[Analysis of specific cellular immunity in renal transplant recipients by MLR & CML].

The specific cellular immunity of renal transplant recipients was investigated by MLR & CML. MLR has been thought to reflect the disparity of HLA-D locus between two individuals and CML to represent an in vitro model for graft rejection and graft adaptation. Anti-donor and anti-control reactivities were diminished in MLR immediately after transplantation even during acute rejection. These MLR suppression are probably due to immuno-suppressive drugs. In acute rejection episode, CML reactivity against donor was elevated and well correlated with clinical findings, but on the other hand, MLR reactivity was suppressed. This discrepancy indicates that strong MLR is not needed to induce high CML. The in vitro development of cytotoxic T cells as a result of proliferation in MLR is impaired in long term survivors and quiescent recipients after transplantation. These suppression is directed to specific donor but not to unrelated third party cells. I have demonstrated the MHC restricted CML suppressor T cell by using HLA identical monozygous twin's lymphocyte. This suppressor T cell may prove to be of value in enhancing graft acceptance and play a part of immunomodulations.

[Pulmonary metastases from osteosarcoma].

On the basis of prognosis in conjunction with tumor behavior, 22 patients with pulmonary metastatic osteosarcoma who had a systematic with combination of surgery and adjuvant chemotherapy were compared with thirteen patients treated randomly in the initial period. The thirteen patients had median survival of 5 months with median tumor-free interval of 6 months. Six of the twenty two patients, however, are alive and 16 had median survival of 15 months with median tumor-free interval of 14 months. In 20 of the twenty two treated with repeated or bilateral thoracotomies, there was a correlation between decrease number of metastatic lesions with number of thoracotomies and survival. Wedge excision performed repeatedly produced only minimal loss of vital capacity, compared with lobectomy. Dn bloc resection of pericardium, chest wall, and diaphragm enhanced no survival. The patients with tumor-free interval of greater than 8 months survived or succumbed to the disease and was associated with mitotic ratio of more than 10 or less. Both of chest X-ray and CT scan had a positive diagnostic rate of 41% and 67%, respectively. Currently, we conclude that modified Thomfold 's indication is applicable to the patient with pulmonary metastatic osteosarcoma with bilateral or multiple nodules and more than 8 months of tumor-free interval.

[Clinical experience in patients with Pancoast's tumor treated by fast neutron therapy].

We review our results with fast neutron therapy in Pancoast's tumor, and compare them with results obtained by photon beam therapy. 13 patients with Pancoast's tumor were divided into two groups; Group I (8 patients) received fast neutron therapy. Group II (5 patients) were treated by voltage X-ray therapy. 1. Group I was comprised of 3 patients receiving mixed beam therapy (TDF 80-100) and 5 patients subjected to boost therapy. All group II patients received 3100 to 8000 rads. 2. Fast neutron therapy was effective in 7 patients. In group II, high voltage X-ray therapy was effective in only 3 patients. 3. Two group I patients are still alive without signs of recurrence. The others died with a mean survival of 11 months. All group II patients died; their mean survival was 4.2 months. Our results suggest that fast neutron therapy is suitable and effective in patients with Pancoast's tumor.

Prosthetic replacement of entire left hemidiaphragm in malignant fibrous histiocytoma of the diaphragm.

A case of primary malignant fibrous histiocytoma of the diaphragm which, to our knowledge, is the first record in the world literature, is presented. It occupied almost the entire left hemidiaphragm and was surgically removed with the left lower pulmonary lobe, stomach, colon, and spleen. The entire left hemidiaphragm then was replaced with Marlex mesh. Prosthetic replacement of the entire hemidiaphragm, which has not been previously reported, was successfully performed with preservation of pulmonary function on the affected side. The patient died of brain metastases of malignant fibrous histiocytoma more than 4 months postoperatively. Diagnosis of diaphragmatic tumors and a technique which involves the use of a prosthesis are discussed.

[Malignant melanoma in Japan: unique distribution and effect of DAV chemoimmunotherapy].

This study, based on a co-operative group project involving 4 major medical institutes in Japan presents the first survey of malignant melanoma (MM) patients (157 cases) where an attempt was made to systemically evaluate the distribution of primary MM, and the response to DAV-chemoimmunotherapy. The distribution of primary MM in Japan is unique in a sense that the MM involving the lower extremities occupied more than 50% of the total male and female cases. The commonest type and site of involvement is the acrolentiginous MM involving the plantar area (30%). The regimen of our group included the combination of DTIC, VCR and ACNU, a new nitrosourea with or without immunoadjuvants OK-432, PSK, or NK-421). By a historical comparison, the DAV-treated group showed a better prognosis in the survival rates of overall (Stages I-IV) and disseminated (Stages III-IV) patients than those of the non-DAV group. However, the effect of combined immunoadjuvants was not statistically significant, though OK-432 showed a significant inhibition of lymphopenia which always occurred during DAV therapy.
