[Spontaneous renal rupture].

OBJECTIVE: To study clinical regularity of spontaneous renal rupture. METHODS: 16 cases of this disease were reviewed. Renal carcinoma was found in 4, angiomyolipoma in 2, undetermined etiology in 3, and invasive colic cancer, kidney cyst, nephrolithiasis, SLE with nephritis, polycythemia vera, renal tuberculosis, portal hypertension in 1 each. RESULTS: CT demonstrated a sensitivity of 87.5% in identifying the underlying kidney mass, which was higher than that (25%) of B-type ultrasound and (0) KUB + IVU. Radical nephrectomy was carried out in 6 cases, renal-reserved operation in 5, and conservative treatment in 5. CONCLUSIONS: 60% of spontaneous renal rupture are due to kidney neoplasia. CT scanning is the most valuable diagnostic procedure. If the primary cause has not been clarified, close follow-up with periodic CT scanning might be mandatory.

[Bone tissue engineering seeded with bone marrow stromal cells].

OBJECTIVE: To assess the ability of bone graft substitutes to support the growth of cells in vitro and new bone formation in vivo. METHODS: The methods of tissue engineering were employed, the bone marrow derived stromal cells were seeded onto blocks of hydroxypatite (HA), cultured for two weeks, and implanted intermuscularly into the rabbit back muscle, scanning electron microscopy and undecalcified bone tissue sections were used to evaluate the the growth of cells in vitro and new bone formation in vivo. RESULTS: The stromal cells were attached to and grew upon the surface of the periphery HA pores, especially in the pores adjacent to the bottom of culture flasks. No cell growth was found on the surface of the central pores. At four weeks after implantation, new bone formation was observed in most HA blocks, almost all of the new bones were lamellar bone and deposited directly on the surface of the HA pore. Active osteoblast and osteoid were found on the surface of the newly formed bone, invasion of bone marrow was observed in some newly formed bone. CONCLUSION: Bone marrow stromal cells are potentially the source of seeded cells for use in bone tissue engineering, and the HA may be considered as a suitable scaffold for these cells. The findings may be useful for detailed researches on seeded cells, scaffold and implanting fashion of bone tissue engineering.

[Effect of escharectomy during burn shock stage on stress response].

OBJECTIVE: To investigate changes in hormones of plasma and the effect of escharectomy during burn shock stage on early stress response after burn. METHODS: Sixteen male minipigs were subjected to a 35% TBSA III degree burn injury. The burned minipigs were randomly divided into two groups: escharectomy during burn shock stage group (S group) and escharectomy group after burn shock stage (N group). The sequential changes in plasma levels of insulin, glucagon, cortisol, GH, IGF-I, albumin were analyzed. RESULTS: The serum concentration of insulin, glucagon, cortisol increased to some degree, whereas the serum concentration of GH, IGF-I decreased obviously after burn. As compared with the N group, no significant difference in the serum concentration of glucagon, cortisol, GH, IGF-I, was observed. CONCLUSIONS: Escharectomy during burn shock stage could not aggravate stress response after burn.

[The effect of early escheractomy on expression of ICAM-1 and E-selectin mRNA of endothelial cells].

OBJECTIVES: To examine the effect of burned serum on change in endothelial cell ICAM-1 and E-selectin mRNA expression and whether these alterations can be corrected by the means of escharectomy during burn shock period. METHODS: One hundred and seventy-six Wistar rats with 30% III degree TBSA on back were studied. Endothelial cell ICAM-1 and E-selectin mRNA were tested with the method of RT-PCR. RESULTS: ICAM-1 and E-selectin expression showed an increase after stimulation by serum of 4 hours post burn, reaching peak levels 168 hours after injury. ICAM-1 and E-selectin expression levels could decrease to baseline within 168 hours in the rats which underwent escharectomy during shock stage, but kept higher than normal in rats without escharectomy or underwent 96 hours post burn. CONCLUSIONS: These findings suggest that eschar can induce endothelial cells to express adhesion molecule. Early escharectomy is important to prevent adhesion molecule expression and SIRS.

[The treatment to esophagus stricture with Ni-Ti shape memory alloy stand(with the report of 19 clinical cases)].

OBJECTIVE: To discuss the use of Ni-Ti shape memory alloy stand in treating the stricture of esophagus. METHOD: After finding the stricture part of esophagus under X-ray, the metal guide string was put through the esophagoscope. Dilatation of esophagus was performed firstly. Then the inserting instrument, which has the Ni-Ti shape memory alloy stand inside, was put through the stricture part along the metal guide string and the stand which can return to its original shape automatically was released. Barium meal examination of esophagus was performed after the operation to determine the site of the stand and to realize the smooth degree of esophagus. RESULT: All 19 patients got a successful operation at one time. They swallowed freely after the operation. The effective rate was 100%. CONCLUSION: Ni-Ti shape memory alloy stand can dilate the stricture of esophagus quickly, improve swallowing function obviously. The operation can be easily performed. The therapeutic effectiveness is reliable, and there is no serious complication. This can be used as a routine method in treating the stricture of esophagus.

[Determination of 5-hydroxytryptamine(5-HT) secretion from activated platelets by high performance liquid chromatography/electrochemical detector].

A method of determination of 5-HT secretion from activated platelets by HPLC/ECD was established. Spherisorb C18 column of 4.6 mm i.d. x 100 mm was used in this experiment. The CV was 4.65%, The linear range was 0.002 5-1 mg/L. This method has the advantages of high speed, high sensitivity; specificity, of requiring a minimum of preparation of samples and of relatively low cost for instrumentation and reagents. It could reflect the activated situation of platelet induced by weak activator and be used in other biological sample.

Testosterone inhibits apoptosis of Leishmania donovani-infected macrophages.

AIM: Investigate the effect of the male sex hormone, testosterone (Te), on apoptosis of bone marrow derived macrophages (BMMs) from female C57BL/6j mice METHODS: Propidium iodide (PI) staining and transmission electron microscopy (TEM) techniques were used to investigate apoptosis specific-morphological changes. BMMs derived from Te-treated mice and Oil-treated mice were challenged with Leishmania donovani (L. d.), Oligo-nucleosomal DNA were extracted 24 h post infection to detect apoptosis. RESULTS: The removal of M-CSF from the medium could induce BMM apoptofsis. And the DNA fragmentation assay also indicated that: 1. there was no difference in the amount of apoptotic cells between Te and Oil group; 2. Te + L. d. group had significantly less dead cells than Oil + L. d. group demonstrating that Te could prevent apoptosis of macrophage infected with L. d. to a greater extent. CONCLUSION: Te inhibits apoptosis of the macrophages infected with L. d., however, this inhibition did not occur in the macrophages uninfected with L. d. Te-induced macrophage apoptosis-inhibition may play an important role in Te-induced immunosuppression.

[Laryngeal function preserving surgery in hypopharyngeal carcinoma].

OBJECTIVE: To investigate the results of the conservation laryngectomy in patients with hypopharyngeal carcinoma. METHODS: Twenty-three cases with hypopharyngeal carcinoma that were treated with function-sparing laryngectomy during a 15-year period were reviewed retrospectively. RESULTS: The 3-year survival rate was 73.9% (17/23) and 5-year survival rate was 47.8% (11/23). Severe complication occurred in 8 of 23 (34.8%). Complete restoration of laryngeal function was achieved in 20 cases (87%) and deglutition restored in all patients (100%). The mean duration of oral intake was 11.8 days (6-33 days) and mean hospitalization was 32 days (14-94 days). CONCLUSION: Conservation laryngectomy and hypopharyngectomy is not only efficacious in the treatment of hypopharyngeal carcinomas, but also reserves laryngeal function and thus improves the quality of patients life.

[Spectroscopic assay, chemical modification and activity of three kinds of polyoses].

Infrared spectra and nuclear magnetic resonance spectra of chitosan, pachyman and amorphalls mannan were studied. The difference among the infrared spectra of chitosan under different conditions was disscused. The modification of Methylolation of the three kinds of polyoses was made and the relationship between the chemical modification and the antitumor activity was also studied.

[Study on the human serums by absorption spectra].

The absorption spectra of normal human and cancer patient serums were measured, and those of human serum after adding some cancer cells were also measured. The results showed some difference in absorbance among the three kinds of serums. Some useful results were obtained.

Nucleotide excision repair of actively transcribed versus nontranscribed DNA in rat hepatocytes: effect of age and dietary restriction.

The ability of primary cultures of rat hepatocytes to remove cyclobutane pyrimidine dimers (CPDs) from DNA fragments containing the transcriptionally active albumin gene and the transcriptionally inactive embryonic myosin heavy chain (MHCemb) and H-ras fragments as well as the genome overall was measured. At all UV doses studied, more CPDs were observed in the three DNA fragments and the genome overall in hepatocytes isolated from old (24-month-old) rats fed ad libitum than in young (6-month-old) rats fed ad libitum or old rats fed a calorie-restricted diet. The cultured hepatocytes preferentially removed CPDs from the albumin fragment compared to the genome overall or the MHCemb and H-ras fragments. The rate of repair (12 h after UV irradiation) of the albumin fragment was approximately 40% less in hepatocytes isolated from old rats than from young rats; this was due to a decrease in repair of the transcribed strand of this fragment, and dietary restriction prevented this decrease. The extent of repair (24 h after UV irradiation) of the MHCemb and H-ras fragments as well as the genome overall was reduced approximately 40% with age, and this decrease was reversed by dietary restriction.

Randomised placebo-controlled trial of use of topical recombinant bovine basic fibroblast growth factor for second-degree burns.

BACKGROUND: Wound healing is a dynamic process that could be accelerated by growth factors. We investigated the effect of recombinant bovine basic fibroblast growth factor (rbFGF) on burn healing in a randomised placebo-controlled trial. METHODS: We recruited 600 patients with superficial or deep second-degree burns. Patients received 150 AU/cm2 daily topical rbFGF (n=300) or placebo (n=300) plus vehicle. We assessed healing by photography, punch-biopsy, and clinical examination. FINDINGS: All patients treated with rbFGF had faster granulation tissue formation and epidermal regeneration than those in the placebo group. Superficial and deep second-degree burns treated with rbFGF healed in a mean of 9.9 (SD 2.5) days and 17.0 (4.6) days, respectively, compared with 12.4 (2.7) and 21.2 (4.9) days (p=0.0008 and p=0.0003, respectively). No adverse effects were seen locally or systemically with rbFGF. INTERPRETATION: rbFGF effectively decreased healing time and improved healing quality. Clinical benefits would be shorter hospital stays and the patient's skin quickly becoming available for harvesting and grafting.

Cognitive impairment, drug use, and the risk of hip fracture in persons over 75 years old: a community-based prospective study.

The authors examined the effects of cognitive function, as assessed by the Mini-Mental State Examination, and drug use on the incidence of hip fracture in a community-based Swedish population of 1,608 subjects who were aged > or = 75 years on October 1, 1987, and who had not had a hip fracture. During the 7,123.8 person-year follow-up, 134 first hip fractures were identified. The Cox proportional hazards model was used to estimate the relative risk of developing hip fracture, taking into account several potential confounders. Compared with those without cognitive impairment, subjects with mild impairment (Mini-Mental State Examination scores 18-23) had a relative risk of 2.04 (95% confidence interval (CI) 1.29-3.24), and subjects with moderate-severe impairment (Mini-Mental State Examination score < 18) had a relative risk of 2.09 (95% CI 1.17-3.72). Subjects using opioid analgesics (97% took propoxyphene) had a relative risk of 2.01 (95% CI 1.19-3.40). Taking potassium supplements (99% took potassium chloride) was related to a reduced risk of hip fracture (relative risk = 0.55, 95% CI 0.31-0.98), while diuretics did not have an independent impact. In summary, the results show that cognitive impairment and use of propoxyphene are associated with increased risk of hip fracture. The observed protection of potassium chloride merits further attention. The limitation of the study was that the assessment of drug use was made only at baseline.

Regulation of tyrosine hydroxylase gene expression during transdifferentiation of striatal neurons: changes in transcription factors binding the AP-1 site.

We have shown previously that the synergistic interaction of acidic fibroblast growth factor (aFGF) and a coactivator (dopamine, protein kinase A, or protein kinase C activator) will induce the novel expression of tyrosine hydroxylase (TH) in neurons of the developing striatum. In this study we sought to determine whether, concomitant with TH expression, there were unique changes in transcription factors binding the AP-1 regulatory element on the TH gene. Indeed, we found a significant recruitment of proteins into TH-AP-1 complexes as well as a shift from low- to high-affinity binding. Supershift experiments further revealed dramatic changes in the proteins comprising the AP-1 complexes, including recruitment of the transcriptional activators c-Fos, a novel Fos protein, Fos-B, and Jun-D. Concomitantly, there was a decrease in repressor-type factors ATF-2 and CREM-1. aFGF appeared to play a central but insufficient role, requiring the further participation of at least one of the coactivating substances. Experiments examining the signal transduction pathway involved in mediating these nuclear events demonstrated that the presence of only an FGF (1, 2, 4, 9) competent to induce TH caused the phosphorylation of mitogen-activated protein kinase (MAPK). Moreover, the treatment of cells with MEK/ERK inhibitors (apigenin or PD98059) eliminated TH expression and the associated AP-1 changes, suggesting that MAPK was a critical mediator of these events. We conclude that, during transdifferentiation, signals may be transmitted via MAPK to the TH-AP-1 site to increase activators and reduce repressors, helping to shift the balance in favor of TH gene expression at this and possibly other important regulatory sites on the gene.

The Xenopus Chk1 protein kinase mediates a caffeine-sensitive pathway of checkpoint control in cell-free extracts.

We have analyzed the role of the protein kinase Chk1 in checkpoint control by using cell-free extracts from Xenopus eggs. Recombinant Xenopus Chk1 (Xchk1) phosphorylates the mitotic inducer Cdc25 in vitro on multiple sites including Ser-287. The Xchk1-catalyzed phosphorylation of Cdc25 on Ser-287 is sufficient to confer the binding of 14-3-3 proteins. Egg extracts from which Xchk1 has been removed by immunodepletion are strongly but not totally compromised in their ability to undergo a cell cycle delay in response to the presence of unreplicated DNA. Cdc25 in Xchk1-depleted extracts remains bound to 14-3-3 due to the action of a distinct Ser-287-specific kinase in addition to Xchk1. Xchk1 is highly phosphorylated in the presence of unreplicated or damaged DNA, and this phosphorylation is abolished by caffeine, an agent which attenuates checkpoint control. The checkpoint response to unreplicated DNA in this system involves both caffeine-sensitive and caffeine-insensitive steps. Our results indicate that caffeine disrupts the checkpoint pathway containing Xchk1.

Relocation of the t-SNARE SNAP-23 from lamellipodia-like cell surface projections regulates compound exocytosis in mast cells.

For regulated secretion, mast cells and several other cell types utilize compound exocytosis, a combination of granule-plasma membrane and granule-granule fusions. The molecular machinery that controls this massive export process has not been identified. We report that SNAP-23, a t-SNARE related to SNAP-25, relocates in response to stimulation from plasma membrane lamellipodia-like projections to granule membranes in permeabilized mast cells. While relocation is a prerequisite for secretion, it can occur without membrane fusion and will expedite a subsequent secretory response. After relocation, SNAP-23 is required for exocytosis, implying a crucial role in promoting membrane fusion. Thus, relocation of this SNARE regulates compound exocytosis and links granule-plasma membrane and granule-granule fusions.
