Homologous and heterologous connective tissue tube for vascular substitute.

A newly-developed vascular graft having many advantages over the fabric vascular prosthesis was evaluated as aortic and vena caval substitutes. Alcohol-preserved connective tissue tubes, obtained from dogs and calves, supported by polyester mesh were implanted into the thoracic aortae of 61 dogs and the superior vena cava of 24 dogs. In the case of the aortic experiment, all the tested animals were healthy and each graft was patent at the time of removal. In the case of the vena caval experiment, all the tested animals were healthy, but 10 of 24 grafts were occluded. Aneurysmal dilatation, immune response and regressive changes of newly formed neointima were not observed in any case. Moreover, no blood leakage through the graft wall was observed. Leakage at the line of anastomosis was very little. The great advantage of the graft was good endothelization of the neointima. Between the grafts obtained from dogs and calves, there was no difference in the process of their healing.

Plasma ADH levels during heart surgery.

1) Plasma ADH levels measured by bioassay in the group with extracorporeal circulation were 2.3 +/- 0.6 muu/ml before surgery and 6.6 +/- 1.8 muu/ml during anesthesia. They increased to 196.5 +/- 62.3 muu/ml or about 100 times greater than before surgery during cardiopulmonary bypass. 2) In the group without extracorporeal circulation, plasma ADH levels were 1.5 +/- 0.9 muu/ml before surgery and increased to 44.1 +/- 15.2 muu/ml during operation. 3) After operation decrease in plasma ADH level was relatively rapid in both groups. It became three times that of the control level in the morning of the next day. 4) Marked increase in plasma ADH level during cardiopulmonary bypass was much the same as it was during hemorrhagic shock in dog experiments. 5) Fall in mean arterial blood pressure and loss of pulsatile blood flow will play main roles in this marked increase in ADH during cardiopulmonary bypass through stimulation of arterial baroreceptors and probably chemoreceptors. 6) In two cases with mitral stenosis, increase in plasma ADH during cardiopulmonary bypass was lesser than the other heart diseases.

Effect of aluminium chloride on metabolism of 4-nitroquinoline 1-oxide.

Aluminium chloride was subcutaneously administered to mice and its effect on the activities of 4-nitroquinoline 1-oxide (4-NQO) reductase and 4-hydroxyaminoquinoline 1-oxide (4-HAQO) reductase, and the organ distribution of carcinogen(s) in mouse lung and liver were examined. Subcutaneous administration of aluminium chloride in mice results in significant elevation of 4-NQO reductase and 4-HAQO reductase activities in their lung and liver, compared with those of the control. Simultaneous subcutaneous administration of aluminium chloride with 4-nitroquinoline[5, 6, 7, 8, 9, 10-14C] 1-oxide (14C-4-NQO) and examination of radioactivity distribution in the lung and liver showed that the radioactivity per tissue, 0.5 and 1 hr after the administration, decreased in the lung but inversely increased in the liver. Radioactivity in the lung and liver 2 hr after the administration was not different from that of the control. Simultaneous subcutaneous administration of 14C-4-NQO and aluminium chloride resulted in decreased distribution of 4-NQO and 4-HAQO in the lung compared with that of the control, while the distribution of their metabolites, 4-aminoquinoline 1-oxide (4-AQO) and 4-hydroxyquinoline 1-oxide (4-OHQO), inversely increased. Distribution of 4-aminoquinoline (4-AQ) and 4-hydroxyquinoline (4-OHQ) in the lung was not different from that of the control. These results suggest that the rapid metabolic changes of carcinogenic 4-NQO and 4-HAQO to noncarcinogenic substance(s) and decrease in the concentration of carcinogenic substances in the lung by the subcutaneous administration of aluminium chloride constitutes one of the factors for the mechanism of the suppression of carcinogenesis by aluminium chloride.

Effect of aluminium chloride on binding of 4-hydroxyamino-quinoline 1-oxide to nucleotides.

Effect of aluminium chloride on the binding of carcinogenic 4-hydroxyaminoquinoline 1-oxide (4-HAQO) with mouse lung DNA, RNA, and various homopolyribonucleotides was examined in vitro, in the presence of seryl-AMP. Mouse lung DNA, RNA, or homopolyribonucleotide [poly(A), poly(G), poly(I), poly(X), poly(C), or poly(U)] was pretreated with aluminium chloride in an ice bath and the binding with 4-HAQO was examined. Binding with DNA, RNA, poly(A), and poly(G) was markedly inhibited, and their binding rates were 46%, 56%, 53%, and 18% of that of the control, respectively. Binding with poly(C) and poly(U) was hardly different from that of the control. Consequently, effect of aluminium chloride in inhibiting the binding of 4-HAQO with mouse lung DNA and RNA is assumed to be due to the inhibition of its binding with guanine. Effect of various metals (Mg2+, Mn2+, Co2+, Ni2+, Cu2+, Zn2+, and Fe3+) on the binding of 4-HAQO with mouse lung DNA was examined and it was found that aluminium chloride had the strongest inhibitory effect, followed by copper and zinc. Trivalent iron showed hardly any inhibition.

Bartter's syndrome--case report.

A 26-year-old female with Bartter's syndrome associated with Graves' disease is reported. This patient had a history of Graves' disease from the age of 22 and anti-thyroid drug (Methimazole) had been administered for 2 years. Thyroid function returned to normal but general fatigue and polyuria continued. Hypokalemia was diagnosed at 25 years of age and she was referred to our hospital for evaluation. Blood pressure was normal and laboratory data revealed normal thyroid function, hypokalemic alkalosis, high plasma renin activity and high plasma aldosterone concentration. She showed normal pressor sensitivity to norepinephrine infusion, grossly diminished pressor sensitivity to exogenous angiotensin II infusion compared with the normal. A renal biopsy specimen showed juxtaglomerular cell hyperplasia. Electron microscopy confirmed lacis cell (agranular cell) proliferation.

A case of ectopic renin-secreting orbital hemangiopericytoma associated with juvenile hypertension and hypokalemia.

An unusual case of orbital tumor with high renin content and severe hypertension is described. The patient was a 15-year-old girl with juvenile hypertension (200-140 mmHg) associated with right exophthalmos and hypokalemia. The patient showed extremely high levels of plasma renin activity and plasma aldosterone concentration. No difference was present in plasma renin activity from either side of the renal veins. Preoperatively, hypertension responded to treatment with spironolactone. The tumor could not be completely removed because of intracranial metastasis and infiltration, and the hyperreninemia and secondary hyperaldosteronism persisted. The renin content in the orbital tissue was 1,403-2,225 ng/angiotensin I generated/h/g wet weight of tissue. The postmortem histopathologic diagnosis was orbital hemangiopericytoma. This is the first case of extrarenal (ectopic) renin-secreting (or -producing) hemangiopericytoma of the orbital origin. Furthermore this case is worthy of note in the point of view of the presence of the extrarenal renin-angiotensin system, particularly in the brain.

A case of myocarditis caused by Mycoplasma pneumoniae.

A 16-year-old girl with myocarditis and hepatitis in the course of mycoplasma pneumoniae infection was reported. She had fever and coughed for ten days prior to admission. At the time of admission infiltrations of the left lower lung field were revealed on the chest X-ray films. The ESR was elevated and CRP+6. There were no leukocytosis and anemia, but S-GOT, S-GPT and LDH were moderately increased. On the 11th day of admission VPC in bigeminy appeared and the third sound was heard. Subsequently biphastic and inverted T waves in leads V2 and V3 and flattening of T waves in leads II and aVF appeared. At the same time, the cardiac shadow was enlarged. Antibody titer to mycoplasma pneumoniae increased to more than 1:640 two weeks after admission and then it decreased gradually. The cold agglutinin test was 1:64 on the 8th day of the disease and then it became normal. ASO, antibodies to DNA and immunoglobulins were normal; ANA, Coombs test and LE test were negative. The abnormal ECG-findings were normalized three months later.
