Postirradiation hearing loss.

Hearing loss due to irradiation of the head-and-neck region is a rarely reported complication of such a treatment. Although experimental work had been performed in laboratory animals as early as at the turn of the century, substantiated clinical data in large series are lacking. The few reports published are somewhat contradictory as to the incidence, time of onset, type and severity of the hearing loss. Although infrequently encountered, the possibility of radiation-induced hearing loss should be kept in mind. The pertinent literature is reviewed.

Amyloid goiter.

A case of a 50-year-old patient suffering from secondary amyloidosis presenting with amyloid goiter is described. The diagnostic and therapeutic implications are reviewed.

Radiological case of the month. Monostotic fibrous dysplasia of the temporal bone.

Computed tomographic scan demonstrating narrowing of the left external auditory canal caused by an expansile bone lesion of the temporal bone (curved arrow). The regional bone was characterized by a "ground-glass" appearance. A small cholesteatoma is located between the tympanic membrane and the bony lesion (black arrow).

Laryngeal reconstruction by composite nasoseptal graft after extended partial laryngectomy. Twelve-year follow-up.

Reconstruction of the larynx after extended frontal, frontolateral, or subtotal laryngectomy is a challenge to the head and neck surgeon. Over the last 12 years we have used autogenous composite nasoseptal grafts for rehabilitation of the larynx after extended partial laryngectomy in carcinoma invading the anterior commissure of the glottis. The composite graft proved to be an excellent replacement tissue and provided a dependable rigid cartilaginous wall lined by respiratory mucosa. The effectiveness of the procedure in bridging the laryngeal defect was successfully demonstrated in 29 patients. Good functional results were achieved in all of them. In our experience the nasoseptal graft is a safe and reliable single-stage procedure that provides a relatively easy and early laryngeal rehabilitation.

An objective method for evaluation of the patulous eustachian tube by using the middle ear analyzer.

Patulous eustachian tube is more common than is usually presumed. The symptoms of this condition can paradoxically simulate an obstruction of the eustachian tube. The characteristic eardrum movements synchronized with breathing can be overlooked or be absent altogether and, therefore, the diagnosis in many cases is missed. We suggest using the middle ear analyzer for diagnosis and objective evaluation of the patent eustachian tube by adjusting it as for acoustic reflex testing. Our method is more sensitive than the more common method of tympanometry. Information is supplied for the patient's evaluation before and after treatment.

Salivary duct carcinoma in major and minor salivary glands. A clinicopathological analysis of four cases.

Salivary duct carcinomas are an uncommon but distinct group of highly malignant salivary gland tumours. We report the clinical course, pathological findings and surgical treatment of this tumour in 4 patients. In one patient an intraductal tumour developed in a minor salivary gland, while in the other three patients, a major salivary gland was involved by an infiltrating salivary duct carcinoma. We point out the highly aggressive biological behaviour of the tumour when occurring in the major salivary glands, in contrast to the benign course of the intraductal carcinoma in the minor salivary gland.

Scrofula revisited.

Mycobacterial cervical lymphadenitis, or scrofula, is a disease dating back thousands of years. In spite of modern treatment and public health measures, scrofula persists but is infrequently seen. Five patients with cervical tuberculous lymphadenitis as a sole presentation have been seen in our department in the last 10 years. The clinical presentation and current methods of diagnosis and treatment are described. A short historical review is given.

Delayed brainstem auditory evoked responses in diabetic patients.

The incidence of subclinical central diabetic neuropathy is unclear due to difficulty in detecting latent alterations of central neural transmission process. The aim of this study was to evaluate a central neuroconductive mechanism in diabetics by brainstem auditory evoked responses (BAER). We found increased latencies of peaks I, III, V in diabetics as compared to control subjects. These BAER abnormalities were demonstrated in 62 per cent of insulin-controlled diabetics and in 33 per cent o patients treated by diet, or peroral drugs. No alterations in brainstem responses were observed in patients with latent diabetes. We did not find any correlation between the BAER abnormalities and the duration of the disease, the blood glucose level or the level of control of the diabetes.

Teratoma of the mastoid region in a newborn.

Teratomas are congenital tumours arising from pluripotent cells and usually consisting of tissue arising from all 3 embryonic germ layers. Teratomas are quite uncommon in head and neck sites and to date only 150 such cases have been reported. A rare case of teratoma of the mastoid region in a newborn is reported. The method of diagnosis and treatment are discussed and the literature is reviewed.

Primary tissue cultures of benign and malignant human thyroid tumors: effect of TSH.

Many thyroid carcinomas seem to be dependent upon the thyroid growth-promoting properties of the thyroid stimulating hormone (TSH). The purpose of the present investigation was to compare the in vitro effect of TSH on tissue cultures derived from malignant and benign thyroid tumors. The results indicate that TSH can affect the morphology and protein synthesis of primary tissue cultures derived from benign and malignant thyroid tumors differently. The addition of TSH to cultures derived from benign tumors resulted in a reorganization of follicle-like structures of the monolayer and in a reduction of protein synthesis. In contrast to this, monolayers derived from carcinomas of the thyroid were not able to reorganize and their protein synthesis was not inhibited in the presence of TSH. For a better understanding of TSH suppressive therapy, we suggest testing the influence of TSH on a large number of tissue cultures derived from benign and malignant tumors of the thyroid.

Microbiologic drinking water quality in Israel: standards, monitoring, and treatment.

Drinking water quality depends on standards, monitoring and treatment. Israel's microbiologic water standards are less stringent than those currently recommended internationally, and have focused primarily on bacteriologic levels. Compliance with routine monitoring requirements averaged 86% in 1985-86. However, compliance with retesting of contaminated samples averaged 49% in 1985-86. Compliance with treatment recommendations of district sanitary engineers has also been poor. Poor water quality continues to be a public health problem in Israel, and has contributed to a relatively high incidence of waterborne disease. Current international guidelines recommend filtration and chlorination, particularly for surface water and other contaminated sources. Israel should make this a major public health priority.
