Epidermal growth factor (EGF) binding, EGF-dependent autophosphorylation and activity of tyrosine-specific protein kinase in hepatic membrane fractions from fetal, newborn, adult and partially hepatectomized rats.

The binding of 125I-epidermal growth factor (EGF) and activities of EGF-receptor autophosphorylation and of tyrosine-specific protein kinases were determined in hepatic membrane fraction from newborn, fetal and hepatectomized adult rats and compared with those of adult control rats. Although the EGF binding was decreased, there was a tendency for the activity of autophosphorylation to be higher and ligand-dependency to be lower in the membranes from growing hepatic tissues. The activity of tyrosine kinases did not differ among animal groups but a supplement of (NH4)2SO4 to the incubation mixture revealed a difference in the EGF-dependency of the activity; the salt inhibited the activity in the control more profoundly than in the newborn and fetus but the activity was partially restored in the presence of EGF, while in the newborn and fetus the activity did not respond to the added EGF. The results suggest that the higher activity with less responsiveness to the ligand of EGF-receptor autophosphorylation and protein-tyrosine kinase is one of the characteristics of growing rat hepatic tissues.

[A questionnaire about the dental treatment for visiting patients at welfare center for the aged people].

A questionnaire about the dental treatment for visiting patients at Welfare center for the aged people. Nowadays, 11 percent of the population is over 65 in Japan. According to the Minister of Health and Welfare's Population Projection for Japan, it will grow to 20% at the beginning of the 21st century. Problems concerning old people will become some of Japan's biggest challenges. In that situations, the dental treatments for visiting aged patients will become more important. Then, authors sent out a questionnaire about it on aged people who have no need of anyone's assistance in their home lives. And we examined concrete countermeasures and problems about the dental treatments for visiting patients. The findings were as follows: 1. Even the aged people who have no need of anyone's assistance in their home lives, they hope much carrying out the dental treatments for visiting them. 2. The dental treatments for visiting aged patients have some problems. But it will become one of the most important way to supply their demand for dental treatments, in spite of their latent requirements, a lot of old people give up to undergo dental treatments.

Studies on the castability of Co-Cr alloy for cast plates. Part 1: The effects of sprue attachment direction to wax plates and the investment direction of refractory models.

Co-Cr alloy is used more frequently than Ni-Cr alloy as a non-precious alloy for cast plates in Japan. However, since the melting point of Co-Cr alloy is very high, about 1300 degrees C, and since it oxidizes easily, a vacuum-pressure casting machine capable of melting this alloy in a reduced atmosphere has recently been developed. Using this vacuum-pressure casting machine, the authors studied the effects on the Co-Cr alloy of the attachment direction of sprues and the investment direction of refractory models. The results clarified that in the vacuum-pressure casting method, sprue attachment direction and the investment direction of refractory models have a significant effect (p less than 0.01) on the castability of Co-Cr alloy.

[Case report of catecholamine-secreting carotid body tumor].

We encountered a case of catecholamine-secreting carotid body tumor which has never been reported in Japan. The patient was a 20-year old man, who complained of a mass located at the right neck and hypertensive symptoms such as loss of consciousness at extension of the neck, fatigability, etc. Carotid angiography and computed tomography (CT scan) revealed a carotid body tumor. Before operation, a mild hypertension was noted, serum and urinary levels of norepinephrine showed abnormal increase, and phentolamine test was positive. After the tumor was dissected by a surgical procedure associated with carotid reconstruction, blood pressure and norepinephrine levels clearly trended to decrease and phentolamine test returned to negative. Histopathologically, the tumor was a paraganglioma with so-called "organoid" pattern. The tumor was not chromaffin, but the presence of neurosecretory granules was confirmed under electron microscope. In one-year follow up after the operation, no sign suggestive of "latent" or newly arising paraganglioma was noted in assay of catecholamines in serum and urine, nor on adrenal CT scan.

[Pathomorphological study of local extension and progression of prostate cancer].

The interrelationship among localization of tumor, local extension, lymph node metastasis and various histological grades was investigated in 31 consecutive series of prostate cancer treated by radical prostatectomy. Surgical specimens were examined by step sections cut perpendicular to the urethra. Each tumor was graded histologically in accordance with WHO-Mostofi grading (evaluated by nuclear anaplasia and structural differentiation), Gleason's and M.D. Anderson's grading systems. The areas of the prostate was divided into 4 quadrants delimited vertically along the urethral axis and transversely at the level of utricle, i.e. anterior/superior, anterior/inferior, posterior/superior and posterior/inferior quadrants. Topographical localization and extension of each tumor was examined in accordance with the above area subdivision. Every tumor was growing at least in the posterior/inferior area. All cases with tumor progression to the anterior/superior area also revealed extensive tumor growth in the other two or three quadrants. Therefore, prostate cancer appears to be originated from the posterior/inferior area of the prostate and terminated in the anterior/superior area as local extension. In 31 cases examined, capsular invasion was noted in 22, seminal vesicle invasion in 16, and lymph node metastasis in 12 cases. All cases with seminal vesicle invasion also showed capsular invasion. None of the cases without capsular and/or seminal vesicle invasion had lymph node metastasis. Thus, it is inferred that prostate cancer is initially growing locally, extending to the capsule and seminal vesicle and then metastasizes to the pelvic lymph nodes. As for the histological grading, higher grade tumors tended to show higher incidence of capsular and seminal vesicle invasion and lymph node metastasis.(ABSTRACT TRUNCATED AT 250 WORDS)

Effects of alpha adrenoceptor and dopamine receptor agonists and antagonists on ganglionic transmission.

Effects of alpha adrenoceptor and dopamine (DA) receptor agonists and antagonists on ganglionic transmission were studied in pentobarbital-anesthetized, open-chest dogs. Changes in tachycardia produced by preganglionic cardiac nerve stimulation were monitored as a measure of ganglionic transmission. Several agonists, injected i.a. into the blood supply of the stellate ganglion, inhibited ganglion transmission. This effect was localized to the ganglion as none of the agonists, in the highest doses studied, inhibited tachycardia produced by postganglionic cardiac nerve stimulation. The potency order of the agonists was UK 14,304 (alpha-2 adrenoceptor agonist) greater than norepinephrine (NE) greater than dipropyl DA (DA2 dopamine receptor agonist) greater than DA greater than or equal to phenylephrine (alpha-1 adrenoceptor agonist) greater than fenoldopam (DA1 dopamine receptor agonist). UK 14,304 was over 200 times more potent than fenoldopam, being active in nanomole doses, whereas NE was more potent than DA. Rauwolscine, an alpha-2 adrenoceptor antagonist, antagonized the inhibitory effects of NE and DA and was the only antagonist, given i.a. or i.v., that facilitated ganglionic transmission; frequency-response curves of tachycardia induced by preganglionic nerve stimulation were dose-dependently augmented. SCH 23390, in double the full DA1 antagonist dose, had no effect on frequency-response curves or on NE- or DA-induced ganglionic inhibition. Domperidone antagonized the effect of DA but had no effect on ganglionic inhibition produced by NE or on frequency-response curves of tachycardia.(ABSTRACT TRUNCATED AT 250 WORDS)

[Effects of sevoflurane anesthesia and surgery on plasma catecholamine levels].

Effects of sevoflurane-nitrous oxide-oxygen anesthesia and surgery on plasma levels of epinephrine, norepinephrine and dopamine were evaluated in twenty four patients who ranged in ages from 15 to 65 years. They underwent non-abdominal surgery (orthopedic surgery) or abdominal surgery (gastrointestinal or gynecological surgery). Anesthesia was induced and maintained with sevoflurane in 50% nitrous oxide and 50% oxygen. Succinylcholine was administered intravenously to facilitate tracheal intubation and pancuronium was given intravenously during surgery. Lactated Ringer's solution at a speed of 5-15 ml.kg-1.h-1 was also administered intravenously throughout the procedures. Neither plasma epinephrine, norepinephrine nor dopamine levels changed significantly during the anesthetic induction with sevoflurane anesthesia. Plasma catecholamines were unchanged during and after surgery in patients who underwent non-abdominal surgery, while plasma epinephrine and norepinephrine levels increased significantly during and after abdominal surgery. Plasma dopamine levels, however, were unchanged during and after surgery in these patients. The findings suggest that epinephrine and norepinephrine secretion was significantly stimulated with abdominal intervention but not with orthopedic one under sevoflurane anesthesia.

[Two cases of small intrahepatic arterial aneurysm].

Two cases with intrahepatic aneurysms were experienced. One was associated with hemobilia, the other was found incidentally. These aneurysms had angiographic appearance of pseudo-aneurysm, and occurred in peripheral branches of hepatic arteries, that are rare location for aneurysm formation. Causative factor was demonstrated in neither of the cases.

[Retinal circulation in rhegmatogenous retinal detachment demonstrated by videofluorescence angiography and image analysis. I. The condition of retinal circulation before retinal detachment surgery].

The retinal circulation in cases of retinal detachment was studied in eight eyes with rhegmatogenous retinal detachment. Retinal mean circulation times of the detachment areas and the non-detachment areas were measured by a system using videofluorescence angiography and image analysis with dye dilution technique. The mean circulation times were 4.05 +/- 1.13 seconds (mean +/- standard deviation) in the non-detachment areas and 6.59 +/- 2.91 seconds in the detachment areas. The difference between the two areas was statistically significant (p less than 0.03). The results demonstrated that the velocity of retinal circulation is slower in retinal detachment area.

The interaction between narrow-band radiation of UVA and that of UVB on erythemal reaction in Japanese subjects.

The interaction between ultraviolet A (UVA) and ultraviolet B (UVB) on erythemal reaction was investigated on the skin of the backs of 24 healthy Japanese volunteers, using narrow-band radiation. The minimal erythema doses (MED) for UVB (MEDB) and UVA (MEDA) were determined and UVA and UVB were both applied to the same site in immediate combination with 2 orders of exposure (A + B, B + A). In experiment 1 the interaction between suberythemogenic doses of UVA and UVB to produce a visible erythema was examined; the sum of the fraction of MEDA and the fraction of MEDB was at most equal to 1 for 3 different doses of UVA (1, 2.4, 4.9 J/cm2) with either order of exposure. This interaction is considered to be photoaddition. In experiment 2 a fixed dose of UVB (1.7 MEDB of each subject) was applied in combination with 4 different doses of UVA (1, 2.4, 4.9, 7.4 J/cm2); the erythemal reaction was compared with the grading scale erythema produced by UVB alone and allotted a score of 1 to 7. The total number of MED irradiated was calculated for each combination exposure and the score obtained was compared with the score expected on the basis of that number of MED. The scores for A + B practically coincided with the expected values, whereas those for B + A were decidedly smaller than the expected values. This suggests photoaddition for A + B and photorecovery for B + A in the bright erythema dose range. The relationship between the Japanese skin type (JST) and the waveband interaction was also examined. In experiment 2, when UVA irradiation was followed by UVB, the subjects in JST group III showed photorecovery for high-dose UVA.

Late onset of skin cancers in 2 xeroderma pigmentosum group F siblings and a review of 30 Japanese xeroderma pigmentosum patients in groups D, E and F.

Sib patients with xeroderma pigmentosum (XP), XP90TO (42 years old, male) and XP92TO (40 years old, female, were assigned to group F by the complementation analysis in hybridized heterodikaryons. The XP90TO and XP92TO fibroblasts exhibited the typical XPF characteristics of a threefold higher sensitivity to the lethal effect of 254 nm UV and a reduced level of 12% unscheduled DNA synthesis (UDS) compared with normal cells. Clinically, both patients manifested moderate to severe acute sun sensitivity by age 8, pigmented freckles by age 10 and skin malignancies at higher ages (6 basaliomas at 42 years in XP90TO; 1 basalioma at 41 years in XP92TO). Despite the still currently sun-sensitive state, the patients showed normal minimal erythema dose (MED) at monochromatic wavelengths of 290, 300 and 305 nm but abnormally delayed peaking of erythema reaction at 48 h after exposure. After irradiation with more than 3 MED, XP92TO showed a long persistence of induced erythema for at least 7 days. A review of the 16 reported XPF patients indicated mild skin manifestations, no neurological abnormalities, and more delayed skin carcinogenesis at a lower frequency than that in XPA patients. In addition, we have collected clinical information from Japanese XP patients in rare complementation groups D and E and reviewed their clinical and photobiological characteristics.

[Evaluation of rapid injection immunotherapy by antigen specific IgE and IgG4 antibodies].

Ten children, aged 3 to 12 years old (mean 8), with house-dust-mite sensitive bronchial asthma were hospitalized and administered a course of rapid injection immunotherapy with house-dust (HD) antigen. In order to evaluate rapid injection immunotherapy, we examined total IgE, anti-HD IgE and IgG4 antibodies, anti-mite (Dermatophagoides farinae: DF) IgE antibodies, and anti-DF IgG4 antibodies in their sera serially from 56 to 80 weeks after rapid injection. The results were as follows: 1) All of their clinical courses were markedly improved by this immunotherapy. 2) Total IgE levels were unchanged. 3) Anti-HD IgE antibodies were decreased 5 weeks after rapid administration (p less than 0.01). And anti-DF IgE antibodies were also decreased (p less than 0.05). 4) Anti-DF IgG4 antibodies were increased more than twofold from 2 to 5 weeks after rapid administration (p less than 0.02). Rapid injection immunotherapy produced positive effects in some of the asthmatic children. This seemed to be attributable to the production of antigen specific IgG4 antibodies (said to be blocking antibodies) and the reduction of antigen specific IgE antibodies.

Different sensitivities to ultraviolet light-induced cytotoxicity and sister chromatid exchanges in xeroderma pigmentosum and Bloom's syndrome fibroblasts.

The relationship between ultraviolet (UV) light-induced sister chromatid exchanges (SCE) and UV cytotoxicity was compared among fibroblast strains from 18 patients with xeroderma pigmentosum (XP) (6 group A, 1 group C, 3 group D, 5 group E, 3 variant), 2 patients with Bloom's syndrome (BS) and 3 normal subjects. Spontaneous SCE frequencies in all of the XP strains were indistinguishable from those in normal cells, while BS cells exhibited about 10 times more spontaneous SCE than did normal cells. SCE were induced dose-dependently by UV at doses of 0-4.0 J/m2 in all the strains. The mean frequencies of UV-induced SCE were much higher in XP-A and -C cells than in normal cells. XP-D and -E cells were slightly more sensitive to UV-induced SCE formation than normal cells. XP-variant cells were almost as sensitive as normal cells. The induced SCE frequencies plotted as a function of surviving fraction were relatively lower in XP-A and -D cells than those in normal cells, though in XP-C, -E, and -variant cells they were almost the same as those in normal cells, suggesting that, in XP-A and -D groups, UV damage is more lethal than producing SCE in surviving cells. By contrast, 2 BS strains exhibited significantly higher frequencies of UV-induced SCE than the other strains as a function of not only UV dose but also surviving fraction. These results reflect the essential differences between XP and BS cells in UV induction of SCE.

[Chronic demyelinating polyradiculoneuropathy associated with malignant histiocytosis].

A case of malignant histiocytosis (MH) presenting with peripheral nerve involvement is described. A 67-year-old man initially noted left facial weakness on July 20, 1986. The symptom was improved within a week, however, two weeks later he noticed numbness in his fingers and a burning pain in his legs, which was followed by double vision and progressive weakness in all four limbs. On examination, he was found to have total ophthalmoparesis of the right eye and a trace of the left facial palsy. There was severe weakness of the leg muscles, as well as mild weakness of the arms. This was accompanied by wasting of his limbs. All tendon reflexes were absent. Planter responses were flexor. Although cutaneous sensation was intact, vibratory sense was markedly impaired in the legs below the knees. The sphincter function was mildly disturbed. The sedimentation rate was 32 mm/hr. The hemoglobin was 11.9 g/dl and the leukocyte count was 5,700/mm3. The platelet count fell to 60,000/mm3. Results of routine biochemical and radiological studies were unremarkable. The cerebrospinal fluid protein level was 129 mg/dl with a normal cell count. Motor nerve conduction study revealed marked reduction in amplitude of the compound muscle action potential, slow motor nerve conduction velocities and multifocal conduction blocks along the nerve trunks (the left ulnar nerve in the forearm, the bilateral tibial nerve in the lower leg). F-wave was absent or elicited with prolonged latency and with increased chrono-dispersion. Sensory nerve had normal or nearly normal conduction. EMG sampling showed an impaired interference pattern during voluntary contraction and a few denervation potentials at rest.(ABSTRACT TRUNCATED AT 250 WORDS)

[Transurethral microwave coagulation of bladder cancer in elderly patients].

Since April 1985, 72 patients with bladder cancer have been treated by transurethral microwave coagulation (TUMC). In this procedure, tumors were irradiated with microwave energy of 100 watts for 2-16 minutes. A total of 20 patients were 70 years old or older, including 5 who were more than 80 years old. These 20 patients have been followed for 6 to 32 months with a median follow-up of 14.7 months. After TUMC, tumors completely disappeared endoscopically and histologically in all patients. Four patients had heterotopic recurrences. In all patients, serious complications were not seen during and after TUMC. Our clinical experience revealed that TUMC was the useful therapeutic method for bladder cancer in elderly or poor-risk patients. In this paper, we describe the technique of TUMC and preliminary clinical results.
