Post-translational processing of tumor necrosis factor production.

To elucidate the mechanism of tumor necrosis factor (TNF) production, we analyzed proteins produced in macrophages sharing the epitope of TNF according to the priming and triggering of TNF production. Rabbit alveolar macrophages primed with Bacillus Calmette-Guerin (BCG) were isolated and cultured in vitro with 35S-methionine, and the proteins produced were analyzed using anti-rabbit TNF monoclonal antibody. Primed with BCG, alveolar macrophages synthesized two proteins with molecular sizes of 50 and 17 kilodaltons (kDa) (p50 and p17) sharing the same epitope with mature TNF within the cells. These two proteins were released into the medium where other proteins were detected without TNF-activity. Cultured with lipopolysaccharide (LPS triggering), the primed alveolar macrophages released TNF-activity into the medium where p17 together with many larger proteins was detected by immunoprecipitation. In vitro translation of messenger ribonucleic acid (mRNA) from BCG-primed macrophages showed that primary TNF has a molecular size of 28 kDa (p28). These results suggest that active TNF of p17 is secreted when triggered via post-translational processing of the precursor molecules synthesized through priming with BCG.

Aortitis syndrome, coronary artery ectasia, and mid-ventricular obstruction.

A 46-year-old Japanese woman, who had been diagnosed as having aortitis syndrome 4 years earlier, was admitted to our hospital in May 1989. The diagnosis of aortitis syndrome was confirmed by intravenous digital subtraction angiography which showed stenotic lesions in each subclavian artery, the left common carotid artery, and the descending aorta. Coronary arteriography revealed diffuse and prominent dilatation of entire coronary artery segments. Moreover, a left ventriculogram showed complete obstruction of the mid-ventricle during systole. Thus, we diagnosed this case as aortitis syndrome complicated by coronary artery ectasia and mid-ventricular obstruction. The causal relations of these findings are discussed.

Hypereosinophilic syndrome associated with obliterative left ventricular chamber and systemic obliterative arteriosclerosis.

An extremely rare case of hypereosinophilic syndrome is reported. The patient had congestive heart failure due to left ventricular obliteration by a giant thrombus in the apex. Arteriosclerosis obliterans resulted in serious lower extremity gangrene. Combination therapy with a corticosteroid and hydroxyurea caused a marked reduction in eosinophil count, and heart failure recovered in stages. Although the patient was discharged, he returned to our hospital 3 months later with acute myeloblastic leukemia and subsequently died of respiratory failure with pneumonia.

[A case of small cell carcinoma originating at the site of pneumoplication of giant bulla five years later].

A 64 year-old man was performed pneumoplication for his left-sided giant bulla on December 15, 1983. After five years, consciousness loss and right hemiplegia occurred. Brain CT revealed a cystic lesion with enhancement in the left occipital lobe. Brain biopsy disclosed metastatic small cell carcinoma. Lung biopsy showed the same histopathology, so primary lesion was considered to be from the site of the previous pneumoplication. Lung cancer associated with emphysematous bulla have been mostly characterized by tumor shadow or niveáu in the bulla. In this case, the postoperative infiltrating shadow of pneumoplication had made early diagnosis difficult.

Muscle weakness after anterior cruciate ligament reconstruction using patellar and quadriceps tendons.

Eighty-five knees of 85 patients who underwent anterior cruciate ligament reconstruction with autologous patellar and quadriceps tendon grafts were followed for 3-7 years. Hamstrings strength improved to normal. Quadriceps strength in men returned to the preoperative level, but not to normal strength. Quadriceps strength in women, at the final follow-up, was significantly less than it had been preoperatively. This weakness may be a consequence of impairment of the knee extensor mechanism resulting from harvesting the graft. Although favorable postoperative stability of the knee was obtained, we do not recommend this technique for ACL reconstruction because of the quadriceps weakness.

Long-term evaluation of high tibial osteotomy for medial osteoarthritis of the knee.

Fifty-five medial osteoarthritic knees in 50 patients who had undergone high tibial osteotomies (HTO) were followed up twice: once, six years after surgery, and again, 10-15 years postoperatively. The results were satisfactory in 85% of the knees at the six-year follow-up and in 62% of the knees at the 10-year follow-up. The results post 10 or more years had deteriorated significantly compared to those seen six years postoperatively. In patients maintaining excellent improvement of knee function post 10 or more years, the mean femorotibial angle (FTA) one year after surgery had been 165 degrees; this value remained constant for 10 years postoperatively. The authors believe that an FTA of 164 degrees - 168 degrees should be set as a goal when performing a high tibial osteotomy to ensure favorable long-term results in HTO.

[Bilateral occipital infarction with central homonymous hemianopia].

A 45-year-old man of bilateral occipital infarction with central homonymous hemianopia is reported. He was admitted to our hospital with complaints of visual loss and large central scotoma on both eyes. Pupillary light reaction and ocular fundi were normal. On admission, bilateral retrobulbar optic neuritis was suspected. However, congruous, irregular central scotomas with vertical step were observed in both eyes with Goldmann perimetry. X-ray computed tomography (CT) and magnetic resonance imaging (MRI) revealed the infarction in bilateral occipital tips. Bilateral central homonymous hemianopia is considered to be rare and a case in which occipital lobe lesions were detected with X-ray CT and MRI has not been reported. We stress that bilateral central scotomas with vertical step indicate bilateral occipital lesions.

[Analysis of 7 cases with intracavernous aneurysm].

Intracavernous sinus aneurysm is rare and consists of approximately 3% of intracranial aneurysm. Since this aneurysm increases gradually in size and causes various neurological signs, differential diagnosis is necessary. In this report, seven cases of intracavernous sinus aneurysm were analyzed. In conclusion, oculomotor or abducens nerve palsy were seen in most cases. Though trigeminal nerve palsy was suspected in many cases, it was actually seen only in one case. Most cases developed slowly and progressively. One case showed an acute onset and the differential diagnosis from internal carotid-posterior communicating aneurysm was needed. To diagnose intracavernous sinus aneurysm, computed tomography scan was very valuable.

[Spontaneous carotid-cavernous sinus fistula; analysis of 16 cases].

We have encountered 16 cases with spontaneous carotid-cavernous sinus fistula. According to the classification reported by Barrow, one case was type A; direct shunt between the internal carotid artery (ICA) and the cavernous sinus (CS), 5 were type B; dural shunt between meningeal branches of the ICA and the CS, 6 were type C; dural shunt between meningeal branches of the external carotid artery (ECA) and the CS, and 4 were type D; dural shunt between meningeal branches of both ICA and ECA and CA. Of all cases, 8 patients with low-flow fistula treated conservatively improved spontaneously. Three patients were treated with irradiation. Consequently, good results were obtained in 2 cases, but no improvement could be obtained in the remaining one with high flow fistula. Another four patients were treated with intravascular embolization via the ECA, and their symptoms improved. But one patient treated with Ivalon embolization died because of complicated pulmonary embolism. As spontaneous CCF had a high rate of spontaneous regression of symptoms, conservative treatment such as Matas maneuver or irradiation should be recommended at first for low flow cases in type B, C, and D. Surgical therapy such as intravascular embolization should be carried out for high-flow cases in type C and D.

Endotracheal expandable metallic stent placement in dogs.

Various types of Gianturco zig-zag wire stent were implanted into the tracheas of 4 dogs to define the suitable characteristics of the endotracheal wire stent in these animals. The stents were constructed of 0.45, 0.40, and 0.33 mm stainless steel wire. The diameter of the fully expanded stents was 3 cm and their lengths were 2, 3, and 4 cm. The 2 cm stent constructed of 0.33 mm wire showed minimum pathologic changes of the trachea of the dog compared to the other stents, and at the same time had a complete covering of ciliated columnar epithelium over the stent surface.

[Experimental study of the intraurethral placement of expandable metallic stents].

Various types of Gianturco expandable metallic stents were implanted into the urethra of four dogs in order to evaluate the suitable expandability of the stents in urethra and also to determine the effect of the stents on the urethral wall. The stent of 1.5 cm in length and constructed of 0.010 inch round stainless steel wire with twelve zigzag bends showed the suitable expandability on the canine urethra compared to the other stents. The urethra remained patent and the inflammatory changes on the urethral wall were moderately noted. No hematuria or calcifications around the stents were noted in any dogs. The experimental data showed a potential clinical application.

[Self-expandable metallic stent for use in venous occlusions].

Expandable metallic stent was successfully introduced to 5 patients including idiopathic obstruction of the hepatic portion of the inferior vena cava (1 case), superior vena cava obstruction from metastatic lymphnodes swelling (2 cases) and IVC obstructions from liver metastases or lymphnodes metastases (2 cases). The obstructions were 4-14 cm long in the case of venous obstructions due to lymphnodes metastases and 2 cm long in the case of idiopathic hepatic IVC obstruction. In all cases, the stents were introduced by using 12-16 Fr catheter percutaneously without any complications. The stent used was connected up to 7 pieces by using wire struts depending length of the obstructions. However the stents connected more than 5 pieces were difficult to advance through the catheter. The patients who had been treated with stents had been free from hazardous complications such as lower extremity swelling, and pain.

[Fundamental experiment of expansile force of expandable metallic stent].

So far expansile force of expandable metallic stent has been estimated by only wall-distending pressure P. We estimated expansile force of stent by not only P but also a force per one unit length of wire F. F corresponds to the degree of digging into the vessel wall. P and F were thought to be useful in selecting the best stent.

Two cases of disseminated Trichosporon beigelii infection treated with combination antifungal therapy.

Two patients with acute leukemia in whom disseminated Trichosporon beigelii infection developed are reported. The T. beigelii infection developed in the first patient while he was receiving 5-fluorocytosine. He was treated with amphotericin B in addition to 5-fluorocytosine. Despite the continued antifungal therapy, multiple organs were invaded by the organisms at autopsy. The second patient was treated with miconazole and norfloxacin. Although this combination antifungal therapy seemed to be effective, this patient required splenectomy for cure of the infection.

Prediction of preserved flow to the infarct area based on admission electrocardiogram in anterior wall acute myocardial infarction.

To determine whether preserved flow to the infarct area could be predicted from the admission electrocardiogram and to define the effect of preserved flow on the late results after reperfusion, 20 anterior myocardial infarction patients who were successfully reperfused were studied. Patients were divided into 3 groups: (1) no-flow group (8 patients), with an occluded infarct-related artery and no easily visible collaterals; (2) intact collateral group (6 patients); and (3) subtotal obstruction group (6 patients). From the admission electrocardiogram, the sum of ST-segment elevation (sigma ST), the sum of R-wave amplitude (sigma R) in leads V1 through V6 and the ratio of these (sigma R/sigma ST) were measured. There was no significant difference in sigma R among the 3 groups. The no-flow group had significantly lower sigma R/sigma ST and higher sigma ST than the intact collateral group or subtotal obstruction group. All patients (6 of 6) with subtotal obstruction and all except 1 patient (5 of 6) with intact collateral showed sigma R/sigma ST greater than 2.5 or sigma ST less than 2.0 mV. All patients (8 of 8) with no flow showed sigma R/sigma ST less than or equal to 2.5 and all except 1 patient with no flow (7 of 8) showed sigma ST greater than or equal to 2.0 mV. The regional wall motion was assessed by the radial method at 4 weeks. The mean percentage systolic shortening in the anterior and apical regions was significantly correlated with sigma R/sigma ST (r = 0.75, p less than 0.001) and sigma ST (r = -0.65, p less than 0.005).(ABSTRACT TRUNCATED AT 250 WORDS)

[Experimental study of endotracheal expandable metallic stent placement].

Various types of Gianturco zig-zag wire stents were implanted into the trachea of three dogs in order to evaluate the suitable conditions of the endotracheal wire stent. The stents were constructed of 0.018, 0.016 and 0.013 inch stainless steel wire respectively. The relaxed diameter of the stents was 3.0 cm and their length were 2 cm, 3 cm, and 4 cm respectively. The stent of 2 cm in length and constructed of 0.013 inch showed minimum pathologic changes to the trachea of the dog compared to the other stents, and also showed the covering of ciliated, columnar epithelium over the stent surface. The experimental data showed that it was possible to use the stent in the tracheobronchial tree of dogs and also showed a potential clinical application.

Possible role of coronary artery spasm in unexplained syncope.

Coronary spasm provocation by intracoronary methylergonovine was performed in 14 patients (8 men and 6 women, mean age 56 +/- 6 years) with syncope that remained unexplained despite neurologic and noninvasive cardiac evaluations. Electrophysiologic testing was also performed in 6 of 14 patients. No patient had structural heart disease or significant fixed stenosis of greater than or equal to 75% in the coronary arteries. Six patients had no history of chest pain even when they developed syncope. Serious arrhythmia was documented in 2 patients, cardiac standstill in 1 and complete atrioventricular block in the other. Coronary spasm was induced in 9 patients using the methylergonovine provocation test. Multivessel spasms were found in 3 patients. Coronary spasm was induced in the artery supplying the inferior wall in 7 of 9 patients with positive results. In 4 of 9 patients who had a positive result, there was no prior history of chest pain. In 1 patient, whose electrocardiogram was recorded during syncope, cardiac standstill was documented and cardiac standstill and syncope also occurred during the provocation test. Monomorphic ventricular tachycardia was not induced by the electrophysiologic study. These results suggest that coronary spasm is involved in unexplained syncope.
