Amino acid sequences for human urinary bikunin (trypsin inhibitor) isomers.

Our previous study has shown that urinary bikunin has four major isomers, which differ with respect to the sulfation ratio in a glycosaminoglycan chain but which do not exhibit any differences in amino acid composition, N-terminal amino acid sequence, C-terminal amino acid, sialic acid and uronic acid content. In this study, the amino acid sequences of the four isomers were examined. Sequence analysis showed that the amino acid sequence of the four isomers was completely identical and was also the same as the amino acid sequence deducted from cDNA encoding bikunin. We conclude that the charge heterogeneity of urinary bikunin is due to only a difference in the sulfation ratio of a glycosaminoglycan chain.

[A case of pulmonary arteriovenous fistula failed in transcatheter embolization and followed by emergency operation].

We reported a 55-year-old male case with pulmonary arteriovenous fistula. The lesion existed in right S9b and was about 6 cm in diameter. The afferent artery and the efferent vein, both were more than 10 mm in diameter, were shown on the chest CT and DSA film. At first transcatheter embolization with steel coils and Histoacryl was tried. Though we succeeded in embolization of the afferent artery, embolization of the fistula was failed because of reflux from the drainage vein. And a coil, a fragment of Histoacryl dropped and floated in the fistula. With concerning about the risk of embolization of the other organ with these materials, we performed the enucleation of the fistula at the next day. Although transcatheter embolization is useful in treating pulmonary arteriovenous fistula, in the case of wide communication with large drainage vein such as our case, it seems to be risky.

[Bronchial circulation following sleeve lobectomy and effects of the main residual bronchus length on this parameter].

In mongrel dogs undergoing left anterior sleeve lobectomy, the role of bronchial circulation at the site of bronchial anastomosis was studied. Mucosal hemoglobin concentration and oxygen saturation were measured by spectral analysis of light reflected from the mucosal surface. The main bronchus was not resected in ten dogs (Group A). In ten other dogs (Group B), the main residual bronchus consisted of one or two rings. The following results were obtained. 1) The length of the main residual bronchus influenced the recovery of bronchial circulation following sleeve lobectomy. In Group B, arterial blood supply to the distal bronchial wall was better. Wound healing showed rapid progress from day 5, as was evident from the finding that bronchial arteries distal to the anastomosis were filled with proximal and mediastenal arterial blood. 2) Mucosal changes in bronchial anastomosis were the same for Groups A and B, as indicated by endoscopic examinations. In Group B, active neovascular proliferation was detected at the distal anastomotic site by microscopic examination. 3) In both groups, the micro-abscee could be seen at the anastomotic site at day 21. Bronchial anastomosis following sleeve lobectomy would thus appear to be complete on day 21 or thereafter.

A 22 kDa human growth hormone specific enzyme-linked immunosorbent assay.

A specific sandwich enzyme-linked immunosorbent assay (ELISA) was developed for human growth hormone (hGH) with molecular weight of 22 kDa (22k hGH). To prepare the 22k hGH specific antibody, a peptide (P-15) corresponding to residues 32-46 in 22k hGH was synthesized and used to immunize rabbits, because a 20k hGH variant was known to lack this peptide sequence. The assay method was developed using a combination of an anti P-15 polyclonal antibody (YK-2) coated on the microtiter well as a catching antibody and another polyclonal antibody against 22k hGH as a peroxidase-conjugated detecting antibody. The assay sensitivity was 1 ng/ml for 22k hGH, and cross-reactivity with other pituitary protein hormones as well as the 20k hGH variant was less than 0.01%, respectively. Furthermore, the 22k hGH content was measured precisely even in the presence of 20k hGH in this ELISA system.

Charge isomers of urinary bikunin (trypsin inhibitor).

It was observed that the purified urinary bikunin (trypsin inhibitor) consisted of four major isomers with different electric charges which could be separated by HPLC using a Mono Q column. These isomers revealed the same antitrypsin activity and did not show any differences in the apparent molecular weight by SDS-PAGE, amino-acid composition, N-terminal amino-acid sequence (1-40) and C-terminal amino acid (Leu). The contents of sialic acid and uronic acid were also identical among these isomers. However, analysis of chondroitin sulfate revealed all the glycosaminoglycan chains of these isomers were undersulfated, comprising nonsulfated and 4-sulfated disaccharide units, and 4-sulfated disaccharide unit ratio varied among these isomers. After the chondroitin ABC lyase digestion, all the isomers were eluted at the same position on a Mono Q column chromatography. These results indicated that charge isomers of urinary bikunin was attributed to the difference on sulfation ratio in a glycosaminoglycan chain.

Nucleotide sequence of the genes encoding the L3, L4, and L23 equivalent ribosomal proteins from the archaebacterium Halobacterium halobium.

A lambda EMBL clone containing a gene cluster coding for the ribosomal proteins L3, L4, L23 and 5' region of L2 was identified in a genomic library for the halophilic archaebacterium Halobacterium halobium using a heterologous hybridization probe from the related organism Halobacterium marismortui. The clone also contains two conserved open reading frames found in H. marismortui, although with still unknown function. Its gene organization is very similar to that of 'S10 operon' of H. marismortui. The deduced amino acid sequence of these ribosomal proteins (HhaL3, HhaL4, HhaL23 and 5' region of HhaL2) shows high similarity (64-71%) to those of the archaebacterium H. marismortui and a lesser degree of similarity to their eukaryotic (31-42%) and eubacterial (17-33%) counterparts.

[Studies on the gamma delta T lymphocytes in the peripheral blood in healthy adults].

Since gamma delta T lymphocytes has been found to be increased in patients with some immunological disorders, there exists clinical needs to determine the population of the cells in peripheral blood in normal subjects. We counted the number of gamma delta T lymphocytes in samples obtained from 151 healthy volunteers, aged from 20 to 69 years, by using a dual-color flow cytometry with fluorescence-conjugated CD3 and anti-TCR-gamma delta antibodies. Distribution ratio of gamma delta T lymphocytes among peripheral blood lymphocytes was 3.8 +/- 2.5% (range, 0.6-16.1%). Difference in the ratio and the number of gamma delta T lymphocytes among sex was not significant, but those decreased slightly but significantly with age.

[A case of intrapleural rupture of pulmonary arteriovenous fistula].

The hemothorax due to rupture of arteriovenous fistula is very rare and only 6 cases of this complication have been reported in Japanese literature. 44-year-old male complained of chest pain and dyspnea. A chest roentgenogram revealed right pleural effusion and an abnormal pulmonary shadow. By further examination including pulmonary angiography, the rupture of pulmonary arteriovenous fistula into right pleural cavity was diagnosed. Partial resection of the right lung containing arteriovenous fistula was successfully performed and postoperative course was uneventful.

[Intracavitary suction for treating giant bulla].

Examination was made of 9 consecutive patients who underwent intracavitary suction for giant bulla. A second operation was required for one patient who had multiple giant bullae. In the other 8 patients, improvement of pulmonary function and symptoms was obtained immediately following the operation. This was particularly more apparent in patients with poor pulmonary function. The following results were obtained. Functional recovery was evident from the early postoperative phase. A one-stage operation was possible. Reduction in postoperative drainage time was realized by bronchial occlusion. The present operation is particularly applicable to compromised patients. Computed tomography scanning is essential for evaluation of the drainage site. For giant bulla with some septation and multivesicular, the present mode of treatment would not be indicated.

Right anterior caudocranial oblique projection for portal venography; its indications and advantages.

Right anterior caudocranial oblique (RACCO) projections were evaluated for transarterial portography with digital subtraction angiography. For RACCO projection, the image intensifier was tilted 25 degrees caudally and 30 degrees to the patient's right with the patient in the supine position. The abnormal findings imaged in 20 patients were analyzed by types of abnormalities and their locations in the portal venous systems by comparing RACCO and PA projections. The RACCO view was superior to the PA projection for demonstrating either encasements or tumor thrombi in relatively proximal segments of the portal venous branches. Two encasement lesions and a tumor thrombus were imaged only on the RACCO projection. However, the RACCO projection had no definite advantage over the PA view for showing encasements or tumor thrombi in the distal segmental branches. The RACCO projection was superior to the PA view for demonstrating defects in the hepatograms of some hepatic segments (S5 and S8). We concluded that the RACCO view is extremely useful and is indicated for angiographic work-ups of hepatic or biliary lesions when abnormalities are suspected in proximal portal venous branches.

[Coagulation and fibrinolysis in pregnancy].

We studied on coagulation and fibrinolysis systems during pregnancy by measuring plasma levels of thrombin-antithrombin III complex (TAT), plasmin-alpha 2 plasmin inhibitor complex (PIC), fibrinogen/fibrin degradation products (FDP), plasminogen (PLG) and antithrombin III (AT-III). Ninety seven pregnant, 5 post-delivery and 32 nonpregnant women aged from 20 to 52 years old were included in this study. Plasma concentrations of TAT and PIC in nonpregnant women were 3.38 +/- 1.02 micrograms/l and 0.65 +/- 0.24 micrograms/ml, respectively. TAT gradually increased with the progression of pregnancy and rapidly decreased after the delivery. Whereas PIC and AT-III concentrations did not change significantly during pregnancy. Fibrinogen, PLG and FDP concentrations changed similarly as TAT. Eight pregnants whose plasma PIC concentrations elevated more than 1.0 micrograms/l were further examined. In 6 women out of them (71.5%), FDP concentrations were elevated. In this particular group of subjects, however, they delivered normally without complications such as toxemia. These observations suggest that, at least, a hypercoagulative state progresses with pregnancy, being normalized after the delivery. Although we could not find the relationship between the hypercoagulation and clinical complications such as thrombosis and toxemia of pregnancy, present findings suggest that special caution should be paid on the pregnants whose TAT and FDP levels are elevated.

[A case of small adenocarcinoma of the lung with giant bullae].

A 52-year-old male underwent right upper lobectomy for giant bullae which involved almost half of thoracic cavity and covered over the right upper lobe. Postoperative histopathological examination revealed emphysematous bullae of right upper lobe and adenocarcinoma (6 mm in diameter) was found in the bullae wall under the pleura. Since there was no metastasis in bronchial lymph nodes of upper lobe, additional procedure for lymph node dissection was not performed. Patient has been healthy for eight years after operation and there is no evidence of recurrent tumor. It is difficult to define the small lesion of neoplasma with giant bullae of lung. Therefore, the patient involved with bullae may require preoperative and intraoperative attentive investigation for possible malignant lesions.

[Superior posterior mediastinal neurinoma with extrathoracic extension].

We have encountered a case of superior mediastinal neurinoma which grows through the intercostal space and advances into the erector spinal muscles. Neurinoma which shows such a growth pattern is rare. MRI is useful in making preoperative evaluation. For operation, non-open chest extrapleural extirpation was successfully performed by the supraclavicular and posterior approach. This type of operation has the following advantages: surgical stress is less; postsurgical pain is mild, etc.

[Transcatheter embolization therapy of pseudoaneurysms in patients with chronic pancreatitis].

Chronic pancreatitis is known to cause vascular disorders including pseudoaneurysm of peripancreatic arteries. Although the incidence of pseudoaneurysms due to pancreatitis detected by angiography has been reported as high as 10% in western literature, they are still considered rare in Japan. We reported two cases of pseudoaneurysm caused by chronic pancreatitis, one in splenic artery and the other in gastroduodenal artery, successfully treated by embolization. The embolizations were performed by occluding proximal splenic artery for splenic arterial aneurysm and gastroduodenal artery distal and proximal to the orifice for the aneurysm of gastroduodenal artery with stainless steel coils. Since the surgical therapy has high mortality rate, we consider transcatheter embolization is the treatment of choice for pseudoaneurysms caused by chronic pancreatitis.

Early and delayed imaging with 123I-IMP SPECT in patients with ischemic cerebrovascular disease.

Cerebral blood flow imaging with N-isopropyl (I-123) p-iodoamphetamine (IMP) was performed in 44 patients with ischemic cerebrovascular disease (CVD) at an acute or subacute stage less than 30 days from the onset. IMP imaging was obtained at 20 minutes (early scan) and 4 hours (delayed scan) after intravenous injection of 222 MBq of IMP. The region of interest (ROI) was selected in a slice compatible with the findings on the CT images, and the lesion to tissue ratio (L/T ratio) was calculated in a comparison with the unaffected side. The redistribution index (RI) was also calculated by dividing the difference between the L/T ratio in early and delayed image by the L/T ratio in the early image. The patients were classified into three groups (Grade 1, 2, 3) on the basis of the CT findings. The L/T ratio in the delayed images and RI was high in grade 1 and 2 groups and low in grade 3 groups both in early and delayed scans. The RI had tendency to grow high as the days after the onset became later. In the duration period from 4 to 7 days, 'reversed' redistribution was observed in 4 cases. Follow up examinations were performed in 6 cases in grade 3 group. The RI became higher in 3 cases and lower in 3 cases in the second examination. In conclusion, good redistribution was observed in grade 1 and 2 groups, and the prognosis was good. [A study of five cases of traumatic diaphragmatic rupture].

During the last 5 years, 5 cases of traumatic diaphragmatic rupture were surgically treated. These cases were reported and the literature concerning traumatic diaphragmatic hernia in the last one decade in Japan, including 80 cases was studied. The purpose of this study is to discuss the most important early diagnostic tools and to consider the choice of incision and approach. The following two results were gotten. (1) Plain chest X-ray, computed tomography and ultrasonography were the most valuable diagnostic tools. (2) The choice of incision and approach depends on the stage at which the rupture is recognized (early or late), the site of rupture and associate injuries.
