Myopathy in Marinesco-Sjögren syndrome: an ultrastructural study.

Seven muscle biopsies from patients with the clinical characteristics of Marinesco-Sjögren syndrome (MSS) revealed myopathic changes of two types; muscle fiber necrosis followed by regeneration and focal myofibrillar degeneration inducing autophagocytosis with rimmed vacuole formation. In two young patients, massive muscle fiber necrosis with phagocytic invasion was the predominant feature and autophagic phenomenon was minimal, resembling the findings in progressive muscular dystrophy. Myofibrillar degeneration with autophagic phenomenon was prominent in five adult patients. The coexistence of these two degenerative processes and the secondarily induced reactive changes of muscle fiber hypertrophy, interstitial fibrosis, occasional ragged-red fibers and type 1 fiber predominance, are responsible for the wide spectrum of muscle pathology in MSS. The dense double-membrane structure surrounding myonuclei, previously reported as being specific to MSS, was present in only one biopsy.

Immunohistochemical study of a rat membrane protein which induces a selective potassium permeation: its localization in the apical membrane portion of epithelial cells.

We previously reported a novel rat membrane protein that exhibits a voltage-dependent potassium channel activity on the basis of molecular cloning combined with an electrophysiological assay. This protein, termed IsK protein, is small and different from the conventional potassium channel proteins but induces selective permeation of potassium ions on its expression in Xenopus oocytes. In this investigation, we examined cellular localization of rat IsK protein by preparing three different types of antibody that specifically reacts with a distinct part of rat IsK protein. Immunohistochemical analysis using these antibody preparations demonstrated that rat IsK protein is confined to the apical membrane portion of epithelial cells in the proximal tubule of the kidney, the submandibular duct and the uterine endometrium. The observed tissue distribution of rat IsK protein was consistent with that of the IsK protein mRNA determined by blot hybridization analysis. In epithelial cells, the sodium, potassium-ATPase pump in the basolateral membrane generates a sodium gradient across the epithelial cell and allows sodium ions to enter the cell through the apical membrane. Thus, taking into account the cellular localization of the IsK protein, together with its electrophysiological properties, we discussed a possible function of the IsK protein, namely that this protein is involved in potassium permeation in the apical membrane of epithelial cells through the depolarizing effect of sodium entry.

Generation of transgenic mice with elevated blood pressure by introduction of the rat renin and angiotensinogen genes.

The role of the renin-angiotensin system in blood pressure control and in the development of hypertension was investigated by generating transgenic mice carrying the rat renin or angiotensinogen gene or both genes under the control of the mouse metallothionein I promoter. The systolic blood pressure was significantly elevated in transgenic mice carrying both transgenes but was maintained normally in those bearing either of the transgenes. The transgene was effectively and properly transcribed to form the mature mRNA in the transgenic mice. The production of rat renin and angiotensinogen in the transgenic mice carrying the corresponding transgene was also verified by immunoanalyses of these proteins. Furthermore, the specific angiotensin-converting enzyme inhibitor captopril was effective in reducing the elevated blood pressure of the hypertensive transgenic mice. These results indicate that the combined action of the exogenous rat renin and angiotensinogen is responsible and necessary for elevation of blood pressure in the hypertensive transgenic mice.

Clinical characteristics and possible role of coronary artery spasm in syncope and/or aborted sudden death.

We investigated the clinical and pathophysiologic characteristics in patients with vasospastic angina who developed syncope and/or experienced aborted sudden death (SD). Vasospastic angina was diagnosed using the methylergonovine test. Syncope was found in 32 (10.4%) patients among 309 who were admitted to our institute in a one-year period. The most frequent cause of syncope was ventricular tachycardia which was found in 10 (31.2%) of the 32 patients. The next important cause of syncope was vasospastic angina which was found in 7 patients (21.8%). Among the 7 patients with vasospastic angina who experienced one or more syncopal episodes, there were 3 patients with aborted SD, 3 with syncope and one with shock. Cardiovascular collapse was observed in 4. Interior wall ischemia was found in 5 and anterior wall ischemia in 2 during the methylergonovine test. None of the 7 patients had significant coronary stenosis. Two patients had no prodromal symptom such as chest pain. Our results suggest that coronary artery spasm may be one of the most frequent cardiovascular diseases that causes syncope which is not always accompanied by a prodromal symptom. Therefore, coronary spasm should be distinguished in patients with unexplained syncope or aborted SD.

[Congenital anomalies of the coronary arteries with associated ischemic ST depression on exercise: a report of three cases].

This report presented evidence of myocardial ischemia as the etiology of angina pectoris in three patients with congenital anomalies of the coronary arteries but without arteriosclerotic disease. All of three cases showed angina pectoris and ST depressions on their exercise electrocardiogram. Case 1: This 58-year-old man developed angina pectoris at the age of 50 years. His treadmill exercise test precipitated chest pain and ST depression. His coronary arteriograms disclosed an ectopic origin of the right coronary artery just anterior to the origin of the left coronary artery in the left coronary sinus. No significant atherosclerotic stenosis was present. An apparent ischemic manifestation appeared to be caused by compression of an aberrant right coronary artery between the aorta and the right ventricular infundibulum. Case 2: A 49-year-old woman had a history of angina. Her treadmill exercise test induced chest pain and an abnormal exercise electrocardiographic finding. Her coronary arteriograms revealed a single left coronary artery. Insufficient perfusion was postulated as a cause of apparent myocardial ischemia in this case though angiographically, there was adequate perfusion. Case 3: This 31-year-old man had a six-year history of angina. His treadmill exercise electrocardiograms revealed ischemic changes accompanied by chest pain. Coronary arteriograms disclosed a coronary artery fistula. The ischemic manifestation was apparently caused by inadequate perfusion due to coronary steal. With the increasing use of coronary arteriography, unusual origins and courses of coronary arteries will be more frequently encountered. Precise knowledge of anomalies is prerequisite for evaluating variations in the location of the coronary artery ostia and their statistical probabilities.

[Effectiveness of TRH analog in a case of early infantile epileptic encephalopathy].

We reported a case of early infantile epileptic encephalopathy (EIEE) due to perinatal hypoxic-ischemic brain damage. This infant had frequent brief tonic seizures in series accompanied by apnea and bradycardia since two days after birth. Her EEG showed a typical suppression burst pattern. We administered PB. VPA and CZP, but could not control her seizures at all. Then, a thyrotropin releasing hormone (TRH) analog, in addition to PB and VPA, was administered intravenously (0.5 mg/day) to her. Three weeks after the administration of this TRH analog, her seizures ceased completely. We suggest that TRH analog therapy should be considered as one of therapeutics for EIEE whose prognosis is expected to be poor.

[Modified microdiffusion method for fluoride analysis of foodstuffs].

In fluoride analysis, there are two procedures for the determination of fluoride depending on nature of samples. One procedure directly can determine fluoride contents by fluoride ion electrode, dissociating fluoride from the sample with the acid (perchloric acid). The other procedure needs to separate fluoride from biological sample using steam-distillation or micro-diffusion method, and then the determination by means of fluoride ion electrode can be applied. In this study, we applied a modified microdiffusion method by Hinoide (1990) for its availability in different temperatures and reaction time, and then estimated to determine fluoride contents of Shrimp (Sakura-shrimp) as a biological sample by this method. It is also very important to determine fluoride contents of foodstuffs and biological samples with regard to bioavailability of fluoride in vivo. The results were as follows: 1. Using the modified microdiffusion method, the fluoride recovery rate attained more than 95% in the range of 0.1-10 micrograms fluoride from a standard solution under the experimental condition of 60 degrees C and 90 degrees C of reaction temperatures and 60, 90 and 120 minutes of reaction time. 2. For the sample of shrimp, fluoride contents analyzed by means of the modified micro-diffusion and steam-distillation methods showed almost similar values, although the fluoride contents by steam-distillation method revealed relatively lower value. The difference might be derived from a loss of fluoride in ashing the sample before applying the steam-distillation method. The modified microdiffusion method used in this experiment can be utilized routinely and precisely in fluoride analysis for small amounts of biological sample.

[Clinical experience of percutaneous transluminal angioplasty and expandable metallic stent placement for Budd-Chiari syndrome with massive thrombus in the inferior vena cava; report of a case].

A 72-year-old female who had suffered from lower extremity edema and liver cirrhosis due to Budd-Chiari syndrome with massive thrombus in IVC was treated by PTA and placement of self-expandable metallic stent. Thrombolysis therapy after PTA was very effective. Five months later, she had no clinical symptom due to IVC stenosis.

[Cerebellar infarction: analysis of 33 cases].

During the past 9 years and 2 months we have encountered 33 cases with cerebellar infarction. These patients were classified into 3 types according to their clinical course. Type 1 (18 cases); The course was benign, and symptoms and signs improved without surgical treatment. Type 2 (11 cases); The course was progressive with deterioration of consciousness between 24-72 hours after the onset. Type 3 (4 cases); The course was rapid resulting in lapse into coma within a few hours. Also, its prognosis was fatal due to coexisting brain-stem infarction regardless of any treatment. Surgical intervention was required for Type 2 in which the lesion included the region of the vermis or occupied more than one-third of the cerebellar hemisphere, and had subsequently compressed the brain-stem and caused obstructive hydrocephalus. In 9 cases out of Type 2, ventricular drainage alone was performed and prompt improvement of consciousness level was detected except in one case. We consider that ventricular drainage is not so invasive a method, and it is beneficial. However, in 2 cases of Type 1 and 4 cases of Type 2, hemorrhagic infarction occurred. Thus, one should be aware of the possibility of hemorrhagic infarction, even though it may be asymptomatic infarction. If prompt improvement of consciousness is not detected after ventricular drainage, suboccipital craniectomy should be recommended.

[Successful treatment of adult T-cell leukemia with interferon-alpha-2b by systemic and local administration].

A 59 years old woman, born in Fukuoka Prefecture, was admitted to our hospital in Aug, 1988 because of diarrhea, fever and skin eruption. Physical examination revealed systemic lymphadenopathy and hepatosplenomegaly. The white blood cell count was 11,200/microliters with 28% atypical lymphocytes with convoluted nuclei. Mild anemia, thrombocytopenia and hypercalcemia were also observed. Antibody against the adult T-cell leukemia (ATL) associated antigen in serum was positive. OKT 4/8 ratio was high. A diagnosis of ATL was made. Because of the complications of pneumonia and herpes simplex, systemic chemotherapy was not given, and interferon (IFN)-alpha-2b was intramuscularly injected daily from Oct, 1988, resulting in the disappearance of atypical lymphocytes and improvement of skin lesions. The effect of IFN therapy lasted for three months, followed by increase of atypical lymphocytes. Although the patient became refractory to systemic IFN therapy, local injection of IFN into a buccal tumor infiltrated with atypical lymphocytes resulted in its regression of size. In spite of continued administration of IFN, the patient died of pneumonia in Jan, 1989.

[Long-term results of cerebral arteriovenous malformations--surgical and conservative treatment].

During the past 20 years, we have treated 187 cases with cerebral arteriovenous malformations (AVMs). These 134 patients were treated surgically and the remaining 53 patients were managed non-surgically. The purpose of this study is to clarify the surgical indications of AVMs, based on a comparison of the long-term follow-up results between the surgical and nonsurgical groups. Consequently, good results were obtained in the surgical group: the operative mortality was 7.5% and the morbidity was 12.7%. The long-term follow-up results were as follows: Of the 124 followed cases in the surgical group, there were 16 cases with improvement of neurological deficits (13%) and 3 cases with neurological aggravation (2%). Of the 47 followed cases in the nonsurgical group, there were 5 cases with improvement of neurological deficits (11%) and 6 cases with neurological aggravation (13%). As for the incidence of AVM hemorrhage, there were 8 cases with hemorrhage in the nonsurgical group including 4 cases (3 rebleeding cases) which were fatal (11%), in contrast to one case (subtotal resection) in the surgical group. On follow-up angiography, the size of AVMs were unchanged in the majority of cases in the nonsurgical group, with the exception of 2 cases in which there were slight regressions. In the surgical group, on the other hand, enlargement in 2 cases and spontaneous regression in 3 of residual AVM occurred following incomplete resection. Particularly in 2 cases involving unresectable AVMs, a newly formed aneurysm was discovered in the follow-up angiography, emphasizing the fact that serious follow-up angiography is required in cases of untreated AVMs.

[Experience of expandable metallic stent for tuberculosis bronchial stenosis].

A 41-year-old female who had suffered from a severe left main bronchial stenosis due to bronchial tuberculosis was treated by implantation of self-expandable metallic stent after dilating the stenosis by balloon catheter. After placing the stent symptoms such as dyspnea were disappeared completely. Four months later, the patient can tolerate to do jogging.

Clinical features and prognosis of hepatocellular carcinoma with reference to serum alpha-fetoprotein levels. Analysis of 606 patients.

Clinical features of hepatocellular carcinoma (HCC) with normal serum alpha-fetoprotein (AFP) levels were compared with those of AFP-positive cases. A total of 606 patients were divided into four groups based on their serum AFP levels at the time of diagnosis: group 1 (less than or equal to 20 ng/ml, N = 125), group 2 (20-1000 g/ml, N = 256), group 3 (1000-10000 ng/ml, N = 149), and group 4 (greater than 10000 ng/ml, N = 76). Increasing prevalence of group 1 patients and decreasing prevalence of group 4 were noted during the last 9 years. The proportion of hepatitis B virus-positive cases was significantly lower in group 1 than in group 4. The serum glutamic oxaloacetic transaminase/serum glutamic pyruvic transaminase ratio was found to be significantly higher in group 4 than in group 1 regardless of the size of the tumors. The survival rates were compared among the four groups in size matched cases since the size of the tumor significantly influenced their prognosis. The median survival in relatively small HCC patients (less than or equal to 5 cm in diameter, N = 199) were 24.6 months for group 1, 20.6 months for group 2, and 13.7 months for group 3 patients; and those in large HCC (greater than 50% in the proportion of the tumor area, N = 200) were 15.1 months for group 1, 6.3 months for group 2, 5.8 months for group 3, and 5.2 months for group 4. Thus, serum AFP values at the time of presentation are not only of diagnostic value, but also of prognostic significance in patients with HCC.

Influence of sinus impulses on the parasystolic cycle length.

Recently, it has been shown that in most clinical cases of parasystole, the parasystolic rhythm is not completely independent of the sinus rhythm. In this study, to disclose the mechanism of such "irregular" parasystole, parasystolic cycles with an intervening sinus QRS complex (XSX) were compared with their immediately adjacent pure parasystolic cycles without any intervening nonparasystolic QRS complexes (XX) in 10 cases of ventricular parasystole. In eight cases, the XSX interval was equal to or nearly equal to the adjacent pure XX interval; in one, the XSX interval was shorter than the XX interval; and in only one, the XSX interval was longer than the XX interval. In six cases in which the XSX interval was almost equal to the XX interval, calculated XSX intervals with a later intervening sinus QRS complex were obtained from the differences between the XSSX interval (ie, interectopic interval with two intervening sinus QRS complexes) and its adjacent XX interval. In five of the six cases, the calculated XSX interval was shorter than the XX interval. These observations suggest that in most cases of parasystole, early intervening sinus impulses do not change the parasystolic cycle, whereas late intervening sinus impulses shorten the parasystolic cycle. This suggests the presence of type I second-degree entrance block as the mechanism of "irregular" parasystole.
