[Clinical trial of 5'-deoxy-5-fluorouridine (5'-DFUR) in advanced cancer patients].

5'-DFUR was administered orally to advanced or recurrent cancer patients at a daily dosage of 600-1200 mg divided into 3 or 4 times a day. Out of 13 evaluable cases 2PR, 2MR, 4NC and 5PD were observed, response rate was 15.4%. PR were obtained in one gastric cancer case and one breast cancer case. Side effects were observed in 6 cases out of 14 cases (42.9%) and major adverse reaction was gastro-intestinal toxicities such as anorexia, nausea-vomiting and diarrhea. Two leukocytopenia and one erythrocytopenia were observed. This study indicated that 5'-DFUR would be useful as a new anticancer agent.

[Induction of ovulation with subcutaneous pulsatile administration of human menopausal gonadotropin].

Induction of ovulation with subcutaneous pulsatile (every 90 min.) administration of HMG (Pergonal) 75 or 150 IU/day using a portable pump (Nipro SP-3I) was performed in 3 PCO patients (6 cycles), 4 first grade amenorrhea (Am-I) patients (7 cycles) and 4 Am-II patients (4 cycles). All patients ovulated except one cycle of Am-I patients and one PCO woman conceived. In regard to the duration of administration and the total dose of HMG until ovulation, the administration of 150 IU/day (M +/- SD=15.2 +/- 5.0 days, 2280 +/- 774 IU) is superior to 75 IU/day (39.5 +/- 11.4 days, 3900 +/- 1357 IU), and there was no significant difference between this method and the daily intramuscular injection of HMG. The group treated with HCG in the luteal phase revealed a longer luteal phase (14.0 +/- 2.3 days) than the nontreated group (12.6 +/- 1.5 days). Ovarian hyperstimulation was observed in one case and subsided spontaneously after admission. There were no other side effects. In conclusion, this method has the following advantages: A high ovulation rate, comparable with daily intramuscular administration. It is a less painful procedure than daily intramuscular injection. It is possible for the patient to lead normal life, insertion and removal being easily done by herself.

Increased fructose-lysine of nail protein in diabetic patients.

Fructose-lysine, which is formed by binding glucose to lysine, is degraded on acid hydrolysis into furosine. Furosine was determined by high-performance liquid chromatography according to the method of Schleicher et al. Furosine values were significantly higher in diabetic patients than in healthy subjects, and significantly correlated with haemoglobin A1 (HbA1) values. These results suggest that furosine, like HbA1, may become an indicator of long-term blood glucose control in diabetic patients and be useful in investigating diabetic complications on the level of tissue.

Effect of coadministration of uracil on the toxicity of tegafur.

The cardiotoxic and neurotoxic effects of tegafur, an anticancer agent, were compared with those of uracil plus tegafur (4:1 mol/mol) in mice, rats, rabbits, cats and dogs. Uracil plus tegafur was shown to be less toxic than the drug alone in all the species, and uracil was found to decrease the toxicity of tegafur. alpha-Fluoro-beta-alanine, a catabolic metabolite of the drug, had toxic effects similar to tegafur. The results suggest that administration of uracil with tegafur prevents the side effects of the drug on the heart and CNS by inhibiting the degradation of 5-fluorouracil.

Surgical and conservative repair of Stahl's ear.

Surgical and non-surgical procedures for the correction of Stahl's ear anomaly, more commonly found in Oriental races, are presented. The final postoperative appearance is aesthetically pleasing, acceptable, and encouraging to the patient with this deformity.

Carbamylation of hemoglobin in renal failure and clinical aspects.

It was revealed that in renal failure hemoglobin A1 (HbA1) was formed by the binding of substances other than glucose to Hb. It was elucidated that the formation of HbA1 increased in urea-added medium and further increased in media containing urea either stored at 37 degrees C for 1 week or heated at 100 degrees C for five minutes. The increment of HbA1 was proportional to the amount of cyanic acid produced by the treatment of the medium after the addition of urea. We found that there was the best correlation between the HbA1 level and the BUN level of 1 to 2 weeks before. Therefore, it appeared that HbA1 could be an indicator of the state of renal failure of 1 to 2 weeks earlier in patients with renal failure without impaired glucose tolerance, especially in patients on hemodialysis.

Effects of glycosylation on physiological and biological activities of substances.

Hemoglobin A1 (HbA1) increases in old erythrocytes. These erythrocytes with increased HbA1 had decreased oxygen dissociation ability, resulting in decreased function. L-Triiodothyronine (T3) underwent glycosylation to form glycosylated T3. The biological activity of glycosylated T3 was assayed utilizing the metamorphosis of tadpoles. It was found that glycosylated T3 was less than one-tenth as biologically active as T3.

The double-Z rhomboid plasty: an improvement in design.

From clinical experiences with the double-Z rhomboid, which was originally described by Cuono, the differences between the original description and our operative results were pointed out. Our improved concept and procedure of the double-Z rhomboid are presented and clinical cases are demonstrated.
