Peripheral neuropathy with predominantly motor manifestations in a patient with carcinoma of the uterus.

An autopsy case of a 56-year-old woman who had carcinoma of the corpus uteri and peripheral neuropathy with predominantly motor manifestations is described. The neurological abnormalities included subacute weakness of the limbs and loss of deep reflexes, which improved after the surgical removal of the uterine carcinoma. Neuropathologically, peripheral nerves mainly presented features of axonal degeneration with a mild loss of myelinated fibres. Anterior horns of the spinal cord showed central chromatolysis of the motor nerve cells and many spheroids without neuronal loss. Axonopathy of peripheral nerves was considered to be the main pathological process in this paraneoplastic syndrome.

Immunolocalization of cathepsins B, H and L in skeletal muscle of X-linked muscular dystrophy (mdx) mouse.

The amounts of non-collagen proteins (muscle structural proteins) and the activity of creatine kinase were significantly decreased in muscles of 28-day-old mdx mice. The activities of lysosomal thiol proteases such as cathepsins B and L were increased in muscles of mdx mice at as early as 10 days of age. Endogenous thiol proteinase inhibitor and various lysosomal hydrolases also showed increased activities. The localization of cathepsins B, H and L, and endogenous thiol proteinase inhibitor was investigated using the respective specific antibodies. While only invading macrophages were stained strongly with anticathepsin B and H, and anti-thiol proteinase inhibitor antibodies, cathepsin L was localized in muscle cells as well as in invading macrophages. Cathepsin L in muscle cells itself may initially degrade muscle structural proteins, before lysosomal thiol proteases, mainly derived from macrophages, degrade them in skeletal muscles of mdx mice.

Gating behaviors of a voltage-dependent and Ca2+-activated cation channel of yeast vacuolar membrane incorporated into planar lipid bilayer.

A voltage-dependent and Ca2+-activated cation channel found in the vacuolar membrane of the yeast, Saccharomyces cerevisiae, was incorporated into planar lipid bilayer and its gating characteristics were studied at the macroscopic and single-channel levels. The open-channel probability at steady state, which was estimated by the macroscopic current measurement, gave a maximum value at -10 mV and decreased in a graded fashion as the voltage became more positive or more negative. The steady-state voltage dependence was explained by assuming two independent gates, which had different rate constants and opposite voltage dependence. The fast-responding gate opened when the voltage of the cis side (the side to which the vesicles were added) was made more negative and the slow-responding gate behaved in the opposite direction. Relatively high concentrations of Ca2+, about 1 mM, were required on the cis side for opening the slow gate in a voltage-dependent manner. DIDS increased the open-channel probability of the fast gate when added to the cis side, but was ineffective on the slow gate.

Growth and achievement of large- and small-headed children in a normal population.

Growth and school achievement were examined at regular intervals in the 926 children enrolled in the Prospective Longitudinal Study on the Development and Growth in Nagoya City, conducted from 1964 to 1981. Among these children, 10 had head circumferences of more than 2 standard deviations (SD) above the mean and 6 ones of less than 2 SD below the mean at least once in each infancy and adolescence. The heights and weights of the large-headed children were greater than the means, and those of the small-headed children lower than the means. These physical characteristics were already observable at birth. Achievement in primary and secondary schools over 9 years was excellent in 5 (50%) and average in the other 5 of the large-headed children, and average in 5 (80%) and poor in the other one of the small-headed children. Therefore, head size is correlated to both physical growth and school achievement.

Effect of packed cell volume on diastolic coronary artery pressure-flow relations in the dog.

To elucidate the role of the haemorheological properties of the perfusate in the coronary circulation, the diastolic pressure-flow relation was studied in nine open chest heart blocked dogs with minimal vasomotor tone when blood with various packed cell volumes (12-67%) was used as perfusate. An electrical analogue model with proximal resistance R1, capacitance C, distal resistance R2, and the zero flow pressure intercept Pint was derived from the observation of the pressure-flow relation to support the data analysis. The diastolic pressure decay was then determined after the perfusion line had been clamped to calculate stop flow coronary artery pressure (Psf). The stop flow coronary artery pressure decreased in relation to packed cell volume (r = 0.45, p less than 0.01), and the value for the lowest packed cell volume (10-29%) was slightly higher than the great cardiac vein pressure (about 3 mmHg). The zero flow pressure intercept of the steady state pressure-flow relation showed a close correlation with the stop flow coronary artery pressure (r = 0.87, p less than 0.001). The value of R1 + R2, which reflects the inverse of the steady state pressure-flow slope, decreased simultaneously with the packed cell volume (r = 0.62, p less than 0.001). The resistance ratio R2/(R1 + R2) by our model prediction decreased in relation to packed cell volume (r = 0.5, p less than 0.001). The values of stop flow coronary artery pressure, zero flow pressure intercept, and R1 + R2 for the highest packed cell volume (50-69%) were 17.8(1.1) mmHg, 25.1(1.3) mmHg, and 0.48(0.05) mmHg.ml-1.min.100 g-1 respectively, whereas those for the lowest packed cell volume (10-29%) were 13.4(0.8) mmHg, 19.7(1.0) mmHg, and 0.24(0.02) mmHg.ml-1.min.100 g-1. The pressure difference between the stop flow coronary artery pressure and the zero flow pressure intercept may be due to the non-linearity in the pressure-flow relation at a low perfusion pressure. The left ventricular end diastolic pressure and great cardiac vein pressure did not change in relation to the packed cell volume of the coronary perfusate. Thus it is concluded that packed cell volume is one factor determining the high zero flow pressure.

Dysarthria after large doses of intravenous diazepam.

This is a description of a 34-month-old girl with dysarthria after a dosage of 37.5 mg/15 hours of intravenous diazepam (DZP). Dysarthria lasted 180 hours after the final dose. However, her respiration and blood pressure were normal, and disturbance of consciousness was mild and normalized at 77 hours. The clinical correlations of the concentrations of DZP and its active metabolite are discussed.

Acute effects of sodium valproate on epileptic photosensitivity in the lateral geniculate-kindled cat.

Epileptic photosensitivity was acquired as a result of kindling in the lateral geniculate body (GL), and a GL-kindled cat pretreated with DL-allylglycine was found to be a useful model of epilepsy for assessing the efficacy of anticonvulsant drugs. In the present study the acute anticonvulsant effects of sodium valproate (VPA) were examined in the GL-kindled cat under DL-allylglycine. An intravenous injection of VPA at 50 mg/kg induced a protective action against photically induced seizures and paroxysmal EEG activities. This protection persisted for up to 4 hours of the observation period and corresponded to plasma concentrations (61 to 123 micrograms/ml) similar to those considered therapeutic in humans. The present findings are consistent with those obtained in other animal models of photosensitive epilepsy, and lend experimental support to the idea that VPA possesses antiepileptic activity in human photosensitive epilepsy.

Piezoelectric polymer curvature sensor for measurement of regional curvature radius of LV wall.

To evaluate regional myocardial function, we developed a curvature sensor for direct and instantaneous measurement of the regional curvature radius of the LV wall. The sensor is a bimorph of two sheets of thin piezoelectric polymer film. The relation between output voltage of the sensor and the reciprocal of the known curvature radius has been shown to be linear. In anesthetized dogs, we inserted the curvature sensor into the subepimyocardium with a specially designed introducer. Special care was taken to insert it parallel to the epicardial surface. Circumferential regional curvature radius, which is defined as the reciprocal of regional curvature, showed the same phasic changes as the short-axis diameter under control conditions. Under regional ischemia caused by transient occlusion of the coronary artery, amplitude of the phasic change in regional curvature radius decreased, whereas that of the short-axis diameter did not change or slightly increased. Phasic changes in regional curvature radius cannot be estimated from short-axis diameter during regional ischemia. We calculated regional wall tension from the directly measured regional curvature radius and LV pressure and found that tension-length loop clearly differentiates between regional myocardial function under control and ischemic conditions. We have concluded that our newly developed curvature sensor is accurate, that its practical use is feasible, and that measurement of the regional curvature radius of the LV wall provides detailed and accurate information on regional shape and function of the left ventricle.

Comparison of blood-flow velocity waveforms in different coronary artery bypass grafts. Sequential saphenous vein grafts and internal mammary artery grafts.

Characteristics of blood-flow velocities were investigated at different sites in two types of coronary artery bypass grafts, sequential saphenous vein grafts (SSVG) and internal mammary artery grafts (IMAG). The latter appear to have the longest life span. The patency rate of the side-to-side anastomosis of the SSVG is better than that of the end-to-side anastomosis. The SSVG was anastomosed to the major diagonal branch by side-to-side anastomosis and to the left anterior descending coronary artery (LAD) by end-to-side anastomosis in 13 patients who had 75-100% and 75-90% stenoses in the LAD and major diagonal branch, respectively. IMAG anastomoses were performed to the LAD in 10 patients with 75-100% stenoses of the artery. The blood-flow velocities were measured by the 20-MHz, eighty-channel ultrasound pulsed Doppler method during surgery. In six patients in the SSVG group, we investigated the configuration of velocity profiles at the region just proximal to the side-to-side anastomosis and at the bridge portion between the side-to-side and end-to-side anastomosis. In the other seven patients, we measured the blood-flow velocity at several centimeters proximal to the side-to-side anastomosis and compared it with that in the IMAG. At the region just proximal to the side-to-side anastomosis, the velocity profile skewed toward the anastomosis side wall in all patients, and the flow velocity near the wall opposite to the side-to-side anastomosis was reversed in five of six patients.(ABSTRACT TRUNCATED AT 250 WORDS)

Human cytotrophoblastic antigens defined by monoclonal antibodies.

Monoclonal antibodies have been raised against cytotrophoblast. Two different antigens, defined on cytotrophoblast but not on syncytiotrophoblast were designated ACT-1 and ACT-2, respectively. Chorionic villi were taken from normal early pregnancy and processed for immunization by two different procedures. ACT-1 was demonstrated to be present in lung alveolar cells, endothelial mucosa of the jejunum, colon, ureter, urinary bladder and the fallopian tube, and endometrial gland of the pregnant uterus. On the other hand, ACT-2 was present in the endothelial mucosa of the stomach, endothelium of the renal vessel, and the decidua of the pregnant uterus. Although the monoclonal antibodies did not react with such established cell lines as Bewo, SCH, OVK-18, HHUA, MK-01, FL, BHK and P3 X 63Ag 8 . 653, they did react with some of the cell lines when the cell membrane was destroyed with Triton X-100. Each antibody, therefore, may recognize the antigen not on the cell membrane but in the cytoplasm. The antigens might be shed or may disappear in the process of differentiation from cytotrophoblast to syncytiotrophoblast.

Effects of androgen on 17 beta-estradiol production by cultured human granulosa cells.

In order to determine whether androgen acts solely as a substrate for aromatization or whether it also influences on the activity of aromatase enzyme, human granulosa cells were incubated in vitro with or without androgen. Although basal production of 17 beta-estradiol (E2) in cultured granulosa cells obtained from follicles about 26 hr after the initiation of the LH surge was restricted in small quantities, a marked increase in E2 production occurred in the presence of testosterone (T) (10(-6) M) as aromatizable substrate. The non-aromatizable androgen, 5 alpha-dihydrotestosterone (DHT) (10(-7) M, 10(-6) M), slightly enhanced E2 production and it did not inhibit T aromatization in these cells. By contrast, DHT did not increase E2 production in granulosa cells obtained from follicles 2-5 days before the LH surge. The results indicate that androgen can enhance the estrogen biosynthesis of granulosa cells obtained from follicles during the LH surge not only by acting as a substrate for aromatization but also by participating in some process of the estrogen synthesis. This effect of androgen was not clearly seen in granulosa cells obtained before the LH surge, suggesting that the response of granulosa cells to exogenous androgen varies with the stage of the cell differentiation.

Prevalence of human papillomavirus type 18 DNA in adenocarcinoma and adenosquamous carcinoma of the uterine cervix occurring in Japan.

To identify whether the incidence of human papillomavirus (HPV) type 18 DNA in adenocarcinoma and adenosquamous carcinoma is attributable to the histological types or geographical differences, the presence of HPV-16 and HPV-18 DNA in carcinoma of the uterine cervix from Japan were studied by in situ hybridization using tritium labeled HPV DNA probes. HPV-18 DNA was detected in 5 of 11 cases (45%) of adenocarcinoma, one case of adenocarcinoma in situ and 2 of 3 cases of adenosquamous carcinoma. In contrast, HPV-16 DNA was detected in 2 of 11 cases (18%) of adenocarcinoma, and 3 of 7 cases (43%) of squamous cell carcinoma. Compared with our previous results (Tase et al. 1988), the present results imply that the prevalence of HPV-18 DNA in carcinoma of the uterine cervix is attributable rather to the histological differences than to the geographical differences.

Characterization of abnormalities in the gamma-globin gene arrangements of Japanese newborns.

Cord blood samples from 889 healthy Japanese newborns from three districts of Honshu Island were studied with the purpose of characterizing the gamma-globin genes in the Japanese. The A gamma T gene frequency was 0.159 which is the same as that found elsewhere in Japan. The haplotype of the chromosome with the A gamma T gene was [-- ++ - + (+ or -) (+ or -)] at eight polymorphic sites. Data from analyses of DNA and the proportion of gamma chains in Hb F showed the existence of various kinds of gamma-globin gene arrangements; six genotypes were observed for individuals with high G gamma values and triple or quadruple gamma-globin gene arrangements, and seven genotypes for babies with low G gamma values and a single gamma-globin gene. The in vivo expression of the gamma-globin gene located at the third or fourth position in the multiple gene arrangement was found to be about 5% and 2.5% of the total, respectively. The haplotype for the chromosomes with a triple gamma-globin gene was [+ --(-)---- +], but that for the chromosomes with a single gamma-globin gene remains unclear. In addition, a new type of Bgl II polymorphism in the G gamma-globin gene was observed.
