[Evaluation of plasma PIVKA-II as a new marker for hepatocellular carcinoma].

We have measured the plasma PIVKA-II levels in 188 cases of various liver disease with HCC and malignant diseases in other organs by an EIA, using a monoclonal antibody (E-1023 kit, Eisai), and also have measured the plasma vitamin K levels in cases of HCC and cholestasis by an HPLC. Plasma PIVKA-II was detected in many cases of HCC (67%, 35 of 52 cases) and cholestasis (60%, 6 of 10 cases). In contrast, the positivities of PIVKA-II in the other diseases including benign liver diseases were very low. Combination assays of PIVKA-II and vitamin K revealed that PIVKA-II correlates with vitamin K in cholestasis but not in HCC, suggesting that PIVKA-II in HCC does not depend on a systemic deficiency of vitamin K. From these results, it was concluded that PIVKA-II is a reliable marker which can reflect the clinical course of HCC.

[A staining method of PIVKA-II by an indirect immunoenzyme technic].

The localization of PIVKA-II in liver tissue was studied by the immuno-staining method using monoclonal antibody. The cell lines derived from hepatoblastoma (huH-6), hepatocellular carcinoma (PLC/PFR/5), and normal liver (Chang) were used as materials for staining. We succeeded to stain PIVKA-II, when the materials were fixed in a formalin solution and frozen. In patients with hepatocellular carcinoma, PIVKA-II was stained in the cytoplasm of both hepatoma and non-hepatoma cells.

A computerized standard flow-generator for spirometer calibration.

We analyzed the dynamic characteristics of a newly designed standard flow generator (SFG) to be used for spirometer calibration. The SFG consisted of a computer, large-bore cylinder, and piston driven by a high torque motor. The computer develops numerical equations to simulate a flow-volume pattern with different convexities of the descending limb. The excursion of the piston was processed according to the computer-developed equation. The computer's numerical solutions were compared to those of the SFG and agreement of the values of the peak flow, forced expiratory volume, and flow-volume pattern was satisfactory in repeated trials. Application of an external resistive load (3.5 cm H2O/sec) did not change the flow output. However, there were several minor disagreements due to the dynamic characteristics of the SFG. In an emphysema pattern the ascending limb of the generated flow-volume curve flattened and the descending limb was less convex compared to the preset pattern. When the SFG was generating sinusoidal waves of high frequency (e.g., 10.0 Hz) the flow output was deformed. The newly designed SFG was used to examine the dynamic characteristics of rolling-seal spirometers. There was an overshoot of the peak flow, over-convection, and oscillations of the descending limb in the spirometer output. These deformations were more prominent with the severe emphysema pattern. It is concluded that the SFG can be very useful in the examination and calibration of spirometers.

Genetic analysis and infection of SIVAGM and SIVMND.

Recently, the authors determined the partial sequence of simian immunodeficiency virus (SIV) from the mandrill (SIVMND) and found SIVMND to be a new member of the HIV/SIV group, equidistant from other members, including SIVAGM. Experimentally, the African green monkey and cynomolgus monkey could be infected with SIVAGM and the cottontop tamarin with SIVMND. However, no clinical sign of an AIDS-like disease was observed in these monkeys.

[The effect of succinylcholine on vecuronium and pancuronium].

In 105 adult patients under halothane anesthesia, the neuromuscular blocking effects of vecuronium and pancuronium were determined with prior succinylcholine 1 mg.kg-1 administration and without. Force of the evoked twitch increased 123.7% of control after recovery from succinylcholine-induced block. Prior administration of succinylcholine was associated with a leftward shift of dose-response curve of vecuronium or pancuronium. Onset of the force reduction from initial dose (0.08 mg.kg-1) was faster and recovery from initial and maintenance doses (0.02 mg.kg-1) were slower. This potentiating effect persisted at least 2 hours.

[A case of lung metastasis of thyroid cancer with endobronchial polypoid growth].

A case of a 73-year-old woman, complaining of exertional dyspnea and hemoptysis, is reported. About twenty years earlier, she had been diagnosed as having a thyroid carcinoma and had undergone a partial thyroidectomy. Bronchoscopic findings showed an endobronchial polypoid tumor which obstructed the intermediate bronchus almost completely. The polyp was about 3cm in length and protruded from the entrance of the middle lobe bronchus. The histologic findings revealed it to be a metastasis of the papillary thyroid carcinoma. After a polypectomy, the hypoxemia and dyspnea showed remarkable improvement.

[A cell kinetic study of liver cells in various liver diseases by the detection of DNA polymerase alpha].

DNA polymerase alpha (DNA-P alpha) in the nuclei of hepatocytes was visualized by the immunoperoxidase method to study the number of liver cells which were at the stage of G1, S, and G2 stage in the cell cycle. Seven liver specimens from patients with acute hepatitis (AH), 17 with chronic persistent hepatitis (CPH), 32 with chronic active hepatitis (CAH), 6 with liver cirrhosis (LC), 4 with hepatocellular carcinoma (HCC) and 4 with hospital controls were studied. The number of DNA-P alpha-positive hepatocytes in 1000 hepatocytes were as follows: 19.1 +/- 18.0 in AH, 8.8 +/- 6.1 in CPH, 27.3 +/- 23.8 in CAH, 21.8 +/- 14.3 in LC, 545.3 +/- 184.0 in HCC and 1.1 +/- 1.1 in hospital controls. The number of DNA-P alpha-positive hepatocytes in HCC were significantly increased compared with other liver diseases. Likewise, those in CAH and LC were higher than those in CPH and hospital controls. The liver cell necrosis was thought to be one of the secondary stimulators for cell division of hepatocytes.

Production of monoclonal antibody to thyroglobulin from malignant thyroid carcinoma.

In this study, the production of anti-human thyroglobulin (Tg) monoclonal antibodies was attempted to detect Tg levels in serum and to study the localization of thyroid metastases in patients with thyroid-gland ablation. As a result four clones of hybridoma cell lines were obtained. By means of enzyme immunoassay (EIA) and immunohistochemical method, it was found that one of them, 2G4, produced a monoclonal antibody recognizing a malignant structural change of Tg and other three clones produced monoclonal antibodies recognizing not only the normal human Tg but also the malignant Tg. Our monoclonal antibody, 2G4, will be a hopeful reagent for the diagnosis in thyroid carcinoma.

[Color vision under different luminosity in various fundus diseases].

Reports on the utilization of the color vision test as a method for detecting early abnormality in fundus diseases have been published. The author performed the 100-hue test at five luminous stage levels set between 10 and 2000 lux in cases of glaucoma, diabetic retinopathy and central chorioretinopathy. The progressive degree of each disease and the level of disorder seen in color vision were assessed comparatively at each luminous level to determine adequate luminosity for performing the 100-hue test. For classification of the degree of progression in each disease, visual acuity, visual field and C/D ratio were used for glaucoma. Visual acuity, type of retinopathy and the period of contracting the disease were used for diabetic retinopathy, and visual acuity was used for central chorioretinopathy. As a result, the total error score in the 100-hue test closely correlated to the degree of progression of each disease and the luminosity for performing the most effective 100-hue test was found to be 100 lux.

Multiple forms of pancreatic polypeptide-related compounds in the lamprey CNS: partial characterization and immunohistochemical localization in the brain stem and spinal cord.

Although neuropeptide Y (NPY) is established as a transmitter in many regions of the nervous system, the role of other peptides of the pancreatic polypeptide (PP) family in the CNS is obscure. This study provides evidence that PP-like peptides in the "primitive" CNS of a cyclostome are composed of different molecular forms, which are stored in separate neuronal populations with apparently different functions. PP-like material was detected in extracts of brain and spinal cord from Lampetra fluviatilis by radioimmunoassay (RIA) using an antiserum to the C-terminal hexapeptide of mammalian PP. The PP-immunoreactive material consisted of several molecular forms, as shown by its complex elution profile on high-performance liquid chromatography (HPLC). The cellular distribution of PP-like immunoreactivity was studied with indirect immunofluorescence histochemistry using antisera toward porcine peptide YY (PYY), porcine neuropeptide Y (NPY), and bovine (BPP), rat (RPP), and avian (APP) pancreatic polypeptide. Adjacent sections from the brain stem and spinal cord of L. fluviatilis and Ichthyomyzon unicuspis, incubated with the different antisera, displayed 2 main patterns of PP immunoreactivity. The PYY and RPP antisera labeled groups of neurons and fibers in the rhombencephalic and mesencephalic reticular formation. One of the PYY/RPP-ir cell groups, located in the anterior rhombencephalic reticular nucleus, had a projection to the dorsolateral spinal cord. Fibers of this reticulospinal system were in close apposition to dendrites of intracellularly stained spinal motoneurons and sensory relay interneurons, indicating that they may receive PPergic input. In contrast, antisera to NPY and APP labeled local neurons systems in the spinal dorsal horn, in the lateral parts of the brain stem, including the rhombencephalic alar plate, and in the retina. The BPP antiserum recognized the NPY/APP as well as the PYY/RPP immunoreactive neuron systems, further supporting that they both contain PP-like peptides.

Ascending and descending projections of the nucleus reticularis gigantocellularis in the cat demonstrated by the anterograde neural tracer, Phaseolus vulgaris leucoagglutinin (PHA-L).

Ascending and descending projections of the nucleus reticularis gigantocellularis (NRGc) were studied in the cat by the anterograde tracer, Phaseolus vulgaris leucoagglutinin. Ascending fibers from the left or the right NRGc coursed through the bilateral medial reticular formation and some of them reached the diencephalon. In the brainstem, PHA-L-labeled fibers and their terminals were observed in the medial reticular formation, the cranial motor nuclei (III, IV, V, VI, VII, XII), the vestibular complex, the LC complex, the raphe nuclei, the periaqueductal gray, the red nucleus, the Edinger-Westphal nucleus and the interstitial nucleus of Cajal. In the diencephalon, they were observed in the dorsal thalamus and the hypothalamic regions. In the caudal medulla, labeled fibers and their terminals were observed in the nucleus prepositus hypoglossi, the nucleus intercalatus and the inferior olive. Descending axons from the NRGc coursed bilaterally through the ventral and ventrolateral funiculi as far caudal as the upper thoracic cord. Single axon collaterals arising from the descending axons gave off terminal fibers to the left or the right gray matter. Their terminals were located in laminae V-X, mainly in laminae VII and VIII. In lamina IX, they were distributed mainly in the medial portion. A few fibers originating from the descending axons ipsilateral to the PHA-L injection side coursed through the anterior or posterior commissure, and ended in laminae VI, VII and VIII. The functional implications of these findings are discussed in relation to the behavioral state control and the generalized motor inhibition.

Adrenergic control of the glucagon response to ammonia in the perfused rat pancreas.

The isolated perfused rat pancreas was used to investigate how adrenergic influences within the pancreas might mediate ammonia-induced glucagon secretion. The addition of 2 mM ammonia to the perfusate increased norepinephrine release and glucagon secretion in the effluent. Upon cessation of ammonia addition, a pronounced burst of glucagon release was observed. Alpha-adrenergic blockade with phentolamine (10 microM) blocked the glucagon response to ammonia. Beta-adrenergic blockade with propranolol (10 microM) had no significant effect on the amount of glucagon release induced by ammonia. Depletion of norepinephrine from sympathetic nerve terminals by pretreatment with 6-hydroxydopamine lowered the pancreatic norepinephrine content to less than 16% of the control value and diminished the glucagon and norepinephrine response to ammonia almost completely. The burst of glucagon release after the removal of ammonia was inhibited to 2% of the control value by phentolamine and to 57% by propranolol. Pretreatment with 6-hydroxydopamine reduced the burst of glucagon secretion to 28% of the control value. Neither phentolamine nor propranolol reduced the magnitude of the ammonia-induced suppression of insulin secretion. We conclude that the effect of ammonia on glucagon release from the isolated rat pancreas is mediated by intrapancreatic adrenergic control.
