Sixteen surgical resected cases of "enterocolitis unclassified" diagnosed on the first pathological examination were re-examined. Their incidence was 3.0% of non-neoplastic bowel diseases registered in our department. On the re-examination including additional microscopic sections, histological reconstruction, special stainings and precise clinical data of each lesion, the 16 lesions were macroscopically divided to nine types and re-diagnosed as follows: (1) definite diseases: ulcerative colitis and phlegmonous enterocolitis, (2) probable diseases: intestinal tuberculosis and infective enterocolitis, (3) possible diseases: healed ischemic colitis, healed ischemic enteritis, healed infective colitis, and healed Crohn's disease or healed ulcerative colitis, (4) enterocolitis unclassified in a narrow sense. In conclusion, patho-morphologically the 16 lesions of enterocolitis unclassified might be subdivided into 13 lesions with the similarity to the already known inflammatory bowel diseases and three lesions with enterocolitis unclassified in a narrow sense.
A 39-year-old woman was admitted to our hospital because of dry cough. She had been quite well until 1 month before admission. She had no history of chronic sinusitis. Chest X-ray film showed a thin-walled cavity in the superior segment of the left lower lobe of the lung. Curettage biopsy of the cavity wall revealed granulomatous change. Mycobacterium intracellulare-avium complex was identified on sputum culture. Anti-tuberculous therapy was given and the cavity disappeared in 3 months. Since atypical mycobacterial infection is quite unusual for a healthy woman to contract, we performed high-resolution CT (HRCT) in search for occult abnormalities of the lung. HRCT detected a lot of thin-walled cystic lesions, and bronchography showed that they were consistent with cystic dilatation of relatively large bronchi, which ballooned up during inspiration, and collapsed during expiration. Airways peripheral to these dilatations were also visible, and were quite different from usual cystic bronchiectasis formed by acquired recurrent infections. Her bronchiectasis is most likely congenital in its etiology, and might be considered as an adult case of Williams-Campbell syndrome.
Epstein-Barr virus (EBV), a ubiquitous human herpesvirus and an aetiological agent of infectious mononucleosis, has a unique tropism for B lymphocytes. Clinical and laboratory features of chronic active EBV infections are chronic or persistent infectious mononucleosis-like symptoms and high antibody titre against early antigens (EA). Kawasaki disease (KD), aetiology unknown, is thought to be self-limited immunologically mediated vasculitis. Clinical features of KD are fever, rash, mucositis, lymphadenopathy and coronary artery damage. We report here a child with chronic active EBV infection accompanied by dilatation of coronary arteries. All the EBV-determined nuclear antigen (EBNA)-positive lymphocytes had exclusively CD4 antigen, as revealed by dual staining immunofluorescence analysis. Southern blot hybridization showed that the purified CD4+ cells harboured EBV genome.
When rat basophilic leukemia (2H3) cells were stimulated by higher oligomer, the chemically cross-linked oligomers of IgE, in the presence of calcium the activity of histidine decarboxylase (HDC, L-histidine carboxylase, E.C.4.1.1.22), a histamine-forming enzyme, was increased by 1 hr, reaching maximum activity by 2 hr, and returning to the original level by 8 hr. A similar increase in enzyme activity was observed in cells treated with phorbol myristate acetate (PMA) or oleoyl-acetylglycerol (OAG), which are known activators of protein kinase C. Removal of calcium from medium abolished the increase in HDC activity in response to higher oligomer but not that induced by PMA or OAG, suggesting that the increase in HDC activity may be mediated by protein kinase C. The increase in the HDC activity probably required induction of enzyme synthesis, because it was prevented by cycloheximide.
A 54-year-old man with a right hemiparesis was found to have an intrasellar intercarotid communicating artery associated with agenesis of the right internal carotid artery. Magnetic resonance imaging (MRI) studies demonstrated the spatial relationship of the anomalous artery to the surrounding structures, thus suggesting an embryonic enlargement of the capsular artery as a source of this anomalous artery.

We reported the result of our discriminant analysis concerning prognostic factors in differentiated thyroid cancer, which based on 134 cases obtained from three hospitals. The sensitivity, specificity and false-negative rate were 59%, 61% and 22%, respectively. When these values were estimated individually in each hospital, the figures were better than those obtained from a whole group of patients. The purpose of this paper is to assess the reliability of the previous result. Simulation was done to assess the relationship between the number of patients and the ratio of criterion variables. New groups were made from the 134 patients by random sampling. In each group, discriminant analysis was done by using the same nine explanatory variables as in the previous report (age, sex, diameter of tumor, site of tumor, histology, local invasion, lymph node metastasis, operative method and lymph node dissection). The results revealed that sensitivity was relatively stable but specificity and false-negative rate were better in the smaller group of patients. The ratio of criterion variables had an effect on the results. In the higher ratio, sensitivity was higher and the false-negative rate was lower.

Ten cases of sigmoid colocystoplasty carried out over a 19-year period have been reviewed. The indications were bladder gangrene (1), interstitial cystitis (2), bladder fibrosis (1), failed vesico-vaginal fistula (1), tuberculous contracture (2), atonic bladder (1) and invasive bladder carcinoma (2). All patients void with control every 2 to 4 h throughout the day and twice at night. There has been no long-term deterioration of the upper tract and no delayed complications.
In clinical liver transplantation, a venous bypass that rechannels the blood flow from the inferior vena cava and portal vein to the superior vena cava has been used to maintain the venous return. However, the usage of mechanical shunt has given rise to derangements of blood coagulability and fibrinolysis. Therefore, changes of coagulability and fibrinolysis during the venous bypass were examined in dogs using a centrifugal pump (Bio-Pump), and the effect of gabexate mesilate for coagulation were studied. Venous bypass from the inferior vena cava and the portal vein to the external jugular vein was performed in mongrel dogs (group A: bypass without pump, group B: bypass with the Bio-Pump, group C: bypass with the Bio-Pump and the addition of gabexate mesilate). In group A, blood pressure gradually decreased, but in groups B and C it maintained the pre-bypass level. All results of coagulation parameters and fibrinolytic measurements of group B were not significantly different from those of group A; only partial thromboplastin time was prolonged in group B. However, this prolongation was effectively prevented by the addition of gabexate mesilate in group C.

To determine the utility of injection of polymers to obliterate the pancreatic duct, we examined this method and others viz, pancreatico-duodenostomy, duct ligation, and the open duct method. We performed the pancreatic duct treatment on the left portion of the pancreas in dogs without any transplantation. The right portion of the pancreas was removed. We used Neoprene and Ethibloc as injected materials. One month after the operation, the polymer injected dogs showed the same endocrine function as the duct ligated and open duct ones. However, three months after the operation, endocrine function of the polymer injected dogs improved and was better than that of the other two groups which showed no changes. The endocrine function of the polymer injected dogs had remained stable for one year and was close to that of the optimal function of pancreatico-duodenostomy. Furthermore, after operation in the polymer injected dogs, the elevated serum amylase descended to normal level. This occurred much less rapidly in the other groups. The fibrosis of the polymer injected dogs' pancreas was completed by 3 months, while the dogs in the other groups still had some degenerated exocrine tissues. Rapid destruction of exocrine tissues, induced by the injection of the polymer, appears to encourage reconstruction of endocrine tissue.
