A transplantable mouse lymphoma with unusual azurophilic granules.

A transplantable malignant lymphoma arising spontaneously in the C3H/Jax Heston mouse line is described. Such a spontaneously arising lymphoma within this strain has not been previously reported. The tumor exhibits 100% transmission in the C3H/HeJax mouse line and has been successfully carried through 113 transfers. No spontaneous regression was observed in 678 animals. The tumor terminates in lymphocytic leukemia after 2 to 3 weeks. The neoplastic cells contain azurophilic granules which stain purple-red with Wright's or tetrachrome blood stains, are not metachromatic with toluidine blue, are not peroxidase positive and show a pleomorphic structure at the level of electron microscopy. While the nature and function of azurophilic granules remains undetermined, their presence in this transmissible tumor offers a better opportunity to study them.

[Therapy optimization of heart failure through the evaluation of the plasma concentration variation of B-type natriuretic peptide].

AIM: Our research was based over the critical evaluation of the plasmatic concentration variation of B-type Natriuretic Peptide in emergency in patients with heart failure during therapy with diuretics, anti-aldosterone, ACE-inhibitors, beta-blockers and nitroderivates. MATERIALS AND METHOD: We selected 108 patients: 30 control subjects (average 58.40 +/- SD13.32 for 20 M, and 65 +/- SD 14.74 for 10 W), and 78 subjects (average 75.90 +/- SD 9.60 for 41 M, and 77.89 +/- SD 8.62 for 37 W) arrived to the emergency and reception department for dyspnea and/or precordialgia and/or palpitations with heart failure diagnosis according to NYHA Classification. The variation of BNP concentration was evaluated in these subjects at the admission, after 1 week, and 1 month from the beginning of the therapy. RESULTS: Patients with heart failure had a BNP concentration high during all measurements. The values were high during admission, but after 1 week and after 1 month, they reduced reaching the balance. CONCLUSIONS: BNP evaluation is a good indicator for the diagnosis of heart failure and for improving the therapy. The main limit of BNP diagnostic role is the need of knowing in advance the specific values for each patient.

Preoperative selection for curable renovascular hypertension.

Clinical research has defined the criteria for identifying "curable" renovascular hypertension. The prediction is based on the measurement of plasma renin activity in peripheral veins, renal veins, and the aorta. Renin profiles can be examined with the method of Laragh and coworkers (the so-called incremental method), in which values for plasma renin activity are incorporated into formulas to compute indexed parameters. A score is attributed to each index and the prediction is based on the final score. Because the identification of curable renovascular hypertension is made according to numerical rules, the method is easily transformed into a computer-assisted process. The program is written in BASIC, and it is short enough to run on a personal computer.

[Dermatopathies in those engaged in domestic work and related risks].

The AA., considering the skin diseases in the subjects employed in the domestic work and similar activity, in a more ample table of general medicine, produced a contribution to the study of etiopathogenetic and nosographic problems. They have estimated the role of the individual predisposition and the etiopathogenetic, chemical, physical, biotic, complex factors. They pointed out the various nosographic, skin expressions in range of contact irritant dermatitis, of contact allergic eczema, skin abrasion, of dermatitis by physical agents, of urticaria, of dermal alterations by infectious and parasitic agents and of various dermal, morbid expressions by chemical agents. They reported the direct experience of personal casuistry and called attention on multiple aspects of preventive medicine.

Phagocytosis of monocytes and platelets in a neutropenic infant with septicemia.

The case is described of a low birth weight infant who developed severe neutropenia, thrombocytopenia and monocytosis in the course of staphylococcal septicemia. Light microscopy examination of the blood revealed bacteria inside the monocytes. On electron microscopy, the monocytes were degranulated and distorted by the presence of large vacuoles containing bacteria. While most of the bacteria were intact, some showed capsular dissolution suggesting the effect of lysosomes. Rarely, the bacteria were also noted within the polymorphonuclear leukocytes and platelets. Based on the finding of electron microscopy in this case, the phagocytic function of the monocytes, neutrophils and possibly the platelets was intact. In view of the fatal outcome, despite the previous findings, it is suggested that granulocyte transfusion be considered in low birth weight infants with persistent sepsis.

Ultrastructural observations in an alcoholic patient with post-surgical pseudomonas infection.

Gram negative bacteria were seen in the peripheral blood and within the neutrophils of a patient with bacteremic shock secondary to Pseudomonas aeruginosa infection. By electron microscopy, bacteria were present either in vacuoles or in the cytoplasm of neutrophils. When seen in the cytoplasm, they were surrounded by amorphous material which most probably represented fused lysosomal granules. In both cases, the microorganisms appeared morphologically normal. The presumption is that there was a pre-existing defect of neutrophilic lysosomal formation or function. These findings indicate the importance of studying neutrophil morphology and function in patients with persistent infections.

Treatment of myocardial infarction: hypotensive effect of different thrombolytic agents.

The incidence of hypotension in patients treated with thrombolytic agents for myocardial infarction was investigated in a series of 71 patients, 17 treated with urokinase, 35 with rtPA and 19 with APSAC. Hypotension was observed in 23.5% of the first group, in 5.5% of the second, and in 42.10% of the third (p < 0.002 between rtPA and APSAC). In the inferior location hypotensive reaction was much more frequent than in anterior one (p < 007) especially if a right ventricular involvement was associated. Even if hypotension is a minor and generally harmless complication, it poses many practical problems, and its occurrence must be taken into account when choosing a fibrinolytic treatment.
