The base view in vertebral angiography. An aid in the diagnosis of extraaxial cerebellopontine angle tumors.

Though the advent of computed tomography has improved the detection of posterior fossa lesions, there are times when it can not be used to differentiate between an intra- and extraaxial lesion, particularly when the mass is anteriorly located. Base view angiography can be used to make this distinction, accurately localize the lesion, and demonstrate involvement or compression of vital adjoining structures.

Persistence of antigen in nonarthritic joints.

The presence of antigen, IgG and C3 was shown by radioautography and immunofluorescence in the collagenous tissues of the joints of animals injected intra-articularly with antigen after having been previously immunized with that antigen in Freund's incomplete adjuvant. Since these joints were shown to be virtually free of inflammatory reactions, we suggest that the persistence of immune complexes activating complement cannot fully explain the chronicity of experimental allergic arthritis.

The non-passage of mycobacteria from Freund's complete adjuvant granuloma depots to arthritic joints.

The intra-articular injection of antigen after immunization with that antigen in FCA, the mycobacterial component of which consisted of live M. avium or dead 125I-M. tuberculosis, resulted in identical chronic disease, experimental allergic arthritis. In the former case live mycobacteria could be isolated from the subcutaneous granuloma which developed at the injection site, and from arthritic joints into which 10 mug wet weight (equivalent of 2 mug dry weight) had been injected with this antigen. However, no mycobacteria could be isolated from other inflamed or normal joints. With 125I-labelled mycobacteria radioactive counting and radioautography clearly showed M. tuberculosis present in the subcutaneous granuloma but not in either injected or uninjected joints. The quantity of mycobacteria which could have been detected if present was estimated to be less than 1 mug dry weight. These results strongly suggest that mycobacteria do not travel from the granuloma depot to the arthritic joint, and that any mechanism one may consider to explain the chronicity of EAA cannot involve the physical presence of mycobacteria in the knee joint.

The timing and frequency of complications after peripheral percutaneous transluminal angioplasty and iliac stenting: is a change from inpatient to outpatient therapy feasible?

PURPOSE: A prospective study was performed to assess the frequency and timing of complications after transluminal angioplasty and stent placement with a view to changing our practice and performing these procedures on an outpatient basis. METHOD: A total of 266 angioplasties and 51 stent deployments were attempted on 240 consecutive patients. Immediate complications were documented by the radiologists. The timing and nature of any complications during and beyond the first 24 hr were reported by the vascular surgeons. RESULTS: There were 14 complications in 240 patients, giving a complication rate of 4.8% per vessel segment dilated. There were five major and nine minor complications. Eighty-six percent of complications were evident before the patient had left the angiography suite. All complications were evident within 4.5 hr of the procedure. CONCLUSION: The timing of complications suggests it would be reasonable to perform percutaneous transluminal angioplasties and iliac stenting on an outpatient basis in suitable patients.

PCDD/PCDF on land around an industrial site and identification of source.

Following the discovery of elevated levels of PCDD/PCDF in dairy products and in flue gases at a nearby industrial site, the levels and spatial distribution of these compounds on surrounding land were determined through soil monitoring. In mass terms, the levels in soil were excessive relative to the UK background but in terms of toxicity they were less significant. There were positive correlations between wind directions and deposition levels. The relative amounts of the prominent congeners/congener groups in the soil were similar to those in the flue gas but different from those in the background. An incinerator of chemical waste was identified as the likely source of the PCDD/PCDF.

Serogroup W135 meningococcal disease in Hajj pilgrims.

An outbreak of W135 meningococcal disease occurred in the spring of 2000 among pilgrims returning from Saudi Arabia and their contacts. Clinical isolates from England and France were examined and compared with reference strains from other countries. Characterisation of isolates by a range of typing methods showed them to be of clonal origin (ET-37) and closely related to other meningococci with an established propensity to cause disease clusters. A reappraisal of vaccination strategies for travellers is required.
