Multiple eccrine hidrocystomas.

Abstract

The clinical and pathological features and treatment of two patients with multiple eccrine hidrocystomas are presented. The first case is characterized by multiple pearly papules with a bluish hue located in the periorbital region and the bridge of the nose. The second case is characterized by multiple, skin-coloured papules located in the periorbital area, forehead, chin and nose. Both were exacerbated by a hot and humid environment. Histopathologically, both demonstrated a unilocular cyst located in the dermis, with a 2-3-layer wall composed of cuboidal epithelium that was non-keratinizing. Treatment with topical atropine sulphate 1% in aqueous solution three times a day was instituted in the first case; however, this was poorly tolerated because of blurred vision and nausea. The lesions were subsequently hyfrecated with a good response. The second case was treated with topical atropine sulphate 1% in aqueous solution three times a day with a good response.

[Hydrofluoric acid burns. A rare chemical emergency situation].

Burns caused by hydrofluoric acid can be life-threatening. Of special significance is the often underestimated local and sometimes delayed deep action of the highly diffusible free fluoride ions and the accompanying systemic toxicity. The specific antidote calcium gluconate can be topically applied, injected into tissue or infused intra-arterially. Because of the extreme danger of systemic toxicity even after seemingly trivial injuries, monitoring in the intensive care station, especially by measuring the calcium concentration in blood and electrocardiography, and therapy is recommended.

[Traumatic tympanic membrane perforactions. Local application of an alginate matrix loaded with epidermal growth factor in an animal model].

BACKGROUND: The aim of this study was to evaluate the effects of an alginate matrix releasing epidermal growth factor on healing after acute tympanic membrane perforation in rats. METHOD: A total of 20 male rats were divided into two groups. In each animal, a randomly chosen tympanic membrane was perforated by heat. A piece of alginate matrix (control group) or alginate matrix loaded with 0.25 microg epidermal growth factor (EGF group) was then placed on the perforation. The rat ears were examined after days 3, 6, 9, and 14 and every week thereafter for a total of 11 weeks. Each matrix was removed on day 9. To examine the status of the tympanic membranes on day 14, one randomly chosen membrane from each group was histopathologically examined. RESULTS: By day 6, complete closure of the tympanic membrane perforation was achieved in 56% of the EGF group, whereas it was achieved in only 10% in the control group. By day 14, all tympanic membrane perforations were closed in both groups. There were no complications and no significant differences in the histopathologic parameters between the EGF group and the control group. CONCLUSION: An alginate matrix seems to be a useful EGF-delivery system to the tympanic membrane.

[Topical application of local anesthetics and opioids after elective tooth extraction].

The aim of this study was to provide evidence for the peripheral effect of opioids in oromaxillary procedures and to investigate whether postoperative morphine sulfate plus lidocaine administered as a local spray achieve better analgesic efficacy than lidocaine applied alone. The double-blind randomized study included 60 patients. The patients exhibited neither preoperative pain nor inflammation (tooth extractions) and when pain occurred received as alternative medication diclofenac retard 50 mg or tramadol. Side effects were documented. No differences between the two groups were observed in the demographic data, visual and numeric analog scale in the first 24 h, or in the consumption of analgesics. Since the patients presented in a pain-free state at the time of surgery, apparently no activation of the peripheral opioid receptors occurs. Thus, a peripheral effect of opioids could not be demonstrated in this model.
