Effect of fetal spinal cord graft with different methods on axonal pathology after spinal cord contusion.

OBJECTIVE: To investigate the effect of fetal spinal cord (FSC) graft with different methods on axonal pathology and neurological function recovery after spinal cord injury (SCI). METHODS: Forty Wistar rats were divided into 4 groups. In Group A, the spinal cord was injured and hemisected. In Group B, fetal spinal cord (FSC) was transferred into the injured site. In Group C, after having done as Group B, the upper and lower spinal nerve roots were anastomosed. And in Group D, after having done as Group B, the pedicled omentum was transferred into the hemisection cavity. At 6 weeks after operation, light and electronic microscopes were used to examine the axonal pathology. The neurological function was assessed with inclined plane tests in the open field. The number of axons was quantitated by a computer image analysis system. RESULTS: A greater loss of axons was observed in Group A than that of other groups at 6 weeks. The sequence of the reduced rate of the axons was as following, Group A>Group B>Group C>Group D (P<0.05). The remaining axons were paralleled with the significant improvement in neurological function recovery of the rats. CONCLUSIONS: It indicates that FSC and pedicled omentum grafts after SCI can protect the axons and promote the neurological function recovery of the rats.

[A study on the risk factors of cerebral infarction as complication of type 2 diabetes mellitu patients].

OBJECTIVE: Cerebral infarction, a common acute cereovascular disease, is one of the main complications of type 2 diabetes mellitus (DM). This paper is to explore the risk factors of cerebral infarction among type 2 DM patients. METHODS: A pair-matched case-control study was carried out. Data was analyzed with conditional logistic regression. RESULTS: One hundred and ten type 2 DM patients with cerebral infarction and controls were selected respectively. Results showed that higher educational level and physical exercises lowered the risk of cerebral infarction among type 2 DM patients. Higher HDL-C and blood sugar under control could also lower the risk of developing cerebral infarction among type 2 DM patients. The above factors seemed to have protective functions. Excessive salt intake and snoring increased the risks of cerebral infarction development among type 2 DM patients. Hypertension served as an independent risk factor while atrial fibrillation and hyperinsulinemia were important risk factors. High LDL-C, Apo-A, Lp(a), Fg increased the risk of cerebral infarction among type 2 DM patients. The study did not discover the relationship of smoking, drinking, oral contraception, menstrual and obstetrical history, TC, TG, Apo-A and cerebral infarction among type 2 DM patients. CONCLUSION: To prevent cerebral infarction among type 2 DM, measures need to be taken as control of salt intake; eating more vegetables; regular physical exercises; control of blood sugar, blood pressure and Hyperinsulinemia.

[A clinicopathologic study on 9 cases of epithelioid hemangioendothelioma].

OBJECTIVE: To study the clinicopathological features of epithelioid (histiocytoid) hemangioendothelioma (EHE). METHODS: 9 cases of EHE were examined by light microscopy and immunohistochemistry, 1 case with ultrastructural observation. RESULTS: The patients' age ranged from 16 to 47 years (mean 32 years). The lesions occurred in both sexes and presented at a variety of sites including head and facial area (4 cases), upper limbs (3 cases) and lower limbs (2 cases). The morphological characters were: (1) "epithelioid" or "histiocytoid" tumor cells; (2) rounded or polygonal tumor cells arranged in nests, cords and irregular patterns; (3) the tumor cells were embedded in myxoid or hyaline matrix; (4) primitive lumen formation within a single tumor cell (intracytoplasmic lumens); (5) lack of pleomorphism, mitotic activity and necrosis; (6) presence of spindle cell hemangioendothelioma in some cases; (7) osteoclast like giant cells scattered throughout the entire lesion were observed in one case. Immunohistochemistry studies on 7 cases found that tumor cells were positive for vimentin (7/7), factor VIII-RAg (5/7), CD31 (5/7), CD34 (5/7), cytokeratin (2/7), CD68 and alpha-AT (1/7). Ultrastructural study of 1 case demonstrated Weibel-Palade bodies and an abundance of intermediate filaments in the cytoplasm. The follow-up of 6 cases found that 3 cases developed local recurrences but none of them metastasized. CONCLUSIONS: EHE is a distinctive low-grade sarcoma whose etiology remains unknown. Recognizing its morphological characters is important to avoid confusion with other similar lesions.

[Analysis of loss of heterozygosity on chromosome 6 in human prostate carcinoma and prostatic intraepithelial neoplasia].

OBJECTIVE: To detect the significance of loss of heterozygosity (LOH) on chromosome 6 in prostate carcinoma and high grade prostatic intraepithelial neoplasia (PIN). METHODS: Pure DNA was obtained from prostate neoplasms and normal tissues after tissue microdissection. LOH of chromosome 6 was detected by PCR based microsatellite polymorphism analysis technique using 20 pairs of microsatellite primers in 10 prostate carcinoma cases and 10 high grade PIN cases. RESULTS: Allelic loss at one or more loci was observed on chromosome 6 in 8 of 10 prostate carcinoma cases. 6q21-6q23 and 6q25-6q27 were two of LOH high frequency regions. 5 high grade PIN cases had LOH detected on chromosome 6. CONCLUSIONS: Two high frequency LOH regions were detected on chromosome 6 of prostate carcinoma. Cyclin C, IGF2R genes were two candidate tumor suppressor genes located in these two regions, they may be involved in the initiation and progression of prostate cancer.

[An experimental study on the construction of composite graft with epithelia and fibroblasts on collagen sponge].

OBJECTIVE: Primarily cultured human epithelia and fibroblasts were inoculated on collagen sponge to construct composite skin substitute. METHODS: Epithelia and fibroblast asts from human foreskin were primarily cultured. Fibroblasts (1 x 10(5)/cm(2)) were inoculated on collagen sponge after digestion and cultured for 3 days prior to inoculation of epithelial cells (2 x 10(5)/cm(2)) to construct composite skin grafting substitute, and it was then grafted onto full skin loss (FSL) wounds of nude mice. Collagen sponge membrane lacking cell inoculation was used as control. The wound healing was evaluated by measuring the residual wound area. Tissue samples were harvested and examined by means of histology, immunohistochemistry and electron microscopy. RESULTS: Epithelia and fibroblasts could be cultured in vitro on the carrier collagen sponge. After being grafted onto the wound, the epithelia and fibroblasts wound further proliferated and differentiated. The composite skin could stick to the carrier fairly well. There exhibited less epidermal scar proliferation and satisfactory contour of the neoskin. Integral basal membrane of the skin could be identified by immunohistochemical staining. The quality and speed of wound healing with the composite skin substitute were superior to that with collagen sponge only. CONCLUSION: The composite grafts constructed by epithelia and fibroblasts on collagen sponge could be a potential neotype composite skin substitute for the repair of FSL.

[Diagnostic significance of measurement of the receptor for urokinase-type plasminogen activator on granulocytes and in plasma from patients with paroxysmal nocturnal hemoglobinuria].

OBJECTIVE: To investigate the expression levels of the receptor for urokinase-type plasminogen activator (uPAR) on granulocytes and the soluble uPAR (suPAR) level in plasma from patients with paroxysmal nocturnal hemoglobinuria (PNH) and its clinical application in the diagnosis of this disorder. METHODS: Expression of uPAR, CD55 and CD59 was measured on granulocytes by using flow cytometry (FCM) and the level of suPAR in plasma by using immunoradiometric assay (IRMA) in 20 patients with PNH, 59 anemic patients with diagnoses other than PNH (18 with autoimmune hemolytic anemia, 6 with other hemolytic anemias, 26 with aplastic anemia and 9 with iron deficiency anemia) and 21 healthy individuals. RESULTS: In PNH patients, both the mean fluorescence intensity and percentage of positive fluorescence-activated granulocytes of uPAR, CD55 and CD59 were remarkably decreased, while in anemic patients with diagnoses other than PNH the results were not different when compared with those of the healthy individuals. The expression level of uPAR was reduced to the same extent as that of CD55 and CD59 on the PNH-affected granulocytes. In addition, some peak shape abnormalities (double peaks and/or peak tailing) in the histogram of fluorescence intensity were also found in PNH patients. The suPAR concentration of PNH plasma was (4.04 +/- 2.47) micrograms/L, being higher than that of the healthy individuals (1.73 +/- 0.96) micrograms/L (P < 0.01). And the percentage of positive fluorescence-activated granulocytes was inversely associated with the plasma suPAR level in PNH patients (r = -0.79, P < 0.01). CONCLUSION: It is suggested that measurement of expression level of uPAR on granulocytes with FCM and suPAR in plasma with IRMA would be alteanative specific techniques for the diagnosis of PNH.

[A study for the relationship between serum calcitonin gene-related peptide and coronary arteriopathy].

OBJECTIVE: To investigate the relationship between serum calcitonin gene-related peptide (CGRP) and coronary angiopathy. METHODS: In a cross-sectional study, serum CGRP levels of 119 patients with coronary heart disease (CHD), 78 with non CHD and 39 healthy subjects were measured with radioassay method. The information on conventional risk factors were collected by interviews. RESULTS: The development of coronary angiopathy was correlated with smoking, diabetes and aging. The mean serum CGRP level was significantly lower in CHD than in NCHD patients [(35.5 +/- 48.8) mumol/L vs (63.8 +/- 47.3) mumol/L, P < 0.01). There was no difference between the mean serum CGRP level in NCHD patients and healthy controls. By multivariate logistic regression, the odds ratios (OR) of aging, smoking and diabetes were all > or = 1 (P < 0.01), that means they are independent risk factors for the development of coronary angiopathy. The OR of CGRP was < or = 1, indicating that it was an independent protective factor. CONCLUSIONS: It is shown that aging, smoking and diabetes are all independent risk factors for the development of angiopathy, while CGRP is an independent protective factor for the development of coronary angiopathy.

[Shoulder pain after laparoscopic cholecystectomy].

OBJECTIVE: To detect the mechanism of shoulder pain after laparoscopic cholecystectomy(LC). METHODS: Ninety patients who had receive LC were randomly divided into three groups (each 30 patients): group A, no management after LC; group B, absorption of residual CO2 after LC; group C, O2 insufflation for three times followed by complete absorption of CO2 after LC. PO2 and PCO2 before and after LC were recorded. Postoperative shoulder pain was assessed with shoulder pain's rate and level. RESULTS: The PO2 difference of group C between before and after LC was significant as compared to groups A and B(P < 0.05). Postoperative shoulder pain developed in 13 patients (43.33%) of group A, in 8 (26.67%) of group B, and 21 (70%) of group C (P > 0.05 A vs B; P < 0.05 A vs C; and P < 0.01 B vs C). The shoulder pain's level was higher in group C than in groups A and B. CONCLUSIONS: Direct irritation to the diaphragm due to the residual CO2 pneumoperitoneum is not the main cause of shoulder pain. The excessive traction of the triangular ligament and over-stretching of the diaphragmatic fibers due to insufflation are the main cause of shoulder pain.
