A case of systemic light chain deposition disease in a 48-year-old man is presented. Clinically, the patient showed the signs of multiple organ (liver, heart and kidney) failure, but multiple myeloma was not diagnosed. Autopsy revealed generalized deposition of hyaline, a Congo red-negative substance, especially in the arterial walls of various organs. In the bone marrow, myelomatous proliferations of plasma cells positive for kappa light chain were recognized. The deposited substance was ultrastructurally different from amyloid fibrils, and was identified as kappa light chain by immunohistochemistry. A liver tissue extract was immunobiochemically examined and the deposited substance was confirmed to consist of kappa light chain, its molecular weight being approximately 14,000 to 17,000 Da.
Ten cases of ameloblastoma including a case of gingival origin and a case of atypical ameloblastoma were studied immunohistochemically, especially with monoclonal and polyclonal anti-keratin antibodies. Stellate reticulum cells reacted strongly with monoclonal and polyclonal anti-keratin antibodies but columnar cells were reacted weakly. The level of keratinization of columnar cells was similar to that of the basal cell layer of the gingival squamous epithelium. Our immunohistochemical studies suggested that the process of keratinization-ameloblastoma follows the sequence of 1) cuboidal cells 2) columnar cells 3) stellate reticulum cells and 4) metaplastic squamous cells. The findings also disclosed the epithelial nature of granular cells. The reactivity of the components of atypical ameloblastoma differed from that of other cases. Other positive "markers" in ameloblastomas included CEA and CA19-9, but no particular histologic differences were observed between positive and negative cases.

The involvement of histamine in mediating gastric function under normal and pathological conditions has been largely established. Histidine decarboxylase (HDC) is the only synthetic enzyme of histamine in the mast cell or enterochromaffin like (ECL) cell. In this study, we examined, the activity of HDC by a method based on the modification of Kobayashi's method, the histological distribution of HDC containing cells was determined by an immunohistochemical method and the histamine concentration by a HPLC method in rat stomach. Histamine concentration in the total gastric layer of the fundus is overwhelming large compared to that in the antrum. On the other hand, histamine concentration of the mucosal layer of the fundus is 72.1 +/- 0.4 micrograms/g (mean +/- S.D.) and that in both the muscular and serosal layers is 7.2 +/- 0.4 micrograms/g. HDC activity of the total gastric layer of the fundus was about ten times as high as that in the antrum. Histological distribution of HDC-containing endocrine cells was observed in the basal part of the gastric mucosa of the fundus, but could not be observed in the muscular and serosal layer. These results suggest that HDC-containing endocrine cells, in only the basal parts of the mucosal layer of the fundus, were similar to ECL cells and have important parts for biosynthesis of internal gastric histamine. Therefore, using the total gastric layer of the stomach was thought to be easier than using only the mucosal layer for measuring gastric histamine and HDC-activity in rats.
Localization of glutathione-peroxidase (GSH-PO) in adrenal cortical cells of neonatal rats were determined by immunocytochemical analysis. GSH-PO first appeared 14 days after birth. Intracellular localization of GSH-PO was mainly in cytosol (cytosol GSH-PO) but no intramitochondrial localization of GSH-PO was detected. Twenty-one days after birth, intramitochondrial localization of GSH-PO (mitochondrial GSH-PO) was present. Mitochondrial GSH-PO depends on ACTH stimulation; therefore, its presence in the adrenal cortical cells of neonatal rats may be the morphologic expression of the gradual acquisition of adult metabolic features during steroidogenesis. The intracellular GSH-PO staining pattern in adrenal cortical cells therefore should be a useful marker for steroidogenesis.
Mouse monoclonal antibodies E29 and 115D8 have been raised in the human milk fat globule membrane. The distribution of the antigen detected by these two monoclonal antibodies has been examined in formalin-fixed paraffin-embeded human ovarian tumors by an indirect immunoperoxidase technique. The highest positive rate of E29 and 115D8 was obtained in the malignant group suggesting that there is relationship between this positive rate and the histological malignancy. As for serous cystadenocarcinomas, a strong staining area of E29 was observed in the diffuse cytoplasm in poorly differentiated cases. These results suggest that the presence of E29 is a useful tumor marker for ovarian cancer.
A 75-year-old woman with anemia admitted to our hospital and was found to have a polyp in the duodenal bulb. The biopsy specimen was histologically diagnosed as an adenoma. A barium enema examination showed an obstruction in the ascending colon and a biopsy done on a specimen of the ascending colon revealed colonic cancer. A histology of the resected specimen of the duodenal bulb revealed a tubular adenocarcinoma in an adenoma, and a similar examination of a colonic resected specimen revealed a mucinous, papillary carcinoma. A primary malignant tumor in the duodenum is an uncommon tumor and our case is first in Japan showing three malignant tumors with an early duodenal cancer.
Deposition of immunoglobulins in the glomeruli by immunoperoxidase (IP) and/or immunofluorescence (IF) using unfixed materials obtained from 25 patients with IgA nephropathy (IgAN) was evaluated. The results indicated that the deposition of IgA, IgM or IgG in the glomerular capillary walls using a peroxidase anti-peroxidase (P-AP) method was more prominently detected than by using IF. The results of double staining indicated that both P-AP and IF were able to stain mesangial and capillary depositions of IgA was more widely detected by P-AP than by IF. It is concluded that the P-AP method has the advantage of indicating the deposition of immunoglobulins in glomerular capillary walls, and evaluating the histopathological significance together with IF in patients with IgAN.

Indirect immunoperoxidase staining was carried out on seminomatous tumors of testes (22 specimens) using anti-placental alkaline phosphatase antibody to search for the usefulness of placental alkaline phosphatase as a tumor marker. All of the seminomas showed positive staining mainly on the cell surface. However, the normal testes and non-seminomas showed negative staining. Of interest, normal tissues outside the tumor mass showed positive staining. This phenomenon is considered to be suggestive of carcinoma in situ. Immunohistochemical study of placental alkaline phosphatase on the seminomas is a useful method for histopathological diagnosis and may be useful for understanding the pathogenesis of the tumor.
A rare case of hemangiopericytoma of the breast is reported with an electron microscopic study. The differential diagnosis is discussed. Ultrastructural study confirmed the diagnosis by revealing that the tumor cells were separated from the endothelial cells of the capillary and had abundant cytoplasmic filaments that formed dense plaques along the plasma membrane. The patient has remained free of tumor for four years after tumorectomy. A review of the literature on reported cases is also presented. Microscopically, benign and malignant forms could not be distinguished easily. It is necessary to followup behavior for more than five years if the tumor does not seem malignant.

We hereby report light and electron-microscopic (EM) studies on an unusual case of molluscum contagiosum occurring in an epidermal cyst. It was considered that the pre-existing cyst was inoculated with molluscum contagiosum viruses, which were confirmed electron-microscopically. We also would like to re-emphasize the usefulness of formalin fixed tissue for EM confirmation of viruses.
The correlation between adrenal cortical steroid metabolism and ultrastructural alterations was studied in rat adrenal glands stimulated by 4-aminopyrazolopyrimidine (4-APP) and ACTH. Ultrastructural alterations were focused on the outer fasciculata (Outer zone) and inner fasciculata-reticularis (Inner zone) of the rat adrenal cortical cells. In 4-APP administered rats, the most striking ultrastructural alteration was noted in the characteristic tubular profiles of smooth endoplasmic reticulum, mainly of the Inner zone. In ACTH administered rats, proliferation of fine tubulo-vesicular profiles of smooth endoplasmic reticulum was frequently seen in intermitochondrial space in both the Outer and the Inner zones. On the other hand, by 4-APP plus ACTH administration, the most striking ultrastructural changes were detected in mitochondria, mainly of the Inner zone. These mitochondria had a highly electron dense matrix and protrusions or blebs of mitochondrial outer membrane were frequently observed. In contrast, no remarkable changes were detected in the Outer zone of the cortex, except a slight enlargement of the mitochondria. On the bases of our findings, it is strongly suggested that the Inner zone of the cortex after 4-APP plus ACTH administration, at least in part, probably corresponds to highly activated steroid synthesis and it is further, speculated that steroidogenesis in the adrenal cortical cells after 4-APP administration is partially impaired in mitochondrial 11 beta-hydroxylase system.
Mitochondrial alteration in aged rat adrenal cortical cells were studied. Severe mitochondrial alterations were predominantly observed in the zonae fasciculata and reticularis of the adrenal cortex. Many adrenal cortical mitochondria from these zones were relatively larger in size and more irregular in shape. The matrix of altered mitochondria appeared denser than normal and the intramitochondrial cristae including myelin-like membrane configurations and parallel arrays were seen. Intramitochondrial inclusions such as paracrystalloid structures were also noted. In addition, giant mitochondria with various inclusions such as highly electron dense lipid-like materials, large vacuoles and other cytoplasmic organelles including small mitochondria were characteristically observed. Based on these data, functional aspects of the altered mitochondria in aged rat adrenal cortical cells were discussed.
