[Two cases of wedge pneumonectomy in primary lung cancer involved into carina].

Right wedge pneumonectomy was performed on two cases of primary lung cancer involved into carina. A 59-year-old male involved with primary lung cancer was found with bloody sputum. Preoperative data confirmed as the superficial spread type of squamous cell carcinoma in carinal lesion and tumor was resectable with wedge pneumonectomy. In the second case, abnormal shadow was pointed out on chest X-ray film of a 61-year-old male patient. Preoperative examination defined as primary lung cancer of rt-S6 with subcarinal lymph node metastasis. The operation indicated wedge pneumonectomy with patch plasty using the wall of right main bronchus. Although wedge pneumonectomy is not common compared to sleeve pneumonectomy, if available this procedure is technically easier and post-operative management may be more successful. Wedge pneumonectomy limits resectable area, because the continuation of tracheo-bronchial wall must be remained in part. Therefore, the indication of this procedure for surgical treatment of lung cancer is limited. However, when this procedure indicates to selected case with limited lesion of carina, this may be an useful procedure as surgical treatment of primary lung cancer.

New projection for portography.

The right anterior caudocranial oblique (RACCO) view is proposed for improved depiction of the portal venous system. It is achieved by tilting the image intensifier 25 degrees caudal and 25 degrees-30 degrees to the right and maintaining the patient in the supine position. Transarterial portography with digital subtraction angiography was performed in the RACCO and posteroanterior (PA) views in 22 patients. Comparison of the two views revealed that the RACCO view was superior to the PA view of the left main branch in 15 patients (68%) and of the right posterior lobe branch in 19 patients (86%). The RACCO view will be useful for the evaluation of the portal system in patients with primary and secondary hepatic neoplasms in whom detailed images are important for diagnosis treatment planning.

[A case of double primary cancer of lung and trachea].

A 57-year-old male was admitted to our hospital with bloody sputum, whose right lung had been resected for squamous cell carcinoma of the lung three and a half years ago. Tomographic examinations and chest CT films showed the presence of tracheal tumor. Histological examinations of the biopsy specimen obtained from tracheal tumor, using bronchofiberscopy, identified squamous cell carcinoma. Trachea was resected in the length of six cartilages and end to end anastomosis was performed. Though lung cancer and tracheal cancer were same histological type, clinical findings confirmed that this case was double primary cancer. Postoperative course was uneventful except for minor leakage of anastomosis site.

[A case of omentopexy for bronchial fistula and pyothorax following the right completion pneumonectomy with combined resection of pericardium, diaphragm and chest wall].

A 68-year-old male underwent the right completion pneumonectomy with combined resection of pericardium, diaphragm and chest wall. Three months later, he was diagnosed as a bronchial fistula with bloody sputum and decreasing of right pleural effusion level on chest X-ray film. The conservative therapy with pleural drainage and endoscopic practice with fibrin matrix was failed to close a fistula and pyothorax was developed. Therefore, surgical treatment with simple omentopexy without thoracoplasty and/or muscle transposition was performed onto fistula and SILASTIC sheet used for the repair of diaphragm at initial operation was left in the thoracic cavity. Although pleural fluid remained the contamination with bacteria for one month postsurgically, infection did not develop and fistula closed successfully. Moreover, infection did not prolong in the presence of artificial SILASTIC sheet followed by simple omentopexy. Omentopexy may be very useful for the treatment of bronchial fistula with the presence of infection.

Fragmentation of the beta-subunit of human chorionic gonadotropin produced by choriocarcinoma.

When human chorionic gonadotropin (hCG) was subjected to sodium dodecyl sulfate-polyacrylamide gel electrophoresis under reducing conditions with dithiothreitol (DTT), a smaller weight material (CTP'), in addition to the beta-subunit, could be detected by Western blot analysis using antiserum for hCG beta-carboxy-terminal peptide (CTP). The CTP' band was much more apparent with urinary hCG from a patient with choriocarcinoma than with that from normal pregnant women. Second-dimensional electrophoresis of the choriocarcinoma hCG (c-hCG) after reduction with DTT indicated that the CTP', Mr 25,000, was released from the beta-subunit. The carbohydrate structure of the CTP' was analyzed by affinity with lectin-peroxidase on a nitrocellulose membrane. The CTP' did not interact with Concanavalin A, but exhibited strong interaction with both RCA120 and Arachis hypogaea after removal of sialic acid, indicating that it was released as a fragment containing an O-linked sugar chain as was found in the hCG beta carboxy-terminal portion. Western blot analysis using the antisera for hCG alpha, hCG beta, and hCG beta-CTP showed that the CTP' contains not only the carboxy-terminal portion but also a part of the internal (core) portion of the beta-subunit molecule. This dissociation of the c-hCG beta was further supported by the presence of a faster moving component (FMC) which may correspond to the NH2-terminal side counterpart. The desialylated FMC could be detected by Concanavalin A and RCA120 but not by Arachis hypogaea, indicating that it contains N-linked rather than O-linked sugar chains. The FMC does not contain any of the epitopes for the antisera examined in Western blot. These results indicate that the beta-subunit of the choriocarcinoma urine hCG has an unusual site which is dissociated into two components of Mr 25,000 (CTP') and Mr 18,000 (FMC) by DTT reduction.

[Molecular weight and fragmentation of human chorionic gonadotropin in urine in patients with trophoblastic diseases].

Molecular weights (MW) of urinary human chorionic gonadotropin (hCG) from trophoblastic diseases were analyzed by sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE), followed by Western blotting using specific antibodies. Although the MW of hCG beta from hydatidiform mole and invasive mole were identical to that of standard hCG beta, those from choriocarcinoma were individually different. These results indicate that the structural changes of carbohydrates, which may be induced by malignant transformation of trophoblast, can be reflected as differences in MW. In SDS-PAGE under nonreducing conditions. Western blotting using anti-hCG beta-carboxy-terminal peptides (CTP) revealed a single band corresponding to hCG beta. Under reducing conditions with dithiothreitol (DTT), however, a second low MW material (CTP') could be observed with anti-hCG beta-CTP. All hCG samples from invasive mole and choriocarcinoma were much more susceptible than standard hCG to release of CTP' by DTT. Estimation of MW and susceptibility to DTT reducing of urinary hCG by Western blotting are useful in the diagnosis of invasive mole and choriocarcinoma.

Novel chiral stationary phases for optical resolution by ligand-exchange high-performance liquid chromatography.

Novel chiral stationary phases, (1R,2S)- and diastereomeric (1S,2S)-2-carboxymethylamino-1,2-diphenylethanol, were prepared from (1R,2S)- and (1S,2S)-2-amino-1,2-diphenylethanol, respectively, and were bound to silica gel pretreated with 3-glycidoxypropyltrimethoxysilane. The chiral stationary phases were found to be very effective for the optical resolution of amino acids, amino acid derivatives and hydroxy acids by ligand-exchange high-performance liquid chromatography.

Effective separation of human chorionic gonadotropin subunits by high performance gel permeation chromatography.

Human chorionic gonadotropin (hCG) was effectively separated into alpha and beta subunits by treating with 8 M guanidine hydrochloride, followed by high performance gel permeation chromatography (GPC) on a 2.5 X 60 cm TSK G 3000 SW column in 6 M guanidine hydrochloride. Their molecular weights estimated by GPC were about 29,000 daltons for the beta subunit and 19,000 daltons for the alpha subunit. Both subunits were examined for SDS-polyacrylamide gel electrophoresis, amino acid analyses and radioimmunoassay, respectively. Neither subunit showed significant binding to the hCG-receptor fraction of rat Leydig cell in vitro. However, the separated subunits easily recombined and recovered about 90% of receptor binding activity of intact hCG. The present results indicate that the GPC technique is an effective, simple and non-denaturing method for the separation of hCG into alpha and beta subunits.

Distribution of intercellular adhesion molecule-1 on leukocytes and corneal endothelium after endotoxin stimulation in rats.

After stimulation with Salmonella typhimurium endotoxin, the intercellular adhesion molecule-1 (ICAM-1) was studied on the corneal endothelium and associated leukocytes in rats using immunoscanning electron microscopy. Two hundred micrograms of the endotoxin was injected in Lewis rats. The corneae were excised at 0-h and 16-h-postinjection time (n = 5, respectively). The corneae were prepared in hypothermic University of Wisconsin (UW) solution for immunoscanning electron microscopy. Histotopographical examination visualized ICAM-1 antigen on cytoplasmic processes of the corneal endothelium, arranged along microfolds, especially at the peaks. In the leukocytes, ICAM-1 was located primarily in morphologically non-specialized domains of the cell body surface, and only rarely scattered on the surface of microvillar projections. We concluded that the endotoxin stimulation can increase ICAM-1 in both corneal endothelium and associated leukocytes. Increased ICAM-1 may be an important factor for the leukocytes to form clustering and adhering to the corneal endothelium.

Embolotherapy for persistent hemoptysis: the significance of pleural thickening.

Elective embolotherapy for recurrent hemoptysis was evaluated for its effectiveness. Forty embolotherapeutic procedures were performed in 30 patients. In the short term, complete hemostasis was achieved in 16 of 40 procedures (40%) and significant improvement in another 16 procedures (40%). Long-term, complete hemostasis was achieved in 14 of 34 procedures (41%) and significant improvement in 12 (35%). Embolotherapy was consistently more effective in patients who had no pleural abnormalities compared with those with pleural thickening. Long-term complete hemostasis was achieved in 7 of 10 procedures (70%) for the patients without pleural thickening and only 7 of 24 (29%) in patients with pleural thickening. Thus, pleural abnormalities negatively influence long-term effectiveness of embolotherapy.

Retroperitoneal leiomyosarcoma. Its MR manifestations.

The magnetic resonance manifestations of retroperitoneal leiomyosarcomas in two patients are described. The tumors were isointense to muscle on T1-weighted images and exhibited a relatively high signal on T2-weighted images. Postcontrast T1-weighted images after the administration of gadolinium-dietilenetriaminepentaacetic acid produced moderate enhancement except in regions of central necrosis.

Pseudo lesions on CTAP secondary to arterio-portal shunts.

PURPOSE: To clarify the characteristics of the defects on computed tomography (CT) during arterial portography (CTAP) due to arterio-portal shunting (APS). METHOD AND PATIENTS: The defects in the hepatic parenchyma on CTAP due to angiography proven APS were characterized according to their configuration, size, sharpness of margins, location of the lesion in the liver, and homogeneity of the lesions. RESULTS: Twenty five patients had non-tumor related APS. The shunts were single on angiography in 15 cases and multiple in 10 cases. The APS were due to liver cirrhosis in 22 patients, and to trauma in the other three. The lesions were irregular in shape in nine cases, wedge shaped in six cases, and round in four cases. In three cases both wedge and round lesions were seen. The margins were sharp in five single defects in five cases. The rest of the lesions were judged to have unsharp margins except for one case in which both clear and unclear lesions were present. In 10 cases, internal enhancement of the lesions were seen on CTAP. The location of the defects were central in five cases, peripheral in nine, and both peripheral and central in eight cases. CONCLUSION: The parenchymal defects due to APS were typically irregular in shape, with unsharp margins and internal enhancement.

Optimal timing for delineation of hepatocellular carcinoma in dynamic CT.

OBJECTIVE: The aim of our study was to ascertain the optimal timing and scanning method for detecting hepatocellular carcinoma (HCC) as a high density mass during the early phase of dynamic hepatic CT and to confirm its clinical effectiveness. MATERIALS AND METHODS: We studied retrospectively 39 patients with clinically diagnosed HCC who underwent dynamic hepatic CT. RESULTS: All nine lesions that were scanned within 60 s after initiating injections were detected as high density masses. However, 15 of 21 lesions (71%) that were scanned > 70 s after initiating injections could not be detected as high density masses. Using helical scanning, which permitted scanning the entire liver within 60 s after initiating injections, 27 of 31 lesions (87%) were detected as high density masses. CONCLUSION: We concluded that most HCCs can be detected as high density masses during the early phase of dynamic CT using helical scanning.

Computed tomography for the assessment of blunt abdominal trauma.

Computed tomography (CT) was carried out on 70 patients with blunt abdominal trauma. Damage to the abdominal organs was clearly demonstrated by CT. The positive rates of CT in traumatic lesions were higher than those of ultrasound (US). Intraperitoneal hemorrhage was well demonstrated by US. Serum GPT levels were significantly elevated in patients with liver damage (p less than 0.001). CT proved to be useful for detecting damage to organs in patients with abdominal trauma.
