[Change of beta-actin gene expression during culture on osteoblast-like cell line, MC3T3-E1].

Multiple forms of actin (alpha, beta, and gamma-actin) have been found in a variety of mammalian cell lines and tissues by the use of high resolution, two-dimensional gel electrophoresis. alpha-Actin is found only in differentiated muscle cells, and its synthesis is induced during myogenesis in culture, beta- and gamma-actin isotypes are the principal components of microfilaments, structures believed to be involved in cellular morphogenesis, motility, and mitosis. Recent studies have shown that mammalian actin mRNA levels are modulated in response to changes in cell shape and during cell differentiation in vitro. On the other hand, Kodama et al. isolated clone MC3T3-E1 cells from newborn mouse calvaria, which have retained the ability to produce and mineralize a bone-like ground substance after many serial passages. Thus clone is useful for studying osteoblast differentiation as well as the mechanism of calcification. It is demonstrated that bone cells assume a stellate morphology after some hormone treatment and these changes were clearly associated with altered reorganization of the microfillaments which resulted in an enhanced osteoblast phenotype. We have now examined the effect of culture day on expression of beta-actin gene in MC3T3-E1 cells and found that expression of beta-actin gene reduced with culture days. These alternative on the expression of actin gene suggest that beta-actin may be involved in bone remodeling in vivo.

Antiinflammatory properties of the new antirheumatic agent 4-acetylaminophenylacetic acid.

The antiinflammatory activity of 4-acetylaminophenylacetic acid (MS-932) was investigated. MS-932 did not suppress the acute inflammation of carrageenin-induced paw edema in rats or of primary swelling in adjuvant arthritic rats. However, prophylactic treatment with MS-932 inhibited secondary inflammation in adjuvant arthritic rats. MS-932 also restored to normal the weight of the spleen and the serum albumin/globulin ratio of adjuvant arthritic rats. In addition to its prophylactic effect, MS-932 had a therapeutic effect on adjuvant arthritis. In in vitro tests, MS-932 did not inhibit prostaglandin E2 biosynthesis from arachidonic acid by sheep seminal vesicle microsomal enzyme or superoxide generation by guinea pig neutrophils stimulated with opsonized zymosan. MS-932 had no analgesic effect in mice and no antipyretic effect in rats. These results indicate that MS-932 suppresses adjuvant arthritis through modulation of the immune system.

[Dermal patch anesthesia to prevent pain from dermal puncture--10% lidocaine aqueous gel with 3% glycyrrhetinic acid 3-0 hemiphthalate disodium].

The clinical efficacies of 10% lidocaine aqueous gel with and without 3% glycyrrhetinic acid 3-0 hemiphthalate disodium (GAHPh) applied as a skin patch for reduction of pain from venous cannulation were evaluated in a double blind study. Twenty-four adult patients, who were scheduled for surgery under general anesthesia, gave informed consent to participate in this study. The patients were allocated randomly into two groups: one for a dermal patch GAHPh (GAHPh group) and the other for a dermal patch without GAHPh (plain group). Approximately 0.3g of either gel with or without GAHPh, soaked in a round sponge (25mm in diameter, 1mm in thickness), was applied over the selected vein on the arm and was covered with an adhesive plastic film (Tegaderm). Pain score was graded by the number of painful spots out of the 5 tests in the treated skin area. In patients with a pain score under 1, venous cannulation was carried out without an intradermal injection of a local anesthetic and pain associated with the cannulation procedure was graded by patients on a scale of 5, where 0 = no pain, 1 = little pain, 2 = moderate pain, 3 = painful, 4 = very painful. The mean application time periods were 59.3 min for the GAHPh group and 60.3 min for the plain group. Transient local redness was observed in 8 patients after removal of the gels; 3 in the GAHPh group and 5 in the plain group. The mean pain score (1.3 +/- 1.5) in the GAHPh group, was significantly lower than that (2.5 +/- 1.7) in the plain group (P less than 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)

[Effect of the lateral hypothalamic area-lesions on the development of gastric mucosal damages by water immersion restraint stress in rats].

The role of the lateral hypothalamic area (LHA) in the development of gastric mucosal damages induced by water immersion restraint stress was evaluated in bilateral LHA-lesioned rats. The ulcer index of lesions (M +/- SEM, mm) was significantly higher in LHA-lesioned rats (21.6 +/- 2.8) than in both thalamus-lesioned rats (9.8 +/- 2.0) and non-treated rats (10.2 +/- 1.5). The gastric acid and pepsin outputs during water immersion for 5 hours were significantly lower in the LHA-lesioned rats than in both the control rats. In the LHA-lesioned rats, PAS-positive mucus content of gastric corpus without lesions was significantly decreased after water immersion restraint stress for 5 hours. All these results suggest that lateral hypothalamic area plays an important role in the process of stress ulcer formation mainly through the disturbance of mucosal defensive mechanism such as changes in gastric mucus contents.

[Analysis of subsets of colonic mucosal lymphocyte in ulcerative colitis by two color flow cytometry].

To assess how the immunological events occur in the colonic mucosa in patients with ulcerative colitis, it is thought to be important to evaluate the subpopulations and/or subsets of mucosal lymphocytes. In this point of view, we assayed those by lymphocyte isolation techniques and two color flow cytometry. Although our results showed no disease-specific abnormalities of the percentages of CD3, CD4, CD8, CD20, and HLA-DR (+) cells in PBL (peripheral blood lymphocyte) nor CML (colonic mucosal lymphocyte), these subsets of CML appeared to be altered according to the grade of severity of inflammation. In our cases, the HLA-DR (+) cell and CD4 population were larger in severely inflamed mucosa. Furthermore, fluorescence intensities of HLA-DR antigen of CD20 population in CML were greater than those in PBL. These results suggest that the B cell-mediated mechanisms may play an important role in maintaining the severe inflammation, and the clinical significance of these studies are discussed.

[Detection of proliferative cells in colorectal carcinomas and adenomas by monoclonal antibody to DNA polymerase alpha].

DNA polymerase alpha is an endogenous DNA replication enzyme expressed in all cells in a proliferation cycle. An immunoperoxidase method and the monoclonal antibody to DNA polymerase alpha were used to identify proliferating cells in colorectal carcinomas (n = 35) and adenomas (n = 43). The labeling index (L.I.) in colorectal carcinomas was 51.6%, being significantly higher than 28.6% in adenomas. The L.I. in colorectal carcinomas correlated with clinical staging (stage I: 33.1%, stage II and III: 49.5%, stage IV and V: 66.9%). Furthermore, the L.I. had a tendency to elevate as carcinoma deeply invaded (pm: 25.8%, ss-s or a1-a2: 52.2%, si or ai: 67.5%). The L.I. in adenoma was related to the degree of atypia. The L.I. in adenomas with mild atypia, with moderate atypia, and cancer in adenoma were 18.3%, 31.5%, and 47.0%, respectively. And the L.I. of cancer in adenoma had no significant difference in advanced carcinomas (47.0% vs 51.6%). These results suggest that the L.I. is useful as a marker for evaluating the degree of biological malignancy of human colorectal carcinomas and the degree of histopathological atypia of adenomas.

Pathological factors influencing survival of carcinoma of the ampulla of Vater.

The resected specimens of 24 consecutive patients, who underwent pancreaticoduodenectomy for ampullary carcinoma from 1977 to 1986, were examined pathohistologically and the factors influencing survival were evaluated. According to the pTNM pathological classification, the 5-year survival at Stage I was 100%, at Stage II 64.8% and at Stage III 15%. Only one patient at Stage IV survived more than 5 years. These indicate that the localization of the tumor within the ampulla of Vater and lymph node metastases are worthwhile prognostic indicators. The other factors, such as shape of the tumor, invasion into the veins and lymphatic vessels in the primary lesion, and type of local extension were indicative of influencing survival, but without statistical significance. Tumor size, histological type, and the ratio of connective tissue included in the primary lesion did not correlate with survival.

[Clinical study on the use of retrograde cardioplegia with St. Thomas' Hospital solution].

One hundred and three consecutive patients with aortic valve disease and twenty seven patients with ischemic heart disease of severe critical coronary stenosis or left main trunk stenosis underwent open heart operations with the use of retrograde cardioplegic technique for myocardial protection. Under complete cardiopulmonary bypass, a balloon catheter was inserted into the coronary sinus through small right atriotomy and secured in place. Retrograde cardioplegia was accomplished using cold St. Thomas' Hospital solution by drip method at height of 60 to 80 cm with topical saline slush. Cardiac resuscitation was very easy and acceptable hemodynamics were obtained in all patients. Even in 8 patients in which aortic crossclamping time was above 180 minutes cardiac recovery was excellent except one who needed IABP support. Eight patients in aortotomy group were died postoperatively from the reasons unrelated to myocardial protection. Postoperative hemodynamic data and enzymatic analyses of CK-MB revealed good myocardial protective effects. Retrograde cardioplegia with the use of cold St. Thomas' Hospital solution is thus an effective alternative of myocardial protection in aortic valve surgery or aortotomy surgery and in coronary revascularization for multiple coronary stenoses or left main trunk lesions.

[Coronary revascularization using an internal mammary artery-saphenous vein composite graft in a patient with severely calcified ascending aorta].

A 52-year-old woman with angina pectoris resulted from complete obstruction of left coronary ostium required double-vessel bypass. Because of severely calcified ascending aorta, avoidance of aortic cross-clamping was needed for the prevention of embolic injury and aortic dissection. Internal mammary artery (IMA)-saphenous vein (SV) composite graft under hypothermic ventricular fibrillation was successfully performed without any complication. IMA-SV composite graft is a good alternative in a case of insufficient IMA length and limited site for proximal vein graft anastomosis, which can avoid or reduce the manipulation of diseased ascending aorta.

Tissue-specific granularity of gap junction cytoplasmic surfaces revealed by rapid-freeze, deep-etch replicas.

Previous rapid-freeze, deep-etch replica studies have revealed the differences between heart and liver gap junctions; cytoplasmic surfaces of in situ and phenylmethylsulfonyl fluoride (PMSF)-unproteolyzed isolated cardiac gap junctions (MW 47 kD) have a particulate substructure, which is absent both in the proteolyzed heart junctions (MW 29 kD) and in the liver junctions isolated with PMSF (MW 28 kD). The present deep-etch replica studies of gap junction cytoplasmic surface (CS) membranes in several tissues of rats and mice were performed to examine whether or not this difference between liver and heart is typical of variations in gap junction proteins from tissue to tissue. In surface mucous cells of the stomach, intestinal epithelial cells, and kidney tubule cells, these epithelial gap junctions always showed smooth cytoplasmic surfaces, similar to the liver gap junctions. In contrast, in the atrial myocardium, aortic endothelium, and the ciliary process, cytoplasmic surface membranes of the gap junctions consistently revealed particulate patterns. Close examinations disclosed that those granular structures were not merely attached to the memvrane surface, but they also protruded from the membrane interior as an integral component of gap junction particles. Furthermore, in the pregnant rat uterus at term, cytoplasmic surface membranes of myometrial smooth muscle gap junctions were particulate, but those of endometrial epithelium were smooth. The present observation suggest that tissue specificity exists in cytoplasmic surface structures of gap junctions between the "true" epithelial and the nonepithelial tissues: the nonepithelial gap junctions contain the additional cytoplasmic surface domain that is absent in the gap junctions of "true" epithelial origin.

Purification and immunological properties of human urinary kallikrein and prokallikrein.

Human urinary prokallikrein and kallikrein have been purified from the same source of urine simultaneously. The anti-kallikrein and anti-prokallikrein antibodies were raised in rabbits using the purified preparations. With respect to solid phase enzyme immunoassay (EIA), immunoaffinity column chromatography, and single radial immunodiffusion, reactivity of each antibody with kallikrein was distinctly different from that with prokallikrein. Kallikrein could be determined by anti-kallikrein antibody-immobilized EIA below 20 ng per ml, whereas prokallikrein was undetectable. Prokallikrein became detectable at higher concentrations, although it was less reactive than kallikrein. The anti-prokallikrein antibody-immobilized EIA detected both kallikrein and prokallikrein with the same sensitivity. However, the binding capacity for kallikrein was about one-third less than that for prokallikrein. The results show that kallikrein in human urine may be determined directly and selectively. Similar difference in reactivity was observed with immunoaffinity column chromatography and single radial immunodiffusion. The presence of 3-4 antigenic sites per molecule was indicated by quantitative precipitin reaction, and it is suggested from analysis of amino acid sequence of kallikrein by the method of Hopp and Woods that four hydrophilic regions exist in kallikrein molecule.
