Visual fields in metastatic choroidal carcinoma.

A large choroidal metastasis was diagnosed in the right eye of a 44-year-old man referred for admission to hospital because of 'retinal detachment.' At the same time in the second, symptomless, eye a very small metastasis was observed. Close follow-up during the next month showed an extremely rapid deterioration of visual acuity and visual field in this eye. This is thought to be characteristic of such metastatic tumours in contrast to the slower progress of choroidal malignant melanoma. The application of local radiotherapy to the same eye led to an impressive improvement in the visual acuity and visual field. The source of these bilateral choroidal metastases, which was found only after the patient's death, proved to a bronchial carcinoma.

Fuch's heterochromic iridocyclitis: an electron microscopic study of the iris.

The irides of two patients with Fuch's heterochromic iridocyclitis were investigated by electron microscopy. The main findings were abnormal melanocytes with relatively few, small, and at times immature melanin granules, abundance of plasma cells, and membranous degeneration of nerve fibers. The defective melanin production may be due to abnormal adrenergic innervation, either primary or secondary to the inflammatory process. The cause for this inflammatory reaction was not evident in this study.

Fluorescein pupillary flow in aphakics with intact and spontaneous openings of the vitreous face.

The passage of fluorescein dye, injected into the general circulation, from the posterior to the anterior chamber was studied in 60 selected aphakic eyes between 2 days and 4 weeks after an uneventful cataract extraction. In a group of 22 eyes with intact anterior hyaloid, fluorescein was seen to pass only through the pupillary margin, mostly within 20 seconds of the beginning of the eye injection. In 24 eyes with rents in the anterior hyaloid the dye was seen percolating only through the vitreous face openings. No fluorescein passed through the pupillary margin or the peripheral iridectomy in this group of eyes. The appearance time of the dye was delayed to about twice the normal appearance time. In 8 eyes with rents in the anterior hyaloid the dye passed only through the pupillary margin. In 4 eyes with anterior vitreous rents the fluorescein appeared through the anterior vitreous face opening. From the analysis of the data it appears that the spontaneous breaks in the anterior vitreous face may represent a 'self-c,re' of pupillary and iridectomy block by the vitreous in aphakics in a high proportion of cases. It is our impression that 3 peripheral iridectomies and a very tight closure of the wound can prevent the complication of spontaneous breaks of the vitreous face.

Subretinal neovascularization developing after prophylactic argon laser photocoagulation of atrophic macular scars.

Two patients who had lost central vision in one eye due to a histoplasmic disciform macular scar received prophylactic argon laser photocoagulation to one or more atrophic lesions (histo spots) in the second macula in an effort to reduce the risk of developing active neovascularization in the second eye. Despite this prophylactic treatment, both patients developed a choroidal neovascular membrane from the photocoagulation scar.

Identification of copper ions in aqueous and vitreous of eyes containing copper and iron foreign bodies.

The reliability of aqueous analysis by the atomic absorption spectrophotometer for copper ion content in samples from eyes containing intravitreal pure copper particles was investigated. It was demonstrated that values of copper ions rose well above the normal in the aqueous as well as in the vitreous of such eyes. The standard deviations of the mean values were generally high. It was also found that a similar copper ion content increase occurred in eyes containing iron particles. The possible clinical implications of these findings are discussed.

The fate of eyes with anterior chamber intra-ocular lenses.

A total of 143 anterior chamber (AC) intra-ocular lenses (IOL) of various designs were implanted in a five-year period and followed for 18 to 76 months. The lenses were inserted as a secondary implant after extracapsular cataract extraction (ECCE) complicated by vitrectomy, or after primary intra-capsular extraction. The complications (corneal edema, uveitis/glaucoma, extrusion/imbedding, cystoid macular edema), the final visual acuity and the need for removal of the IOL were analyzed. Only small differences were found between the different lenses but some association was found between complicated surgery (ECCE + vitrectomy) and a higher rate of complication, poorer visual outcome and more frequent need for IOL removal. Cystoid macular edema was encountered only in cases with complicated ECCE. A high percentage of other ocular pathologies was found among the cases, possibly implying that diseased eyes are more prone to surgical complications. An alternative to the AC-IOL in cases with a ruptured posterior capsule is the suture-supported posterior chamber IOL, but the flexible-loop AC-IOL may not yet be obsolete.

Keratopathy associated with hypoparathyroidism and Addison's disease.

Two siblings suffering from keratopathy, one with hypoparathyroidism associated with Addison's disease and the other with Addison's disease alone, are described. The hypothesis that there was an autoimmune etiology with variant manifestations in each subject is suggested and discussed.

Traumatic total hyphema in a patient with severe hemophilia.

A 12-year-old hemophilic boy suffered from secondary glaucoma in his right eye due to total hyphema following trauma. An emergency operation was performed to evacuate the hyphema under a tight anti-hemophilic treatment. A careful postoperative treatment with factor VIII concentrate, antiglaucomatous drugs and aminocaproic acid was administered. Intraocular pressure returned to normal and remained so during the four months follow-up period. Visual acuity returned to 6/9 and there was no rebleeding into the anterior chamber. To the best of our knowledge, this is the first reported case of traumatic hyphema and of an operation to wash this hyphema in a hemophiliac. The dilemma of management of such a case is discussed.

Acquired Brown's syndrome caused by frontal sinus osteoma.

A 32-year-old man had a history of acquired Brown's syndrome associated with diplopia, proptosis, downward displacement of the globe, and lid edema. A CT scan of the orbit revealed an osteoma arising from the left frontal sinus and extending into the left orbit. After surgical extirpation the proptosis and diplopia disappeared and the globe returned to normal. To our knowledge, such a case has not been reported previously.

Medial rectus re-recession in undercorrected esotropia.

Conventional surgical management of surgically undercorrected esotropia includes bilateral lateral rectus resection, marginal myotomy, and Faden operation. During a 3-year period, 10 patients with surgically undercorrected esotropia had medial rectus re-recession 13.5 mm from the limbus. Successful binocular realignment (to 10 prism diopters or less residual binocular misalignment) was achieved in 9 of the 10 cases for both near and distance measurements.

Retinopathy of prematurity and surfactant treatment.

PURPOSE: Surfactant therapy in premature infants has reduced the severity of respiratory distress syndrome (RDS), thus leading to a reduction in mortality. However, the anticipated effect of surfactant therapy on the incidence and severity of retinopathy of prematurity (ROP) is ambiguous. The acute rise in PaO2 and the increased survival of low-birth-weight infants may augment the risk of ROP, whereas their improved health and respiratory status may lower it. METHODS: We reviewed the findings of sequential ophthalmologic examinations performed in our neonatal intensive care unit. Premature infants of gestational age under 32 weeks and weighing less than 1500 g at birth who received surfactant treatment were compared with a group of historical controls consisting of premature infants of the same mean birth weight and gestational age who did not get this supplement. RESULTS: ROP was present in 13 (65%) of the 20 surfactant-treated babies, and threshold disease was noted in six (30%). In the historical control group, 20 (77%) of 25 infants had ROP, of whom 10 (40%) were treated for threshold disease. These differences were not statistically significant. CONCLUSION: Surfactant therapy was not associated with an increased incidence of ROP in our series.

Retinopathy of prematurity in multiple-gestation, very low birth weight infants.

Ninety-nine infants from multiple gestation births and weighing < or = 1500 g at birth were matched with infants from single births to clarify the relationship of multiple gestation to retinopathy of prematurity (ROP). There was no significant difference in the incidence of ROP between the twins and the singletons (relative risk [RR] = 0.84, 95% confidence intervals [CI] = 0.61, 1.16). Logistic regression analysis confirmed that very low birth weight (VLBW), not single or multiple gestation, was the most significant predictor of ROP occurrence in either group. In 27 pairs of twin siblings from the original twin group, where both weighed < or = 1500 g at birth, we found that the second-born twin seemed at higher risk for developing ROP, but logistic regression showed that the lower birth weight of the second twin, not birth order, was the more significant predictor of ROP. These results indicate that ROP screening in VLBW twins may be conducted according to the same standard protocols as for singletons.
