Oral health comparison between the institutionalized and non institutionalized elderly.

AIM: The aim of the present epidemiological study is to gather information on the state of oral health in both hospitalized and non-hospitalized patients in Milan, in order to evaluate the prevalence of periodontal disease and to carry out a comparison between the 2 groups. METHODS: The study involved 364 patients (111 males, 253 females) aged between 65 and 104, subdivided into 2 groups: 1) the 1(st) group of 258 hospitalized patients; 2) the 2(nd) group consisted of 106 non-hospitalized patients. In order to carry out the examinations 5 orthodontic mirrors, traditional orthodontic O.M.S. probes, and a source of white light were utilized. The epidemiological parameters utilized, according to O.M.S. criteria, were D.M.F.T. and C.P.I.T.N. RESULTS: The results obtained by the dental examinations were registered by means of the ''Scheda di Valutazione O.M.S'' (O.M.S. Evaluation Report). Hospitalized patients D.M.F.T. 24,99, C.P.I.T.N. 1,40; non-hospitalized patients D.M.F.T. 19,82, C.P.I.T.N. 1,78. 42.3% of patients proved to be edentulous (49.6% hospitalized patients; 28.3% non-hospitalized patients). Of these, 45% wore dentures. CONCLUSION: The value of D.M.F.T. in elderly hospitalized patients was higher than in non-hospitalized patients; decay is thus prevalent in hospitalized patients. If we observe the values of C.P.I.T.N., a worrisome datum emerges: neither of the 2 groups are aware of correct oral hygiene techniques. Therefore, the clinical results derived from our study highlight the need to create a specific oral health program, directed at the elderly.

[Alternatives to institutionalization in accordance with the Institutionalization Law: problems, recommendations for a solution and medical anthropologic interpretations].

Police authorities, Public Health offices and Law Courts cooperate in solving the problem of psychiatric patients who have to be referred to a psychiatric hospital according to state mental health acts. Such referral, however, is only permissible as a last resort, i.e. if all other alternatives fail. The present study first presents this legal situation and points out that the alternatives are not always fully explored with the necessary energy. The importance of these alternatives is explained from a psychiatrist's point of view. A practical approach is suggested with the main burden resting with the relevant police authorities who (in West Germany) are responsible for arranging proper sociopsychiatric care of such persons. The problem is then discussed in accordance with a medico-anthropological system model demonstrating the typical characteristics of social, medical and legal attitudes and those of the pertaining "subcultures" to such problems.

[Cerebral hypoperfusion symptoms in elderly subjects with hyperkinetic and/or hypokinetic arrhythmia. Study of institutionalized and non- institutionalized patients].

BACKGROUND: We evaluated the symptomatology caused by cerebral hypoperfusion in a group of over-65 year old hospitalized and non-patients, with hyperkinetic and hypokinetic arrhythmias. METHODS: 2441 clinical records of hospitalized and ambulatory patients at the unit of Cardiology, "Pugliese-Ciaccio" Hospital of Catanzaro between January 1st 1991 and March 31st 1995 were examined. The clinical records of those patients who had showed anamnestic episodes of syncope, lipothymia and dizziness were selected. The selected sample was made of 36 hospitalized patients and 36 ambulatory patients. The Holter-ECGs of these patients were examined. RESULTS: Six episodes of syncope (16.7%) were found in the hospitalized patients and 4 in the ambulatory patients (11.1%). Ten (27.8%) and 8 (22.2%) episodes of lipothymia, were found in the hospitalized and ambulatory patients respectively. Dizziness was found in 20 (55.5%) hospitalized and in 24 (66.6%) ambulatory patients. In all the patients the symptoms appeared during the recording and were linked to hyperkinetic arrhythmias in 22 (61.1%) hospitalized patients and in 25 (69.4%) ambulatory patients and to hypokinetic arrhythmias in 14 (39.9%) and 11 (30.5%) hospitalized and ambulatory patients. CONCLUSIONS: The present study pointed out that patients with hyperkinetic arrhythmias (both hospitalized and ambulatory) show symptoms of cerebral hypoperfusion more frequently than those with hyperkinetic arrhythmias.

Low circulating levels of insulin-like growth factors and testosterone in chronically institutionalized elderly men.

OBJECTIVE: To determine the prevalences of and the associations between hyposomatomedinemia and hypogonadism in healthy young men, healthy old men, and chronically institutionalized old men. DESIGN: Survey with serial blood tests. SETTING: Veterans Affairs nursing home and young and old men living in the community. SUBJECTS: Three groups were studied: healthy young men (20-29 years old, n = 32), healthy old men (59-98 years old, n = 30), and chronically institutionalized old men (59-95 years old, n = 112). MEASUREMENTS: Plasma insulin-like growth factor-I (IGF-I), total testosterone (TT), free testosterone (FT), and plasma insulin-like growth factor-II (IGF-II) were measured. In subjects with low testosterone level, serum luteinizing hormone (LH) was also determined. In a subset of chronically institutionalized old men with low IGF-I, the serum growth hormone (GH) level was analyzed during the first 4 hours of sleep. RESULTS: A low IGF-I level (defined as a value below the lower 2.5 percentile of the comparison group) occurred in 85% of the healthy old men when compared with healthy young men (P < 0.001), in 90% of the chronically institutionalized old men when compared with healthy young men (P < 0.001), and in 26% of the chronically institutionalized old men when compared with healthy old men (P < 0.001). In chronically institutionalized old men with low IGF-I compared with healthy young men, nocturnal peaks of serum GH were < 2 ng/mL in most cases. Low TT (defined as a value below the lower 2.5 percentile of the comparison group) occurred in 86% of the healthy old men when compared with healthy young men (P < 0.001), in 88% of the chronically institutionalized old men when compared with healthy young men (P < 0.001), and in 28% of the chronically institutionalized old men when compared with healthy old men (P < 0.001). The results of FT were similar. In 80% of the institutionalized old men with low TT and FT, the serum LH level was low (< 20 mU/mL). In 53% of the institutionalized old men, the IGF-II level was below the lower 2.5 percentile of the healthy old men (P < 0.001). In both healthy and institutionalized old men, IGF-I and IGF-II levels were significantly correlated to each other (r = 0.6), but neither was significantly correlated to TT or FT. In the institutionalized old men, IGF-I was inversely correlated with age and with a diagnosis of dementia; TT and FT were inversely correlated with age and with the degree of dependency in ADL's. CONCLUSIONS: Compared with healthy young men, most healthy old men have low serum IGF-I, TT, and FT levels. The geriatric hyposomatomedinemia and hypogonadism are more severe in institutionalized old men. In the latter group, both endocrine deficiencies are usually of central origin, but their occurrences are not significantly associated. Healthy old men usually have a low level of IGF-I compared with healthy young men, but a similar level of IGF-II; institutionalized old men are usually low in both values.

Prevalence of hepatitis A in an institution for the mentally retarded in an intermediate endemicity area: influence of age length of institutionalization.

OBJECTIVE: This study aimed to examine the prevalence of antibodies against hepatitis A (anti-HAV) in a population of institutionalized mentally retarded persons compared with that of institutionalized non-mentally retarded persons in an area with moderate endemicity of HAV infection. METHODS: The study population was a group of mentally retarded persons aged between 10 and 30 years, institutionalized in one residence in Madrid, Spain. A group of non-mentally retarded subjects in the same age range was chosen as controls. Information about demography and duration of institutionalization was obtained by the investigator. Four drops of whole blood were obtained from each person by finger-stick, collected on filter paper, air-dried and stored at +4 degrees C until antibody determination. Eluates were tested by an ELISA method. RESULTS: A total of 314 institutionalized subjects were included: 157 mentally retarded (mean age and mean duration of institutionalization were 24.4+/-3 and 9.7+/-5 years respectively) and 157 non-mentally retarded (mean age and mean duration of institutionalization were 19.2+/-5 and 4.6+/-3 years respectively). The prevalence of anti-HAV antibody was 54% (95% confidence interval (CI) 50-58) in mentally retarded and 22% (95% CI 19-25) in non-mentally retarded [P<0.001; odds ratio (OR): 4.2 (95% CI 2.5-7)]. In both groups, these differences were not statistically significant for anti-HAV antibody prevalence between persons institutionalized for >5 years compared with those institutionalized for < or = 5 years. CONCLUSIONS: There results indicate that in an area of moderate endemicity the institutionalized mentally retarded are at increased risk of having acquired hepatitis A infection compared to the non-mentally retarded. In these institutionalized persons, regardless whether they are mentally retarded or not, prevalence is not influenced by age or length of stay in institutions.

Institutionalization in Taiwan. The role of caregiver gender.

The role of caregiver gender in the likelihood of institutionalization of Taiwanese older adults was explored in this study. A sample of 78 male and 69 female primary caregivers of elderly patients who had experienced a stroke at least 6 months prior to the study were interviewed. Logistic regression analyses were applied to examine direct and interaction effects of the elderly adult's functioning the caregiver's available resources, the degree of caregiver burden, perceived public opinion toward institutionalization, and precipitating events on the likelihood of institutionalization among Taiwanese male and female caregivers. Women were more likely to institutionalize the older adult for whom they cared. The proposed model correctly predicted the likelihood of institutionalization of an elderly adult based on male versus female caregivers at the 92% level. Perceived public opinion toward institutionalization was the most significant predictor of institutionalization for both genders. Perceived public opinion toward institutionalization has a strong influence on whether or not caregivers institutionalize an elderly relative. This is consistent with Chinese culture in which public opinion has a much stronger effect on individual behavior than in the United States. American concepts of "minding one's own business" do not exist in Taiwan. It is logical that the older adults' level of functioning would predict the likelihood of institutionalization regardless of caregiver gender. In terms of caregiver characteristics, working hours in male caregivers is more predictive, and the quality of the relationship with the older adult was more predictive of institutionalization for female caregivers.

Depressive symptoms and cognitive performance of the elderly: relationship between institutionalization and activity programs.

OBJECTIVE: The aim of this study was to assess the frequency of depressive symptoms and to evaluate cognitive performance of institutionalized versus non-institutionalized elderly subjects and to compare the effect of institutionalization and participation in the institution's activity programs on their cognitive performance. METHOD: A group of 120 elderly subjects with a mean age of 71 years and average schooling of 4.2 years was evaluated. The participants were divided into 3 groups: non-institutionalized (n = 37); institutionalized with activities (n = 37); institutionalized without activities (n = 46). The groups were matched for age, gender and educational level. The following assessment instruments were used: the Geriatric Depression Scale, the Mini-Mental State Examination, the Verbal Fluency Test and the computerized versions of the Hooper Visual Organization Test and the Boston Naming Test. The data were analyzed using one-way ANOVA and the Pearson's correlation test. RESULTS: The two groups of institutionalized elderly showed higher frequency of depressive symptoms when compared to non-institutionalized subjects and worse performance on the Verbal Fluency Test. The institutionalized group without activities had lower scores on Mental State Examination, Boston Naming Test and Hooper Visual Organization Test when compared to the other two groups (p < 0.05). CONCLUSIONS: Institutionalization of the elderly seems to be related to worse cognitive performance. Activity programs during institutionalization may be effective in minimizing cognitive functional loss.

The institutionalization of operations research.

When the conceptualization of OR is inappropriate, institutionalization will not take place. For this reason, the discussion here has focused as much on the pitfalls in OR design as on factors that facilitate institutionalization. Research that is inappropriately adapted to the societal or programmatic context will not be institutionalized, nor will OR lead to institutionalized research capabilities if theory is ignored or the research lacks credibility. The institutionalization of credible and scientifically sound OR requires well-trained social scientists who can carry out this task. Career opportunities must persist in OR so that well-trained scientists will find in an attractive and rewarding pursuit. Long-term projects facilitate institutionalization by providing a setting where work teams can be developed and career tracks defined. Academics and universities institutionalize OR by sponsoring training in applied research and developing high-level research leadership. For this reason, institutional linkages and mechanisms that nurture collaboration facilitate the institutionalization of OR. Finally, institutionalization of OR implies that research is being utilized for action by program management and the policy community. There is extensive literature on the determinants of research utilization, much of which is relevant to the question of how OR can be institutionalized.

Calcidiol, calcitriol and parathyroid hormone serum concentrations in institutionalized and ambulatory elderly in Switzerland.

AIM: The aim of the present study was to measure concentrations of vitamin D metabolites and intact parathyroid hormone (iPTH) serum concentrations and an urinary bone resorption marker in two groups of elderly subjects, who differed markedly in their sedentary status and seasonality. DESIGN: 193 institutionalized elderly people of a long-stay geriatric ward (106 women: mean age 82; 87 men: mean age 78) were studied during wintertime. 312 ambulatory elderly people (109 women: mean age 74; 203 men: mean age 76) were studied during summertime. Concentrations of calcidiol (25(OH)D), calcitriol (1,25(OH)2D) and serum iPTH, as well as urinary N-telopeptides (NTX) were measured. RESULTS: Vitamin D deficiency (defined as serum 25(OH)D < 12 ng/ml) was present in 86% of institutionalized at the expected nadir (wintertime), compared to 15% of the ambulatory elderly subjects at the expected maximum (summertime). Serum calcitriol concentrations were significantly lower in institutionalized subjects (p = .0001). However intact PTH concentrations did not differ significantly between institutionalized and ambulatory elderly. Institutionalized and female subjects showed higher urinary NTX excretion (female institutionalized: 131.9; female ambulatory: 66.8/male institutionalized: 76.3 male ambulatory: 45.8 nmol/mmol). CONCLUSION: This cross-sectional study documented very low serum calcidiol and calcitriol concentrations and high urinary N-telopeptide excretion in institutionalized elderly people. There was no difference in serum iPTH concentrations between institutionalized and ambulatory elderly. This finding could not be explained by the differences in calcidiol and calcitriol concentration, nor urinary NTX excretion. These results suggest that other factors than vitamin D deficiency, such as lower mobility status and sedentary life style, might have an important role in the regulation of iPTH and mechanisms of bone loss in the elderly.

Ethnicity and time to institutionalization of dementia patients: a comparison of Latina and Caucasian female family caregivers.

OBJECTIVES: To compare rates of institutionalization of dementia patients cared for by Latina and Caucasian female caregivers and to explore which caregiver and care-recipient characteristics predicted institutionalization. DESIGN: Longitudinal. SETTING: San Francisco Bay area, California. PARTICIPANTS: Two hundred sixty-four female caregivers of dementia patients (154 Caucasian women, 110 Latinas) who participated in an intervention project designed to reduce caregiver stress and may represent a sample that is more stressed and motivated than a general sample of caregivers. MEASUREMENTS: Number of days between baseline interview and institutionalization was recorded over an 18-month period. Kaplan-Meier survival analysis with the log rank statistic was used to test for ethnic differences in time to institutionalization. Cox regression analyses were conducted to determine whether care-recipient or caregiver characteristics (e.g., care-recipient age and memory and behavior problems; caregiver depression, years of education, income, and views of the caregiving role) significantly interacted with ethnicity to explain time to institutionalization. RESULTS: Latinas delayed institutionalization significantly longer than their Caucasian counterparts; care-recipient characteristics or caregiver demographics did not explain these results, although Latinas who identified greater benefits or more-positive aspects of the caregiving process at baseline were less likely to institutionalize their loved one than those who reported fewer benefits of caregiving. Less-acculturated Latinas were significantly more likely to identify positive aspects of caregiving than more-acculturated Latinas. CONCLUSION: Latina dementia caregivers delay institutionalization significantly longer than female Caucasian caregivers. In addition, Latino cultural values and positive views of the caregiving role are important factors that may significantly influence their decision to institutionalize loved ones with dementia.
