The role of periodontal plastic microsurgery in oral facial esthetics.

Periodontal plastic microsurgery incorporates the use of a surgical dissecting microscope in an attempt to increase visibility, minimize trauma, and enhance surgical results. This paper will attempt to demonstrate how periodontal plastic surgery utilizing periodontal microsurgery could contribute in the role of soft tissue modification to enhance the smile and ideally allow for improved oral facial esthetics.

Development of gingival esthetics in the edentulous patient prior to dental implant placement using a flangeless removable prosthesis: a case report.

Development of gingival contours found in healthy natural dentitions enhances the esthetic results achieved with implant-supported fixed prostheses. However, this endeavor is frequently difficult to achieve, especially in the completely edentulous patient. Edentulous patients with optimal hard and soft tissue can be treated with a specially designed removable prosthesis that will develop gingival contours prior to implant placement. By means of a transitional complete removable prosthesis with ovate pontics and no labial flange, a natural-looking soft tissue profile can be developed prior to dental implant placement. A minimally invasive tissue punch surgical technique is used to place the implants, which are immediately restored with a 1-piece, cross-arch, provisional fixed prosthesis. This article presents the prosthodontic and surgical protocols utilized to improve the appearance of the definitive implant rehabilitation.

Esthetic periodontal considerations in orthodontic treatment--the management of excessive gingival display.

This paper examines various esthetic periodontal considerations during orthodontic treatment. The management of excessive gingival display caused by altered passive eruption is reviewed, with emphasis on causes, recognition, diagnosis and surgical management of this problem. A case of orthodontic treatment of excessive gingival display associated with altered passive eruption of the maxillary incisors is reviewed to demonstrate appropriate management. With proper diagnosis, soft-tissue periodontal procedures after completion of orthodontic treatment can enhance the patient's final appearance.

Interimplant papilla preservation in the esthetic zone: a report of six consecutive cases.

Simultaneous removal of multiple adjacent teeth in the anterior maxilla often leads to collapse of the labial bony plate as well as flattening of the interproximal bony scallop, resulting in long implant restorations with missing interimplant papillae. An interimplant papilla preservation technique involving alternate immediate implant placement and provisionalization, one following the osseointegration healing period of the other, was introduced. All 14 implants placed in six consecutive patients maintained osseointegration after a mean functioning time of 22.6 months (range 12 to 34 months). A highly satisfactory esthetic outcome and a papilla index score of 3 was observed in all patients.

Facial esthetics in a selected Saudi population.

OBJECTIVE: The purpose of this study is to analyze the soft tissue cephalometric norms of the Saudi population and to evaluate whether significant cephalometric soft tissue differences exist between Saudi and Caucasian population. METHODS: Lateral cephalometric radiographs of 40 selected Saudis with esthetically pleasing faces (20 males and 20 females). The study was carried out in the College of Dentistry, King Saud University, Riyadh, Kingdom of Saudi Arabia between 1999 - 2001. The age range between 20 and 30 years was analyzed using Burstone system of soft tissue analysis. The means, standard deviations and ranges of the measurements were compared between males and females, and both figures correlated with the results obtained for a Caucasian population. RESULTS: Statistically, the facial convexity angle (glabella-subnasale-soft tissue pogonion) was found to be less convex for the Saudi group; the maxillary prognathism was more anterior, and the upper and lower lip positions were more anterior. CONCLUSION: The results of the present study are significant, and showed that the normal Saudis have a slightly lesser obtuse nasolabial angle, and a more anteriorly positioned upper and lower lips. These results have a clinical implication in the diagnosis and treatment of adult Saudis with dentofacial deformities.

All-ceramic restorative system for esthetic implant-supported crowns: in vitro evaluations and clinical case report.

Patient demands for improved esthetics have prompted the development of all-ceramic restorative systems for dental implants, but material strength and restorative costs have presented clinical challenges. Therefore, a new restorative system with tooth-shaped ceramic copings for the anterior and premolar jaw regions has been introduced to address these problems. Fatigue and 17o. compression tests were conducted in vitro to assess the mechanical strength of the 6 tooth-shaped copings and several luting agents of the system. A case report on the clinical use of the components is presented. All 6 tooth-shaped copings significantly exceeded the range of forces associated with restoration in the anterior jaw. Crown-endurance limits for fatigue and 17o. compression were 70% higher and 46% higher, respectively, than the established minimum-fatigue-endurance limits in those categories. In clinical evaluation, the ceramic restorative system performed well and produced excellent results; it has potential for implant restorations in the anterior and premolar regions of the jaw.

Predictable esthetic indirect restorations.

Patients are increasingly requesting esthetic tooth-colored restorations. Direct composite restorations have limitations based on the size of the restoration, the ability of the patient to tolerate procedures, and the ability of the dentist to provide functional restorations. The indirect composite restoration is ideal for replacing multiple failing restorations in teeth with existing restorations that are too large to be restored with direct composite restorations, yet too small to warrant the preparation for complete crowns.

Tooth fragment reattachment--an esthetic alternative: report of a case.

Trauma to the anterior teeth is relatively common among children and teenagers. Reattachment of a fractured fragment to the remaining tooth can provide better and long lasting esthetics, improved function, a positive psychological response and is a faster and less complicated procedure. This article discusses fragment reattachment technique and presents clinical case of coronal fracture involving enamel and dentin.

Esthetic evaluation and treatment of the maxillary anterior--a case report.

Achieving an optimal esthetic result can be very challenging. As in other dental disciplines, attention to detail must be followed at all stages, starting with thorough diagnosis and treatment planning. Better understanding and fewer potential problems result when dentists communicate with patients by educating them about their treatment options and keeping them involved. In this case, decisions regarding orthodontics and periodontics were made easily because of a complete understanding between the patient and dentist. Informing patients of the advantages and disadvantages of various treatment options allows them to make more informed decisions.

The use of cast gold pinledge retainers with pontics as an esthetic and functional restorative option in the maxillary anterior.

A restorative technique for replacing congenitally missing lateral incisors was necessary when implants were not an option. Two three-unit bridges were considered but were rejected in favor of the much more conservative pinledge retainers. Only a relatively small amount of tooth structure was removed during the preparation of the canines, compared to the amount of reduction necessary for crown abutments. Maryland-type bridges (both all-ceramic and metal-based) also were considered but were rejected due to the tight occlusion and the need to prepare the lingual of both abutments in greater amounts than necessary for the thin gold castings. From an esthetic standpoint, there was a small amount of gold showing but not enough to bother the patient, his parents, or the dentist.

Successful dentist-patient communication for optimal esthetic results.

Dentist-patient communication must be at the highest level to ensure efficient, functional, and esthetic dentistry. Understanding the patient and his or her needs and helping that patient understand them as well is the first step to providing proper dental care.

The utilization of an interdisciplinary team approach in esthetic, implant, and restorative dentistry.

To execute interdisciplinary care for complex dentofacial abnormalities, a team of providers must develop a consistent philosophy and mission regarding examination, diagnostic, and treatment planning procedures. To obtain optimum therapeutic results, patients must rely on the collaborative expertise of a team. This article and case study illustrates a multidisciplinary team's role in the prosthetic and esthetic rehabilitation of the maxillary anterior region.

The restoration of functional occlusion and esthetics.

Severe occlusal destruction, whether it results from attrition, abrasion, or erosion, must be met with definitive diagnosis and treatment. This case report illustrates the step-by-step diagnosis and treatment of a patient with severe occlusal wear. Restoration of a functional occlusion and esthetics involved endodontic treatment, reestablishment of the vertical dimension of occlusion, osseous periodontal surgery, and placement of single-tooth cast glass-ceramic crowns.

Making esthetic provisionals.

The problems that arise from using traditional methyl methacrylate material to construct temporary restorations are discussed. The need for exceptional, esthetic and well-fitting temporaries in a contemporary dental practice is stressed. A practical method for constructing temporary restorations is described in detail. The advantages of the author's method are described.

Esthetics in fixed and removable prosthodontics shade selection in metal-ceramics.

Many factors affect selection of the proper shade for metal-ceramic restorations. The purpose of this paper was to present the inherent limitations in accurate shade determination and discuss methods to control these limitations. A systematic approach is presented as an aid for attaining accurate shade selection. Using this approach will enable the dentist to place more esthetic restorations.

Esthetic restoration of traumatized permanent teeth in children using composite vestibular veneers (preliminary communication).

UNLABELLED: All people are eager to have attractive dental looks. Esthetic corrections of defects caused by colour alteration are required more and more frequently by dental patients. Very often even the form of viable or devitalised teeth in children is wanted to be changed. The modern composite materials allow this to be done by vestibular veneers satisfying even the highest requirements in this respect. THE AIM of the study was to address the matter of the restoration of traumatised permanent children's teeth by composite veneers. MATERIALS AND METHODS: The study included 30 traumatized permanent anterior teeth in children which, after dealing with the trauma, were restored by means of composite veneers. The teeth were first prepared using a suitable method and the composite veneers were fabricated during the same visit. The patients were evaluated at one, three and six months. RESULTS: The colour, margin integrity, surface structure, and anatomic form were rated using the California Dental Association's criteria. CONCLUSIONS. Restoration of traumatized permanent teeth in children with vestibular veneers yields very good clinical results. The composite veneers is an alternative method in restoration of such teeth in children.
