[Comparison of two-dimensional echocardiography and Tl-201 SPECT in the evaluation of ischemic heart disease].

To ascertain the influence of severity of coronary stenosis and the presence or absence of collateral circulations on echocardiographic (2DE) asynergy or abnormal myocardial perfusion by exercise Tl-201 SPECT, we performed a correlative study of 40 patients undergoing coronary angiography, including 27 with myocardial infarction and 13 with angina pectoris, whose mean age was 55 years. Each view of the left ventricle (LV) obtained by both methods was assigned to five segments; i.e., anterior, septal, inferior, lateral and apical. The correlation of abnormal segments was investigated using both methods. The segments showing a severe perfusion defect coincided those with severe asynergy. Among 116 segments with normal perfusion, 99 (85%) revealed normal wall motion, but the remaining 15% showed asynergy. Among 84 segments with abnormal myocardial perfusion, 26 (31%) had normal wall motion, and most of these were non-infarcted regions. Concerning the severity of coronary artery stenosis, greater than 99% coronary arterial stenoses existed in most of the segments showing a complete defect or incomplete redistribution on Tl-201 SPECT and akinesis or hypokinesis on 2DE. Collaterals were observed in 22% of the segments with a complete defect and 66% of segments with incomplete redistribution; whereas, there was no significant correlation between the severity of LV wall motion abnormality and the presence or absence of collaterals. The severity of asynergy is possibly related to the severity of abnormal myocardial perfusion, however, asynergy could occur at the site of ischemic regions where Tl-201 SPECT can not detect the abnormalities. In patients with myocardial infarction who show incomplete redistribution by exercise Tl-201 SPECT, temporal ischemia of collaterals might cause such a perfusion abnormality.

[Objective evaluation of the mandibular position by the change in the function of the middle ear. 1. Centric relation and lateral eccentric position].

Tympanometric measurement has been made on centric relation and working side and non-working side in 19 healthy subjects (12 males and 7 females aged 23-27) and the following conclusions have been obtained: 1. The Tympanogram in the healthy subjects showed a symmetrical pattern, being of type A bilaterally, in all the cases, and no lateral or sexual difference was seen any item of measurement. 2. S. C. value increased in order of centric relation, working side and non-working side, but decreased in IMP., PRS. and EAC.. 3. Significant differences were observed in the items of measurement regarding S. C. and IMP. all between centic relation and working side, centric relation and non-working side, working side and non-working side. These findings show a correlation between the function of middle ear and the horizontal mandibular position. Therefore these findings suggest the possibility of tympanometric measurement becoming one of the parameters as the method for objective evaluation of the mandibular position as the change in the function of the middle ear.

[The regulation of re-selection form in the aesthetic factor of upper anterior artificial teeth].

Sixty one cases of selection error due to the esthetic factors of artificial tooth forms were investigated out of the 72 cases of selection error which were committed at the department of Prodosthontics in Nihon University school of dentistry at Matsudo for 17 years from 1971 to 1988. The following were the results of investigation, 1. The cases of S-type were changed to those of ST-type, which emphasized the Tapering type factors. 2. The those of ST-type were changed to those of SO-type and S-type which emphasized the Ovoid type factors and got rid of the Tapering type factors. 3. The those of SO-type were changed to those of STO-type which emphasized the Tapering type factors. 4. The those of T-type were changed to those of TO-type which emphasized the Ovoid type factors. 5. The those of O-type were changed to those of TO-type and STO-type which emphasized the Tapering and Square type factors. The regulations of re-selection showed the tendency of changing from single form to complex form, emphasizing the Tapering type factors.

[Interferon-alpha for the treatment of retinal vasculitis associated with human T-lymphotropic virus type I myelopathy (HAM)].

Interferon-alpha was effective for the treatment of retinal vasculitis and vitreous opacity in a patient with human T-lymphotropic virus type I associated myelopathy (HAM). The patient was a 40-year-old male with retinal vasculitis, vitreous opacity and Koeppe's iris nodules. Systemic administration of corticosteroid hormone was not fully effective for the treatment of ocular involvements except for iris nodules. After Interferon-alpha treatment, given 3 million IU/day as the intramuscular injection for 4 weeks, the retinal vasculitis subsided and vitreous opacity disappeared.

[A case of spontaneous regression of malignant lymphoma, complicated with miliary tuberculosis, liver dysfunction and pancytopenia].

An 82-year-old woman was admitted to Iwamizawa Rosai Hospital, Cardiovascular Medicine, for detailed examination of an aneurysm of abdominal aorta. CT scan revealed marked swelling of para-aortic lymph nodes which regressed spontaneously in three months. She was re-admitted to our hospital because of general malaise and gait disturbance. Her chest X-ray on the second admission revealed miliary disseminated shadows, which were confirmed to be tuberculous granuloma by lung biopsy. She was initially well controlled with anti-tuberculosis drugs, followed by severe liver dysfunction and pancytopenia, and died of respiratory and cardiac failure. At autopsy, wide-spread malignant lymphoma and miliary tuberculosis of the lung were found. The cause of liver dysfunction was strongly suspected to be due to infiltration of the lymphoma cells to portal triads of the liver. Hemophagocytosis found in the bone marrow, spleen and liver might be consistent with clinically so-called "hemophagocytic syndrome", causing pancytopenia, associated with infection.

[A case of tracheoesophageal fistula in Behçet's disease repaired with pericardial patch and gastric roll].

A 23-year-old man suffering from Behçet's disease developed a tracheoesophageal fistula. Subtotal esophagus was stripped and replaced with a gastric tube along greater curvature. The defect of membranous portion of the trachea (2 X 1 cm) was closed with a pedicled pericardial patch and covered with omentum. Transient postoperative recurrent nerve palsy was recovered in about 2 months. He is going well without any significant complaints. He received steroid hormone administration and platelets transfusion because of complicated ITP (Idiopathic Thrombocytopenic Purpura).

[High-dose leucovorin and 5-fluorouracil in advanced gastric and colorectal cancer. High-Dose Leucovorin and 5-FU Study Group].

A multicenter cooperative study was conducted from June 1988 to July 1989 to evaluate the clinical efficacy of high-dose dl-Leucovorin (dl-LV) and 5-FU treatment in 61 cases of advanced gastric and colorectal cancer. The administration schedule was a 2-hour infusion of dl-LV (500 mg/m2) and an IV bolus of 5-FU (600 mg/m2), given 1 hour after the beginning of LV infusion. Patients (pts.) were treated q week x 6 then evaluated for response. Thirty one gastric cancer pts. were divided into two groups; nine pts. treated with 30 min. infusion of 5-FU, and the remaining 23 pts. treated with IV bolus. PR was obtained in 2/9 (22.2%) and in 7/22 (31.8%) of the first and second group, respectively. An overall response rate was 9/31 (29%). Thirty colorectal cancer pts. were divided the same: 13 pts. treated with 30 min. infusion of 5-FU and the remaining 17 pts. treated with IV bolus. PR was obtained in 2/13 (15.4%) and in 7/17 (41.2) of the first and second groups, respectively. An overall response rate was 9/30 (30%). Median survival time for the gastric cancer group was 9.4 months, and for the colorectal cancer group was 13.6 months. Toxicity was within acceptable limits. Toxic effects included diarrhea, stomatitis, anorexia and myelohypoplasia. Our data suggests that high dose LV and 5-FU seems to be a very promising combination and warrants a further investigation.

[Iris nevus (Cogan-Reese) syndrome--clinicopathological correlations].

Clinicopathological correlations of iris nevus (Cogan-Reese) syndrome were studied by light and electron microscopy, using tissues obtained surgically by trabeculectomy and peripheral iridectomy. The patient was a 52-year-old female, who had a typical appearance of the disease with nodular iris nevi, distorted pupil, ectropion uveae, peripheral anterior synechia, and intractable glaucoma. The histopathological studies of the specimen revealed abnormal corneal endothelialization and basal lamina formation continuously covering the peripheral cornea, trabecular meshwork, and anterior iris surface. Lymphocyte infiltration was found in the layer of the endothelialization and in the anterior iris stroma. The nodular iris nevi were found to consist of mainly integration of degenerated iris melanocytes and clump cells of Koganei. Neovascularization associated with infiltration of lymphocytes and macrophages was also observed in the trabecular meshwork. The present study suggests that the disease might occur by chronic inflammation primarily as corneal endothelitis and iritis.

Ubiquitous formation of catalase compound II in hemoglobin-free perfused rat liver and detection of novel spectral species.

Spectral examinations of hemoglobin-free perfused rat livers with a high-sensitivity reflectance spectrophotometer have revealed an accumulation of catalase Compound II to an amount comparable to that of Compound I under the aerobic steady state. This finding is in contrast to a recent proposal that NADPH associated with catalase both prevents and reverses the accumulation of Compound II (Kirkman, H. N., Galiano, S., and Gaetani, G.F. (1987) J. Biol. Chem. 262, 660-666). Furthermore, spectral species with a broad peak extending from 550 to 600 nm were observed in a time range between the methanol-induced decays of Compound I and Compound II. When rats were treated with 3-amino-1,2,4-triazole, catalase Compound I was not detected but spectral species with peaks at 570, 556 and 530 nm were observed. These novel spectral profiles suggest contributions from "peroxy" and "ferryl" forms of cytochrome oxidase.
