Laboratory and clinical evaluation of modified New York City medium (Henderson formulation) for the isolation of Neisseria gonorrhoeae.

The growth-promoting properties of several ingredients of New York City (NYC) medium were investigated by using 100 recently isolated clinical strains of Neisseria gonorrhoeae. Difco GC medium base promoted the growth of large colonies of gonococci significantly better than other commercial GC medium bases. A 1% agar concentration resulted in the growth of larger colonies than 2% agar, without affecting the stability of the gel. Neither replacement of horse plasma with horse serum nor reducing the concentration of added serum from 12 to 3% affected the growth of even the most fastidious strains tested. From these observations, a modified NYC medium (Henderson) has been formulated which is easy to prepare and less expensive than NYC or Thayer-Martin medium. In a direct clinical comparison with 1,275 specimens, an isolation rate of 6.5% was achieved with both NYC medium and the Henderson formulation.

Antenatal infections with Candida species.

The clinical, pathological, and microbiological features of 18 pregnancies complicated by intrauterine infection with Candida sp. are described. Chorioamnionitis with Candida sp. can be recognised macroscopically at birth. Penetration of the umbilical cord and membranes is associated with an intense fetal inflammatory response. The infection characteristically presents in infants of very low birthweight as pneumonia or a skin infection. In nearly every case the organism can be recovered from the gastric aspirate. A case control study showed that there is a striking association between chorioamnionitis caused by Candida sp. and the presence of a foreign body (an intrauterine contraceptive device or a cervical suture) in the mother's genital tract in pregnancy. This feature in the perinatal history of an infant of short gestation who exhibits a very high neutrophil count should alert the clinician to the possible presence of chorioamnionitis due to Candida sp.

[Single-dose gentamicin treatment and simultaneous localization of urinary-tract infections (author's transl)].

Single-dose gentamicin (80--120 mg; Refobacin) can be used successfully to treat significant bacteriuria in women, while at the same time differentiating between upper and lower urinary tract infection. Localization was achieved by demonstrating antibody-carrying bacteria in urine by immunofluorescence. In all 31 women with lower urinary-tract infection so treated urinary sample continued to be bacteria-free when retested one to for weeks later. On the other hand, in 22 of 25 women with renal bacteriuria the original bacteria were repeatedly isolated after treatment. Using single-dose gentamicin and subsequently re-testing urine samples, renal infections in four women were correctly diagnosed in this manner, although the initial immunofluorescence test was negative.

Fourier-transform analysis of normal photoelectron diffraction data for surface-structure determination.

A direct method for surface-structure determination from normal emission photoelectron diffraction (NPD) data is presented. Fourier transforms of the calculated NPD intensities yield peaks at adsorbate-substrate normal interlayer distances. Applications are demonstrated, using theoretical NPD curves for the Se/Ni system calculated by dynamical theory. These results show that interplanar spacings between the overlayer and as many as four substrate layers can be determined with an accuracy of better than 3%.

Simple lignocaine regimen for transit to hospital after myocardial infarction.

A simple combined intravenous/intramuscular regimen is described for lignocaine administration in the early vulnerable stage of myocardial infarction. Plasma levels in the therapeutic range are attained. This allows adequate drug protection during transport to hospital when an intravenous regimen may be impractical or impossible.

The involvement of the anticodon adjacent modified nucleoside N-(9-(BETA-D-ribofuranosyl) purine-6-ylcarbamoyl)-threonine in the biological function of E. coli tRNAile.

tRNAile was isolated from E. coli Cp 79 (leu-, arg-, thr-, his-, thiamin-, RCrel) which had been grown on a sub-optimal concentration of thr and was found to contain an average of 50% less N-[9-(beta-D-ribofuranosyl)- purin-6-ylcarbamoyl]threonine, t6Ado, than tRNAile from cells grown on an optimum concentration of thr and containing a normal complement of t6Ado. The two tRNA's were identical in their ability to be aminoacylated, to accept the 3'-terminal dinucleotide, and to form an ile-tRNAile-Tu-GTP complex. In contrast, the t6Ado-deficient-tRNA was significantly less efficient in binding to ribosomes compared to the normal tRNA. This difference was seen in the binding of deacylated tRNA and in the nonenzymatic and enzymatic binding of ile-tRNA, all in response to poly AUC. The t6Ado-deficient ile-tRNA demonstrated no binding at Mg2+ concentrations less than or equal to 10 mM, while the normal ile-tRNA bound at low Mg2+ concentrations. Tetracycline had the same effect on the normal as on the t6Ado-deficient ile-tRNA binding. As a control, the binding of phe-tRNA (which does not contain t6Ado) from normal and thr-starved cells in response to poly U was identical. It was concluded that t6Ado is required for proper codon-anticodon interaction.

Terminal oxidation-reduction of N-acetyl-phenylalanyl-tRNA blocks initiation complex formation with Escherichia coli 30S ribosomal subunits.

Terminally oxidized-reduced tRNA(Phe) of yeast, exclusively acylated at the 2'-hydroxyl of the 3'-terminal ribose of the tRNA, is a useful model for investigating the stereospecificity of AA-tRNA in protein synthesis. In this work, the ability of N-acetyl-Phe-tRNA(ox-red) to form an initiation complex with Escherichiacoli 30S ribosomal subunits was investigated. Thirty per cent of added control N-acetyl-Phe-tRNA was bound in a reaction dependent on initiation factors, GTP, and poly U, but no binding of the oxidized-reduced analog could be detected. These results imply that initiation complex formation may be specific for the 3'-ester of initiator tRNA.

Campylobacter jejuni infected bursitis.

Campylobacter jejuni is a common cause of enteritis, and has been isolated from patients with bacteremia, meningitis, and cholecystitis. We describe here an unusual case of a chronically inflamed bursitis infected with C. jejuni.

Accurate combination of CT and MR data of the head: validation and applications in surgical and therapy planning.

A method is presented for the accurate combination of magnetic resonance (MR) and computed tomographic (CT) images of the head. Our technique is based on user identified 3D landmarks followed by data combination and display as adjacent slices, a single fused slice representation, colour overlay and three-dimensional (3D) rendered scenes. Validation with a point phantom and computer simulation has established the relationship of registration accuracy with point location accuracy, the number of points used and their spatial distribution. The technique is in clinical use in the planning of skull base surgery, transferring MR images acquired without a stereotaxic frame to stereotaxic space, and staging and planning therapy of nasopharyngeal tumours.
