. Further studies by plasma-atomic emission spectrometry showed that the tapetal cells are very rich in zinc, with similar levels in pigmented and albino ferrets. Excessive concentrations of other metals were not observed. Histochemical demonstration of heavy metal showed that the zinc is present in the tapetal rods and indicated a localization mainly in the rod membranes.

Solubility of copper and zinc containing proteins before and after 2-mercaptoethanol treatment of liver samples from normal and from chronically copper poisoned sheep.

The distribution between cytosol and pellet of Cu- and Zn-containing proteins was studied in livers from normal sheep and sheep with chronic copper poisoning (CCP). It was found that the relative amount of soluble Cu- and Zn-containing proteins was lower in liver homogenates from CCP sheep than in homogenates from normal sheep. Treatment of homogenate from the livers of 2-mercaptoethanol (ME) increased the solubility of Cu- and Zn-containing proteins, especially in CCP sheep. Oxidation of the liver homogenates from normal sheep with air oxygen gave a decrease in the relative amount of soluble CU-containing proteins and an increase in Zn-containing proteins present in the cytosol. It is proposed that oxidation in vivo of Cu and/or the metal-binding protein causes the observed decrease in soluble CU- and Zn-containing proteins seen in CCP sheep.

History and screen memories.

I propose that shared historical data can figure in a way comparable to personal history simultaneously to represent and conceal a person's childhood conflicts and memories. This phenomenon is compared to that of screen memories as described and defined by Freud. Two clinical examples demonstrating this vicissitude have been given. One is taken from the analytic transferential process, the other from an (extra-transference) consultation. The questions of the specific suitability of history for such a use is explored.

[Epiphyseal fixation with an external fixator. Biomechanical and clinical study].

Fifty one fractures of an epiphysis were treated by fixation using a new external fixator. The stability of the fixation was compared with that obtained by other types of fixator. The basis of the technique was the insertion of five pins into the epiphysis and of a series of pins in the diaphysis. Fixation between the epiphyseal and diaphyseal pins was achieved by three bars. The results obtained in 38 cases, half of them being at the proximal end of the tibia were analysed and were found to be encouraging. Only three had to be grafted secondarily. There was a low number of mal-unions. The main advantage of this method was to allow early mobilisation of the joint. The risk of sepsis did not give rise to concern provided that the rods do not cross the joint.

Psychotherapy of schizophrenia. What happens to treatment dropouts.

Sixty-nine nonchronic schizophrenic patients who had dropped out of psychotherapy were studied with regard both to reasons for dropping out and their subsequent clinical course, assessed by means of a retrospective, semistructured interview. Their 2-year outcomes were compared with those of a group of similar patients who remained in psychotherapy. It was found that the patients who dropped out were markedly heterogeneous in terms of their reasons for dropping out, and their subsequent outcomes. As a group, they received an unexpectedly substantial amount of subsequent psychiatric treatment, and after 2 years did not generally differ from those remaining in psychotherapy on measures of social or occupational functioning.

Autoradiography in mice of tritiated 2-(2-furyl)benzimidazole and melanin-binding in vitro.

The distribution of tritiated 2-(2-furyl)benzimidazole ([3H]FB) in male and pregnant albino mice and male pigmented mice was studied by whole-body autoradiography. The most notable finding was the accumulation of radioactivity in the melanin-containing tissues of the eye, the inner ear and the hair follicles. An additional study in vitro on isolated bovine-eye melanin showed a substantial binding of [3H]FB to the melanin. The general distribution was characterized by a rapid excretion pattern, except for uptake in the thyroid and nasal mucosa. Transfer to fetuses was significant and the fetal distribution was dominated by excretion.

The Indians have many terms for it: stuttering among the Bannock-Shoshoni.

This report follows by 23 years correspondence between Wendell Johnson and Sven Liljeblad in which Liljeblad pointed out that among North American "nonstuttering" Indians there were Indians who stuttered. He also reported several terms that referred to stuttering. An unpublished study reported by Frank confirmed Liljeblad's claims. Since we must rely on published reports, it is important to get these findings into print for future scholars. Though these findings in no way refute the claimed importance of linguistics and cultural variables as contributing to the development of stuttering, they do call to question evidence supporting the view that stuttering is a diagnosogenic disorder. Historical and sociological issues related to these contradictory findings are similar to those discussed by Freeman.

Similarity of obesity indices in clinical studies of obese adults: a factor analytic study.

The similarity of measurements obtained with six commonly used obesity indices was assessed with correlational and factor analyses performed on data for 951 obese adults participating in a weight reduction study. Intercorrelations among the indices were found to be very high, with a mean of 0.96. A factor analysis of the six indices resulted in a single factor which accounts for 97% of the aggregate variance in the six indices. A factor analysis of the six indices plus height and weight resulted in two factors. The six indices loaded nearly perfectly on one factor and not at all on the second. Height loaded perfectly on the second factor. The results of these analyses constitute strong empirical evidence that the obesity indices are measuring the same thing and that factor is independent of height. Although anthropological or other special studies may necessitate the use of a particular index, these results suggest that it should make little difference which of the six indices is used in a clinical study of obesity with obese adults.
