[Endodontic-periodontic problems].

Abstract

The relationship between periodontium and endodontium. This article discusses the influence of periodontal pathology and therapy on the dental pulp and endodontium. It also gives a differential diagnosis description of perio-endo lesions and the implications of the perio-endo relationship for treatment.

Teeth: malignant neoplasms in the dental pulp?

Common pathologies of the dental pulp differentiate between acute and chronic inflammatory states caused by caries or dental trauma. Inflammations of the dental pulp as a result of neoplastic alterations are generally considered non-existent. In fact, using the search phrase "dental pulp" combined with "sarcoma", "carcinoma", or "neoplasms" in PubMed when using the MeSH search mode yielded no reports on primary malignant neoplasms. However, a hand search yields clinical reports on pulpal tumours that were published over a century ago. In this Essay, the results of a hand search in historic published work are presented. Furthermore, deductive reflections are done on general tumour pathogenesis with respect to specific anatomical prerequisites of the dental pulp. Because of the restricted space in a tooth, tumour expansion will probably lead to the formation of irritation dentine by secondary odontoblasts and, subsequently, to a haemorrhage infarct of the pulp. One hypothesis states that a purported neoplasm of the dental pulp leads to a chronic appositive pulpitis and-sooner or later-will be treated likewise by root-canal treatment or extraction. Further research, including stem-cell studies, is recommended.

State of the art and science of endodontics.

BACKGROUND: Advances in the art and science of endodontics have facilitated better understanding of disease processes and have led to development of treatment modalities aimed to restore health to the pulp and periradicular tissues. This article presents a summary of both basic and clinical breakthroughs in endodontics. RESULTS: The author indicates that, on the basis of the reviewed literature, pulp and periradicular diseases are primarily microbial. Basic research has led to development of methodologies that have allowed for specific and accurate identification of pathogens that are likely to cause persistent infections. Close examination of clinical data shows that meticulous débridement of the infected root canal system will result in a high probability of successful treatment outcome. Progress in visualization, mechanical and chemical disinfection, and biological seal of portals of entry will improve quality of care further. CONCLUSIONS AND CLINICAL IMPLICATIONS: A better understanding of the pathogenesis of pulp and periradicular diseases, inflammation and healing, as well as of pain pathways, will improve patient care and result in preservation of natural tissues.
