[Orthodontic treatment as prevention of periodontal changes].

Even the general dental practitioners consider the orthodontic therapeutical interventions carried out with removable appliances as a harmful procedure. It is much less known how the lack of adequate orthodontic treatment endangers the peridontium. Clinical data show a cause related correlation between the maxillary incisor protrusion, mandibular frontal jammed dentition, deep vertical overbite, openbite, crossbite early deciduous tooth extractions and the periodontal pathological happenings. So it is very important to start an early orthodontic treatment to avoid the periodontal consequences of malocclusion before it has become irreversible.

Guidelines for management of impacted canines.

Though the management of impacted canine is considered a multi-disciplinary entity requiring the expertise of a number of specialists, the general practitioner needs to coordinate care and must know about the diagnosis and management of canine impaction. The management of canine impactions can be simplified by following a logical and systematic approach. The schematic treatment plan for impacted canines and the factors influencing and determining the plan are discussed in this article.

Failure of tooth eruption involving a mandibular primary first molar: a case report.

PURPOSE: Primary failure of tooth eruption involving primary teeth is unusual. The purpose of this report was to describe uneruption of a single mandibular primary first molar and the treatment outcome following limited orthodontic care. METHODS: A 5-year, 3-month-old boy was referred to the authors' clinic for treatment of an unerupted primary first molar, 3 months after fenestration by an oral surgeon. Orthodontic traction was performed on the affected molar in the authors' clinic. The affected mandibular primary first molar was fully erupted and in occlusion 16 months after the first fenestration. RESULTS: The histopathological diagnosis of the overlying tissue was pericoronal myxofibrous hyperplasia (PMH) without any calcified obstacles. CONCLUSIONS: Of the 26 primary tooth eruption failure cases treated in the authors' clinic between 1979 and 2003, the present case was the only example of a primary first molar.

Mesiodens in the primary dentition stage: a radiographic study.

Five hundred and forty-three Chinese patients in the primary dentition stage were evaluated for mesiodens with maxillary, anterior occlusal films. Compared with other studies, a higher prevalence was found, using different age-groups and diagnostic methods. The distributions of mesiodens by sex, location, and direction were also studied. Because mesiodens could interfere with normal development of the occlusion, especially in the early mixed dentition stage, early diagnosis, using appropriate radiographs during the primary dentition stage, could lead to more comprehensive treatment planning.

Multispecialty team management of a case with impacted maxillary permanent canines.

A fifteen-year-old Chinese girl presented with unerupted maxillary permanent canines impacted against the roots of the central incisors, causing malalignment of the maxillary incisors. The canines were fully formed and their apices closed. The potential path of eruption of the canines contraindicated surgical exposure, followed by orthodontic traction. It was decided to transplant the impacted canines to their normal positions, and then align the maxillary anterior teeth, using a fixed orthodontic appliance. The transplantations and the orthodontic treatment were successful, and neither transplant showed signs of root resorption, periodontal pockets, mobility, or pain, three years after surgery. Root canal therapy was performed on the canines after transplantation, because their apices were closed. They were stained by the amalgam in the access cavities, but both responded well to nonvital bleaching using 30 percent H2O2.

Ectopic eruption of a mandibular first permanent molar: report of an unusual case.

A case history of an ectopically erupting mandibular left first permanent molar is described. Most notable are the unusual findings of a missing mandibular left second premolar and an epithelial cyst and a horizontally impacted mandibular left second permanent molar. A unique treatment plan called for the extraction of the mandibular second primary molar, excision of the cyst, and removal of the mandibular left second and third molars. In hindsight, other procedures might have served the patient better.

[The relations of socioeconomic status to health status, health behaviors in the elderly].

OBJECTIVES: To analyze the relationships of socioeconomic status(SES) to health status and health behaviors in the elderly. METHODS: Data were obtained from self-administered questionnaire of 4,587 persons, older than 65 years, living in a community. We measured the sociodemographic characteristics, socioeconomic status, health status (subjective health status, acute disease, admission experience, dental state, chronic disease etc.), activities of daily living (ADL), instrumental activities of daily living (IADL), and mini-mental state examination-Korean (MMSE-K). Binary and multinominal logistic regression analyses were employed to analyze factors affecting on the socioeconomic status of the elderly. RESULTS: With regard to the SES and health status, those with a low SES had poorer subjective health states and lower satisfaction about their physical health. Also, acute disease experiences, admission rates and tooth deciduation rates were higher in those of low SES. In the view of physical and cognitive functions, the ADL, IADL and MMSE-K scores were also lower in those of low SES. However, with regard to health behaviors, lower smoking and alcohol drinking rates were found in the low SES group, and a similar trend was shown with regular physical exercise, eating breakfast, and regular physical health check-up. From these findings, we surmise that those with low SES have a poorer health condition and less money to spend on health, therefore, they can not smoke or drink alcohol, exercise and or have a physical health check-up. CONCLUSION: This study suggests that socioeconomic status plays an important role in health behaviors and status of the elderly. Low socioeconomic status bring about unhealthy behavior and poor health status in the elderly. Therefore, more specific target oriented(esp. low SES persons) health promotion activities for the elderly are very important to improve not only their health status, but their health inequity also.

Impaired tooth eruption: a review.

Eruption is the continuous process of movement of a tooth from its developmental location inside the jaw to its functional location in the mouth. Impaired tooth eruption, where this process is disturbed, is common in dental practice. It may manifest either as delayed or complete absence of eruption. Although unerupted teeth are usually asymptomatic, they may cause cosmetic and pathologic complications. The purpose of this article is to provide a review on the pathogenesis and differential radiographic interpretation of impaired tooth eruption.

Mandibular premolar impaction: 2 case reports.

The purpose of this article is to review the principles of case management of impacted mandibular premolars and to illustrate their potential to respond well to treatment. Although the scope of treatment may be influenced by the patient"s age, past dental history, severity of impaction, dentoalveolar development and root form, the 2 case reports demonstrate the inherent potential for good treatment outcome even in the most unfavourable circumstances.

A retrospective assessment of zinc oxide-eugenol pulpectomies in vital maxillary primary incisors successfully restored with composite resin crowns.

PURPOSE: The purpose of this retrospective study was to evaluate, via clinical and radiographic assessments, the treatment outcome of zinc oxide-eugenol (ZOE) pulpectomies performed in vital maxillary primary incisors successfully restored with composite resin crowns. METHODS: Pulpectomized vital primary incisors were treated by a uniformed technique, filled with ZOE paste, and successfully restored with composite resin crowns. Those that remained intact and noncarious for the assessment interval were evaluated for the outcome (success or failure) based on clinical and radiographic findings and compared to: (1) the reason for treatment; (2) the canal filling extent; (3) the type of composite resin crown restoration performed; and (4) the eruption status of its succedaneous tooth. RESULTS: For 104 maxillary primary incisors meeting the inclusion criteria, failure, as judged by presence of pathologic root resorption and/or apical lucency, was determined to be 24% (25/104), for a mean duration of 18 months observation. Failures were statistically associated with the reason for treatment (higher for trauma), the extent of ZOE paste filler in the pulp canal (higher for gross overfill), and the eruption status of the associated succedaneous permanent incisor (higher for delayed eruption). CONCLUSIONS: This study determined a failure rate (24%) for pulpectomies-using ZOE paste and performed on vital primary incisors-comparable to that reported for nonvital pulpectomies. A statistically significant increase in failure rates was found for: (1) incisors treated for trauma (42%) vs those treated for dental caries (19%); and (2) grossly overfilled canals (80%) vs canals filled to the apex (0%).

Treatment of a pseudo-class III relationship in the primary dentition: a case history.

In conclusion, posterior and anterior crossbites of the primary dentition should be corrected, once recognized, to allow for normal dental base development and subsequent favorable skeletal growth, provided the analysis of the case reveals no gross boney discrepancies. To quiet any archaic ideas of delaying posterior crossbite correction until the eruption of the permanent first molars, a pilot is being undertaken to determine whether there is a statistical difference in the need for retreatment of patients, when the permanent first molars erupt with the use of either the Porter, Quadhelix, or R.P.E.-type appliances. It is hoped that this will support the low retreatment frequency of 7.6 percent reported by Kutin and Hawes.

[A supernumerary primary tooth inducing impaction of primary incisor: a case report].

A case of a primary supernumerary tooth in a 18 month old girl was reported. 1. The supernumerary tooth was located in the maxillary incisor area. 2. The maxillary left primary central incisor and the primary supernumerary tooth were impacted. 3. The clinical and radiographic findings indicated that the primary tooth in the mesial position was the supernumerary tooth, and therefore it was removed. 4. Three days after extraction of the mesiodens, the left primary central incisor erupted. The tooth completely erupted in two months. 5. The radiographic examination showed no supernumerary succedenous teeth.

Extraction of 'nylon' teeth and associated abnormalities in Tanzanian children.

The aim of this study is to determine the prevalence of extraction of 'nylon' teeth and its long term adverse effects on the permanent dentition of Tanzanians. A total of 1890 subjects, aged between 3 and 5 years, and 1377 subjects aged between 12 and 19 years, in different parts of the country were examined. The prevalence of missing teeth in the primary dentition due to 'nylon' teeth practice ranged from 0% to 16.9%, with a weighted mean of 9.5%. Canines were the most involved teeth, accounting for 95% of all missing teeth. Mutilated permanent teeth, a result of traditional extraction of deciduous tooth germs, were observed only in Singida district in 8% of the examined subjects. 22.8% of these mutilations presented as malformations, while the remaining 77.2% presented as missing teeth. Most missing teeth and all the malformed teeth were seen in the mandible. The mandibular canines were the most affected, followed by the mandibular lateral incisors. It is suggested that 'nylon' teeth practice in Tanzania may be a recent invention rather than a traditional tribe related custom. The precariously high prevalence of 'nylon' teeth practice in Tanzania, and the people's perception of this malpractice, requires that particular attention be paid to this issue during oral health educational programmes in the affected communities.
