[The clinical picture and correction of paroxysmal neuropsychic disorders in children suffering from neurodermatitis].

Clinical and electroencephalographic examinations of 182 children with neurodermatitis have revealed paroxysmal neuropsychic disturbances, differing from the psychopathologic disorders, in 16. These disturbances, that may be described as the night fears syndrome, may be cured by long treatment including anticonvulsive therapy. This permits a conclusion that neurodermatitis in children is associated with deep organic involvement of the brain.

[Atopic neurodermatitis].

Neurodermatitis atopica is a skin disease caused by various different factors and which is characterized by the simultaneous presence of different clinical features. The treatment is directed towards the various aetiopathological factors and oriented on the clinical picture. The main symptom is dry skin, which has to be rehydrated and provided with an artificial film of grease. Dermosteroids are the most efficient antiinflammatory agents, but they can only be used as one of various components of a complete neurodermatitis therapy. New knowledge gained over the past few years has shown the important role of Staphylococcus aureus in this condition, and as a result antibiotic therapy has increased in importance. The use of soap, which was previously not recommended, is now considered to be a useful and beneficial addition to the treatment, thanks to the availability of new alkali-free, disinfectant soaps. In certain cases there is a causal connection with exposure to allergens. Besides treatment of the itching, which is crucial for the patient, stabilization of the psychological factor and, if necessary, changes in the patient's environmental situation are extremely important. Substitution of essential fatty acids and UV therapy are discussed.

[Long-term study "Neurodermatitis therapy using urea-containing Externa". Experience report of a pediatrician].

The experience gained in the pediatric sector within the framework of a neurodermatitis study on the use of urea and urea/hydrocortisone combinations will be reported. Particularly in young children, these preparations yield very good results, even after short-term use. The compliance of the patients is increased because, using alternating therapy, these preparations quickly make it possible for the patients to responsibly help determine their own therapy. The active participation of the parents in therapy makes it easier for the therapist to improve access to the psychosomatic environment of the disease. The consistent use of urea/hydrocortisone and urea preparations is a valuable enhancement of an all-encompassing neurodermatic therapy.

[Moisture retaining effect and tolerance of urea-containing Externa in neurodermatitis patients].

Urea ointments and emulsions are excellently suited for the care and supportive therapy of neurodermatitis patients. In treating children and in the case of acutely irritated skin conditions, we have arrived at a concentration of 5% urea. All other dermatological conditions can be easily and effectively treated with a 10% urea preparation. It goes without saying that galenics also play an important role. An unbalanced composition can lead to poor results. In a short-term test (180 minutes) and in a long-term test (7 days), we tested the moisture level of the corneal layer with the corneometer in a climate-controlled room. We were able to show that urea in hydrous topical agents ensures acceleration of corneal hydration and helps retain water in the corneal layer. Thus, urea preparations can also be especially recommended for use in patients with neurodermatitis.

[Epidemiology of constitutional atopic neurodermatitis].

There are more and more reports of an increase in atopic diseases, and particularly atopic constitutional neurodermatitis. In this context, evidence is accumulating that atopic constitutional neurodermatitis exhibits a dominant prevalence, particularly in children. In addition to verified genetic disposition with impairment of IgE regulation, realization factors of greatly variant quality and quantity appear to be characteristically responsible for the phenotypic expression and determination of the course of the disease. In order to definitively assess the global situation, subtle longitudinal studies on representative numbers of the population are needed, with the primary objective of obtaining parameters for real preventative measures.

Neurodermatitis and intralesional steroids.

The present work describes the results of treatment of 30 cases of neurodermatitis circumscripta by intralesional injections which were given by Dermo-Jet. 15 cases were treated with triamcinolone acetonide (0.25 mg/cm2 of the skin) and the rest with aqua distillata every week for 8 weeks and were followed for another 4 weeks. The results of treatment were excellent in 66% of triamcinolone treated cases and 46% of aqua distillata treated cases. The difference in the response was statistically insignificant. So we conclude that aqua distillata might have worked as suggestion therapy in emotionally labile individuals.

[Clinical use of catomas and biotrit in a combined treatment of patients with chronic neurodermatitis].

In 170 patients with neurodermatitis, therapeutic benefit was studied of two new antioxidant drug preparations catomas (containing beta-carotene and tocopheroli acetas) and biotrit (extractum from the sprouting wheat). Catomas was used in applications to the area of affection with the aid of phonophoresis, biotrit in injections. It has been shown by the studies made that employment of the above drugs combined with sedatives, desensitizing and anti-inflammatory means of remediation enhances efficiency of the treatments administered, which fact was evidenced by the reduction of the mean duration of the treatment undergone by 4.4 days and by 30% increase in the treatment efficiency. Greater benefit from catomas and biotrit will be seen in those patients having been placed on a general treatment.

[Atopic neurodermatitis, basic concepts in pediatrics].

Atopic neurodermatitis (AND) is the first or second most frequent skin diseases in childhood. It represented 15% of all skin diseases seen at the Hospital del Nino, IMAN. The clinical and epidemiologic aspects of AND, the different hypothesis of its pathogenesis, management and treatment are discussed. The importance of emotional factors in AND have been over stressed, resulting in therapeutic abuses of psychyatric drugs and psychotherapy.

Effectiveness of ketotifen in the treatment of neurodermatitis in childhood.

17 children suffering from neurodermatitis, aged 2 to 14 years, were treated with ketotifen at a dosage of 1 mg twice daily. In 7 children polyvalent therapy was given. Ketotifen therapy produced a pronounced alleviation of the itching within only two weeks and the patients became entirely itching free after 20 days on an average. The improvement of skin lesions appeared after two months of treatment. Patients became entirely skin lesions free between seven and nine months. In three patients the deterioration of skin lesions was triggered with infection. With regard to long-lasting of Ketotifen therapy, the side effects were not observed.

[The treatment of neurodermatitis and prurigo patients with the mineral springs of Andizhan Province].

The efficacy of balneotherapy with local mineral water was estimated in 328 patients with neurodermatitis and 42 ones with prurigo. Besides balneotherapy, one group of patients was administered polychemotherapy including alpha-tocopherol, 30% sodium thiosulfate, ascorbic and nicotinic acids. Balneotherapy resulted in clinical remission in 25.7% of patients; a considerable improvement was achieved in 42% and less marked improvement in 28.1% of patients. The treatment efficacy depended on the disease duration and process dissemination. The mean duration of a remission was 12.2 months. Late results were much better after repeated courses of balneotherapy.

[The blood rheological properties in children with neurodermatitis undergoing naphthalan therapy at Naftalan health resort].

Shifts in the blood rheologic characteristics were examined in 50 children suffering from neurodermatitis during remission over the course of health resort treatment. The treatment included bromine electrophoresis of the collar area and therapeutic exercises. A clinical improvement was observed by the end of naphthalane therapy course. The examined parameters improved at the end of treatment in all the children.

[The rehabilitation of children with neurodermatitis by using local natural conditions].

Local natural factors, used at a seasonal hospital, are a highly effective means of rehabilitation therapy of children suffering from neurodermatitis, as evidenced by clinical observations and the time course of the immunity and nonspecific defense parameters. A relationship between the efficacy of heliothalassotherapy and the HLA classification of the patients has been revealed.

[Radioimmunologic study of the hormonal and immune status in eczema, psoriasis and neurodermatitis].

The hormonal and immune status was investigated by a radioimmunoassay in 105 patients with dermatosis (55 female and 50 male patients aged 15 to 80): 51 suffered from eczema, 41--from psoriasis, and 13--from neurodermatitis. The results were compared with those of 32 controls. Serum concentrations of T3, T4, TSH, insulin, trypsin, C-peptide, cortisol, and IgE were investigated. Disorders of the hormonal and immune status were noted in the examinees with relation to sex, type of disease, season, time-period and extent of disease.

[Comparative effectiveness of current pathogenetic methods of treating neurodermatitis].

The authors compare various present-day pathogenetic methods used in the treatment of 258 patients with disseminated neurodermatitis (81 cases with the atopic form and 177 ones with the nonatopic form). Clinical and laboratory studies have demonstrated the efficacy of adrenoblockers, pyrroxan and butyroxan, included in multiple-modality therapy for patients with manifest disorders of the autonomic nervous system. Combinations of glycyrrham and voltaren and decimetric wave exposure of the adrenal area have been also effective, as well as parmidine, an antibradykinin drug, in a daily dose of 2.0 g. In the most severe and stubborn cases corticosteroids may be administered; the schemes of therapy should be sparing and physiological, with the drugs given every other day, this helping reduce the total dose of the drug.

[The leukocyte migration inhibition reaction in children with neurodermatitis].

This test (LMIT) with epidermal antigens and ConA, made in capillaries, has been carried out in examinations of 70 children aged 3-10, suffering from neurodermatitis. It has revealed a specific dose-dependent sensitization to epidermal antigens and reduced parameters of ConA-effected LMIT in such patients and a relationship between the ConA-effected leukocyte migration activity and the dermatosis stage.
