Oncocytic lesions of the larynx.

Three cases of oncocytic lesions of the larynx diagnosed at the Section of Pathology, Dep. of Otolaryngology, Padua University from November 1974 until December 1980 are reported. The lesion is uncommon but not exceptional and occurs in elderly people. Problems connected with pathology and the clinico-therapeutic approach are dealt with. The pathogenesis of the lesion is also discussed, with particular stress on whether it is a true neoplasm or a process of metaplasia-hyperplasia from the ductal epithelium in connection with senile involution of the upper respiratory tract.

Synovial sarcoma of the soft palate in a child: a light and electron microscopic study.

An extremely rare case of synovial sarcoma localized in the soft palate of a 9-year-old boy is reported, describing all details noted during light and electron microscopic observation. All features characteristic of this mesenchymal tumour were evident in the light microscopy. Electron microscopy also confirmed the diagnosis. The text includes full particulars of the therapy administered.

Laryngeal salivary duct carcinoma: a light and electron microscopic study.

The light microscopic and ultrastructural features of a laryngeal salivary duct carcinoma are presented. By light microscopy, the neoplasm bore a marked resemblance to the infiltrating duct carcinoma of the breast. Ultrastructurally, the tumour was characterized by the presence of two principal types of cells, i.e. epithelial and myoepithelial cells, arranged in duct-formation.

Primary eosinophilic granuloma of the larynx.

A rare case of primary eosinophilic granuloma of the larynx in a 49-year-old man is reported. The patient remains asymptomatic after a follow-up of three years. This is the third case reported in the world literature.

Diagnosis and assessment of non-Hodgkin's malignant lymphomas of the larynx.

The present paper reports the clinicopathologic features of six cases of non-Hodgkin's lymphoma of the larynx observed during the last 14 years. After careful clinical staging only 3 patients had isolated laryngeal lymphoma (stage IE). Radiotherapy was administered as initial treatment in all cases. These patients are alive and well after 12, 6 and 1 years, respectively. Histologically, all laryngeal lymphomas of this study showed a diffuse pattern of growth. All lymphomas were of follicular center cell origin according to Lukes and Collins (29). Four cases were of low grade malignancy according to Lennert and Mohri (25), or grade I, according to the British (18) classification. The remaining two were of high grade malignancy or grade II. A careful histopathological diagnosis, which may separate other pathological conditions, and an appropriate staging, which may exclude dissemination of disease, are stressed as representing the basis for effective treatment. The applicability of current functional nomenclature is possible also in these extranodal lymphomas.

Primary Small Cell ("Oat Cell") carcinoma of the larynx and hypopharynx.

The present paper is a clinico-pathological study of 8 cases of small cell("oat cell") carcinoma of the larynx and hypopharynx investigates at the Section of Pathology of the ENT Department of Padua University during the last 14 years (from January 1966 to December 1969). Besides a review of the world literature, several problems connected with terminology, histogenesis and pathologic patterns (microscopic, histochemical and ultrastructural) and examined, together with the differential diagnosis, clinical aspects and therapeutic approach. The association of small cell carcinoma with squamous cell carcinoma both in the larynx and hypopharynx is reported.

Mucoepidermoid carcinoma of the larynx. A clinicopathological study of 11 cases with review of the literature.

A series of 11 cases of mucoepidermoid carcinoma of the larynx is presented. The neoplasm is uncommon and the present series has been selected from 872 primary malignant neoplasms of the larynx evaluated at the Section of Pathology of ENT Department of Padua University from January 1975 to December 1979. The median age of the patients was 59 years. The most common location was the epiglottis. The tumors were classified on the basis of cellular differentiation, and common and/or uncommon pathologic features were recorded. Differential diagnosis must be made from several neoplasms, in particular from squamous cell carcinoma, adenoid cystic carcinoma and adenosquamous carcinoma. The treatment of choice is surgery. The prognosis is better than that of the commoner squamous carcinoma and depends both upon the stage and the histologic grading.

Ackerman's tumor (verrucous carcinoma) of the larynx: a clinicopathologic study of 77 cases.

A series of 77 cases of verrucous carcinoma of the larynx is presented. The tumor is relatively rare and the present series has been selected from 2,398 primary and recurrent malignant neoplasms of the larynx and hypopharynx evaluated at the Department of Otolaryngology of Padua University during the period from January 1966 to December 1978. The median age of patients was 58 years. The tumor appears to be exophytic, broadly implanted and fungating in aspect, with papillary fronds and a locally invasive character. It is composed of highly differentiated epithelial squamous cells covered by a thick keratinized layer arranged in deeply invaginated folds. Cellular response in tumor stroma is marked, and the neoplasm has a low-grade malignancy. Close cooperation between the pathologist and the clinician is needed in order to establish the nature of the lesion, which looks clinically malignant but may appear histologically benign. Benign reponses such as hyperkeratosis, acanthosis, or the presence of a benign papillomatous area necessitate further investigation with another biopsy examination from deeper layers. The elective treatment is surgery, as irradiation may cause anaplastic transformation with metastatic spread. Neck dissection is not indicated as this laryngeal tumor has so far never metastasized to cervical lymph nodes or to other organs.

Extramedullary plasmacytoma of the parotid gland.

A case of solitary extramedullary plasmacytoma of the parotid gland occurring in a 47-year-old male patient is reported. Clinical examination failed to reveal evidence of this neoplasm elsewhere in the body. Problems connected with the clinical, pathological and therapeutic aspects of the neoplasm with particular regard to the rare parotid location are taken into consideration. Furthermore, stress is laid on the fact that patients with extramedullary plasmacytoma should be thoroughly followed-up as multiple myeloma may occur even decades later in life. This is the fourth case of extramedullary plasmacytoma located in salivary glands to be reported in the world literature.

Double primary synchronous and metachronous cancer of the larynx and hypopharynx with dissimilar histology. Five case reports and review of the literature.

Five cases of double primary synchronous and metachronous cancers of the larynx and hypopharynx with dissimilar histology encountered at the Section of Pathology of the Department of Otolaryngology of Padua University from 1966 through 1979 are reported. The pathogenesis of these double primary malignancies is discussed, and in particular stress is laid on the perhaps not purely coincidental association of squamous cell and oat cell carcinoma of the larynx and hypopharynx. Furthermore, a thorough review of the literature is made on the reported cases of two primary simultaneous or metachronous malignancies of the larynx and hypopharynx of different histological type.

Observations on the epidemiology of oesophageal carcinoma in the province of Trieste.

After describing the enormous variation in incidence of oesophageal carcinoma in different populations, the authors report data on the incidence of carcinoma of the oesophagus in the province of Trieste, Italy. There is a high incidence in men, this being among the highest in the world, excluding areas with so-called 'endemic' disease. On the other hand, the incidence in women is low, even compared to other European regions. The epidemiological data reveal in particular that, in the province of Trieste, carcinoma of the oesophagus affects predominantly young and middle aged men. This may be related to high alcohol and tobacco consumption.

Acinic cell carcinoma of minor salivary glands.

Three cases of acinic cell carcinoma of minor salivary glands, located respectively in the larynx, base of the tongue, and right tonsil, were encountered at the Section of Pathology of the ENT Department of the University of Padua between 1974 and 1978. Such neoplasms arising in minor salivary gland tissue are extremely rare, but may be found in all sites containing normal or aberrant salivary gland tissue. The histological and histochemical characteristics of the tumour are discussed together with the differential diagnosis from other neoplasms, particularly clear-celled tumours such as glycogen rich adenocarcinoma, mucoepidermoid carcinoma, metastatic clear cell renal adenocarcinoma and clear cell squamous carcinoma. In general, acinic cell carcinoma can be defined as a tumour of low-grade malignancy, though not too rarely it may recur and occasionally metastasize.
