[The clinicopathological effects of infusion of cis-platinum into the bronchial artery on lung cancer].

Ten patients with histologically and/or cytologically confirmed lung cancer were treated with cis-platinum (CDDP) infused into the bronchial artery (BAI). Radiological findings on chest X-rays taken two weeks after BAI were evaluated according to Takeuchi's criteria. The radiological results were judged to be quite effective in one case of squamous cell carcinoma, with more than 50% shrinkage of the tumor shadow, and to be effective in nine others, with more than 20% shrinkage. As for the histological effects, grade III in one case of squamous cell carcinoma and grade IIa in two cases of adenocarcinoma and one of large cell carcinoma were obtained by Shimosato's criteria. In conclusion, BAI with CDDP was sufficiently useful as a chemotherapeutic modality for lung cancer, especially squamous cell carcinoma, because higher local dosage of CDDP can be delivered without systemic toxicity.

[Results of surgical treatment for large cell carcinoma of the lung].

Twenty-six patients with histologically confirmed large cell carcinoma of the lung were evaluated. The overall five-year survival of these cases was 19.9%. Fifteen patients who had undergone surgical resection, excluding one operative death, had a five-year survival rate of 33.8%. None of the nonresected cases survived more than six months after the beginning of treatment. Among prognostic factors in surgical treatment of large cell carcinoma, survival correlated well with clinical stage, extent of lymph-node metastasis, curative resection and histological subtypes. As for histological subtypes, stratified subtype by division of nongiant cell carcinoma revealed poorer prognosis than that of nonstratified subtype and appeared to have some correlation with the frequency of vascular invasion by the tumor. From the clinical viewpoint, giant cell carcinoma should be regarded as a distinct clinical entity. Two cases, however, had benefit of long-term survival due to surgical modality. The clinicopathological analysis of those cases remains to be resolved in the future.

[Anti-platelet therapy in ischemic cerebrovascular disorder--clinical and hematological study].

Anti-platelet therapy (Aspirin 250 mg/every other day approximately 1000 mg/day + Dipyridamole 150 mg/day) was performed on 51 patients with ischemic cerebrovascular disorders (ICVD). Among these patients, 41 cases showed no recurrence of ICVD attack, whereas the remaining 10 cases had re-attacks. These 2 groups were compared from the viewpoint of various risk factors and other clinical findings: e.g. CT scan and angiographic appearance. But there was no statistically significant difference between them. Platelet aggregability and platelet survival time were examined in twenty-eight patients out of cases. We used a simple nonradioisotope technic for the determination of platelet survival time. Platelet survival time was shortened in 22 of 28 (79%) patients. On the other hand, platelet aggregability was found to have no definite tendency. In 9 of 10 cases, platelet survival time was revealed to be prolonged several months after administration of anti-platelet drugs. There was no re-attack in 7 of these 9 patients. Some reports also showed that platelet survival time was shortened in various thromboembolism and recovered by anti-platelet drugs. Measurement of platelet survival time is thought to be an important method as to the indication and the monitoring of anti-platelet therapy.

An epidemiological study on carbon disulfide angiopathy in a Chinese viscose rayon factory.

To assess long-term vascular effects of occupational carbon disulfide (CS2) exposure, we examined 376 exposed and 325 unexposed Chinese workers with a medical and work-history questionnaire, a chest-pain questionnaire, blood pressure and blood cholesterol measurements, routine urinary tests, monochromatic fundus photography and other tests. Data from 354 exposed and 177 age- and sex-matched reference workers were evaluated. Personal exposure monitorings were performed simultaneously with a passive dosimeter using gas chromatography. Monochromatic fundus photography revealed no CS2-retinopathy characterized by microaneurysms and/or small dot hemorrhages, and no effects of CS2 on blood pressure and blood cholesterol were found. These negative results may have been due to the low daily exposure averages, which varied between 0.01 and 12.8, with a mean of 1.5 ppm. The present study suggested that the no-effect level of CS2 exposure on the development of retinopathy was around 2 ppm (8-h time weighted average), and the absence of adverse CS2 effects among the workers studied supported the current standard for occupational CS2 exposure of 10 mg m-3 in China.

Oncocytic carcinoid of the lung. An ultrastructural observation.

A case of a 55-year-old female patient with an oncocytic variety of carcinoid tumor of the lung is reported. Electron microscopic observation of marked hyperplasia and increase in number of mitochondria in the tumor cells suggested it to be of oncocytic variety and membrane bound electron dense granules were characteristically found in the tumor cells which were histologically diagnosed as carcinoid.

[Primary intracranial hypotension associated with chronic subdural hematoma--report of 2 cases].

Low spinal fluid pressure syndrome is characterized by orthostatic headache aggravated in upright position. It is classified into two from etiological standpoint i.e. primary and secondary (most often seen after lumbar puncture). On the other hand, low spinal fluid pressure is one of the promoting factors of chronic subdural hematoma. We report 2 cases of primary low spinal fluid pressure syndrome (primary intracranial hypotension) associated with chronic subdural hematoma. Case 1 is a 47-year-old man who was admitted with disorientation following 2 week history of orthostatic headache. Spinal fluid pressure was 7mmH2O in the lateral recumbent position. CT scan revealed bilateral isodense chronic subdural hematoma. The subdural hematoma reaccumulated 17 days after the first operation. Case 2 is a 31-year-old woman who was admitted with 4 week history of progressive orthostatic headache accompanied by nausea and vomiting. Spinal fluid pressure was 0 mmH2O. CT scan and cerebral angiography showed bilateral chronic subdural hematoma. The hematoma reaccumulated 20 days after the first operation. Six cases including our two cases of primary intracranial hypotention associated with chronic subdural hematoma have been reported. When changes of characters of headache, especially mental symptoms and disturbances of consciousness occurred in patients with chronic orthostatic headache, association of chronic subdural hematoma should be suspected. In cases with chronic subdural hematoma associated with low spinal fluid pressure syndrome, the reaccumulation of hematoma tends to occur after burr hole opening and irrigation of hematoma.

[Effects of infusion of cis-platinum into the bronchial artery in lung cancer].

5 patients with histologically and/or cytologically confirmed lung cancer were treated with Cis-platinum (CDDP) infused into the bronchial artery (BAI). 2 patients with squamous cell carcinoma disclosed hypervascular type and 3 patients with adenocarcinoma demonstrated avascular or hypovascular type on bronchial arterial angiography. All patients showed more than 20% shrinkage of the tumor shadow on chest X-ray taken 2 weeks after BAI and were evaluated to be radiologically effective by Takeuchi's criteria. One patient with squamous cell carcinoma who underwent lobectomy after BAI with 100 mg of CDDP showed grade III histological effects by Shimosato's criteria. The other patient had superior vena cava syndrome and showed regression of a tumor which occluded the orifice of the right upper and showed lobe after BAI with 100 mg of CDDP. By of contrast, only 1 out of the 3 patients with adenocarcinoma who under went resection showed a grade IIa histological effect after BAI with 50 mg of CDDP. Side effects included mild gastrointestinal toxicity in 1 out of the 5 patients. In conclusion, BAI with CDDP was shown to be sufficiently useful as a chemotherapeutic modality for lung cancer, especially for patients with squamous cell carcinoma, because higher local doses can be delivered without systemic toxicity.

[Malignant melanoma in Japan: unique distribution and effect of DAV chemoimmunotherapy (part II)].

This study, based on a cooperative group project involving 4 major medical institutes in Japan, presents the second survey of malignant melanoma patients (198 cases) where an attempt is made to systemically evaluate the survival rates of these patients with respect to the tumor thickness and location of primary lesions, and the response to chemoimmunotherapy. More than 50% of total collected cases showed the primary lesions on the limbs. The most common type and site of involvement is the acral lentiginous melanomas involving the plantar areas (more than 30). The survival rates affecting the limbs were better than those affecting the non-limb areas. However, the comparison of the survival rates did not reveal any difference between those cases affecting the plantar and non-plantar areas. The difference in the prognosis of melanoma patients appeared to be related to the tumor thickness. By historical comparison, the DAV (DTIC, ACNU, VCR) treated group exhibited a better survival rate than the non-DAV treated group. Furthermore, the DAV group with immunoadjuvant therapy (mainly OK-432) showed a better prognosis than the DAV group without any immunoadjuvant therapy.

Uroporphyrinogen decarboxylase. Purification, properties, and inhibition by polychlorinated biphenyl isomers.

Uroporphyrinogen decarboxylase (EC 4.1.1.37) which converts uroporphyrinogen I or III into coproporphyrinogen I or III, respectively, was purified about 5,500-fold from chicken erythrocytes. Purification was accomplished by chromatography on DEAE-cellulose, ammonium sulfate fractionation, chromatography on Sephadex G-100, and chromatofocusing. The most purified preparation was homogeneous on polyacrylamide gel electrophoresis and had a specific activity of 1,420 units/mg of protein, the highest value so far reported. The molecular weight, as determined by Sephadex G-150 gel chromatography, is 79,000. The subunit molecular weight, as determined by sodium dodecyl sulfate-polyacrylamide gel electrophoresis, is 39,700, suggesting that uroporphyrinogen decarboxylase is dimeric in form. The purified enzyme had an isoelectric point of 6.2 and a pH optimum of 6.8. The SH reagents inhibited the enzyme activity, but neither metal ions nor cofactor requirements could be demonstrated. A new and simple method for the separation of free uroporphyrin, hepta-, hexa-, and pentacarboxylic porphyrins and coproporphyrin was developed using a high pressure liquid chromatograph equipped with a spectrofluorometric detector. Kinetic studies of the sequential decarboxylation of uroporphyrinogen with purified enzyme were performed. 3,4,3',4'-Tetrachlorobiphenyl and 3,4,5,3',4'5'-hexachlorobiphenyl which specifically induce delta-aminolevulinic acid synthetase also strongly inhibit uroporphyrinogen decarboxylase directly at two steps, i.e. first in the formation of hexacarboxylic porphyrinogen III from heptacarboxylic porphyrinogen III and second in the formation of heptacarboxylic porphyrinogen III from uroporphyrinogen III.
