[Permanent stomata].

Abstract

The implantation of preternatural orifices is a procedure frequently performed by the visceral surgeon in spite of the advances of modern surgery concerning preservation of continence. In order to avoid unnecessary complications, certain strict principles of operating technique must be followed. The cumulative rate of complications is found to be at least 50%. Especially prolapse, peristomal hernias and stenoses are of profound surgical relevance. A well-implanted and adequately cared for stoma will have a positive influence on a patient's quality of life.

The skin bridge loop colostomy.

Loop colostomy formation commonly involves temporary support over a plastic bridge. We describe a technique of loop colostomy using a permanent skin bridge. By joining two inter-digitating skin flaps raised from the margins of the stoma, an adjustable bridge is formed that can accommodate colon loops of any diameter. In a series of twenty cases, our technique has been complication free and well tolerated by patients.

Intraperitoneal ePTFE mesh repair of parastomal hernias.

Parastomal hernia represents a common complication of colostomy formation. Surgical techniques such as facial repair and stoma relocation have almost been abandoned because of high recurrence rates. Extraperitoneal prosthetic mesh repair had better results but was accompanied by high rates of mesh contamination. A new technique, with intraperitoneal onlay position of expanded polytetrafluoroethylene (ePTFE) was therefore established. We report herein two cases of symptomatic large parastomal hernias treated in our department.
