Analysis of an appliance therapy program in a dental school pedodontic clinic.

It is evident from the data assembled in this paper that a program of appliance therapy in the pedodontic department of a dental school can provide valuable clinical experience for students and benefits for a high percentage of the children who are treated. It is clear that selection of cases, faculty supervision, diagnosis, patient cooperation, and administrative control of patient assignment are the key to a successful program.

Undergraduate clinical pedodontic education: an overview of two teaching systems.

This paper compares and contrasts two clinical teaching systems in undergraduate pedodontics: the block system and the total patient care system (TPC). Descriptions and rationales are given for specific block and TPC systems at two universities. Theoretical advantages and disadvantages are reviewed. Clinical data are presented that represent experiences of the graduating classes of 1978 at The University of Iowa (block) and the University of North Carolina (TPC). From this limited sample there appears to be little quantitative evidence that one system is better than the other when examining clinical experiences in the two schools studied.

Some characteristics of children with dental behaviour problems. Five-year follow-up of pedodontic treatment.

The aim of the study was to find out how 146 children with dental fear, aged 3-13 years and referred to a Pedodontic Clinic during 1 year, accepted dental treatment after five years. The main background variables causing dental fear were social factors in 45%, personality factors in 29%, and previous negative experience in 26%. Caries incidence was high in 71% of the patients. Most patients received up to three training sessions with the "tell-show-do" method. Conventional treatment was given in 48%, 25% needed conscious sedation, and 27%, were treated under general anaesthesia. Significantly more children with low caries incidence were treated conventionally. After five years, 78% of the children showed good acceptance of conventional dental treatment while 18% received conscious sedation and 4% general anaesthesia. Significantly more children referred at the age of 3-8 years showed good cooperation than did the older children. Good acceptance was also found significantly more often when conventional treatment had been given. Caries incidence had decreased in 41% and increased in 6% of the patients. The patients who continue to show negative acceptance should receive special attention in an attempt to change their attitude to dentistry in positive direction.

[Clinico-statistical observation of the dental surgery in the pedodontics clinic of Niigata University Dental Hospital during the period from 1979 to 1989].

Clinico-statistical observation was performed on the dental surgery in the Pedodontic Clinic of Niigata University Dental Hospital during the period from August 1979 to September 1989. 1. The number of surgical operations amounted to 656 cases (males 384 and female 272) and the highest age incidence was in the 6-8 year old range. 2. The incidence of the impacted teeth (62.8%) and the abnormality of the frenum (22.8%) occupied over 85% of all the cases. 3. Based on sex, the impacted supernumerary teeth and the abnormality of the buccal frenum, odotoma were more frequent in males, while the abnormality of the upper frenum and the mucous cysts were more frequent in females. 4. About 40% of the operative patients accepted were introduced from other medical institutions. 5. Regarding the methods of surgery, the impacted permanent teeth were frequent revealed by surgical exposure, while the diseases of salivary glands were revealed by extirpation. The benign tumors were removed by extirpation, the diseases of soft tissues by excision, cysts in the jaws were all surgically removed by marsupialization. 6. In regard to the age at the time of the operation, the abnormal frenum were often seen under 4 years of age but large increases were found in the impacted teeth from 4 to 8 years of age. The diseases of salivary glands and of soft tissues were seen more frequently under 10 years of age, and cysts in the jaws were noted after 7 years of age. 7. Children undergoing surgery who suffered from general diseases amounted to 31 cases and were found most commonly in cases of nervous mental diseases.(ABSTRACT TRUNCATED AT 250 WORDS)

[An investigation into the actual condition of children with blood diseases in the pedodontic clinic of Niigata University Dental Hospital].

Children with blood diseases do not infrequently visit the dental clinics. Therefore, it is important to recognize the actual condition of blood diseases in association with the oral treatment of patients. During the period from Sept. 1979 to June 1989, 68 patients who had blood diseases visited the Pedodontic Clinic of Niigata University Dental Hospital. This report is the investigation of the dental situation of these patients. The result were as follows: 1. The ratio of patients with blood diseases in our clinic was 0.7%. The patients with leukemia were most frequent. 2. Most of the first visit-patients with blood diseases were children aged 6, while the general patients group was younger. The ages of first visit-patients corresponded to the most frequent ages of primary diseases. 3. The majority of their chief complaints were "the treatment of decayed teeth for caries". Others were "the extraction of loose teeth for exchange" or "the diseases of oral soft tissues". 4. The patients visited our clinic from all over Niigata prefecture, some of which visited from a great distance and were introduced by Niigata University Medical Hospital. 5. As to the treatment of decayed teeth for caries, the patients with hemophilia had a large number of treated teeth as compared with the other diseases. In case of leukemia, anaphylactoid purpura and idiopathic thrombocytopenic purpura (ITP), the outpatients generally tend to be subject to more severe caries than the inpatients in Niigata University Medical Hospital. 6. The percentage of recall of patients with blood disease was lower than that of general children at the rate of about 20%.(ABSTRACT TRUNCATED AT 250 WORDS)

[Investigation into the actual condition of patients with occlusal disharmony at the Pedodontic Clinic of Kyushu University. Cases of anterior cross-bite in the deciduous dentition].

The total number of subjects was 2754 children (1354 boys and 1402 girls), who visited the Pedodontic Clinic of Kyushu University during the 5 year period from April 1st 1981 to March 31st 1986. We investigated the actual conditions of patients who complained of occlusal disharmony and who were ascertained to be associated with occlusal disharmony. The results were as follows: 1. The percentage ratio of patients with chief complaints concerning occlusions was 14.5% and the percentage of patients with various type of complaints as well as the problem of occlusion was 8.1%. The ratio of the sex of the patients who complained of their occlusal disharmony was 43.0% for boys and 57.0% for girls. 2. The age distributions of the patients who had as the chief complaint occlusal disharmony were at the ages of 3 and 6 to 7 at the maximum. 3. The number of patients who were diagnosed with occlusal disharmony at the first examination was 28.5%. These included anterior cross-bite 32.5%, edge to edge bite 14.1%, cross-bite 13.0%, crowding 11.8% and et al. 4. The distribution of anterior cross-bites due to the developmental stage was divided into the deciduous dentition 59.9%, the mixed dentition 39.3% and the permanent dentition 0.8%. 5. The sex ratios of anterior cross-bites in the deciduous dentition were 45.6% for boys and 54.4% for girls. 6. The treatment of occlusal guidance for anterior cross-bite in the deciduous dentition started from ages 3 to 5. 7. The appliances used for the treatment of anterior cross-bite in the deciduous dentition were maxillary protraction 25.0%, the chin cap 20.0%, the FKO 17.5%, and the lingual arch with herical springs 15.0% and et al. 8. The status of the patients with the anterior cross-bite in the deciduous dentition as of August 1986 was that the 36.2% was under the observation, and 24.4% was under the treatment of occlusal guidance or under the observation after the treatment was done.

[The deciduous tooth. Diagnosis in pedodontics and its problems. The role of histopathology].

Accurate diagnosis of pulpal and periodontal pathology in children is an intricate question, in fact of mixed dentures and physiological and symptomatological dissimilarity between permanent and deciduous teeth. Some figures of most frequently encountered tissue modifications of deciduous teeth are proposed in order to be considered in the way of diagnosis and therapeutics in pedodontics.

[Study on the longitudinal changes of the dental caries attack patterns of young child patients in pedodontic clinic, School of Dentistry, Ohu University].

The status of dental caries prevalence in the child patients, having visited the pedodontic clinic of Ohu University Dental Hospital, was surveyed longitudinally for 2 years (between 3 and 5 years of age) or 3 years (between 2 and 5 years). Also the dental caries attack patterns used at the dental health examination in infants aged 3 years, were investigated longitudinally. The surveyed child patients were 132 boys and 143 girls who had 20 deciduous teeth in their mouth through the surveyed period. The results were as follows: 1. The dental caries prevalence rate in the child patients was high at each age. 2. B and C caries attack patterns showing extensive caries increased remarkably at 3 or 4 years of age, and O patterns decreased with advanced age. 3. The child patients having no change in the caries attack patterns for 3 years decreased about 70%, and the severe type group transmitted to C caries attack patterns increased with advanced age. 4. Most of O caries attack patterns at 2 years of age changed to other patterns between 3 and 4 years of age, and they were few at 5 years.

[Discriminant analysis concerning the background factors of dental caries prevalence in young child-patients visiting the pedodontic clinic of Ohu University Dental Hospital].

In order to get the appropriate clinical information for dental caries prevention, 38 boys and 30 girls who visited the pedodontic clinic of Ohu University Dental Hospital were surveyed for 3 years, they ranged from 2 to 5 years of age. They had all deciduous teeth and were classified into 4 groups, desirable, light, moderate and severe, on the basis of the longitudinal changes of the dental caries attack patterns, after 1 year and 3 years, which is used at the dental health examination of 3-year old children in Japan. The relationships between the groups and the investigations from documents of their past history, health and usual condition of living at their first visit to the hospital were analysed by means of discriminant analysis of the first class of Hayashi's quantifying theory. The results were as follows: 1. O caries attack patterns at 2 years of age transmitted to all caries attack patterns except B patterns at 3 years, and O caries attack patterns at 3 and 4 years never transmitted to C patterns at 5 years. 2. The desirable group having no change in the caries attack patterns was 64.7%, at 2 to 3 years, but was 25.1% and decreased markedly at 2 to 5 years. The severe group transmitting to C caries attack patterns was 17.7% at 2 to 3 years, but was 42.6% and increased remarkably at 2 to 5 years.(ABSTRACT TRUNCATED AT 250 WORDS)

[Functional analysis in orthodontics and pedodontics].

The special examination of occlusion in the gnathological sense is less important for functional analysis in orthodontics and pedodontics. At the beginning of and during therapy, simple clinical examinations and observations provide the necessary diagnostic and prognostic information on the functioning of the individual parts of the masticatory system or their relation to each other. The clinical relevance of these findings was tested by electromyographic examinations, electronic pressure and volume measurements and tele- and cineradiographic tests. In the retention period, i.e. after therapy had been concluded, a gnathological examination was made in some cases in addition to the general functional analysis. The examination mainly centered on anatomic regions: lips, tongue, masticatory muscles, and temporomandibular joint. The following functions were examined: closing of the mouth, breathing, swallowing, speech, mandibular movements and the relation of rest position to habitual occlusion. In addition, general posture and habits like suckling and nibbling were studied.

Hand-over-mouth behavior management technique in a solo pedodontic practice: a study.

The hand-over-the-mouth technique (HOM) was studied in a single-dentist pedodontic practice. The study included the frequency of its use relative to the patient's age, sex, previous dental experience and history of a significant medical experience. Observations were also made of the nature of subsequent appointments. HOM was used for almost 10 percent of the patients studied in less than 2 percent of the total number of dental appointments. It was used most often for three-year-old patients, especially female. In the majority of cases (89 percent), it was used on a single occasion and that incident was followed by appointments of a positive nature.

Special considerations in pedodontic anesthesia.

Children are not small adults. Many anatomic, physiologic, pathophysiologic, pharmacologic, and psychological differences exist that require alterations in routine adult anesthetic procedures. An appreciation of these differences and their implications for general anesthesia allow the anesthetist to prevent most morbidity and mortality during pedodontic anesthesia, reduce dental and anesthesia-related fears and phobias, and assuage parental anxiety.
