[Thrombohemorrhagic syndrome as the 1st manifestation of stomach cancer with multiple tumor embolisms in the microcirculation].

Two patients were described that died of gastric carcinoma, the first clinical manifestations of the disease being a hemorrhagic diathesis. The morphological examination revealed multiple tumour emboli in microcirculation of bone morrow and internal organs, that led to the development of disseminated intravasal coagulation with consumptive coagulopathy. Megacarycytosis was observed in bone marrow and spleen as well as megacaryocytemia in lungs as a manifestation of compensatory reaction.

Changes in the P3-wave in a situation of verbal stimulus prediction.

The changes in the P3-wave of averaged auditory evoked potentials (AEP) from Fz, Cz and Pz were studied under conditions when: (I) the subject was required to predict which of the two alternative sound stimuli would appear. After presentation of a series of 50 high (1200 Hz) and 50 low (800 Hz) tones, in random order (mixed signal series) we proceeded suddenly to the presentation of the low tone only, without warning the subject (pseudo-mixed series); (II) the same mixed signal series was presented again requiring quick motor response by the subject only to the appearance of the high tone and not requiring verbal prediction (choice RT task). It has been found that in the three recordings: (1) the P3-wave increases for both types of stimuli, both in the situation of verbal stimulus prediction without motor response and in the situation of choice RT task; (2) the increase in P3 (assessed by the N1-P3 amplitude) is greater in AEP from stimuli disconfirming the prediction compared with stimuli confirming it; (3) preforming of the verbal prediction, in the course of the pseudo-mixed series, to prediction of the appearance of the low tone only is accompanied by a sharp decrease or disappearance of the P3-wave. The results show that the P3-wave is not connected with the development of motor potentials but with the processes of verbal prediction, in particular with the non-coincidence between the event expected by the subject and the really appearing one.

Dependence between the P3-wave and the prestimulus EEG negative shift under conditions of probability prediction.

The connection between the prestimulus slow potential shift (SPS) and the P3-wave of auditory averaged evoked potentials (AEP) from Fz and Cz is studied upon changing the probability and signal characteristics of the stimuli. It has been found that while the changes in the P3-wave, depending on the character of the experimental tasks, are unidirectional for all subjects studied in both recordings and repeat our earlier results, the changes in SPS have different directions. The same changes in P3 are accompanied in some cases by a slightly expressed negativization, in other cases by a certain positivization and in a third group of cases by lack of SPS. The results obtained show that the observed changed in the P3-wave under the conditions of our experiments are not connected with pre-stimulus SPS of the CNV type, being generated in the central nervous system irrespectively of them. They support our hypothesis that the P3-wave is connected with the processes of probability prediction in the brain functioning.

[The single daily dose of gentamicin in the treatment of urinary tract infections (author's transl)].

Gentamicin was administered intravenously or intramuscularly to 165 patients with non-obstructive urinary tract infections. 3 prospectively randomised groups were formed in which gentamicin was given either once daily (120 mg), twice daily (40/80 mg) or three times daily (40 mg). Clinically and bacteriologically an equally good improvement of the condition appeared in all groups. The mode of application selected had no influence on the therapeutic success in 86--95%. Since with the single daily injection of 120 mg gentamicin the proportion of side effects did not increase, this therapeutic plan can be recommended for outpatient treatment in urological and internal medical practice.

[Embolisms and thromboses in the lesser circulation and disseminated intravascular coagulation].

Investigations were carried out on 601 deceased with embolism and thrombosis in pulmonary circulation (ETPC), in 53 of the cases (8.82%) a generalized, in two or more organs, disseminated intravasal coagulation (DIC) was found. Generalized DIC develops most often in autochthon pulmonary thrombosis (in 30.91%) and more rarely in pulmonary embolism with additional autochthon thrombformation (in 15.19%). In a great part of pulmonary embolism with additional autochthon thromb-formation (96.62%) and autochthon pulmonary thrombosis (87.27%) DIC develops locally only in pulmonary microcirculation. The local DIC in lungs is characterized morphologically by the formation of multiple fibrin microthrombus (2--3 microthrombuses per square cm section) in the microcirculatory system of lungs. The development of local or generalized DIC aggravates the course of ETPC and could play an essential role in thrombogenesis, both by mechanic occlusion of pulmonary vessels and by serotonun elimination from the destroying thrombocytes.

[Ischemic heart disease and embolisms and thomboses in the lesser circulation].

All deceased with ischemic heart disease (IHD) -- 450 cases and with embolism and thrombosis in pulmonary circulation (ETPC) -- 601 cases were purposefully examined for a period of 10 years (1963--1972). In 75 of the examined a combination of IHD and ETPC was present. In 41 out of them only cicatrices of past infarctions were found and in 34 -- recent myocardial infarctions. In 14 of the examined (41%) it was established, clinically and morphologically, that ETPC are with a longer duration than the recent myocardial infarctions and very likely have played a role in their pathogenesis. The confirmation of acute ischemic changes in myocardium gives a support -- undulation of the myofibrils and fuchsinophylic necrosis in the deceased of ETPC without preceeding IHD. The cardiac hypertrophy and coronary atherosclerosis lead to an intensification of the myocardial ischemic alterations in ETPC.

[Frequency, etiological factors and the morphological characteristics of myocardial infarct in youth].

Examinations were carried out on 827 deceased with myocardial infarction (MI), 27 (3.26%) out of them being at a rather early age (26-44). The average age of those examined with MI at that early age is 39. The males are affected more frequently than females -- ratio 3.5:1. In 89 per cent of the examined the coronary insufficiency was manifested clinically and morphologically before the age of 40 and two or more MI, with a different duration were established. The most frequent causes of the development of MI are coronary atherosclerosis in 48.15 per cent, rheumatism (coronary embolism) in 14.81 per cent, endarteritis obliterans in 7.14 per cent and leutic coronaritis in 3.70 per cent. In 25.93 per cent of the examined only lipoidosis or completely intact coronary arteries were established. Almost in all of the examined cardiac hypertrophia was present, the average weight of the heart being 434 g. In a part of the examined, morphological changes in microcirculation were observed -- namely -- formation of microthrombosis, manifested intimal cushions, fibrosis of the walls and perivasal fibrosis of the muscular arteries and arteriols.

[Morphological changes in diplococcosis in calves].

Diplococcous infection was observed in 15 calves aged 3 to 30 days. Diplococcous pneumoniae was isolated from synovial fluid and parenchymal organs in affected joints of dead calves. Clinically and morphologically the disease had an active course in calves aged up to 20 days, while in the older ones a subacute or chronic course of the disease was observed. The morphological changes depended on the type of infection. In acute infections a septic course of the disease, including hemorrhagic diathesis, enlarged spleen, myocarditis, hyperaemia, oedema and isolated hemorrhages of the brain meninges was observed, while in the chronic cases pneumonia, pleutisy, pericarditis, arthritis of the knee, elbow, dorsal and carpal joints and ofien suppurative meningoencephalitis were encountered.

[Fibrous suppurative arthritis in sheep].

Described are cases of fibrinous supprative arthritis in sheep characterized by thickenning of the joint, resulting from the involvement of the capsule (periarthritis). Histopathologically, there have been suppurative and necrotic processes combined with fibrinoid necrotizing of the collagen fibers and leukocyte infiltration of the joint capsule. Slight leukocyte infiltration has also been said to occur in the parenchymal organs. Microbiologic studies have revealed the presence of pyogenous bacteria (Corynebacterium pyogenes, Streptococcus pyogenes, Pasteurella multocida).

[Enterotoxic form of colibacteriosis in calves].

The enterotoxemic from of calf colibacteriosis was observed on a total of 18 occasions in newborn animals. Clinically, the disease was manifested with diarrhea, depression of the eyes, fast dehydration, the calves tending to go down and lie for a prolonged periods, with high death rates. Some of the diseased calves showed nervous symptoms. Escherichia coli 0126 was isolated from the intestinal content and the mesenterial lymph of the affected calves. It proved virulent for albino mice. Morphologically, there were strongly expressed circulatory disturbances--hyperemia, perivascular edemas, hemorrhages (Fig. 1 and 2) thrombi (Fig. 3 and 5), with a vacuolar and parenchymal dystrophia of the liver, kidneys, and heart.

[Light microscopic and ultrastructural changes in the myocardium in experimental myocardiopathy in rats].

The authors examined the morphological changes in the myocardium of rats with myocardiopathy, caused by electrolytes, steroids and catecholamines. There were fresh and organized microinfarctions, occupying around 17--18% of the volume of the left heart ventricle. Scattered fuxinophilic necrosis were present in the zones outside the infarctions. The electrone microscopic examination of these areas showed vacuolization and crystolisis of mitochondria, dilatation of cysterns of the sarcoplasmic reticulum after dilatation of Z discs of miofibrils and increased folds with herniation of the intercanicular discs. The obtained model belongs to the group of the infarction-like myocardiopathies and resembles the changes in the human myocardium during involvment of the small intramural myocardial arteries and microcirculatory bed.
