Arterial chemoembolization with mitomycin C microcapsules followed by transcatheter hepatic artery embolization for hepatocellular carcinoma.

Of 66 patients with hepatocellular carcinoma, 32 were treated by intraarterial injection of mitomycin C microcapsules (group A) and 34 patients by intraarterial injection of mitomycin C microcapsules followed by transcatheter hepatic artery embolization (group B). Measurable tumor regression greater than 50% in area on liver image occurred in 28% of group A and 57% of group B (p less than 0.05). A decrease of serum alpha-fetoprotein to less than half the initial level was demonstrated in 59% of group A and 91% of group B (p less than 0.05). The survival rate was better in group B compared with group A (p less than 0.05). These results indicate that arterial chemoembolization with mitomycin C microcapsules plus transcatheter hepatic artery embolization will be an improvement over arterial chemoembolization with mitomycin C microcapsules alone in the treatment of hepatocellular carcinoma.

Fatty liver in rats induced by excessive intake of a nutritionally adequate liquid diet.

In order to test whether or not overeating of a nutritionally adequate diet with reasonable fat content could result in significant fat accumulation in the liver, male Sprague-Dawley rats were provided with free access to either a nutritionally adequate liquid diet with 35 per cent of calories as fat or a regular diet (controls) for 3 months. After the feeding period, body weight, Lee index, and epididymal adipose tissue weight, were significantly greater in rats fed with the liquid diet than in the controls. Liver weight, hepatic triglyceride levels were also greater in the liquid diet group. Histologically, remarkable fatty infiltration was observed predominantly in periportal areas in rats fed with the liquid diet ad libitum for 3 months. Compared to a large body of the literature concerning diet-induced obesity in experimental animals, information on animal models of fatty liver by dietary manipulations is insufficient. The results of this study clearly indicate that the overeating of a nutritionally adequate diet with reasonable fat content could result in remarkable fat accumulation in the liver in rats.

DDAVP in acquired von Willebrand syndrome associated with multiple myeloma.

The response to a single intravenous infusion of 1-deamino-8-D-arginine vasopressin (DDAVP, desmopressin) was studied in two patients with acquired von Willebrand syndrome associated with IgG-kappa myeloma. Following infusion of DDAVP (0.3-0.4 micrograms/kg), prolonged bleeding time was normalized; plasma ristocetin cofactor activity, von Willebrand factor antigen, and factor VIII activity were remarkably increased; and high-molecular-weight forms of von Willebrand factor were demonstrated by crossed immunoelectrophoresis in both patients. Excellent hemostasis was achieved following administration of DDAVP in one patient when it was used for the treatment of gum bleeding and for the prophylaxis of bleeding during and after dental extractions. These observations suggest that DDAVP is an effective alternative to blood products for at least some patients with acquired von Willebrand syndrome in addition to patients with inherited von Willebrand disease, hemophilia A, and uremia.

Skeletal muscle pathology in X chromosome-linked muscular dystrophy (mdx) mouse.

Histological, histochemical, and morphometric analyses were performed chronologically on muscles from mutant mice with X chromosome-linked muscular dystrophy (mdx), and the findings were compared with those in nondystrophic control animals (C57BL/10ScSn). Massive grouped muscle fiber destruction, followed by complete regeneration, occurred abruptly at 20 days of age. There were no preceding changes in body weight, the number and mean diameter of fibers, and fiber type differentiation before the initial episode of muscle fiber necrosis. Muscle fiber necrosis decreased in intensity after 60 days of age. Even after repeated muscle fiber necrosis and regeneration, the most striking finding was that interstitial fibrosis and adipose tissue replacement were minimal, and there was no apparent fiber loss. Since the necrosis was probably well compensated by the active regenerative process, the mdx mice developed no obvious muscle weakness and thus differed from human and other animal muscular dystrophies with the exception of the dystrophic hamster.

Ontogenetic steroidogenesis by testes, ovary, and adrenals of embryonic and postembryonic chickens (Gallus domesticus).

To elucidate the relationship between steroidogenesis and sex differentiation in the chicken, pituitary and plasma LH, plasma, testicular, ovarian, and adrenal testosterone, estradiol-17 beta, estrone, and progesterone; and plasma and adrenal corticosterone and cortisol concentrations from 8-, 10-, 12-, 14-, 16-, 18-, and 20-day-old embryonic and 1-, 3-, 7-, and 14-day-old chicks were determined by radioimmunoassay. The plasma LH level was high in 10- to 12-day-old embryos and decreased with advancing age. No sex difference was observed in plasma, testicular, and ovarian testosterone concentrations at embryonic ages. After hatching, a higher testosterone level was observed in the testes than in the ovary, while a similar testosterone level was observed in the plasma of both sexes. Both plasma estradiol-17 beta and estrone concentrations were significantly higher in the female embryo than in the male embryo. Higher values of the two estrogens were observed in the left ovary of the female embryo than in the testes of the male embryo, and the estradiol concentration in the female embryonic ovary was much higher than the testosterone concentration in the male embryonic testes. No sex difference was observed in plasma progesterone, corticosterone, or cortisol. Both plasma and adrenal corticosterone concentrations reached peaks at hatching time. These results indicate that the chicken embryonic ovary is much more active in production and secretion of sex hormones with special reference to estrogenic hormones than are the embryonic testes. It is strongly suggested that the sex of the avian species is basically male, having homozygosity of sex chromosomes (ZZ), and that the estrogens secreted by the embryonic ovary have important roles in female sex differentiation.

Mechanisms of concealed ventricular bigeminy: the concept of concealed conduction in the reentrant pathway.

To clarify the presence of concealed conduction in the reentrant pathway of extrasystoles, 20 patients with ventricular extrasystoles were studied in whom two forms of interectopic periods were found in the same recording. One form is the XS1S2X period, in which two sinus QRS complexes (S1 and S2) intervene between an interpolated extrasystole (the first X) and the next extrasystole (the second X). The other is the XS2X or XS1X period, in which one sinus QRS complex intervenes between two extrasystoles. In all patients except one, the XX interval in the XS1S2X period was longer than that in the XS2X or XS1X period though shorter than twice the latter XX interval. This strongly suggests the presence of two-level block in the reentrant pathway of the extrasystoles. It appears that the sinus impulse S1 in the XS1S2X period invaded a large portion of the reentrant pathway and then was blocked at a distal site of the pathway; namely, that concealed conduction of the impulse S1 occurred in the pathway. It is suggested that such concealed conduction prolonged the conduction time of the following sinus impulse, S2, in the reentrant pathway, resulting in lengthening of the XX interval. The presence of three- or four-level block is also suggested. By the use of such multilevel block, mechanisms of concealed ventricular bigeminy are explained.

[Results of specular microscopy in 5 cases of the iridocorneal endothelial syndrome].

Five cases of iridocorneal endothelial syndrome have been examined with grand angle specular microscope. The first case presented normal appearance unlike the second case which showed diminution of the cellular density with 1.060/mm2 in comparison with 3,487/mm2 in the fellow eye. Pleomorphism and irregular size of endothelial cells associated with excrescences were noted in case 3 and 4. No endothelial cell could be observed in case 5: the authors introduce the possibility that time changes in corneal endothelium could be masked by a predescemetic membrane.
