Secondary thalamic degeneration after cerebral infarction in the middle cerebral artery distribution: evaluation with MR imaging.

PURPOSE: To evaluate secondary degeneration of the ipsilateral thalamus after cerebral infarction in the middle cerebral artery (MCA) distribution by using magnetic resonance (MR) imaging. MATERIALS AND METHODS: Thirty patients (17 men, 13 women; aged 30-85 years) with embolic cerebral infarction in the MCA distribution underwent serial MR imaging 2 hours to 12 months after a stroke. In 23 of the 30 patients, the authors evaluated cerebral blood flow with single photon emission computed tomography (SPECT). RESULTS: T2-weighted spin-echo images disclosed a hyperintense area in the ipsilateral thalamus in 14 patients (47%) 1-12 months after stroke. The hyperintense area was confined to the dorsomedial or anterior and dorsomedial nuclei of the thalamus in nine of the 14 patients; it extended from the dorsomedial or anterior and dorsomedial nuclei to the ventral lateral nucleus or pulvinar in the remaining five patients. Hypoperfusion of the ipsilateral thalamus was observed in 21 of the 23 patients who underwent SPECT. Twelve of the 21 patients also showed a hyperintense area in the ipsilateral thalamus on the MR images. CONCLUSION: MR imaging is useful in evaluating secondary thalamic degeneration after cerebral infarction. In clinical practice, this secondary degeneration should not be mistaken for other lesions such as further cerebral infarction.

Degeneration of the ipsilateral substantia nigra after striatal infarction: evaluation with MR imaging.

PURPOSE: To evaluate the degeneration of the ipsilateral substantia nigra after striatal infarction by using magnetic resonance (MR) imaging. MATERIALS AND METHODS: Twenty-five adult patients with embolic cerebral infarction of the middle cerebral artery distribution underwent MR imaging 0-4, 5-9, 12-15, and 27-29 days after the stroke. Sixteen of them also underwent follow-up MR imaging 2-12 months after the stroke. RESULTS: Ten patients had an infarct in the striatum with or without a cortical infarct (striatal infarction group); the other 15 patients had an infarct in the cerebral cortex of the middle cerebral artery distribution without a striatal infarct (cortical infarction group). In all 10 patients with striatal infarction, a hyperintense spot appeared in the ipsilateral substantia nigra on T2-weighted fast spin-echo images 7-12 days after the onset. This area became less intense and smaller 3 months later. In the cortical infarction group, no hyperintense spot in the ipsilateral substantia nigra was observed at any time. CONCLUSION: Degeneration of the substantia nigra ipsilateral to the striatal infarction was clearly demonstrated at MR imaging. This finding should not be mistaken for further cerebral infarction.

Setting up and features of a database of images of plastinated materials.

A database of plastinated materials was set up and distributed with a CD-ROM on the Internet. A total of 197 specimens, prepared at the Medical Museum of the University of Tokyo between 1991 and 1995, were catalogued in the database on a personal computer with commercial and original software. The database was originally designed to catalogue anatomical specimens and drawings for curators and researchers, but it also services as a worldwide digital museum.

The potentially best stereographic presentation of the plastinated anatomical specimen.

Volumegraph, which is an improved version of the parallax panoramagram, has been studied as a method of stereophotography of a plastinated human body specimen. The volumegraph provides a steric view of the specimen in a natural color appearance. The potential advantages in using the volumegraph for the presentation of anatomical specimens are discussed taking account of its technical details.

The effect of hemodialysis upon serum levels of fluoride.

Serum and dialysate ionic fluoride (F-) were determined in 29 patients under hemodialysis (HD) treatment. Serum creatinine (Cr), blood urea nitrogen (BUN) and phosphorus (P) were also examined before and after HD in 92 patients including the above 29 patients under the same treatment. Results reveal that serum F- levels of the patients before and after HD were statistically higher than those of the healthy subjects. The dialysis clearance of F- was 55.6 +/- 101.3 ml/min and the value was statistically significantly lower than those of Cr, BUN (95%, p < 0.001) and P (95%, p < 0.01). Based on these results, it is concluded that despite a net clearance of F- during a given HD procedure, serum F- failed to return to normal.

Patency of the human accessory pancreatic duct as determined by dye-injection endoscopic retrograde pancreatography.

The accessory pancreatic duct (APD) is the smaller and less constant pancreatic duct. The patency of the APD was investigated clinically in an effort to determine its role in pancreatic pathophysiology. Dye-injection endoscopic retrograde pancreatography (ERP) was performed in 190 cases. In the patients who exhibited filling of the fine branches of the ducts on ERP, contrast medium with indigo carmine was injected into the major duodenal papilla. The patency of the APD was determined by observing the excretion of the dye from the minor duodenal papilla. Of the 123 control cases studied, 41% had a patent APD. According to the shape of the terminal portion of the APD on accessory pancreatogram, it was classified as either the stick type (n = 63), branch type (n = 15), saccular type (n = 15), spindle type (n = 11), or cudgel type (n = 8). In these groups, 49, 0, 27, 82, and 87% of the APD were patent, respectively. The patency of the APD in the patients with acute pancreatitis was 6% (1 of 17). The difference in patency between this group and the control group was significant (p < 0.01). The patency of the APD varies with the shape of the terminal portion of the APD. A patent APD may prevent acute pancreatitis by lowering the pressure in the main pancreatic ducts.

Inactivation of human viruses by povidone-iodine in comparison with other antiseptics.

Inactivation of a range of viruses, such as adeno-, mumps, rota-, polio- (types 1 and 3), coxsackie-, rhino-, herpes simplex, rubella, measles, influenza and human immunodeficiency viruses, by povidone-iodine (PVP-I) and other commercially available antiseptics in Japan was studied in accordance with the standardized protocol in vitro. In these experiments, antiseptics such as PVP-I solution, PVP-I gargle, PVP-I cream, chlorhexidine gluconate, alkyldiaminoethyl-glycine hydrochloride, benzalkonium chloride (BAC) and benzethonium chloride (BEC) were used. PVP-I was effective against all the virus species tested. PVP-I drug products, which were examined in these experiments, inactivated all the viruses within a short period of time. Rubella, measles, mumps viruses and HIV were sensitive to all of the antiseptics, and rotavirus was inactivated by BAC and BEC, while adeno-, polio- and rhinoviruses did not respond to the other antiseptics. PVP-I had a wider virucidal spectrum, covering both enveloped and nonenveloped viruses, than the other commercially available antiseptics.

Genetic cholesteryl ester transfer protein deficiency is extremely frequent in the Omagari area of Japan. Marked hyperalphalipoproteinemia caused by CETP gene mutation is not associated with longevity.

Low levels of HDL cholesterol have been clearly demonstrated to be associated with an increased incidence of coronary heart disease, strongly suggesting that HDL particles have an antiatherogenic function. However, little information has been available concerning the atherogenicity of a marked hyperalphalipoproteinemia (HALP). There is no agreement about whether plasma cholesteryl ester transfer protein (CETP) deficiency is associated with an antiatherogenic state or not, although this disorder was reported to be one of the major causes of marked HALP. In the current study, we have found a unique area (Omagari City, Akita Prefecture, Japan) where CETP deficiency caused by a G-to-A mutation at the 5' splice donor site of intron 14 in the CETP gene is extremely frequent. In Omagari City, the mutation was detected more than 20 times more frequently and the prevalence of a marked HALP with plasma HDL cholesterol > or = 2.58 mmol/L (100 mg/dL) was 5 to 10 times higher than in other areas of Japan. This discovery has made it possible to perform a large population-based study concerning the atherogenicity of a marked elevation of HDL cholesterol in a genetically more homogeneous population. There was a statistically significant U-shaped relationship between HDL cholesterol levels and the incidence of ischemic changes in electrocardiograms. In cases of HDL cholesterol < 1.81 mmol/L (70 mg/dL), the incidence increased in proportion to the levels of HDL cholesterol. The frequency of the CETP gene mutation was higher in patients with coronary heart disease than in healthy control subjects. In subjects aged > 80 years, the prevalence of both marked HALP and the intron 14 splicing defect was significantly lower than in the younger generation. The current study indicated for the first time that a marked HALP caused by CETP gene mutation may not represent a longevity syndrome, suggesting the importance of reevaluation of the clinical significance and pathophysiology of a marked HALP.

Expression of inducible nitric oxide synthase in rat experimental autoimmune myocarditis with special reference to changes in cardiac hemodynamics.

Excessive NO produced by an inducible NO synthase (iNOS) has been implicated in many types of immune-associated disorders of the cardiovascular system, but it remains to be determined whether NO plays a role in myocarditis. Thus, the significance of iNOS expression in the development of experimental autoimmune myocarditis (EAM), an animal model of human giant cell myocarditis, was investigated. Lewis rats were immunized with cardiac myosin and were killed 7, 14, 21, 28, and 49 days after immunization. The development of severe myocarditis was observed on days 14, 21, and 28 in association with significant deterioration of hemodynamics determined by cardiac catheterization, which peaked on day 21. In parallel with histological severity of myocarditis and deterioration of cardiac performance, iNOS activity in the heart measured by [14C]L-citrulline formation was markedly increased on days 14, 21, and 28. The expression of iNOS was confirmed by immunoblotting and was localized to the infiltrating inflammatory cells found in the vicinity of necrotic myocytes by immunohistochemical analysis. Aminoguanidine, a selective inhibitor of iNOS, significantly decreased the iNOS activity (1.04 +/- 0.37 compared with 29.1 +/- 8.62 pmol.min-1.mg protein-1 in untreated myosin-immunized rats, P < .01) and effectively attenuated histopathological changes of EAM on day 21. Hemodynamic parameters were also improved from 64 +/- 3 to 89 +/- 3 mm Hg for mean blood pressure, from 80 +/- 2 to 113 +/- 4 mm Hg for left ventricular systolic pressure, from 7.8 +/- 0.3 to 3.2 +/- 0.3 mm Hg for left ventricular end-diastolic pressure, from 2867 +/- 137 to 4180 +/- 102 mm Hg/s for +dP/dt, and from 2717 +/- 132 to 4180 +/- 184 mm Hg/s for -dP/dt (P < .01). The values after aminoguanidine treatment were not significantly different from the control values. These results suggest an important role for NO in mediating pathophysiological changes in myocarditis of autoimmune origin.

StyD4I restriction-modification system of Salmonella typhi D4: cloning and sequence analysis.

A plasmid (5.4 kbp) from Salmonella Typhi D4 has been identified as encoding a restriction and modification (R-M) system. DNA fragments (2537 bp) that carried the genes for restriction endonuclease and methyltransferase encoded on the plasmid were sequenced. Two divergently arranged open reading frames of 957 bp for the restriction endonuclease consisting of 318 aa (amino acids) and 1140 bp for the DNA methyltransferase consisting of 379 aa were identified. These sequences were similar to the sequences of the SsoII R-M system, including the interspace between the two genes.

Structure and function of inositol 1,4,5-trisphosphate receptor.

Generation of intracellular Ca2+ signals in response to Ca(2+)-mobilizing stimuli is a critical event in the control of many cellular processes. Inositol 1,4,5-trisphosphate (IP3) represents a dominant second messenger subserving the release of Ca2+ from intracellular store sites. The protein on the surface of which the IP3 receptor is located comprises an IP3-gated Ca2+ channel, and binding of IP3 to this receptor triggers the release of Ca2+ through this channel. The receptor for IP3 displays a close resemblance to the ryanodine receptor, another intracellular Ca2+ channel, in many molecular and physiological properties. Many lines of evidence strongly suggest the central role that the IP3 receptor plays in the conversion of numerous external stimuli to intracellular Ca2+ signals characterized by complex spatiotemporal patterns such as Ca2+ waves and oscillations. In this review, we shall summarize our current knowledge of the structure and function of the IP3 receptor in order to understand the way how the activity of this important receptor is regulated to accomodate itself to the generation of diverse intracellular Ca2+ signals.

Relation between smoking and periodontal disease by oral hygiene status in Japanese factory workers.

The relation between smoking and periodontal disease as classified by the Community Periodontal Index of Treatment Needs (CPITN) code was investigated in 1,611 Japanese male factory workers in Osaka, 20-59 years of age. The Simplified Debris Index (DI-S) according to the Simplified Oral Hygiene Index was employed as an index for the quantity of dental plaque. For the items related to smoking, a self-reporting form on smoking, the number of years of smoking, and the number of cigarettes smoked was distributed prior to the oral examination. Our results indicated the following: in the group with a poor state of oral hygiene, the negative effects of smoking were evident, resulting in unhealthy periodontal tissue. We believe that this result further confirms the contention that the removal of dental plaque is most important as a prophylaxis for periodontal diseases, and public education about this negative consequence of smoking will further strengthen prophylactic measures.

Trochlear and abducens nerve neurinomas accompanied by a cerebellopontine angle meningioma--case report.

A 66-year-old male presented with cerebellovestibular symptoms and hypesthesia of the V2 area of his face. Neuroimaging only detected a cerebellopontine angle (CPA) meningioma. The CPA meningioma was removed using the lateral suboccipital approach, which exposed small neurinomas arising from the trochlear and abducens nerves. Both neurinomas were removed intracapsularly. Postoperatively hypesthesia resolved, but other symptoms were unchanged. A karyotypic analysis of chromosome 22 and estrogen receptor analysis suggested absence of neurofibromatosis II, but the cause(s) of the genesis of the multiple diverse tumors was not determined. This extremely rare combination of neurinomas and meningioma was probably incidental, as there are no reports of any case which a combination of the trochlear and the abducens nerve neurinomas, much less one accompanied by a meningioma.

Evidence for association between the class I subset of the insulin gene minisatellite (IDDM2 locus) and IDDM in the Japanese population.

Although the shortest (class I) minisatellite (i.e., variable number of tandem repeats [VNTR]) alleles in the 5' region of the insulin gene are positively associated with IDDM in Caucasians, the majority of Japanese are homozygous for class I alleles. Here, we determined the exact length, in number of repeat units (RUs), of class I alleles in Japanese subjects. The distribution of class I alleles in Japanese was trimodal, with peaks located at 32/33, 41, and 44 RUs. The shortest component (i.e., 1S [25-38 RUs]) alleles were significantly increased in the IDDM group compared with the control group (54 vs. 46%; P = 0.040). The 1S/1S genotype was significantly increased in the IDDM patients (34 vs. 20%; P = 0.005; relative risk 2.1). Furthermore, the transmission disequilibrium test of Japanese families with 1S/1M or 1S/1L heterozygous parents confirmed the association of 1S alleles; 17 alleles of 1S and 6 alleles of 1M (39-41 RUs) or 1L (42-44 RUs) were transmitted to affected offspring (P = 0.022). In addition, we found tight linkage of 1S with allele 9 of the tyrosine hydroxylase gene microsatellite and allele (-) of the IGF-II gene Apa I polymorphism, but neither 9 nor (-) alleles were significantly associated with IDDM. The present study suggests that a class I subset may have a role in IDDM susceptibility in Japan. It was revealed that the difference between 1S alleles and 1M or 1L alleles is almost consistently characterized by a sequence variation generated by deletion of two copies of an ACAGGGGTCC CGGGG repeat element, implying that sequence variation of class I alleles may influence disease susceptibility.

Significance of serum iron and ferritin in patients with colorectal adenomas.

BACKGROUND: Colorectal adenomas are often detected on mass screening, although detection rates with fecal occult blood tests are low. The relationship between colorectal adenomas and the resulting blood loss was examined indirectly, using serum iron and ferritin levels. METHODS: Serum iron and ferritin concentrations were measured in 184 men with colorectal adenomas (> or = 1 cm in 92; < 1 cm in 92) and in 92 healthy male controls. Values in the three groups were compared. In the patients with adenomas > or = 1 cm, serum iron and ferritin levels were compared on the basis of the site, number, histology, and degree of dysplasia of the adenoma. RESULTS: The mean serum iron level was significantly lower in patients with adenomas > or = 1 cm than in controls (P < 0.05), although this level did not differ significantly between those with adenomas < 1 cm and controls. The mean serum ferritin level also was significantly lower in patients with adenomas > or = 1 cm than in those with adenomas < 1 cm and controls (P < 0.05, P < 0.01, respectively), although this level did not differ between those with adenomas < 1 cm and controls. There was no difference in mean serum iron or ferritin levels on the basis of the site, number, histology, or degree of dysplasia of the adenoma. CONCLUSIONS: We conclude that decreased serum iron and ferritin levels are related only to adenoma size and that adenomas > or = 1 cm may bleed steadily, resulting in iron deficiency. However, low dietary intake of iron and fiber may be one of the causes of low serum iron and ferritin.

Geographical distribution of vectors and sero-strains of tsutsugamushi disease at mid-south inland of Korea.

Studies of geographical distributions and relative population densities of the vector mites of tsutsugamushi disease were carried out in October 1996 at 12 locations of the mid-south inland of the Korean peninsula, where chigger mites have been never studied. Of 177 field rodents and insectivores collected, 154 (87.0%) were Apodemus agrarius. Total 25,707 chigger mites were collected and 14 species were identified, of which Leptotrombidium pallidum was predominant (79.8%) and L. palpale the next (8.9%). L. pallidum, the vector species, was widely distributed in all study areas, showing the highest density at Cho-o 2-dong, Sangju-si (chigger index 201.8), and the lowest at Tanwol-dong. Chungju-si (chigger index 40.7). The other vector species, L. scutellare was found only at the southern part of the study area such as Yobae and Mipyong, Kumrunggun and Unsu, Kimchon-si. The northernmost areas of the L. scutellare distribution were coincided with the areas where anual mean air temperature is above 10.0 C. Among 157 A. agrarius sera tested, 48.3% was Karp, 1.7% Gilliam and 3.3% Kuroki. The rest of the sera were not able to determine the sero-type because of the cross antigen-antibody reactions among the tested sero-types.

Anti-Leukemia Chemotherapy of High-Risk Myelodysplastic Syndromes.

We evaluated the outcome of anti-leukemia chemotherapy on 42 patients with the high-risk myelodysplastic syndromes (MDS)-refractory anemia with excess of blasts (RAEB), 8 cases; RAEB in transformation (RAEB-T), 18 cases; chronic myelomonocytic leukemia (CMMOL), 6 cases; and leukemic transformation of MDS, 10 cases. The median age was 67 (range 20 to 84). As a remission-induction therapy, 35 patients received low-dose chemotherapy, such as low-dose cytarabine infusion, and seven patients received high-dose combination chemotherapy. The complete remission (CR) rates of the low-dose chemotherapy group and the high-dose combination chemotherapy group were 29% and 57%, respectively, and the overall CR rate was 33%. The median survival durations after induction chemotherapy of the CR group (14 cases), the partial remission (PR) group (11 cases), and non-remission (NR) group (17 cases) were 19 months, 8 months, and 3 months, respectively. As a post-remission consolidation chemotherapy, high-dose combination chemotherapy seemed to be superior to low-dose chemotherapy judging from the median CR duration (16 months versus 4 months), but a long-term disease-free survival is hardly expected, in contrast with results in cases of de novo acute myeloid leukemia.

[A case of Wilson-Mikity syndrome with marked and monoclonal elevation of IgM in blood].

Wilson-Mikity syndrome, a chronic respiratory insufficiency occurring in premature newborn infants, is known to show an elevation of serum IgM, but its etiology is obscure. We studied the immunological properties of IgM in a female infant of this syndrome, whose IgM in cord blood was very high level at 639.1 mg/dl with monoclonal elevation. The IgM bound to protein(s) in human umbilical venous endothelial cells and in human lung tissue. However, IgMs from this patient after recovery, from patient's mother and from another normal newborn did not bind to the proteins. Tissue-binding activity of the patient's IgM may be associated with tissue damages in this syndrome.
