Characterization of Epstein-Barr virus-induced lymphoproliferation derived from human peripheral blood mononuclear cells transferred to severe combined immunodeficient mice.

Mice with severe combined immunodeficiency (SCID) received 6 X 10(7) fresh human peripheral blood mononuclear cells (PBMC) intraperitoneally from Epstein-Barr virus (EBV)-seropositive and -seronegative donors. B95-8 EBV was inoculated intraperitoneally and intravenously to the mice 6 weeks after transfer of seronegative PBMC. Three of four mice transferred with PBMC from two EBV-seropositive donors and two of four mice from two EBV-seronegative donors inoculated with EBV developed fatal EBV-induced lymphoproliferative disease within 6 to 10 weeks. These tumors were oligoclonal or polyclonal by cytoplasmic immunoglobulin expression. Furthermore no consistent clonal chromosomal abnormalities were shown. Cell lines established from these tumors showed low cloning efficiency in soft agarose. In addition, latent membrane protein, B-lymphocyte activation antigen (CD23), and cell-adhesion molecules (ICAM-1, CD18) all were expressed in the EBV-positive infiltrating lymphoproliferative lesions in each mouse. These results suggest that lymphoid tumors are comparable to lymphoblastoid cell lines immortalized by EBV and are not malignant lymphomas such as Burkitt's lymphoma. This model may be useful for investigating mechanisms responsible for the growing numbers of lymphoproliferative diseases that are occurring in patients with inherited or acquired immunodeficiencies.

[Bronchiectasis with normal paranasal sinus roentgenogram].

Bronchiectasis has come to be considered as a type of sinobronchial syndrome in Japan, but there exist some cases without chronic sinusitis. We studied the clinical features of 14 cases of bronchiectasis with definitely normal paranasal sinus roentgenogram, diagnosed during the past ten years. There were eleven middle-aged women and three men. Ten patients (71%) complained of hemoptysis, one (7%) of dry cough, one (7%) of productive cough, and the two (14%) had no complaint. In seven patients (50%) CT and bronchography showed localized cylindrical bronchiectasis in the right middle lobe and/or left upper lobe lingular division. They were considered to be middle lobe lingular syndrome. Three patients (22%) with localized varicose or cystic bronchiectasis had a history of pneumonia or pertussis in their infancy, so their bronchiectasis were considered secondary to infantile bronchopulmonary disease. Two patients (14%) had diffuse cystic bronchiectasis and were almost asymptomatic. They might be cases congenital bronchiectasis or Williams-Campbell syndrome. Pulmonary function tests were normal in most of the cases and sputum culture revealed no cases of persistent bacterial infection. These clinical features are quite different from those of bronchiectasis reported as sinobronchial syndrome, in which chronic productive cough, poor pulmonary function, persistent bacterial infection, etc. are significant. So we conclude that there are two distinct groups in bronchiectasis.

[A case of desquamative interstitial pneumonia (DIP) suggesting a relationship to chronic eosinophilic pneumonia (CEP)].

A 43-year-old man was admitted to our hospital because of cough and dyspnea. His chest X-ray showed bilateral basal infiltrative shadows. The increased eosinophils in BAL suggested chronic eosinophilic pneumonia (CEP), whereas the TBLB specimens showed findings compatible with desquamative interstitial pneumonia (DIP). The open lung biopsy specimens showed typical findings of DIP with a peripheral zone, suggestive of CEP. These two diseases may be only phasal differences of reaction to the same extrinsic stimulation.

[A case of acute lupus pneumonitis followed by high-resolution CT].

A 43-year-old woman was admitted to our hospital because of fever and dyspnea. She had facial rash, hair-loss, arthralgia, urinary casts, thrombocytopenia, lymphocytopenia, and deposits of IgG, IgM, C3 in the epidermis. SLE was diagnosed. She also had severe dyspnea, which gradually progressed. In spite of steroid pulse therapy and cyclophosphamide, she eventually died about two months after admission. Chest X-ray film showed bilateral ground-glass shadow and volume-loss. High-resolution CT of the lung showed marked high density area with airbronchogram in the posterior part and a gradually enlarging slight high density area in the anterior area. Pathological findings of the posterior area were atelectasis and dense fibrosis. The findings of the anterior area were collapsed alveoli and dilated alveolar ducts. These findings might suggest that alveolar collapse play a major role in the course of pulmonary fibrosis of this patient.

Characterization of histamine H1-receptors on astrocytes in primary culture: [3H]mepyramine binding studies.

The characteristics of histamine H1-receptors on astrocytes from the cerebral cortex of newborn rats in primary culture were analyzed with a [3H]mepyramine binding assay, and compared with those in the cerebral cortex. The apparent dissociation constant (KD) of [3H]mepyramine binding, the apparent inhibition constants (Ki) of various H1-ligands for [3H]mepyramine binding and the stereoselectivity of d- and l-chlorpheniramine for the inhibition of [3H]mepyramine binding to receptors on cultured astrocytes and to receptors in the brain tissue were very similar, indicating that these receptors are identical. The apparent density of H1-receptors (Bmax) on astrocytes was 262 +/- 60 fmol/mg protein, which was comparable to that in the brain tissue (194 +/- 24 fmol/mg protein). The development of H1-receptors on cultured astrocytes resembled the postnatal development of the receptors in the rat brain. These results suggest that astrocytes could be one of the main targets of the central histaminergic system.

Release and synthesis of histamine in rat basophilic leukemia (2H3) cells.

On the stimulation of rat basophilic leukemia (2H3) cells with an IgE oligomer, the intracellular Ca2+ concentration, which was measured with fura-2 as a fluorescent probe, began to increase after 1 min lag period, reached its maximum at 4 min, and gradually decreased to the original level. The histamine release occurred slightly more slowly than the increase in the intracellular Ca2+. It started 2-3 min after the stimulation, reaching a peak at 6 min and thereafter decreasing slowly. To supplement the released histamine, the activity of histidine decarboxylase, a histamine-forming enzyme, increased 5-fold several hours after the stimulation by an IgE oligomer, and this increase may be mediated by protein kinase C system.

[A clinical study of terminal pneumonia].

A total of 210 cases of terminal pneumonia were studied out of 1183 autopsied cases at Tenri Yorozu Hospital from 1978 to 1985. Underlying diseases included lung cancer (77 patients), gastric cancer (26 patients), leukemia (24 patients). There was no statistical significance between the time from death until autopsy and the bacterial examination of autopsied lung and blood. P. aeruginosa and Klebsiella sp. were the most frequently isolated organisms. Seventy percent of isolated organisms were gram negative bacilli. In spite of administration of antibiotics, bacteria isolated from specimens before death were sometimes the same as the one isolated from specimens after death. In addition it was recognized that multiple intensive examinations of sputum are necessary for rapid diagnosis of pneumonia. It was also noted that the longer the duration of antibiotic administration, the more frequently P. aeruginosa was isolated. Finally the possibility of pneumonia should be kept in mind in compromised hosts.

[Examination and treatment of dental caries in the Niigata Prefectural Institute of Total Training for the Mentally Retarded, colony Niigata Shiroiwa-no-Sato].

The purpose of this study was to learn about the prevalence of dental caries in mentally retarded individuals and to follow up the treatment of the caries. The study group comprised 144 severely retarded individuals (79 males and 65 females) aged 6-19 at the time of their admission to the Institute. Following results were obtained by analyses of their dental examination over a period of records in 15 years from 1973 to 1987 and of their treatment charts over a period of 8 years from 1980 to 1987. 1. The DMFT index at the time of their admission showed a tendency to increase gradually with age. In the DMFT index by age, there was no significant difference between that of the Institute and that of the Survey of Dental Disease by Ministry of Health and Welfare Japan in 1981. 2. The FT index was lower and the MT index was higher than the indices of the Survey, and each of differences showed a statistical significance for almost of all ages. Also in the proportion of both indices to the DMFT index, there were significant differences respectively. 3. During the 8 years between 1980 and 1987, 740 teeth were treated with 388 amalgam fillings (52.4%), 174 resin fillings (23.5%), 22 metal crowns (3.0%), 55 endodontic therapies (7.4%) and 101 extractions (13.7%). 4. After Our department started the treatment program in 1980, there were under 0.5 untreated teeth in each individual for each year. Moreover, missing teeth increased slightly in individuals who were on the register more than 2 years. 5. One of the 237 filled teeth (0.4%) and 2 of 21 endodontically treated teeth (9.5%) were extracted because of unfavorable prognosis during 8 years of the follow-up survey period.

[Surgical application of low-power laser: experimental study, development of equipments and clinical application].

We carried out experimental and clinical studies with low-power laser to perform the surgical application. The developed device for microvascular anastomosis with semiconductor laser diode made of In-Ga-As-P was used to perform 15 end-to-end laser assisted microvascular anastomoses of carotid arteries in rabbits. In four of 7 cases which were irradiated with power output 50mW showed patency immediately after surgery with no wound disruption. A new medical device with Ga-A1-As semiconductor laser diode was developed for pain relief. A double-blind evaluation of the test was initiated in 124 patients suffering from chronic inflammatory pain. Result is that of 63 patients irradiated laser, 45(71%) were effective and of 61 cases not irradiated laser, 8(13%) were relieved pain. The data were analyzed statistically by chi square, and criterion for statistical significance was the 0.01 level. The spectral properties of laser-excited fluorescence of three photosensitizers, namely tetracycline, hematoporphyrin free base and chlorin e6Na, taken up in human arterial intima were detected. Photosensitizers were administrated to resected human arteries for 2 hours in vitro. The main results are following: 1) Photosensitizers are confirmed to be taken up in arterial intima and the fluorescence intensity decreases as the time goes on. 2) The fluorescence intensity from atheromatous plaque is usually stronger than that from normal region.

[A case of horizontal impaction of mandibular left and right deciduous central incisors].

This paper reports on a rare case of horizontal impaction of the mandibular left and right deciduous central incisors. The patient was a 4 year and 7-month-old-girl at the time of first visit to our clinic. She had no previous history, and there was no family history or anomaly in her general physical condition. All the deciduous teeth except the mandibular left and right deciduous central incisors had already erupted. The right deciduous central incisor was impacted completely and the left one erupted with its incisal edge about 2 mm at the lingual gingiva. In the X-ray examination, the horizontal impaction of the mandibular left and right deciduous incisors were noted. There were no cysts, odontoma or tumors. Both of them were extracted to prevent loss of space for the permanent central incisors. Each root of both teeth was curved 90 degrees in the lingual direction at half of length of the root. There was no anomaly of figure and color in their crowns. In the histopathologic examination, there was no anomaly except for the small root resorption at the labial and lingual sides. 3 years and 3 months after extraction of the deciduous central incisors, approximately two thirds of the crowns of the left and right mandibular permanent central incisors have erupted. Both of them incline to the lingual direction and rotate and there is a space of about 5 mm between them. This case is still under observation.

[Protective effect of ventilation on warm ischemic lungs].

This study was undertaken to determine whether lung ventilation during warm ischemia provides a protective effect on lung preservation. The mongrel dogs were made left thoracotomy incision. Following a complete dissection of left pulmonary artery and vein, left lung was subjected to room temperature ischemia for 4 hours or 6 hours by clamping vasculature. Left lung was ventilated with oxygen or collapsed during warm ischemic period. The isolated ventilation of left lung was secured by ligation of left main bronchus over Carlens tube. After the ischemic period, blood supply of the left lung was reestablished by unclamping the pulmonary artery and vein. The contralateral pulmonary artery was ligated 10 min later, and blood gas, pulmonary pressure, and pulmonary shunt were monitored for additional 4 hours while left lungs ventilated with oxygen. In lungs collapsed during 4 hour ischemic period, massive pulmonary edema occurred within 30 minutes after reperfusion. Partial oxygen tension was only 63 mmHg at 1 hour after reperfusion. In contrast, lungs ventilated during 4 hours ischemic period did not manifest pulmonary edema, and PaO2 was 407 mmHg at 4 hours after reperfusion. The lungs collapsed during 6 hour ischemic period showed massive edema. When the lung was ventilated, the lung behaved variably but still better than the collapsed lungs. These results suggest that the ventilation of the lung during warm ischemia provide a protective effect.
