Magnetic resonance imaging of a non-pigmented adenoma of the ciliary epithelium.

PURPOSE: A non-pigmented adenoma of the ciliary epithelium is rare. No good documentation has been reported in which magnetic resonance imaging (MRI), histological examination, and immunohistological evaluation have all been conducted. We showed how this tumor differed from ciliary body melanoma. METHODS: We evaluated the ciliary body tumor by MRI, histological examination, and immunohistochemical examination. The data was compared with previously reported cases. RESULT: The tumor was classified as an adenoma of the non-pigmented ciliary epithelium with extracellular pools of hyaluronidase-sensitive mucopolysaccharide. The MRI appearance of the vitreous in our patient showed hyperintensity on T1, isointensity on T2 and marked enhancement with gadolinium. None of 3 other reported patients exhibited the same signals. CONCLUSION: MRI appearance is most variable in adenomas and no microscopical diagnosis can be based on that. Our patient showed isointensity on T2 with marked enhancement by gadolinium when compared to malignant melanoma.

Controlled local application of basic fibroblast growth factor (FGF-2) accelerates the healing of GBR. An experimental study in beagle dogs.

This animal study was performed to ascertain whether the regeneration of membrane-protected bone defects can be accelerated by the controlled application of basic fibroblast growth factor (FGF-2) using a new drug delivery system. Standardized alveolar bone defects were made surgically in 9 beagle dogs, and FGF-2 was administered using specially made collagen minipellets. A minipellet containing either 0.15 microgram FGF-2 (FGF) or 0 microgram FGF-2 (placebo) was placed in the defect or no minipellet was used (control), and bone regeneration was evaluated radiologically, histologically, and histometrically 8 weeks after the operation. Radiographs showed a surprisingly large radiopaque region in FGF sites compared with placebo or control sites. Histologically, mature bone filled the majority of the inner space of the membrane-protected defect in FGF sites. New bone formation was also seen in the control and the placebo sites, however, it filled less than half the area of the defect. Histometrically, the area of regenerated bone in FGF sites was significantly higher than in the other sites (P < 0.01). These results demonstrate that the controlled application of FGF-2 accelerates bone regeneration in membrane-protected bone defects in the canine model.

Xylitol-induced elevated expression of the gbpC gene in a population of Streptococcus mutans cells.

Xylitol possesses a unique property distinct from the other caries-preventive sweeteners. This sugar alcohol cannot be metabolized to acids but is taken up by Streptococcus mutans and accumulated as a toxic sugar-phosphate in the cells, resulting in growth inhibition. Due to the accumulation, xylitol induces biological responses including the emergence of xylitol-insensitive populations. Therefore, we expected another response induced by xylitol and found a new phenomenon, that cells repeatedly cultured in the presence of xylitol evolved into those exhibiting an elevated dextran-dependent aggregation phenotype. This phenotype was found to result from expression of the gbpC gene, which was previously reported to be expressed only under certain stress conditions. Construction of a Strep. mutans isogenic mutant carrying the gbpC::lacZ fusion gene indicated that gbpC expression of cells repeatedly cultured in the presence of xylitol was elevated 20-fold. DNA transfer experiments indicated that this phenotypic change did not appear to be due to a mutation. These cells also exhibited decreased adhesion to glass surfaces when grown in the presence of sucrose. This may be one of the ways by which some populations of Strep. mutans are removed from dental plaques.

Effects of walking on bone quality as determined by ultrasound in the elderly.

In the present study, measurements of broadband ultrasound attenuation (BUA) were used as indications of bone quality in elderly residents of an old people's home. To investigate the possibility of using number of steps walked as an indicator of the role played by exercise on bone quality, we studied the relationship between BUA and the number of steps, taken as a measure of the number of impacts against the ground placing a direct mechanical stress on the bones. The subjects were 59 healthy elderly women with a mean age of 78+/-8 years who had no impairments in walking or daily life. A statistical analysis of the relationship between age, number of steps, BUA, stride length, and walking speed revealed that number of steps and BUA declined with age, and that BUA increased with a greater number of steps. Among other factors, age was found to be negatively correlated with weight (r= -0.343, P<0.01), BUA (r=-0.542, P<0.001), total number of steps (r= -0.524, P<0.001) and past exercise habits (r=-0.425, P<0.001). There were positive correlations between BUA and total number of steps (r=0.606, P<0.001), walking speed (r=0.460, P<0.001), stride length (r=0.373, P<0.01) and past exercise habits (r=0.429, P<0.001). These results show that the benefit to bone quality increases the more a person walks in daily life. The above demonstrates that degree of walking activity is positively correlated to bone quality among the elderly. These results also suggest that, when investigating the relationship between exercise and bone quality, one should look not only at distance and walking speed, but also number of steps as the number of impacts against the ground that place a direct mechanical stress on the bones. This may prove to be a useful indicator of bone quality in future use.

Induction therapy consisting of alternating cycles of ranimustine, vincristine, melphalan, dexamethasone and interferon alpha (ROAD-IN) and a randomized comparison of interferon alpha maintenance in multiple myeloma: a co-operative study in Japan.

This pilot study evaluated the efficacy of a new combination chemotherapy with a newly developed nitrosourea derivative ranimustine and evaluated the efficacy of interferon alpha (IFN-alpha) maintenance in previously untreated patients with multiple myeloma (MM). The induction therapy (ROAD-IN) was a 6-week regimen consisting of chemotherapy with ranimustine, vincristine (Oncovin), melphalan (Alkeran) and dexamethasone starting on day 1 and IFN-alpha, which was administered three times weekly for 3 weeks starting on day 22. This was repeated for three cycles. The responders were subsequently randomized into two groups that received or did not receive IFN-alpha as maintenance therapy. Of the 164 patients registered, 161 were evaluated. An objective response to induction therapy was seen in 75% of patients; complete remission (CR) in 38 (24%) and partial remission (PR) in 82 (51%). The median survival for all patients was 3.6 years from registration. The survival of responders (CR + PR) was significantly better than that of non-responders (median survival 4.3 years vs. 1.4 years; 7-year survival rate 32% vs. 9%; P < 0.0001). The IFN-alpha maintenance did not show any advantage for either response duration or survival. This pilot study demonstrated that a comparatively short period of induction therapy with the ROAD-IN regimen produced a rather high response rate and a similar survival rate to those achieved with other longer induction regimens, and that good responders to the initial therapy survived significantly longer than non-responders.

A biomechanical effect of wide implant placement and offset placement of three implants in the posterior partially edentulous region.

To prevent loosening or fracture of screws retaining the prosthesis to the implants in the posterior partially edentulous region, the use of staggered buccal and lingual offset placement or wide implants is suggested. However, it is not known how this usage compensates for the torque produced by lateral occlusal forces. This study evaluated the effectiveness of offset placement of three implants and a wide implant placement at the most posterior site. Three-dimensional geometric analysis was used to calculate the tensile force applied to gold screws in clinical situations with buccal or lingual loading perpendicular to cuspal inclination (10 or 20 degrees ). Four variations of the placement of three implants (3.75 mm) are: (1) straight; (2) buccal offset of the second implant; (3) lingual offset of the second implant; (4) a wide implant (5 mm) placement at most posterior site. The offset placement did not always decrease tensile force at the gold screw, but wide implant placement and decrease in cuspal inclination did.

Finite element analysis of stress relaxation in soft denture liner.

To gather knowledge related to establishing criteria for selecting soft denture liners for individual patients, the effects of certain properties of soft denture liners on stress distribution were evaluated by two-dimensional finite element analysis. A partial mandibular edentulous ridge crest was modelled. Six combinations of thickness (mucosa: 1 or 2 mm; soft denture liner: 1, 2, or 3 mm) and 18 combinations of Young's modulus (mucosa: three kinds; soft denture liner: six kinds) were analysed. The ratio of maximum to minimum stress in the mucosa (stress ratio) was calculated to estimate stress concentration. In the case of thin mucosa (1 mm thickness), the lower the Young's modulus of the soft denture liner, the lower the stress ratio. However, if the soft denture liner had a lower Young's modulus than the mucosa, stress concentrated adversely. These results suggest that the elasticity of the soft denture liner should match the elasticity of the mucosa to obtain the optimum cushioning effect.

A method for quantifying overall satisfaction of complete denture patients.

Reproducible and quantitative evaluation of patient satisfaction with their complete dentures is of great importance for preoperative diagnosis, treatment planning and assessment of complete denture treatment outcome. This study attempted (1) to clarify the degree of contribution of various factors to overall satisfaction, and (2) to develop a method for quantitative assessment of overall satisfaction with complete dentures. Twelve satisfaction factors and a three-grade scale were used to assess 302 complete denture patients. The contribution of each grade of the 12 factors to the overall satisfaction level was determined by multiple regression analysis. Seven factors were highly correlated to the overall satisfaction. Based on the level of contribution by these seven significant factors, a scoring method for satisfaction was established. Category scores for these factors were calculated and the sum of the category scores was converted to an integer between 0 and 100. The resulting quantification score was closely correlated with overall satisfaction. A protocol for scoring overall satisfaction was developed based on the scientifically analysed contribution of each satisfaction factor.

Biomechanical effects of double or wide implants for single molar replacement in the posterior mandibular region.

Double implants have been thought to have biomechanical advantages for single molar replacement. To evaluate the effectiveness of double implants versus a wide implant, the vertical forces and torque on each implant were calculated by three-dimensional geometric analysis. Buccal load (100N) perpendicular to cuspal inclination (20 degrees) was applied at the occlusal surface of the superstructure. The three kinds of load points (A, B, C) were 1.5, 3.5, and 5.5 mm from the mesial contact point, respectively. Three implants were compared: mesial and distal double implants (phi 3.3 mm), and a wide implant (phi 5 mm). The wide implant showed torque around the long axis (1.8-15.0 N x cm) whereas double implants had no torque. On the other hand, the vertical forces on the mesial double implant were both smaller (60%: loaded at point C) and larger (140%: loaded at point A) than the wide implant. Given the smaller surface area of the mesial double implant, this large force may generate much higher stress in the peri-implant bone. These results suggest that the biomechanical advantage of double implants for single molar replacement is questionable when the occlusal force is loaded at the occlusal surface near the contact point.

Dimensional measurement and finite element analysis of I-bar clasps in clinical use.

An I-bar clasp is a popular retainer for distal-extension removable partial dentures. However, there have been almost no evidence-based criteria on the mechanically preferable shape. The present study aimed to investigate the variations of dimension in I-bar clasps used in patients, and to clarify the effect of the variations on stiffness and stress of I-bar clasps by finite element analysis. Dimensions (thickness, width, taper, radius of curvature, length, relation to oral structures) of 23 I-bar clasps were measured. A three-dimensional finite element model was made for each measured I-bar clasp with vertical and horizontal straight sections connected with a curved section. A concentrated load of 5 N was applied at the lowest point of the tip that contacted the abutment in the buccal direction. Maximal equivalent stress and stiffness of each clasp were evaluated. The measured dimension, stiffness, and maximum stress showed wide variations. Mean stiffness was far from the proper one, and mean stress was near the proportional limit of Co-Cr alloy. Considering the stiffness and stresses in this study, only six clasps out of 23 were appropriate. These results suggest that evidence-based criteria of preferable shape of I-bar clasps should be determined.

Malaria prevalence and a brief entomological survey in a village surrounded by rice fields in Khammouan province, Lao PDR.

We surveyed Nongceng, a village in a south-eastern province of Lao PDR, for malaria and its vectors. Nongceng is situated in a basin and surrounded by rice fields. In February 1998 (dry season), 28.6% of 126 villagers were infected with malaria, and in September 1998 (rainy season), 16.3% of 147 villagers. The prevalence of malaria infection was consistently high in children under 10, and the predominant malaria species was Plasmodium falciparum. In brief surveys of the mosquitoes performed on the same day as the malaria surveys, 2007 Anopheles females from 12 species were collected by means of human bait, animal bait and resting collections. Of the vector species known to be important in transmitting malaria in neighbouring Thailand - An. minimus, An. dirus, and An. maculatus groups - only An. minimus was found. Its density was, however, very low in both seasons and it was therefore unlikely to be the vector. In fact, An. nivipes accounted for more than 65% of all mosquitoes collected and was the most common species collected from human baits. The results of this study show that endemic areas of malaria in Lao PDR are not necessarily related to forest. Rather, An. nivipes is suspected to be the most important vector.

Delayed resolution of intracranial germinoma after radiotherapy: a preliminary study of the correlation between histology and magnetic resonance imaging.

A central nervous system germinoma is curable in most cases by radiotherapy, and most of the tumor mass disappears promptly after 20-30 doses. However, some germinomas take a longer period to vanish completely from magnetic resonance imaging (MRI) or computed tomography (CT) scans. In such cases, the tumor may contain components such as teratoma. The aim of this study was to clarify the nature of the persistence of some germinomas. Five cases of histologically verified germinoma in which radiotherapy was performed to treat residual tumor were selected. The doses of focal radiotherapy and whole brain radiotherapy were 10-20 Gy and 20-34 Gy, respectively. In these cases, correlation was made between the degree of persistence of the tumor when assessed by MRI and the amount of interstitial content, as determined by histology. The histological evaluation, using hematoxylin-eosin stain, silver impregnation and Azan staining was carried out independently of clinical information. The tumor vanished soon after radiotherapy in three cases, but 3-15 months passed before the tumor completely vanished from the MRI scans in the other two cases. The histology of the cases in which the tumor disappeared rapidly was predominantly of large tumor cells and only small amounts of reticulin. However, in the persistent tumors, large amounts of reticulin and vascular components were present. Thus, it is proposed that tumors with a large parenchymal component disappear soon after radiotherapy, whereas tumors composed mainly of interstitial component persist. Long-standing enhancement seen on MRI or CT scans of patients with an intracranial germinoma is indicative of a large amount of interstitial component in the tumor.

Brain surface ependymoma.

A case of brain surface ependymoma is reported here. A 41-year-old woman presented with weakness of the right upper limb. Magnetic resonance imaging revealed a left fronto-parietal cystic tumor with a calcified mural nodule. Total removal of the tumor was performed. This tumor was located within the surface portion of the fronto-parietal parenchyma and was sharply demarcated from the surrounding brain tissue, and there was no continuity with the ventricles. Histological examination of this tumor showed monotonous proliferation of tumor cells with moderate cellularity and occasional mitoses, forming true ependymal rosettes and perivascular pseudorosettes with fibrillary background. A clear cell component resembling oligodendroglioma was partly noted. The tumor cells showed positive immunoreaction for glial fibrillary acid protein and vimentin.

Semiquantitative immunoblot analysis of nm23-H1 and -H2 isoforms in adenocarcinomas of the lung: prognostic significance.

Total amounts of nm23 protein and relative levels of H1 and H2 isoforms were studied in 27 fresh-frozen samples of pulmonary adenocarcinoma and adjacent non-neoplastic tissues that were obtained at surgery. Semiquantitative immunoblotting with a monoclonal antibody (Pan-242) against nm23 protein demonstrated both isoforms, recognized as 20.5 kDa for H1 and 18.5 kDa for H2, to be present in all cases. Both H1 and H2 levels in neoplastic tissues were higher than in the corresponding non-neoplastic samples. Expression of H2 was usually greater than of H1. The H2/H1 ratio varied from 1.9 to 14.1 (mean value 5.2) in non-neoplastic tissues and 1.0-5.9 (mean value 2.5) in neoplastic tissues, although this ratio did not correlate with any prognostic factor like tumor size, nodal status or distant metastasis (TNM tumor stage). H1 and H2 levels were significantly lower (mean values 4.3 and 2.4) in well-differentiated than in moderately and poorly differentiated adenocarcinomas (8.3 and 3.0) (P < 0.03 and P < 0.05, respectively). These data indicate that H1 and H2 isoform levels correlate with histological differentiation, but not the metastatic potential or stage of pulmonary adenocarcinoma.

Inhalation of nebulized nitroglycerin in dogs with experimental pulmonary hypertension induced by U46619.

BACKGROUND: Pulmonary hypertension (PH) causes mortality in some congenital and acquired heart and lung diseases. However, inhalation of NO gas requires complicated and expensive instruments and elaborate preparations to avoid toxic gas administration. We tested the effectiveness and safety of inhaled nebulized nitroglycerin (Neb-NTG) in dogs with experimental PH. METHODS: Experimental PH was induced by continuous infusion of a thromboxane analog (U46619). The U46619 infusion rate was adjusted to maintain a systolic pulmonary artery pressure (PAP) at 40 mmHg in 10 anesthetized and mechanically ventilated dogs. Then, 20 micrograms/kg of NTG liquid nebulized by compressed air was inhaled. RESULTS: After infusion of U46619, the systolic, diastolic and mean PAP increased by 119%, 228% and 169%, respectively, and the systolic, diastolic and mean systemic arterial pressures (SAP) increased by 19%, 29% and 23%, respectively. The systolic pulmonary to systemic pressure ratio (Pp/Ps) and mean Pp/Ps increased by 83% and 113%, respectively, and the pulmonary vascular resistance (PVR), systemic vascular resistance (SVR) and pulmonary to systemic resistance ratio (Rp/Rs) increased by 341%, 100% and 145%, respectively. After inhalation of Neb-NTG in dogs with experimental PH, systolic, diastolic and mean PAP and PVR decreased by 25 +/- 4, 26 +/- 11, 25 +/- 9 and 31 +/- 21%, respectively. There were no significant changes in systolic, diastolic and mean SAP, SVR, cardiac output and plasma methemoglobin concentrations. The systolic and mean Pp/Ps decreased by 18 +/- 7 and 20 +/- 7%, respectively. The Rp/Rs decreased by 25 +/- 13%. CONCLUSIONS: The results of this study demonstrate that Neb-NTG is an effective and selective pulmonary vasodilator and may offer a new therapeutic option for PH.

Exercise training prevents maturation-induced decreases in insulin receptor substrate-1 and phosphatidylinositol 3-kinase in rat skeletal muscle.

We have previously reported that exercise training prevents a maturation-induced decrease in insulin sensitivity and suggested that an improvement of insulin sensitivity by exercise training was attributable, in part, to an increase in insulin-sensitive GLUT-4 on the skeletal muscle plasma membrane. In this study, we examined the effects of maturation and exercise training on the gene expression and protein content of the components of post-insulin receptor signal transduction in rat skeletal muscle. Rats aged 3 weeks were sedentary or trained by voluntary running through 4 or 27 weeks of age, and then the rats in both the sedentary and trained groups were killed and the gastrocnemius muscle was immediately removed for analysis of mRNA and protein content. The concentration of mRNA and protein for insulin receptor substrate-1 (IRS-1) in sedentary rats significantly decreased with maturation (49% and 63%, respectively, at age 27 weeks v age 4 weeks), but in trained rats they did not decrease with maturation. Although the level of phosphatidylinositol 3-kinase (PI 3-kinase) mRNA in sedentary rats was not altered with maturation, PI 3-kinase protein in sedentary rats significantly decreased with maturation (73% at 27 weeks v 4 weeks). However, PI 3-kinase protein in trained rats did not decrease with maturation. These results suggest that the prevention of maturation-induced decreases in the protein content of IRS-1 and PI 3-kinase is involved in the mechanisms responsible for the improvement of insulin sensitivity by exercise training, and exercise training may affect transcriptional regulation of the IRS-1 gene and posttranscriptional regulation of PI 3-kinase expression.

Insulin resistance in patients with depression and its changes during the clinical course of depression: minimal model analysis.

A high proportion of patients with depression develop glucose intolerance accompanied by hyperinsulinemia, suggestive of reduced insulin sensitivity (insulin resistance). The aim of this study was to evaluate insulin sensitivity in patients with depression and its changes during the clinical course of depression. Twenty nondiabetic patients with depression (13 males and 7 females aged 44+/-14 years; body mass index [BMI] 23.2+/-2.8 kg/m2) were prospectively studied by the frequently sampled intravenous glucose tolerance test (FSIGT) and the oral glucose tolerance test (OGTT) before and after treatment of depression, and an age-, sex-, and BMI-matched control group (n = 13) was examined once by the FSIGT. Metabolic indices measuring glucose effectiveness at basal insulin (SG) and insulin sensitivity (SI) were derived from minimal model analysis. Each patient was treated by cyclic antidepressants with an 1,800 to 2,200 kcal/d food intake and underwent no exercise therapy. SI was significantly lower in patients before treatment versus control subjects (6.0+/-2.5 v 13.8+/-8.6 x 10(-5) min(-1) x mol(-1) x L, P < .01). After treatment of depression, a significant increase in SI (10.7+/-7.5 x 10(-5) min(-1) x mol(-1) x 1, P <t.01) was observed without changes in the BMI, fasting blood glucose, and SG. This was associated with a decrease in the insulin response during the OGTT and FSIGT. We conclude that patients with depression have impaired insulin sensitivity and resultant hyperinsulinemia and that these abnormalities can be resolved after recovery from depression.
