Monoclonal antibody highly sensitive for the detection of ras p21 in immunoblotting analysis.

Four murine monoclonal antibodies (NCC-RAS-001, -004, -005, -017) reactive with ras p21 were produced by using recombinant c-Ha-ras p21 as an immunogen. Among these antibodies, NCC-RAS-004 was extremely sensitive when used in immunoblotting analysis, facilitating semiquantitative detection of c-Ha-, c-Ki- and N-ras p21 in cell and tissue lysates. In cells carrying a point-mutationally activated ras, p21 with abnormal mobility upon sodium dodecyl sulfate-polyacrylamide gel electrophoresis was clearly detected.

A case of epithelioid hemangioendothelioma of the liver.

A case of epithelioid hemangioendothelioma of the liver is reported. Both immunohistochemical and electron-microscopic examinations revealed the neoplastic cells to share many characteristics with endothelial cells. We discuss and compare this rare tumor, which represents a low-grade sclerosing hemangiosarcoma of the liver, with its pulmonary counterpart, which is known as intravascular bronchioloalveolar tumor (IVBAT).

Immunoblastic lymphadenopathy, angioimmunoblastic lymphadenopathy, and IBL-like T-cell lymphoma. A spectrum of T-cell neoplasia.

Thirty cases of immunologically determined and histologically diagnosed immunoblastic lymphadenopathy (IBL), angioimmunoblastic lymphadenopathy (AILD), and IBL-like T-cell lymphoma were clinicopathologically reviewed. Clinical manifestations and laboratory findings did not reveal significant differences in these three groups. IBL, AILD, and IBL-like T-cell lymphoma showed a spectrum of histologic changes, in which proliferation of pale cells was a critical diagnostic point for the histologic malignancy. Immunostaining for their subsets revealed that 3 of 21 cases showed T4+ phenotype and the remaining 19 cases showed T8+ phenotype. Three of seven immunohistochemically determined T8+ cases simultaneously expressed Leu7+ phenotype. The latter cells were consistent with large granular lymphocytes in one case, but no clinicopathological differences from the other T8+ cases were present. IBL and AILD were considered to be T-cell malignancies, which show a spectrum of histologic features from T-cell dysplasia to peripheral T-cell lymphoma (IBL-like T-cell lymphoma). Despite intensive chemotherapy, prognosis was poor in T8+ cases of which half of the patients died within 1 year. T4+ cases showed better prognosis, but a higher incidence of synchronous second primary cancers was recognized.

A human monoclonal antibody directed to blood group i antigen: heterohybridoma between human lymphocytes from regional lymph nodes of a lung cancer patient and mouse myeloma.

A fusion of human lymphocytes released from regional lymph nodes of papillary adenocarcinoma of lung cancer with mouse myeloma P3-X63-Ag8-U1 cells resulted in a stable hybridoma-secreting human IgM antibody (NCC-1004) that reacts with a large proportion of squamous cell carcinomas of lung and esophagus as well as carcinoma of thyroid glands. However, the antibody also reacts with normal red blood cells, B lymphocytes, and a few other limited loci in normal tissues such as the basal cells of bronchial epithelium and the basal cell layer of stratified squamous epithelium, as well as endothelium and alveolar lining epithelium. The antigen defined by NCC-1004 has been characterized as blood group i antigen on the basis of the following results. The antibody preferentially agglutinates cord erythrocytes in contrast to adult erythrocytes. The agglutination was obvious at 4 degrees C, but diminished greatly at 37 degrees C, and was enhanced after sialidase treatment. The antibody specifically reacts with lacto-norhexaosylceramide (nLc6) and sialosyllacto-norhexaosylceramide (IV3NeuAcnLc6), but does not react with lacto-neotetraosylceramide (nLc4), sialosyllacto-neotetraosylceramide (IV3NeuAcnLc4), lacto-isooctaosylceramide (IV6Gal beta 1----4GlcNAcnLc6; I antigen), and other standard glycolipids so far tested. The properties of the antibody and its antigen are identical to those previously described for the i blood group system. Inasmuch as the hybridoma was established by hybridization of lymphocytes derived from regional lymph nodes of lung cancer, and the antigen was found in the patient's lung cancer tissue, the i antigen in lung cancer is probably recognized as a tumor-associated antigen by the host's immune cell system.

High concentrations of enolase, alpha- and gamma-subunits, in the aqueous humor in cases of retinoblastoma.

Because retinoblastomas contain a large amount of the gamma-subunit of enolase or neuronspecific enolase, we studied the aqueous humor levels of the gamma-subunit and alpha-subunit in 12 cases of retinoblastoma. Enzyme immunoassay demonstrated that both the gamma-subunit and the alpha-subunit were greatly increased in all 12 (minimum levels, 87.8 ng/ml and 354 ng/ml, respectively). Thus, aqueous enolase levels, especially that of the gamma-subunit, are useful in the diagnosis of retinoblastoma and may be useful as monitoring markers in retinoblastoma involving the central nervous system.

Histiocytes in nasopharyngeal carcinoma in relation to prognosis.

Forty-nine biopsy specimens of nasopharyngeal carcinoma and 20 biopsy specimens of nasopharyngeal mucosa obtained from non-cancer patients were studied for the presence of T-zone histiocytes (Langerhans' cells and their precursors) and macrophages by immunohistochemical methods with the use of antibodies against S-100 protein and lysozyme (lys), respectively. Patients with dense infiltration of T-zone histiocytes in the primary sites survived longer than those without such infiltration (mean survival times, 8, 39, and 72 months in the patients with the density of -, +, and ++, respectively; P less than 0.005). In contrast, there was no relationship between patient prognosis and density of lys+ macrophages in the tumor tissue (P = 0.33). The distribution of T-zone histiocytes in the tumor tissue was different from that of macrophages. T-zone histiocytes may play an important role in the immune reaction against cancer, probably acting through mechanism different from that of macrophages.

Morphological variation of immunoreactive cells positive to cholecystokinin 33 (10-20) and gastrin 34 (1-15) in human duodenum.

Human duodenal endocrine cells reactive with antibodies to cholecystokinin (CCK) 33 (10-20) and/or gastrin 34 (1-15) were studied by a combination of immunohistochemical and electron-microscopic methods. By immunohistochemistry, three types of endocrine cells were distinguished in human duodenal mucosa, i.e., those only positive for only CCK, those positive for both CCK and gastrin and those only positive for only gastrin. Ultrastructurally, the first cell type is characterized by many secretory granules with an eccentric dense core (mean diameter; 271 +/- 74 nm). The second cell type, which was less frequent than the other two, has ultrastructural features that resemble type-I cells. The last cell type was composed of two types of cells containing small secretory granules identical to those of IG cells (mean diameter; 171 +/- 31 nm) or large secretory granules indistinguishable from those of I cells (mean diameter; 286 +/- 50 nm).

Biological, pathological and clinical features of small cell lung cancer.

Small cell lung cancer, which is not uncommon, and is one of the most malignant and relatively well investigated solid tumors of adults, has been reviewed concerning its biology, pathology and clinical aspects. Although it is histologically very simple, its poorly differentiated epithelial cell characteristics are complicated by features of neuroendocrine cells, such as amine and peptide hormone production and specific enzyme activities, some of which have been found to be good monitoring markers during and after treatment. Because of the relative ease of establishing cell lines in vitro, cell characteristics have been studied in detail. This has led to subtyping of cell lines and may further lead to subtyping of histology. However, accumulation of further evidence has disclosed exceptions and unclassifiable cell lines. The same can be said about chromosomal abnormality. The reactivity of monoclonal antibodies and also oncogenes supports the prevalent concept discriminating small cell cancer from non-small cell cancer. However, concepts concerning histogenesis are still changing. Although it is one of the solid tumors most sensitive to radiation and chemotherapy, the response rate of the tumor to non-surgical treatment appears to have reached a plateau. In order to make a breakthrough in the treatment, strategies based on biological findings must be applied.

Subtypes of small cell carcinoma of the lung: morphometric, ultrastructural, and immunohistochemical analyses.

Fifty-three surgically resected small cell carcinomas of the lung were studied morphometrically, electron microscopically, immunohistochemically, and in terms of possible site of origin. Four subtypes of small cell carcinomas were identified: oat cell carcinoma (OAT), small cell carcinoma of the intermediate cell type (INT), combined oat cell carcinoma, and undifferentiated carcinoma of the small cell type (UCS). The latter type is presumed to be non-neuroendocrine. Morphometric analysis showed considerable overlap among OAT, INT, and UCS with respect to nuclear area, cell area, and nuclear/cytoplasmic ratio. Ultrastructurally, significantly more carcinomas categorized as OAT and INT contained neurosecretory granules than did those in the UCS category (P less than 0.01 and 0.05, respectively). Cells with tonofibrils were more frequent in UCSs than in OATs and INTs. Immunohistochemically, fewer UCSs than OATs contained cells with gastrin-releasing peptide, neuron-specific enolase, and Leu-7 (P = 0.5, P less than 0.05, and P less than 0.05, respectively). UCSs were located more frequently at the periphery of the lung than were OATs (P less than 0.01) and INTs (P = 0.06). These findings suggest that UCS may be a pathologic entity distinct from the typical neuroendocrine-type small cell carcinoma and that this subtype probably corresponds to small cell carcinoma with a "large cell component," and to very poorly differentiated adenocarcinoma and squamous cell carcinoma of the small cell type.

Morphometric study of adenocarcinomas and hyperplastic epithelial lesions in the peripheral lung.

Mean nuclear areas (MNA) were calculated morphometrically in ten cases of well-differentiated adenocarcinoma, ten cases of atypical alveolar cuboidal cell hyperplasia (AAH), and five cases of alveolar cuboidal cell hyperplasia (AH) and were compared with each other. In cases of adenocarcinoma, the MNA were significantly larger than those of AAH and AH, and the standard deviation (SD) of nuclear area was greater in adenocarcinoma than that in AAH and AH, thus implying greater "scatter" of the nuclear areas in the former than the latter two. The MNA and the SD of nuclear area in AH were smallest in these three groups. In one case each of AAH and adenocarcinoma, there were two different populations of nuclear size in the same lesions, that is, histograms showed both adenocarcinoma and AAH in the same tumor. Through the use of the morphometric method, many cases of AAH were easily distinguishable from adenocarcinoma cases, and there were two cases in which foci of adenocarcinoma and AAH coexisted.

Immunohistochemical cell kinetic study of human lung cancer by using monoclonal antibody to bromodeoxyuridine.

In vitro cell kinetic studies of 27 surgically removed human lung cancers were performed by the immunohistochemical procedure using monoclonal antibody to bromodeoxyuridine (BrdU). BrdU was incorporated under hyperbaric oxygenation, and labeled cells were found to be evenly distributed in the tissue blocks. A comparison of the in vitro and in vivo labeling index (L.I.) in human tumors transplanted to nude mice showed good correlation. Surgically resected adenocarcinoma of the human lung had a significantly lower L.I. than squamous cell carcinoma, the mean values being 6.0% and 17.2%, respectively. In adenocarcinoma, L.I. values disclosed a trend toward a higher L.I. in poorly differentiated carcinoma.

An immunohistochemical study of thymic epithelial tumors. I. Epithelial component.

Twenty-four cases of thymic epithelial tumors, including 18 cases of thymoma, five cases of squamous cell carcinoma, and one case of undifferentiated carcinoma, were studied immunohistochemically using monoclonal antibody Leu-7 (HNK-1) and antikeratin antibody. Seven cases of non-neoplastic thymic tissues were also studied. Leu-7 antibody stained epithelial cells in the outer cortex of the normal thymus, and antikeratin antibody stained thymic epithelial cells in both cortex and medulla of the normal thymus. Seventeen thymomas and one undifferentiated carcinoma were focally or diffusely stained with Leu-7, some showing cortical and medullary differentiation as seen in the normal thymus. On the other hand, none of the five squamous cell carcinomas were stained with Leu-7. All thymomas stained for keratin in varying degrees, and all squamous cell carcinomas were diffusely and strongly stained with antikeratin antibody. It is concluded that normal thymic epithelial cells showed zonal differentiation, and neoplastic cells were considered to retain these characteristics to some extent; i.e., thymomas had the same phenotype of epithelial cells of the cortex, especially the outer cortex of the thymus in some instances (Leu-7-positive, keratin-positive) and of both cortex and medulla (mixture of Leu-7-positive and -negative cells) with organoid arrangement in other instances, and thymic squamous cell carcinoma had the same phenotype as epithelial cells in the thymic medulla (Leu-7-negative, keratin-positive).

Papillary adenoma of type 2 pneumocytes.

A case of papillary adenoma of type 2 pneumocytes is reported. A 57-year-old man had an unusual coin lesion in the periphery of the right lung without any symptoms. When detected in a mass survey examination, it was approximately 1.5 cm in diameter, well circumscribed, and located in S4, involving the sixth-order bronchus of B4. Light-microscopic examination revealed cuboidal tumor cells arranged in a papillary pattern. Ultrastructurally, the cells had characteristic osmiophilic lamellar bodies. By immunoperoxidase staining, the tumor cells were shown to have intracytoplasmic surfactant apoproteins. The postoperative course was uneventful, and there is no evidence of disease 8 years later.

Multiple sclerosing hemangiomas of the lung.

A case of multiple "sclerosing hemangiomas" of the lung is reported. A 48-year-old housewife had several coin lesions in the right lower lung field, which were detected in a mass survey examination. There were no definite symptoms or signs except for hemoptysis. The resected right lower lobe of the lung contained innumerable lesions varying in size from microscopic to 3.7 cm in diameter, all of which were diagnosed as "sclerosing hemangioma." Although larger nodules revealed hemorrhagic, solid, papillary, and sclerotic features, smaller lesions, particularly minute ones, tended to be composed only of epithelial cells arranged in papillary patterns and so-called pale cells in solid areas, both of which are suggested to be main components of sclerosing hemangioma. Some lesions were associated with tumorlet or adenomatous hyperplasia (or adenoma) probably of the type 2 epithelial cell. It is not certain from this case study whether the lesions were multicentric in origin or intralobar metastases from one primary lesion. The patient's postoperative course was uneventful, and there is no evidence of recurrence 2 years after surgery.

Expression of vimentin in surgically resected adenocarcinomas and large cell carcinomas of lung.

The expression of vimentin in pulmonary carcinomas was studied in 285 cases of surgically resected lung cancer from our hospital files. Formalin fixed, paraffin-embedded sections were studied by immunoreactive staining techniques using two monoclonal antibodies against vimentin. Cases demonstrating vimentin positivity by the avidin-biotin-peroxidase method included 11 of 129 adenocarcinomas studied (8.5%), and 15 of 61 large cell carcinomas studied (24.6%). Vimentin expression was not seen in any of the 51 squamous cell carcinomas or 35 small cell carcinomas in our series. The positive cases of adenocarcinoma were in moderately and poorly differentiated cancers. Four of the eight giant cell carcinomas (50%) demonstrated vimentin expression. All cases that exhibited vimentin positivity were studied for cytokeratin expression. Coexpression of vimentin and cytokeratin was demonstrated not only within the same tumor but also within the same cells in some cases stained by double antibody technique, including both adenocarcinomas and large cell carcinomas. Similar immunoreactive methods were also applied to sections from human lung cancer transplants grown in the nude mouse. Of 28 tumors studied, four of 11 adenocarcinomas (36%) and all 4 large cell carcinomas demonstrated coexpression of vimentin and cytokeratin, while none of the five squamous cell carcinomas or eight small cell carcinomas expressed vimentin.

An immunohistochemical study of thymic epithelial tumors. II. Lymphoid component.

The cellular composition and characteristics of lymphocytes and histiocytes in nine cases of thymoma and one case of squamous cell carcinoma of the thymus were studied immunohistochemically, using a panel of monoclonal and polyclonal antibodies. Nine cases of thymoma were subclassified into seven cases of mixed type and one case each of predominantly lymphocytic type and predominantly epithelial type. Two metastatic tumors and one recurrent tumor were included. Almost all the lymphocytes in the mixed type and lymphocytic type categories, either in primary site or metastasis, were T lymphocytes which showed the immature thymic lymphocyte-phenotype of the normal thymic cortex. On the other hand, the one epithelial thymoma contained many mature T lymphocytes along with immature T cells, as in the normal thymic medulla. In thymic squamous cell carcinoma, all infiltrating T lymphocytes were mature T lymphocytes. These findings suggest that epithelial cells of thymomas have an intimate relationship with the coexisting lymphocytes and retain functional properties of the normal thymic epithelial counterpart. On the other hand, the specific function of thymic epithelial cells is no longer maintained in thymic squamous cell carcinoma.
