Magnifying endoscopic observation of the gastric mucosa, particularly in patients with atrophic gastritis.

The gastric mucosal surface was observed using the magnifying fibergastroscope (FGS-ML), and the fine gastric mucosal patterns, which were even smaller than one unit of gastric area, were examined at a magnification of about 30. For simplicification, we classified these patterns by magnifying endoscopy in the following ways; FP, FIP, FSP, SP and MP, modifying Yoshii's classification under the dissecting microscope. The FIP, which was found to have round and long elliptical gastric pits, is a new addition to our endoscopic classification. The relationship between the FIP and the intermediate zone was evaluated by superficial and histological studies of surgical and biopsy specimens. The width of the band of FIP seems to be related to the severity of atrophic gastritis. Also, the transformation of FP to FIP was assessed by comparing specimens taken from the resected and residual parts of the stomach, respectively. Moreover, it appears that severe gastritis occurs in the gastric mucosa which shows a FIP. Therefore, we consider that the FIP indicates the position of the atrophic border.

Carcino-fetal proteins and gastric cancer: the site of alpha-fetoprotein synthesis in gastric cancer.

Although several investigators have reported serum alpha-fetoprotein positive gastric cancer with or without metastasis to the liver, its site of production is still unclear. We studied on the site of alpha-fetoprotein synthesis in our cases which showed remarkably high level of serum alpha-fetoprotein, and clarified the presence of alpha-fetoprotein producing gastric cancer by means of direct immunofluorescent technique. However, we also knew the hepatocytes adjacent ot the metastatic lesions could also synthesize alpha-fetoprotein, although these hepatocytes were not known whether under regeneration or degeneration. Through this study, the other carcinofetal proteins such as carcino-placental alkaline phosphatase and CEA were examined using the sera or tumor tissues. Our results will support the idea that cancer is the disease of cell differentiation.

[Pharmacological study of 9 alpha-fluoro-11 beta, 17, 21-trihydroxy-16 beta-methylpregna-1, 4-diene-3,20-dione-17-benzoate (betamethasone-17-benzoate, MS-1112), a local anti-inflammatory agent. (1). Its anti-inflammatory and other pharmacological properties].

The anti-inflammatory activity and the other pharmacological properties of MS-1112, a new steroid compound, were examined and compared with other glucocorticoid analogues such as hydrocortisone acetate (Hydr), betamethasone 17-valerate (Val) and dexamethasone (Dexa). Systemically administered MS-1112 and glucocorticoids had a significant effect in inhibiting rat paw edema induced by various phlogistic stimulations and increasing vascular permeabilities and granuloma formation by cotton pellet or granuloma pouch. The order of those inhibiting activity was, in general, Dexa greater than MS-1112 greater than Val greater than Hydr. Concomitantly, Xthymolysis and adrenal weight suppression and reduced rate of body weight gain after multiple systemical administration were also observed. Locally administered MS-1112 caused an inhibiting activity, without systemic side-effects. This activity was approximately 3 to 5 times more potent than that of Dexa in rat carrageenin paw edema and cotton pellet granuloma. MS-1112 was less active than Dexa in glycogen liver deposition activity and in the depression of plasma level of corticosterone but more active than Val and Hydr. In the dose administered, MS-1112 had neither androgenic and anabolic nor estrogenic and anti-estrogenic activity. From this data, it concluded that MS-1112 is a very potent agent applicable regarding permeability and retention of steroids in a local site.

Endoscopic treatment of early gastric cancer.

Laser therapy and endoscopic strip biopsy for early gastric cancer are useful measures from our experience. It is considered that the larger lesion of early cancer should be treated by laser therapy and the smaller lesion by strip biopsy. However, endoscopic therapy as a curative treatment still presents problems. Clinical trials with laser therapy has to accumulate to ascertain the effectiveness of laser on the gastric wall. Also, endoscopic treatment methods are limited to patients who cannot be admitted to surgery. Furthermore, it is difficult to judge the metastasis to lymph nodes surrounding the stomach with present diagnostic capabilities. Wider application of these endoscopic treatment methods will follow when these difficulties are overcome.
