Dopamine-induced paroxysmal supraventricular tachycardia in an infant.

Dopamine is an effective inotropic agent in the management of circulatory collapse. Ventricular dysrhythmias are reported to occur during dopamine infusion. The following report describes an infant with circulatory failure who experienced an episode of paroxysmal supraventricular tachycardia while receiving dopamine. Instability or immaturity of the cardiac conduction system during early infancy may contribute to the induction of an arrhythmia of supraventricular origin.

Congenital lactic acidosis associated with pyruvate carboxylase deficiency.

Two cases of congenital lactic acidosis associated with pyruvate carboxylase deficiency are described. One 2-mo-old infant had a fulminant clinical course with extremely severe intractable acidosis and died after 48 h in hospital. The second infant, aged 2 1/2 mo, had a milder clinical course, characterized by moderate acidosis and frequent convulsive episodes. He died at the age of 3 mo due to respiratory arrest following prolonged status epilepticus. Pyruvate carboxylase activity in liver biopsy specimens obtained from the two patients was 1 and 50% of normal, respectively. Both patients failed to respond to treatment, including massive doses of thiamine and high serum levels of lactate and pyruvate were found throughout their illnesses. Cerebral autopsy performed in both cases was unremarkable. Absence of neuropathological findings ruled out the possibility of Leigh's disease.

Fatal neonatal central nervous system infection caused by Citrobacter diversus.

Citrobacter diversus is a serious, albeit rare, offender of the central nervous system (CNS) in the neonatal period and in early infancy. We report here a case of neonatal CNS infection caused by C. diversus. The course of the illness was one of rapid deterioration, leading to recurrent seizures, coma and death. Since neonatal C. diversus CNS infection carries a grave prognosis, intraventricular administration of gentamicin should be considered.

Neurologic complications of Staphylococcus aureus septicaemia in childhood.

Two previously healthy children experienced serious neurologic complications in the course of severe S. aureus septicaemia. The first patient developed right hemiparesis and stupor due to a left frontal abscess. He recovered completely following surgery. The second patient, admitted with signs of meningeal infection, developed acute bacterial endocarditis followed by late cerebral embolism causing coma and death. In both patients, cerebral CT was a useful rapid diagnostic acid. When the S. aureus septicaemic patient develops acute bacterial endocarditis, regardless of his haemodynamic state, early valve replacement may prevent harmful cerebral embolic phenomena.

Direct subdural scintigraphy. Report of a case with subdural effusion following pneumococcal meningitis.

We report a case of pneumococcal meningitis in an 8 weeks olf female infant followed by persistent subdural effusion successfully treated by repeated subdural taps. The initial delineation of the subdural effusion, the decrease in size and the disappearance of the fluid were demonstrated by direct subdural scintigraphy (D.S.S.). The literature of pneumococcal meningitis, its diagnosis, treatment and complications are reviewed, and it is suggested that direct subdural scintigraphy should be employed as a diagnostic aid in the evaluation and follow up of subdural effusions.

A new white forelock (poliosis) syndrome with multiple congenital malformations in two sibs.

Two Jewish Ashkenazi male sibs are reported as having a new syndrome consisting of a white forelock, distinct facial features associated with congenital malformations involving the eye, cardio-pulmonary and skeletal systems. It is postulated that the etiology of this disorder is genetic and transmitted either as an autosomal recessive or X-linked recessive conditions.
